[Secreting parathyroid carcinoma. Apropos of a case].

Parathyroid carcinoma is rare and hits diagnosis often difficult. A case is reported, remarkable by the age of the patient (17 years), and the main peculiarities of the disease are reviewed: frequency of complete bone syndrome, palpable tumour of the neck, calcemia more than 140 mg/l, occurrence of recurrent palsy; relapsing course, death being dependent on hypercalcemia rather than metastasis; median survival time about five years, non influenced by chemotherapy. Ultrastructural study, previously realized in about thirty cases, is specially evaluated: it seems able to concur on anatomopathologic diagnosis, when assertion of malignancy is difficult.

[Arterial hypertension with secondary hyperaldosteronism, reversible by nifedipine, in systemic scleroderma].

A case of severe hypertension with secondary hyperaldosteronism developing in a 71 year old woman affected by "Crest" syndrome is reported. The patient was treated by nifedipine, which led to the correction of blood pressure, plasma renin activity and serum aldosterone. The role of renal arteries vasospasm and renin-angiotension system in the genesis of scleroderma is discussed.

[Bone metastasis of breast cancer. Apropos of a series of 50 cases].

Among fifty patients with bone metastases from breast cancer who were hospitalized between 1971 and 1980, 19 were given intermittent chemotherapy including both hormone-therapy (drostanolon propionate and tamoxifen) and a combination of cyclophosphamide, methotrexate, 5 FU, doxorubicin and vincristine (CMFDV). Comparison of these patients with two other groups given either miscellaneous chemotherapy programs (19 cases) or no chemotherapy (12 cases) exhibited the value of the standardized protocol, especially concerning median survival which exceeded three years (versus one year with the two other methods). Cobalt-therapy, chemotherapy and hormone-therapy in bone metastases from breast cancer are reviewed.

[Dermatological manifestations of synovial cysts of the lower limbs (author's transl)].

Two cases of synovial cysts of the knees are reported. Thier emergeing point located far away from the joint they originate from can make the diagnosis be difficult. Puncture allows to remove synovial fluid and to inject a radio-active iodine compound to realise a cystography. The radiography of the joint demonstrates an already existing joint disease. The atypical aspects of synovial cysts are emphasized on the basis of then two cases such as pseudo-phlebitic, pseudo-arteritic, inflammatory and fistulised. The presentation of synovial cysts of the legs is frequently dermatological. The diagnosis must be suspected in order to avoid a dermatological biopsy.

Sedation for outpatient conservative dentistry. A trial of pentazocine supplementation to diazepam and local analgesia techniques.

Pentazocine 30 mg. or 15 mg or a placebo, was administered randomly to forty-nine patients undergoing conservative dental treatment in combination with a local analgesic block and intravenous diazepam. Simple cardiorespiratory measurements were made throughout the treatment period. Patients in the 30 mg pentazocine group required some 6 mg diazepam less than the placebo (control) group (P less than 0.05). Patients receiving 15 mg pentazocine also required less diazepam compared to the control group, but this difference was not statistically significant. There were no significant differences between the three groups either in recovery times or the cardiorespiratory measurements.

[Changes in blood calcium, phosphorus and alkaline phosphatase levels in rheumatoid polyarthritis and other types of inflammatory rheumatism].

Albuminemia, calcemia, phosphoremia and alcaline phosphatasemia were measured in three groups of 52 subjects each : rheumatoid arthrits, inflammatory rheumatisms other than rheumatoid arthritis and lumbarthrosics serving as a reference group. Calcemia and albuminemia were significantly lower in patients suffering from rheumatoid arthritis, whose calcemia corrected in relation to albuminemia is, on the other hand, normal : the increase in corrected calcemia pointed out by Kennedy, was not noted. Corrected calcemia was also normal in ankylosing spondylitis, but it was significantly higher in polymyalgia rheumatica. Phosphoremia was shown to be normal but alkaline phosphatases were higher than normal in the three groups.

[Hypocomplementaemic leucocytoclasic vasculitis. (Mac Duffie's syndrome). One case with bacterial lymphadenitis (author's transl)].

Mac Duffie's syndrome includes a hypocomplementaemic allergic vasculitis with essentially cutaneous and articular manifestations. Its pathogenesis, still incompletely elucidated, involves the precipitation of immune complexes in the walls of the all vessels. The problem remains as to whether the alteration in the complement system is merely a reflection of this formation of immune complexes, or if it is primary, favourising the chronic infections which produce such complexes. The significance of the presence of low molecular weight precipitins reacting with C1g in the serum of a number of patients remains uncertain. The case reported, with its new clinical features, does not provide an answer to these questions but emphasises the possible role of chronic infections in the origin of this syndrome.

[Synoviorthesis of the knee using osmic acid: apropos of 61 injections].

The authors report their experience of 61 injections of osmic acid into the knee during the course of rheumatoid polyarthritis. The results stated to be very good and good, are classified by the disappearance of pain, swelling and stiffness, or a improvement in any one of these criteria. As observed in 67% of cases at the third month the result following changed with time. While these favourable results persisted into the fifth month, there was a remarkable maintenance of them to the 30th month in those patients who it was able to follow so long. The most distinct and lasting results were seen in those joints which showed the least radiological change. Tolerance of the injection of osmic acid was satisfactory apart from several painful reactions which improved in the 24 hours following injection. Two cases of thrombo-phlebitis were seen and one case of rupture of the synovium. If the results obtained with osmic acid appear less impressive than those obtained with isotopes, the simplicity of the method attracts attention as it needs no complicated equipment and is applicable to young invalids.

[Treatment of rheumatoid arthritis with levamisole. Preliminary study of clinical and immunological results].

From September 1975 to September 1976, eleven patients suffering from rheumatoid arthritis in whom gold salts were either inactive or contraindicated were treated with levamisole. Treatment was interrupted in one case because of an allergic rash and in one as a precaution. There was an improvement in six, taking the form of a decrease in clinical symptoms and signs and a slowing of sedimentation rate. Certain immunitary changes were seen, in the sense of a reinforcement of humoural and cellular immunity. Fear of sudden agranulocytosis should lead to caution in further clinical trials.
