HIV-1, HIV-2, and HTLV-I infection in high-risk groups in Brazil.

We conducted a serologic survey for antibodies to human immunodeficiency virus types 1 and 2 (HIV-1 and HIV-2) and human T-cell lymphotropic virus Type I (HTLV-I) in 704 Brazilians with the acquired immunodeficiency syndrome (AIDS) or at risk for it. The study population included 70 homosexual men (11 of whom were prostitutes), 58 bisexual men (19 of whom were prostitutes), 101 female prostitutes from three socioeconomic groups, 13 wives of men with hemophilia who were seropositive for HIV-1 antibodies, and 47 blood donors with positive Venereal Disease Research Laboratory tests for syphilis, all from Rio de Janeiro; 86 female prostitutes from two rural towns in Minas Gerais; 133 patients with AIDS from São Paulo; and 196 men with bleeding disorders who were seropositive for HIV-1 antibodies on enzyme-linked immunosorbent assay, from São Paulo and Rio de Janeiro. The prevalence of HIV-1 infection was highest in the homosexual male prostitutes (45 percent), the wives of patients with hemophilia (38 percent), the bisexual men (28 percent), the homosexual men who were not prostitutes (19 percent), and the female prostitutes from the lower class (9 percent). Combined HIV-1 and HIV-2 infection was found in 3 percent of the patients with AIDS and in 1 percent of the homosexual men. The prevalence of HTLV-I infection ranged from 1 percent in rural female prostitutes to 13 percent in HIV-1-positive men with bleeding disorders in Rio de Janeiro. Combined HIV-1 and HTLV-I infection occurred in 1 to 11 percent of some male subgroups. We conclude that in Brazil HIV-1 infection is already well established among homosexuals, bisexuals, and lower-class female prostitutes, with bisexual men probably acting as a bridge between the heterosexual and homosexual communities, that HTLV-I infection is prevalent in groups at risk for AIDS, and that HIV-2 infection has already been introduced into the country.

Methyl-accepting taxis proteins in Halobacterium halobium.

Methyl-accepting taxis proteins were identified and characterized in Halobacterium halobium, an archaebacterial species that is both chemotactic and phototactic. The data suggest direct involvement of methylation and demethylation in mechanisms of both chemotaxis and phototaxis and identify adaptation as the sensory process in which those reactions are likely to be involved. Analysis by electrophoresis and fluorography revealed methyl-accepting species, of apparent Mr between 90,000 and 135,000, that exhibited characteristics of sensory components. Those methyl-3H-labeled species were absent in a mutant blocked in taxis. Methylation of specific bands increased after positive chemostimuli and decreased after negative stimuli. Other methyl-3H-labeled bands, from 17 to 29 kd, exhibited features of biosynthetic intermediates, not of sensory components. Assay of rates of demethylation by measuring release of volatile forms of radiolabeled methyl groups revealed transient changes following chemo- or photostimuli that persisted for periods roughly equivalent to adaptation times. Negative chemostimuli induced increased rates of demethylation, as expected from fluorographic analysis, but positive chemostimuli also resulted in an increase. Photostimuli of either sign were followed by increases in rates of demethylation of shorter duration and lesser magnitude than chemostimuli-induced increases, a relationship that corresponded to differences in adaptation time.

Entamoeba histolytica depresses chemiluminescence in stimulated human polymorphonuclear leukocytes.

Effect of Entamoeba histolytica proteinase/toxin (Ehp/t) on the luminol-dependent chemiluminescence (CL) in stimulated human polymorphonuclear leukocytes (PMN) was studied. The role of superoxide (SO) and hydroxyl (OH) anions in the Ehp/t-associated enhancement/inhibition of CL was also studied using specific scavengers and a biological response modifier, muramyldipeptide (MDP). Ehp/t was isolated from axenic trophozoites of the HM-1:IMSS strain of virulent strain of E. histolytica. Proteinase activity was assayed on a synthetic substrate, Z-arg-arg-AFC and cytotoxicity was tested on HeLa cell monolayers. PMN isolated from blood were incubated with Ehp/t prior to stimulation by phorbol myristateacetate (PMA, 2 micrograms/ml), serum-treated zymosan (2.5 mg/ml) and glucan (2 mg/ml). CL was monitored in an LKB (Wallac) Luminometer. Ehp/t was found to depress up to 90% of CL induced by PMA, glucan and zymosan. Such a depression was Ehp/t concentration-dependent. A 150 micrograms/ml concentration of Ehp/t, obtained from a 0.015-1.5 mg/ml concentration range tested at different incubation times and temperatures, was used in most of our experiments. Incubation time and temperature optima were 15 min and 37 degrees C, respectively. Ehp/t partially inhibited the CL associated with SO and OH. MDP, in the presence of Ehp/t, enhanced CL response in human PMN to about 67% with reference to normal CL without inhibitor. PMN were confirmed to play a vital role in amebic tissue invasion mechanisms.

The porcine bioprosthetic heart valve: experience at 15 years.

The porcine bioprosthetic valve has been in use at Henry Ford Hospital since 1971. In this review, 980 patients with 1,081 porcine bioprosthetic valves were examined from 1 month to 16.4 years after implantation with a 99% complete follow-up. Patient survival was 59% +/- 2.2% (+/- standard error of the mean) at 10 years and 38% +/- 4.0% at 15 years. Factors associated with decreased survival after hospital discharge were age greater than 35 years and New York Heart Association functional class IV. Freedom from thromboembolism was 92% +/- 1.2% at 10 years and 89% +/- 3.2% at 15 years. Freedom from endocarditis was 93% +/- 1.2% at 10 years and 92% +/- 1.3% at 15 years. Freedom from structural valve degeneration for all valves was 71% +/- 2.6% at 10 years and 31% +/- 5.6% at 15 years. Factors associated with increased risk of structural valve degeneration were age younger than 35 years, female sex, and preoperative cardiac index greater than 2 L/min/m2. Among a total of 172 patients undergoing removal of a degenerated valve, mortality was 12.5%, and significant risk factors for death at reoperation were emergency operation and functional class IV. Experience with the porcine bioprosthetic valve after 15 years suggests that its use be confined to older patients or patients with a contraindication of anticoagulation.

Comparability of echocardiography and chest X-ray following myocardial infarction.

In a group of 23 patients with first-time myocardial infarction (MI) we compared the results of echocardiography and chest X-ray as measured 1 week, 2 months, and 6 months following acute MI. Left ventricular end-diastolic dimension (LVEDd) and left atrial (LA) dimension were measured from the echocardiogram, and the cardiac volume in ml/m2 body surface area (BSA) was calculated from the chest X-ray. A progressive increase in LA dimension was noticed during the 6-month period: a significant increase after 2 months (P less than 0.001) with a further increase at 6 months compared with after 2 months (P less than 0.001). The changes in LA dimension were more pronounced in anterior and Q-wave infarction (P less than 0.001) than in inferior and non-Q-wave infarction (P less than 0.01). On the other hand, LVEDd showed a less conspicuous change: a moderate increase (P less than 0.05) at 2 and 6 months, also with a more pronounced change in anterior wall and Q-wave infarction (P less than 0.01). There was no significant concurrent change in the calculated heart volume in ml/m2 BSA, as measured from the chest X-ray. It is suggested that the observed changes in LA dimension reflect reduced left ventricular compliance after MI.

Effects of acute myocardial infarction on the displacement of the atrioventricular plane: an echocardiographic study.

The effects of acute myocardial infarction (MI) on the displacement of the left ventricular (LV) atrioventricular (AV) plane is presented. The material consisted of 40 patients with first-time Q-wave MI (26 pts with ant. MI and 14 with post. MI). Nineteen age-matched healthy subjects were used as controls. The displacement of the AV plane was determined at four sites corresponding to anterior (AV-A), septal (AV-S), posterior (AV-P) and lateral (AV-L) parts of the LV wall. In anterior and posterior MI all the sites showed significantly reduced AV-plane displacement compared to healthy subjects (P less than 0.001). Moreover, within the anterior MI group the AV-A and AV-S displacements were significantly reduced compared to the points AV-L and AV-P (P less than 0.001) and the AV-L displacement was reduced compared to AV-P (P less than 0.001). In posterior MI the displacement at AV-P was reduced compared to other points (P less than 0.001) and, to a certain extent, AV-S was reduced compared to the points AV-A and AV-L (P less than 0.001). The reduced magnitude of the AV plane displacement seems to be an expression of regionally reduced systolic function. The method described may provide a simple means of defining regional wall motion abnormalities of the LV following MI.

Color Doppler features of left ventricular pseudoaneurysm.

A patient had left ventricular pseudoaneurysm after mitral valve replacement surgery. The diagnosis was made by color flow Doppler demonstration of systolic flow between the left ventricular chamber and an echo-free space posterior to the heart which was initially interpreted as localized pleural effusion. Color Doppler features of this entity have not been previously described. Color flow cardiac Doppler enhances echocardiographic and pulsed Doppler diagnosis of left ventricular pseudoaneurysms.

Urinary tract infection.

Over two years, 9892 mid-stream urine samples from patients attending the Aga Khan University Hospital, Karachi were cultured. Significant bacterial growth was seen in 23.5% samples. Further identification of these organisms revealed 40% of E. coli, 16% Pseudomonas aeruginosa, 11% Klebsiella aerogenes, 5.0% Enterobacter sp., 13% Proteus sp., 4.0% Serratia liquifaciens, 1.0% Acinetobacter sp., 3.0% Citrobacter sp., 4.0% Enterococci, 0.5% Staphylococcus aureus. Results of sensitivity tests performed with antibiotics Ampicillin, Cotrimoxazole, Nitrofurantoin, Nalidixic acid, Gentamicin, Amikacin, Pipemedic acid, Cefotaxime, Azactam and Carbenicillin did not reveal any distinct pattern.

Neisseria meningitidis storage identification and MIC determination.

The significance Minimum Inhibitory Concentration (MIC) of determination for Meningococci is described and according to the present study a shift in the sensitivity of Meningococcus is occurring, local isolates show higher Minimum Inhibitory Concentration for Chloramphenicol and Azactam. Strains of Meningococcus can successfully be stored in a domestic freezer.

Echo-Doppler features of pulmonary valve endocarditis.

Three patients with fever and pneumonitis were found to have pulmonary valve vegetations by echocardiography. Pulsed and color flow Doppler studies demonstrated mild to moderate pulmonary valve insufficiency in these patients. All patients responded to the appropriate antibiotics without requiring valve surgery. Echocardiography is of value in diagnosing pulmonary valve endocarditis and should be considered in every patient with recurrent pneumonitis.

Structure of 3-O-acetylgibberellin A3.

C21H24O7, Mr = 388.4, orthorhombic, P2(1)2(1)2(1), a = 13.279 (10), b = 15.615 (10), c = 9.427 (8) A, V = 1954.7 A3, Z = 4, D chi = 1.319 g cm-3, lambda(Mo K alpha) = 0.71069 A, mu = 1.0 cm-1, F(000) = 824, T = 293 K, R = 0.049 for 1805 observed reflections. The gibberellane skeleton shows the conformational variety characteristic of many other gibberellin derivatives. Ring A is in a sofa conformation while the five-membered ring B assumes an intermediate conformation between an envelope and a half-chair. The lactone ring is in an ideal envelope conformation and shows a marked difference in its two C-O distances. Each molecule is hydrogen bonded to two others to form continuous spiral chains along the c axis.

Influence of cationic superoxide generation enhancing protein (SGEP) on phagocytic and intracellular bactericidal activity of human polymorphonuclear cells.

Cationic fraction III from the lysosomes of normal human peripheral blood polymorphonuclear cells (PMNs) was found to contain superoxide generation enhancing protein (SGEP). Herein, we report on the influence of partially purified SGEP obtained from fraction III (subfractions III-5 and III-6), on various phagocytic functions of human PMNs. SGEP markedly enhanced intracellular bactericidal activity of human peripheral PMNs. The enhancement was time and dose dependent. It also reduced adhesiveness of the PMNs. SGEP did not influence chemotaxis, phagocytosis or phagocytic index. These findings are compatible with our original observation regarding superoxide generation enhancement properties of SGEP.

Atrioventricular valve plane displacement in healthy persons. An echocardiographic study.

A method of measuring the displacement of the atrioventricular (AV) of the left ventricle plane during the cardiac cycle in 71 healthy persons is described. An echocardiographic equipment with two cursors was used. Measurements were performed from four sites in the AV plane situated about 90 degrees apart and corresponding to the septal, anterior, lateral and posterior myocardial walls. The mean displacement during systole was 16 mm towards the apex. There was no significant difference in the recordings from the four sites. The study population was divided into three groups with mean ages 28, 42 and 60 years (group I, II and III). The displacement was significantly smaller in group III compared with groups I and II. Fractional shortening, however, could not demonstrate such a difference. The determination of displacement of the AV plane may imply the introduction of a new and simple method in assessment of left ventricular function.
