Expression of bcl-2 in papillary thyroid cancers and its prognostic value.

PURPOSE: Papillary thyroid cancer has a good prognosis. This favourable prognosis may be attributed to the apoptotic tendency of the cancer cells. This study aims to evaluate the expression of bcl-2, which is an antidote of apoptosis, and aims to evaluate the value of bcl-2 as a prognostic marker in papillary thyroid cancer. MATERIAL-METHODS: Bcl-2 expression in the archival materials of 31 patients with papillary thyroid cancer was examined with immunohistochemical methods using bcl-2 and p-53 stains. The results were compared with 31 normal thyroid tissue specimens, which consisted of the contralateral lobes of these patients. The results were then analyzed in accordance with the clinical features of the patients. RESULTS: Thirty (96.7%) patients of the control group were positive for bcl-2 whilst one (3.3%) was negative. The staining for bcl-2 was positive in 12 (%75) patients with microcarcinomas (p < 0.05) and 13 (86.6%) with papillary cancers (p > 0.05). Two cases of the papillary cancer group were admitted to the hospital with local recurrence (6.4%) and both were positive for bcl-2 (p > 0.05). All cases (4/31), whose MACIS scores were higher than 7 were positive for bcl-2. Twenty-one of 27 cases whose MACIS scores were lower than 7 (77.7%) were positive for bcl-2 (p > 0.05). All tumours of this series were negative for p-53 immunstaining. CONCLUSION: The rate of bcl-2 expression in microcarcinomas of papillary thyroid cancer decreases when compared to normal thyroid tissue. This may be an early sign of oncogenesis, and a reason for the favourable prognosis in microcarcinomas. However, bcl-2 cannot be used as a prognostic marker.

Aplastic anemia due to chemicals and drugs: a study of 108 patients.

To illustrate the etiologic role of drugs and chemicals in the development of aplastic anemia, we analyzed 108 cases of aplastic anemia among 3,715 hematologic patients during a ten-year period at the hematology section of Istanbul Medical School. Among these 3,715 patients, 695 had leukemia, a result indicating the relative rarity of aplastic anemia as compared with leukemia. Of the 108 patients, 58.3% were male and 41.7% were female. Their ages ranged from six months to 82 years. Pancytopenia was severe in 42.6% of the patients, and moderate or mild in 57.4%. Bone marrow was hypocellular in 84 patients, normocellular in 16, and hypercellular in eight. In 52 (48.1%) of the patients with aplastic anemia, the following etiologic factors were implicated: benzene (25 patients), antirheumatic drugs (ten), chloramphenicol (four), chloramphenicol plus hepatitis or chromosome anomalies (two), thiamphenicol plus sulfonamide (one), antituberculous drugs (three), daraprim (three), insecticides (two), hepatitis (one), and sulfonamide (one). Data indicate that the degree of bone-marrow cellularity is not always related to functional capacity, and numerous agents may have etiologic roles in the development of aplastic anemia.

Spermatic granuloma in the ram: diagnosis by ultrasonography and semen characteristics.

Ultrasonographic appearance of the testis and epididymis, and seminal characteristics, with regard to localization of spermatic granuloma were studied. In rams with spermatic granuloma (n = 9), diagnosed by clinical or ultrasonographic examinations before histopathological confirmation, localization of each lesion was recorded. Epididymal granulomas, caput and cauda, were seen as anechoic or hyperechoic areas with a distinct margin with or without a hyperechoic capsule. Granulomas in the testis were microscopic and, therefore, could not be detected by ultrasonography. Enlargement in the mediastinum testis was detected in all rams when there were granulomas in the caput. Heterogeneous testis parenchyma invaded by numerous hyperechoic foci, representing testicular degeneration, was associated with granulomas both in the epididymis and testis. Ultrasonographic appearance of the lesions did not differ with regard to season. Seminal characteristics varied between rams. It was concluded that ultrasonographic evaluation may give valuable information in the diagnosis of sperm granuloma in the ram.

The history of beta-thalassemia in Turkey.

The first two patients with beta-thalassemia major in Turkey, were reported in 1941. However, the importance of beta-thalassemia as a health problem was brought to the attention of physicians only after 1950. In Turkey, the clinical and hematological pictures of beta thalassemia were found to be mostly heterogenous. Hematological data of patients with mild disease (beta-thalassemia intermedia) severe beta-thalassemia (beta-thalassemia major) and heterozygotes indicate that patients can be grouped according to the content of Hb F, Hb A2 and red cell indices.

The distribution of haptoglobin types in various regions of Turkey.

The distribution of haptoglobin (Hp) types and the gene frequencies of Hp1 in various regions' of Turkey are discussed. The Hp1 gene frequency ranged between 20.4 percent for Turks living in the eastern part of Turkey and 32.6 percent in those who either are at present living in or have migrated from Western Thrace. Generally, the Hp1 gene frequency in the Turkish population is 26.5 percent, a figure similar to that recorded for Asians. Minor variations in the Hp1 gene frequencies in various parts of Turkey may be assumed to have been caused in certain regions by intermingling with the local populations.

Effects of octreotide and a-tocopherol on bacterial translocation in experimental intestinal obstruction: a microbiological, light and electronmicroscopical study.

BACKGROUND/AIMS: Bacterial translocation induced by intestinal obstruction is suggested to be due to increased intestinal luminal volume, leading to intestinal overgrowth with certain enteric microorganisms and intestinal mucosal damage. If this suggestion is true, maintenance of intestinal mucosal integrity by a cytoprotective agent, a-tocopherol, and inhibition of gastrointestinal secretions by octreotide should decrease the incidence of bacterial translocation and extent of mucosal injury due to intestinal obstruction. METHODS: Complete intestinal obstruction was created in the distal ileum of male Wistar Albino rats by a single 3-0 silk suture. The animals received subcutaneous injections of 1 ml of physiologic saline (group 1) (PS 24) and 1 ml of saline containing octreotide acetate (100 micrograms/kg) (group 2) (OC 24), at 0, 12 and 24 hours of obstruction. In group 3 (PS 48) and group 4 (OC 48), the rats were treated with subcutaneous physiologic saline (1 ml) and octreotide acetate (100 micrograms/kg), respectively, beginning at the time of obstruction and every 12 hours for 48 hours. The rats in group 5 (Toc 24), were pretreated with intramuscular a-tocopherol 500 mg/kg on day 1 and 8, and underwent laparotomy on day 9. A third dose of a-tocopherol was injected at the time of obstruction on day 9 and no treatment was given thereafter. We tested the incidence of bacterial translocation in systemic organs and circulation and evaluated the histopathological changes in all groups. RESULTS: Treatment with octreotide acetate was found to be ineffective in reducing the incidence of translocation, with no histopathological improvement. Mucosal damage scores, on the other hand, in the a-tocopherol group were statistically less than those in the octreotide and control groups (p < 0.05). Additionally, a-tocopherol treatment decreased the incidence of organ invasion with translocating bacteria, although this difference did not reach statistical significance. CONCLUSION: Octreotide acetate treatment in complete intestinal obstruction has no effect on the incidence of bacterial translocation. a-Tocopherol, on the other hand, has a cytoprotective effect on intestinal mucosa in intestinal obstruction which, in turn, is thought to decrease bacterial translocation when used in physiological doses and prophylactically.
