Present status of PACS activities in Japan.

Three PACS organizations in Japan are: J-PACS, JAMIT-PACS and JRS-PACS. J-PACS committee is composed of representatives of manufacturing companies. Many Japanese companies are actively engaged in developing PACS-related products and services. The annual JAMIT Symposium includes an International PACS day. The JRS-PACS Committee is working to bring academics and industry closer together in developing and implementing PACS technology. MIPS has two working groups which started in 1985. The Survey Working Group issued the draft on Survey study of integral technology and present state and trends of existing systems. The Standardization Working Group, which is to complete work in 1989, issued the draft of image data format on the magnetic tape and discussion about the standardization of online digital interface for PACS. The draft of the MIPS Standard was mostly based on ACR/NEMA Standard, but simplified and reduced. However, the number of minimum parameters essential in image transfer were retained. Early experiences with PACS in Tominaga Memorial Hospital, Kyoto University Hospital and Kochi Medical School Hospital was briefly mentioned. Japan is facing a period in the early 1990s in which PACS is to be widely introduced into the university hospitals.

MIPS Standard activities in Japan.

The Medical Image Processing System (MIPS) plan is a standard plan for the online communication of imaging data in Japan. The ACR-NEMA Standard is not always easy for manufacturers in Japan to comply with. This situation led to the formulation of MIPS Plan Minimum Set-87 (MIPS Standard-87), which uses reciprocal corresponding patterns to ACR-NEMA Standards. The working groups on current and future trends study data collected with a questionnaire in 1985 regarding (1) current technology and (2) its future direction as well as (3) the status of utilization of medical imaging and its data management. In 1986, they engaged in further investigations on the above subjects and presented their views on standardization. Conformance tests have been discussed and the conclusions were published in 1987 and 1988. Protocol conversion between MIPS Standard-87 and ACR-NEMA Standard was also discussed and the conclusions were reported. At the beginning of 1988, validation from medical viewpoints, e.g. the setting of anatomical position codes, was requested by medical members. Each layer of MIPS is compared briefly with the ACR-NEMA Standard. A small picture archiving and communication (PAC) system for three X-ray computer tomography units (X-CTs) has been constructed at the University of Tokyo Hospital. In this implementation, X-CTs and an imaging filing system are successfully connected by MIPS Standard-87. Some problems of MIPS Standard-87 are mentioned.

PACS in Japan and the Nordic Countries. First Japan-Nordic PACS symposium.

The ingredients seem to exist for switching from film-based radiology to digital radiology utilizing the concept Picture Archiving and Communication System (PACS). Why has this not happened? What are the barriers limiting the diffusion of PACS into health care? This was the theme of the 1st Japan-Nordic Symposium in PACS, which was held in Tokyo, March 29-30, 1990. Over 40 radiologists and technical specialists in PACS participated. The topics discussed ranged from the present status of PACS in Japan and the Nordic Countries, to functional specifications and user requirements for PACS, to organizational and other changes brought about by PACS and to teleradiology as a solution for communication of images over wide distances. The symposium was quite successful and will be followed by a 2nd symposium in Finland in the summer of 1991.

Lymphangiomatosis of bone--lymphographic and CT findings.

A case of lymphangiomatosis of bone associated with arachnoid cysts of the spinal cord is presented. This disease is very rare and in some cases the definitive diagnosis has been made radiologically rather than histologically. Lymphography and CT scanning showed on inflow of the oily contrast medium within the bony trabeculae. CT was also effective in detecting other anomalies that were otherwise not expected.
