Effect of tungsten absorption edge filter on diagnostic x-ray spectra, image quality and absorbed dose to the patient.

The X-ray spectra from a tungsten-target diagnostic tube were measured with a lithium-drifted silicon detector. Four characteristic X-ray peaks were clearly observed. When a 0.05-0.3 mm thick tungsten absorption edge filter was added to the tube, the number of photons in the spectra above the K-absorption edge decreased dramatically. The effect of the absorption edge filter on image quality and on the absorbed dose were investigated by both measurement and Monte Carlo calculation. The absorbed dose to the patient is reduced without image quality, being degraded.

[Radiotherapy combined with pepleomycin administration for the treatment of esophageal cancer].

A combined therapy of pepleomycin (NK-613) and radiation was performed in 15 cases of esophageal and cancer. Twelve cases out of 15 were inoperable, and 3 cases were operable. NK-631 was administered by drip intravenous injection at a dose of 5 mg per day for 3 consecutive days weekly, aiming at total dose of 60-120 mg. Tumor regression rates, which were measured by planimeter on esophagogram, were 42-92% (mean 72%): two cases were more than 90%, and more than 50% in 12 cases. An average of the survival period of 15 cases was 57 weeks with 7 cases (46.7%) of 1 year survival, 2 cases (13.3%) of 2 year survival. The side effects of NK-631 observed in the present study consisted of fever 6, stomatitis 2, skin rash 2, and reversible pneumonitis 2. This study suggests that NK-631 exhibit remarkable anti-tumor effects on esophageal carcinoma, and seem to be less toxic.

Venous angioma of the neck in a child.

Venous vascular masses in the neck are rare entities in the pediatric population. Aneurysmal dilatation or ectasia of the superficial cervical vein, the so-called "venous angioma" is a very uncommon condition. A 12-year old female with venous angioma involving the right neck is described. The preoperative diagnosis was confirmed by angiography.

Lymphography of the edematous extremities.

The mechanism of secondary lymphedema of the extremities is not always simple to understand. The unsolved mechanism of lymphedema should be further studied from exploring relationship between lymphatic and venous systems each of which has superficial and deep systems respectively.

Physical and radiological findings specific for medullary carcinoma of the thyroid gland.

Preoperative physical and radiological findings, if specific to a certain extent, are important for detecting patients with sporadic form of medullary thyroid carcinoma and especially for the first patient in the family having a hereditary form of medullary thyroid carcinoma and pheochromocytoma syndrome. To delineate clinical features of medullary thyroid carcinoma, a total of 9 patients with this tumor were reviewed retrospectively. In most patients, the thyroid lesions were located in the upper two thirds of the lobe, which was determined by careful palpation or 131I scintiscanning of the thyroid. The primary lesion in the thyroid could be felt more or less as a round, sharply demarcated nodule with fairly good mobility. These findings suggested rather a benign thyroid nodule when there was no lymph node involvement. However, it could be considered a sign suggesting medullary thyroid carcinoma when accompanied by marked lymph node metastasis. In our recent 2 cases, the diagnosis of medullary thyroid carcinoma was strongly suspected on these clinical bases, one of the cases being presented in detail. In 4 patients, lymph node metastasis in the central neck extended to either submandibular or upper mediastinal regions or both. In about one third of the patients, calcified deposits were shown in the cervical roentgenograms. With the use of soft tissue roentgenography, grossly punctate calcific deposits associated with psammoma-like shadows were recognized and the pattern was a criterion for definitive diagnosis of medullary thyroid carcinoma.
