[Medico-surgical attitude in the treatment of cancer of the bile ducts].

In order to evaluate the operative possibilities in patients with obstructive carcinoma of the biliary tract a precise diagnosis of local extent of the tumor and metastasis is essential (cholangiography, US, CTscan). The treatment is preferably surgical whether the tumor be resectable or not. However some patients who, for any reason, are not surgical candidates can be helped by new endoscopic intubation techniques. Medical and surgical consultation is essential to set the criteria for the best treatment modality in all patients with this disease, especially those who are not surgical candidates.

The significance of electron dense deposits in mild lupus nephritis.

In patients with lupus nephritis, progression from a mild lesion to a diffuse proliferative glomerulonephritis has been reported in one to 35 percent of patients. Because of the wide variation in the rate of progression, this study was undertaken to determine those factors which would identify the patients most likely to progress. Of 21 patients with a mild lupus nephritis by light microscopy, progression to a diffuse proliferative lesion was seen in only those patients who had subendothelial deposits. While not all patients with subendothelial deposits had a deteriorating course, the persistence of such deposits on subsequent biopsies indicated a poor prognosis.

The role of infectious agents in Crohn's disease.

Environmental factors certainly play a role in the appearance of Crohn's disease. Wether or not those factors are infectious agents remains uncertain. Broadly, two classes of infectious hypothesis are currently under investigation. The first one, concerning specific microorganisms (such as mycobacteria and virus) dates back to the first description of the disease in 1913. The second one studies the possible involvement of compounds derived from the intestinal microflora, irrespective to speciation. It appeared more recently and receives more and more attention. These hypothesis are reviewed by the authors with regard to data obtained from epidemiology, clinical and experimental investigations.

[Hepatic toxicity of drugs].

This review deals in its first part with the hepatic mechanisms of drug detoxification and toxification as well as with the physiopathological mechanisms of drug-induced liver diseases. In the second part, the clinical and biological pictures of drug-induced pathological lesions are reviewed. Any liver abnormality should alert the clinician towards a possible toxic aetiology so that the risk of fulminant, eventually lethal, or of severe chronic liver disease can be avoided.

Endoscopic varix ligation: a promising new technique?

Endoscopic varix ligation is a new promising endoscopic method which compete with endoscopic sclerotherapy both during acute bleeding and the chronic setting where the aim is to obliterate varices as soon as possible with minimal side effects.
