The prediction of the need for postoperative mechanical ventilation in 23 myasthenia gravis patients, who had undergone thymectomy from January 1983 to December 1989, was evaluated. Five patients who had needed postoperative mechanical ventilation over 24 hours were compared with the patients in whom the tracheal tubes had been uneventfully extubated within 24 hours postoperatively. In this study, 47.8%, 78.3%, 91.3% of the patients were correctly predicted using the scoring systems proposed by Leventhal et al., Makii et al. and Kimura et al., respectively. The product of %VC by FEV1.0% was proposed in this study for the prediction of the need of postoperative mechanical ventilation in patients with myasthenia gravis. If the product is less than 8,300, the patient is expected to need postoperative mechanical ventilation. Nineteen patients out of 23 cases (82.6%) were predicted correctly by this value. Only one patient, who had been predicted to be in no need, was actually ventilated for 3 days. In conclusion, the product of %VC by FEV1.0% is a simple and practical method of predicting whether a patient with myasthenia gravis undergoing thymectomy requires postoperative mechanical ventilation or not.
To determine whether neutrophils are involved in airway hyperresponsiveness after ozone exposure, we studied the effect of neutrophil depletion by cyclophosphamide treatment on ozone-induced airway hyperresponsiveness in seven dogs. Airway responsiveness to inhaled methacholine was determined by modified Astograph (7 Hz oscillation method), and the numbers of neutrophils in the peripheral blood, total cell counts and differential cell counts in BALF were measured before and after ozone exposure, and after ozone exposure of dogs undergoing cyclophosphamide treatment. Ozone exposure was carried out for 2 hrs at an ozone level of 3.07 +/- 0.01 ppm (mean +/- SE), and cyclophosphamide (10 mg/kg) was given intravenously daily for five days starting six days before ozone exposure. There was a significant increase in airway responsiveness to inhaled methacholine after ozone exposure in the seven dogs (p less than 0.001), and the numbers of neutrophils in the peripheral blood and the neutrophil counts in BALF increased significantly after ozone exposure (p less than 0.01). After ozone exposure of dogs undergoing cyclophosphamide treatment, the numbers of neutrophils in the peripheral blood decreased significantly (p less than 0.001) and the neutrophil counts in BALF did not increase, but the airway responsiveness to inhaled methacholine increased significantly (p less than 0.01). There was no significant correlation between the degree of the increase in airway responsiveness and the degree of the increase in neutrophil counts in BALF after ozone exposure. These results suggest that neutrophil infiltration into the airway is not necessary for an increase in airway responsiveness after ozone exposure in dogs.
Two cases of spontaneous pneumomediastinum and subcutaneous emphysema were reported. The first cases was a 21-year-old man who had a past history of recurrent spontaneous pneumothorax. The second was a healthy 20-year-old man. They were admitted to our hospital because of dyspnea and chest pain, which occurred at or after vocal exercise for Xiao-lin Temple boxing. Physically, a mediastinal crunch on auscultation heard over the cardiac apex and the left sternal border (Hamman's sign) was recognized in the first case, and subcutaneous emphysema was palpated in both cases. The chest X-ray films revealed intramediastinal air and subcutaneous emphysema in both cases. We diagnosed these patients as spontaneous pneumomediastinum because of no underlying disease. This is a report of rare cases of spontaneous pneumomediastinum occurring at the same time, and one of the mechanism causing them was considered to be a sudden increase in intrathoracic pressure due to the vocal exercise of chinese boxing.
In order to determine the clinical features of patients whose tubercle bacilli were found after admission, the characteristics of 23 cases (17 males, 6 females) diagnosed after admission (Group I) were compared with 16 other cases (12 males, 4 females) diagnosed before admission (Group II). The patients in Group I were more elderly and had severer underlying diseases than those in Group II. In addition, the majority of the patients of Group I showed manifestations of respiratory infections similar to bacterial pneumonia. Therefore, 14 cases (61%) in Group I were diagnosed as bacterial pneumonia on admission. Radiological findings showed that less than half of Group I showed typical X-ray findings of tuberculosis such as location of lesions in the apex and upper lung field, cavity formation, and mixture of or new and old infiltrative lesions. Also on the PPD skin test, 7 cases (30%) of Group I were negative. Due to the above results, in many cases it took more than 10 days from admission to make a final diagnosis as tuberculosis. There were several differences in findings between two groups, but there was no definite signs to diagnose all patients at the OPD. It is therefore considered to be important to keep in mind the following to prevent admission of open tuberculosis patients into a general hospital: (1) Admission of patients suspected to have open tuberculosis at the OPD should be postponed until their smears are proven negative. (2) Sputum examinations for AFB always should be carried out in patients with respiratory symptoms.

The content of islet amyloid polypeptide (IAPP) in isolated rat pancreatic islets was determined by a radioimmunoassay. Reverse-phase high-performance liquid chromatography analysis revealed that a main peak of IAPP immunoreactivity in the extracts from the islets corresponded to a synthetic rat IAPP. Secretion of IAPP from the cells is regulated by the extracellular glucose concentration. Thus, IAPP may be a novel regulator for glucose homeostasis and changes in the secretion perhaps relate to insular amyloid deposits and impaired glucose tolerance in type 2 diabetes mellitus.

In the present study, we examined the effect of prostaglandin E1 (PGE1) on Ca2+ mobilization in a human megakaryocyte (the progenitor of platelets) leukemia cell line, designated as CMK. PGE1 caused a rapid and dose-dependent increase in the intracellular free calcium level ([Ca2+]i) associated with the elevation of cyclic AMP. The PGE1-induced elevation of [Ca2+]i was decreased by the prior addition of ethylene glycol bis(2-aminoethylether)tetraacetic acid to the medium by approximately 25% of the control. This result indicates that the PGE1-induced elevation of [Ca2+]i is due to influx of Ca2+ from the external medium and to mobilization of Ca2+ from intracellular stores. Pretreatment of CMK cells with 12-O-tetradecanoylphorbol-13-acetate (TPA), a stimulus for protein kinase C, further enhanced the PGE1-induced increase in the cellular cyclic AMP level. Inversely, pretreatment of CMK cells with TPA (10 nM), prior to the addition of PGE1, inhibited the PGE1-induced elevation of [Ca2+]i. Dibutyryl cyclic AMP and forskolin did not elevate [Ca2+]i or affect the PGE1-induced Ca2+ mobilization. The inhibitory action of TPA in the PGE1-induced elevation of [Ca2+]i was mimicked by other protein kinase C-activating agents, such as 1-oleoyl-2-acetylglycerol and N-(6-phenylhexyl)-5-chloro-1-naphthalenesulfonamide, and was selectively restored by protein kinase C inhibitors, such as 1-(5-isoquinolinesulfonyl)-2-methylpiperazine dihydrochloride and N-[2-(methylamino)ethyl]-5-isoquinolinesulfonamide dihydrochloride. Thus, the inhibitory modulation of TPA on the PGE1-induced elevation of [Ca2+]i is mediated through protein kinase C activation. PGE1 had no inductional effect of megakaryocytic phenotypic changes in CMK cells. The biological role of PGE1, which increased [Ca2+]i and cyclic AMP levels in the CMK cells, remains to be determined.
A human glial brain cell line derived from a Tay-Sachs disease fetal cerebellum was transformed with SV-40 virus in order to obtain a transformed brain cell line which reflected the characteristics of the disease. It was shown that the transformed TSD cell line maintained an elevated level of GM2 which was similar to that shown by the nontransformed precursor. In addition, the TSD transformed line lacked hexosaminidase A.
The bcl-2 gene has been identified as a gene directly involved in the consistent chromosome translocation t(14;18), which is found in approximately 90% of human follicular lymphoma cases, and is a prime candidate for the oncogene playing a crucial role in follicular lymphomagenesis. In this paper, we describe a case of chronic lymphocytic leukemia showing the juxtaposition of the bcl-2 gene on chromosome 18 to immunoglobulin lambda light chain (Ig lambda) gene on chromosome 22 in a head-to-head configuration. Sequencing analysis of the joining site of the bcl-2 gene and Ig lambda gene has shown that the breakpoint is within the 5' flanking region of the bcl-2 gene and about 2.2 kilobases 5' to the joining segment of Ig lambda locus in a germ-line configuration. The extranucleotide, commonly appearing at the joining site of the t(14;18) translocation involving the IgH locus, is absent from the joining site of bcl-2 and Ig lambda. The lack of extranucleotide suggests that the juxtaposition of the bcl-2 and Ig lambda genes occurred during physiological rearrangement of the Ig lambda gene since it has been shown that the rearrangement of the Ig lambda locus is not accompanied by extranucleotides.

A new megakaryoblastic cell line (CMK), which also exhibits erythroid and myeloid markers, was established from a Down's syndrome patient suffering from acute megakaryoblastic leukaemia. The CMK cells were found to be positive in reactions with anti-platelet antibodies (anti-glycoproteins IIb/IIIa and Ib, and Plt-1). Platelet peroxidase (PPO) reactivity was found to be associated with the nuclear envelope and the endoplasmic reticulum but not with the Golgi apparatus. Some cells possessed cytoplasmic granules with the characteristics of alpha-granules and demarcation membranes. Karyotyping revealed near-tetraploidy (modal chromosome number of 95; ranging 87-98) and a translocation der(17)t(11;17), also found in the original leukaemic cells, confirming that the cells were derived from the patient's malignant blasts. The CMK cells were also found to be positive in reaction with anti-glycophorin A antibody, as well as with anti-myeloid antibodies (MY4, MY7 and MY9). Treatment of CMK cells with phorbol ester 12-O-tetradecanoylphorbol-13-acetate (TPA) greatly enhanced the reactivity with anti-platelet antibodies, increased the number of cells in which cytoplasm was dissociated into numerous segments and suppressed the reactivity with anti-glycophorin A. The proliferation of CMK cells was stimulated by interleukin-3 (IL-3) and granulocyte-macrophage colony stimulation factor (GM-CSF). This cell line should be a useful tool for analysing the basis of the afferent association between megakaryoblastic leukaemia and Down's syndrome, as well as for further study of megakaryocytic differentiation.
The effects of TSH on peroxidase activity (PO) and thyroglobulin (TG) production were investigated using primary cultures of thyroid cells obtained from patients with Basedow's disease (Basedow's cells). PO activity of cultured cells and TG concentration in the culture medium were measured by biochemical and sandwich enzyme immunoassays, respectively. The addition of TSH (10 mU/ml/day) to the medium did not increase the cell number but did increase the PO activity and TG concentration. It took more than 3 days for the PO activity of cells cultured with TSH (stimulated group) to reach a level twice that of cells cultured without TSH (control group), whereas 2 days of incubation with TSH was sufficient for increasing the TG concentration. When actinomycin D (AD) was added to the medium on the first day of 3-day incubation with TSH, the stimulatory effect of TSH on PO was completely blocked and the TG concentration was reduced to half that of the control group. AD given to the stimulated group on the last day of induction produced no inhibitory effect on the induction of PO activity by TSH, but reduced the TG concentration to almost half that in the stimulated group. An electron microscopic study of Basedow's cells cultured with AD and TSH failed to reveal any cytopathic change. The findings of the present study suggested that in cultured Basedow's cells TSH induces PO activity and TG production through the synthesis of new messenger RNA.
Septicemia encountered at Kawasaki Municipal Hospital between 1985 and 1986 were studied clinically. Forty six patients had monomicrobial and 5 has polymicrobial infections, respectively. Out of these 46 patients with septicemia, 17 were due to Escherichia coli, 7 were due to Klebsiella pneumoniae and 4 were due to Staphylococcus aureus. Ten patients had hepatobiliary, 7 had hematological, 7 had malignant diseases as underlying diseases, respectively. Out of 10 patients complicated with septic shock, 7 died. Twenty three patients were community acquired infections. The age of most of the patients were over 50. The mortality rate of more than 65-year-old patients were higher than that of other patients. Our of 5 patients with septicemia due to polymicrobial infection, only 1 patient with erythroleukemia died. Fifty patients were treated mainly with beta-lactam antibiotics such as piperacillin or cefmetazole alone or in combination with aminoglycosides and so on. Three patients with infective endocarditis were encountered during this period. Two were due to alpha-streptococcus and 1 was due to Enterococcus. A 41-year-old patient with mitral valve insufficiency and metastatic gastric carcinoma to the bone marrow were complicated with disseminated intravascular coagulation. This patient, however, was successfully treated with a daily dose of 24 mega units of benzylpenicillin, and was given gabexate mesilate, concomitantly.
1. Experiments were conducted to examine the possibility that the taste-sensitive neurons with similar taste-selectivity are preferentially innervated by common driving neurons whose taste-selectivity is also similar. 2. Multiple microelectrodes, in most cases a pair of glued electrodes, were inserted into the unilateral solitary tract nucleus (NTS) of the rat, and simultaneous recordings were made in neuron pairs responding to the four basic taste stimuli. The spike response density (RD) of each neuron during tastant stimulation was determined. Correlation coefficients of spike occurrence were calculated for each neuron pair during application of tastants and distilled water and also during spontaneous background activity. The frequency of correlated discharge (FC) of a neuron pair was measured as the area of the peak appearing on the cross-correlogram (CC). The FC value was divided by the RD value to calculate the weight of the correlated discharges in the output of each neuron (WC value). 3. Eleven pairs showed peaks in the CC constructed during tastant stimulation, whereas in other 11 pairs no peaks were found. The cross-correlation-positive group with peaks was composed of 18 NaCl-best (responding most vigorously to NaCl) and 4 HCl-best neurons, whereas the negative group without peaks included 9 NaCl-best, 9 HCl-best, and 4 sucrose-best neurons. 4. In the cross-correlation-positive pairs the taste quality most effective for one of the component neurons was often (13 NaCl-best and 2 HCl-best, 15/22 = 0.681) identical to the taste quality giving the highest probability of correlated discharge, i.e., highest FC value, in the neuron pair. 5. There were five cross-correlation-positive pairs (5/11 = 0.455) in which both of the component neurons were NaCl-best and the FC value was highest during NaCl stimulation. 6. The CCs constructed during water application exhibited peaks for all the pairs which gave positive cross-correlation in response to stimulation with tastants, whereas all pairs with negative cross-correlation during tastant stimulation never gave a detectable peak during water application. 7. In three pairs of the cross-correlation-positive group, the CCs constructed during spontaneous background activity without application of any liquid showed a small peak. 8. During NaCl stimulation some neurons exhibited relatively high FC values, but the WC values were always low. In contrast, during sucrose stimulation, the FC value was always low, but the WC value was quite high in some neurons.(ABSTRACT TRUNCATED AT 400 WORDS)
A 72-year-old man with refractory anemia (RA) developed overt megakaryoblastic leukemia after the course of RA with excess of blasts. The blasts were positive for platelet peroxidase activity and had platelet glycoproteins (GPs) such as GPIIb/IIIa and GPIIIa. The bone marrow biopsy at terminal stage disclosed marked fibrosis. The nature of the megakaryoblasts was investigated. The blasts did not differentiate morphologically into mature megakaryocytes with TPA addition. In vitro colony assay showed the failure of colony-forming unit, megakaryocyte growth in peripheral blood. The pathogenesis of myelofibrosis in our patient is discussed.
