Effect of thyroid-stimulating hormone on cultured thyrocytes obtained from patients with Graves' disease and inhibitive effect by sodium iodide: a functional study.

Thyrocytes obtained from patients with Graves' disease were cultured for 3 days. This was followed by culture with 10 mU/mL thyroid stimulating hormone (TSH) (TSH group), TSH and sodium iodide (Nal group), or without (control group) for 3 additional days. On the 8th culture day, the amounts of intra- and extra-cellular cyclic adenosine monophosphate (cAMP), extracellular cAMP and thyroglobulin (TG), peroxidase (PO) activity, and cell numbers were measured. The amounts of intra- and extra-cellular cAMP correlated well. TSH increased the values of cAMP, TG and PO to levels higher than those of the control group. As the amount of Nal added to the medium increased, these values decreased. Addition of 10(-5) mol/L Nal lowered the value of cAMP only. When 10(-4) mol/L Nal was added, these three levels were lower than those of the TSH group and the value of cAMP was almost equal to that of the control group. On cell number, no difference was found between the cells cultured with TSH, TSH and Nal, and without TSH or Nal. When the thyrocytes were cultured with 1 mmol/L dibutyryl cAMP sodium salt or 8-bromoadenosine 3',5'-cyclic monophosphate instead of TSH, 10(-4) mol/L Nal did not lower the values of thyroglobulin and peroxidase activity. These results suggest that the Nal blocks the intracellular signal transduction provoked by TSH, only at the cAMP production level.

Properties of the binding sites of [3H]9-methyl-7-bromoeudistomin D in bovine aortic smooth muscle microsomes.

[3H]9-Methyl-7-bromoeudistomin D ([3H]MBED), a powerful caffeine-like Ca2+ releaser, binds to the caffeine binding site of terminal cisternae of skeletal muscle sarcoplasmic reticulum and activates Ca(2+)-induced Ca2+ release. Properties of the binding site of [3H]MBED were investigated in aortic smooth muscle. The specific activity was higher in microsomes than in other fractions. [3H]MBED binding sites in smooth muscle microsomes were of a single class with a high affinity (KD 50nM), comparable with that in skeletal muscle sarcoplasmic reticulum. Caffeine competitively inhibited [3H]MBED binding, indicating MBED shares the same binding site with caffeine. Solubilization and fractionation of the microsomes gave two fractions of [3H]MBED binding activities. These results suggest that, in smooth muscle, there are multiple binding sites of [3H]MBED and caffeine, which might correspond to different pharmacological actions of caffeine on smooth muscle. Therefore, [3H]MBED, which binds to the different binding sites of caffeine, is useful as a probe for investigation of the actions of caffeine at the molecular level.

Tissue- and subcellular-distribution of the binding site of [3H]9- methyl-7-bromoeudistomin D, a potent caffeine-like Ca2+ releaser, in rabbits.

Tissue and subcellular distribution of the binding site of 3H-labelled 9-methyl-7-bromoeudistomin D ([3H]MBED), a powerful caffeine-like Ca2+ releaser, were investigated in rabbits. The order of specific activities of total homogenates was liver > brain > other tissues. All binding was completely suppressed by 10 mM caffeine, indicating that all [3H]MBED binding sites are modulated by caffeine. [3H]MBED binding sites distributed mainly in membrane fractions rather than soluble fractions in most tissues. In lung and liver, [3H]MBED binding was enriched in microsomes. [3H]MBED may be useful as a probe to investigate the actions of caffeine at the molecular level not only in muscles but also in a variety of tissues including liver, kidney and lung.

[Comparison of the prevalence of symptoms, elevated serum antibody, antibody types and complications between two outbreaks of influenza in a ward].

We experienced two outbreaks of influenza in the respiratory ward of our hospital in Feb. 1990 and Feb. 1993. Influenza-like symptoms were recognized in 42 of 67 cases (63%) in 1990, and in 22 of 56 cases (39%) in 1993. In the former outbreak, the prevalence of the serum CF titer for anti-influenza antibody was elevated in 25 of 42 cases (60%) (only the A2 antibody titer in 2 cases, B antibody titer in 4 cases in the latter one). Among asymptomatic inpatients, the serum titer was elevated in 3 of 25 cases (12%) in the former outbreak, but in 1 of 8 cases which the serum CF titer for anti-influenza antibody was measured in the latter one. A respiratory complications secondary to influenza were observed in 6 (2 cases pneumonia, 3 cases lower respiratory tract infection, 1 case asthma attack) of 67 cases during the former period, but only 1 of 56 cases during the latter period. In the two outbreaks of influenza in the respiratory ward we found the same results concerning the occurrence of influenza-like symptoms, an elevation of the serum antibody titer, but a significant difference was noted with regard to the phenotype of the prevalent influenza and the occurrence of complications.

Antiasthmatic activity of a macrolide antibiotic, roxithromycin: analysis of possible mechanisms in vitro and in vivo.

This study was designed to examine the possible mechanisms by which macrolide antibiotics favorably influence the clinical course of asthmatic patients. In the first set of experiments, we investigated the effect of roxithromycin (RXM), a newly synthesized macrolide antibiotic, on in vitro cytokine secretion by mitogen-activated human peripheral blood leukocytes. RXM suppressed the secretion of T cell cytokine interleukins (IL) 2-4 and monocyte cytokine tumor necrosis factor alpha. This inhibitory effects on cytokine secretion was dose dependent and firstly noted at a concentration of as little as 0.5 microgram/ml which is much lower than therapeutic blood levels. In the second part of experiments, we examined the influence of RXM on cytokine appearance in mouse lung extract induced by lipopolysaccharide (LPS) inhalation and on bronchial responsiveness to methacholine in LPS-treated mice. As compared with mice pretreated with phosphate-buffered saline, RXM administered orally at a single dose of 5 mg/kg once a day for 21 days inhibited the appearance of IL-3, IL-4, IL-5, and tumor necrosis factor alpha in aqueous lung extracts. Pretreatment with RXM also decreased the bronchial responsiveness to methacholine induced by intratracheal injection of LPS. We conclude that the attenuating effect of macrolide antibiotics on asthmatic syndromes might be explained partially by their inhibitory effects on cytokine secretion from leukocytes.

The specific binding site of 9-[3H]methyl-7-bromoeudistomin D, a caffeine-like Ca2+ releaser, in liver microsomes in distinct from that in skeletal sarcoplasmic reticulum.

3H-labeled 9-methyl-7-bromoeudistomin D ([3H]MBED), a powerful caffeine-like Ca2+ releaser, binds to the caffeine binding site of terminal cisternae (TC) of skeletal muscle sarcoplasmic reticulum (SR) (Fang, Y-I., Adachi, M., Kobayashi, J., and Ohizumi, Y. (1993). J. Biol. Chem. 268, 18622-18625.) and activates Ca(2+)-induced Ca2+ release (CICR). [3H]MBED, however, bound to rabbit hepatic microsomes with a comparable affinity (Kd = 50 nM) and with a more than 30-fold greater receptor density (Bmax = 350 pmol/mg of protein), compared with those in SR. Caffeine (0.1-10 mM) caused a concentration dependent inhibition of [3H]MBED binding to hepatic microsomes with the IC50 value of 0.3 mM. The mode of inhibition by caffeine was allosteric, indicating that the binding site of the ligand is distinct from but related to that of caffeine. Procaine (1-10 mM), a representative inhibitor of CICR, which suppresses [3H]MBED binding to TC-SR, inhibited ligand binding to hepatic microsomes only slightly. Moreover, ligand binding to the hepatic binding site was not affected by adenosine 5'-(beta, gamma-methylene) triphosphate (AMP-PCP) (10-100 microM), which is an activator of CICR and potentiates [3H]MBED binding to TC-SR. Inhibitors of [3H]MBED binding to liver microsomes other than caffeine were nucleotides such as ADP, ATP, GTP, UTP (1 mM), while CTP, cAMP, AMP, adenosine (1 mM), ryanodine (0.1-100 mM) and inositol 1,4,5-trisphosphate (1 microM) were not effective. These features of the hepatic microsomal [3H]MBED binding site distinguish it from that of skeletal muscle SR. [3H]MBED, which binds to the different sites which are both sensitive to caffeine, is useful as a probe to investigate the actions of caffeine at the molecular level.

Lymphocyte membrane modifications induced by HIV infection.

Novel carbohydrate antigen expressions were observed on T lymphocytes from HIV infected patients using flowcytometric analysis with four mAbs; BM-1, ACFH-18, FH-2 and C-6. These carbohydrate antigens were also expressed on oncogenic transformed cells but were either not expressed or were weakly expressed in lymphocyte populations from healthy subjects. A dramatic change in glycosylation was induced on CD8+T cells from HIV infected patients. The glycosylation change correlated with the progression of the disease. The incidence of Ley antigen expression on CD8+T cells increased as the disease progressed with the ongoing impairment of immune function. The phenotype change that occurred with Ley antigen expression might reflect the abnormal activation of T lymphocytes of some specific, but unknown, population of CD8+T cells. Thus, carbohydrate changes on the cell surface may induce immunological abnormality and accelerate the damage within the CD4+T cell subset, resulting in an impairment of the antigen specific immune system.

Effect of dietary magnesium supplementation on intralymphocytic free calcium and magnesium in stroke-prone spontaneously hypertensive rats.

The effects of dietary magnesium (Mg) supplementation on intralymphocytic free Ca2+ ([Ca2+]i) and Mg2+ ([Mg2+]i) were examined in the stroke-prone spontaneously hypertensive rats (SHRSP) at the age of 10 weeks. After 40 day Mg supplementation (0.8% Mg in the diet), systolic blood pressure (SBP) was significantly lower in Mg supplemented group (Mg group) than the control group (0.2% Mg). [Ca2+]i was significantly lower and [Mg2+]i was significantly higher in Mg group than in the control group. Further, [Ca2+]i was positively and [Mg2+]i was negatively correlated with SBP. These results suggest that dietary Mg supplementation modifies [Ca2+]i and [Mg2+]i, and modulates the development of hypertension.

A new approach for the detection of type III hyperlipoproteinemia by RLP-cholesterol assay.

Type III is a remnant hyperlipoproteinemia identified by the presence of beta-VLDL (remnant lipoprotein) as well as a genetic variant of apo E (apo E2/2). The RLP isolated from the serum of Type III patients by a new method we have developed, the RLPcholesterol assay, was identified as chylomicron and VLDL remnant. In addition, the RLP-C levels of the Type III patients were significantly higher than other hyperlipidemic patients with similar serum TG levels, while the ratio of TC/TG in RLP-C of both groups was not significantly different. The RLP-cholesterol assay appears to be useful for the screening and monitoring of Type III hyperlipoproteinemia when used in conjunction with the assays of serum TG level and genetic apo E isoform analysis.

[Urological problems in Prader-Willi syndrome].

Prader-Willi syndrome is rather a rare disease. However, as it includes 4 features (hypogonadism, hypomentia, hypotonia, and obesity), urologist may see the patients with this syndrome for their gonadal problem. We studied all the 27 cases in our hospital of which data were precisely collected. Among males, 67% of patients had presented themselves first to the department of pediatric internal medicine. One third of the patients were not diagnosed as the syndrome and referred to our clinic because of urological abnormalities. Chromosomal abnormality was seen in 40.9%. We found cryptorchism in all the cases and micropenis in 56%. In females, delayed menarche over 15-year-old was seen in 75%. From endocrinological studies, 75% male showed low reaction against HCG stimulation. Low gonadotropin responses to LH-RH were seen in 80% of all patients. We performed orohiopexy for cryptorchism, but testicular development was poor. And we do not actively utilize hormonal therapies for these patients.

The usefulness of the Noise-Field Test as a screening method for visual field defects.

In 1990, we reported the usefulness of the noise-field spontaneously generated on a home television screen for the subjective perception of visual field defects (VFD) in glaucoma patients (Noise-Field Test: NFT). Recently, the sensitivity and specificity of the NFT for testing 555 eyes of primary open angle glaucoma patients and 300 eyes of subjects with normal visual field were 89.2% and 93.7%, respectively. We also performed a comparative study among the NFT and Tübingen electric campimetry (TEC) and oculokinetic perimetry (OKP), and also evaluated the efficiency of the NFT using participants examined in general health check-up systems. In the comparative study testing 136 eyes of glaucoma patients who had never perceived their VFD, and 114 eyes of subjects with normal visual field, the sensitivity of the NFT, TEC (fine), TEC (coarse), and OKP was 85.3%, 75.0%, 71.3% or 59.5%, respectively. The specificity was 88.6%, 78.9%, 82.5% or 95.6%, respectively. In the study made in the general health check-up system, the NFT was carried out on 935 participants (1870 eyes). After the NFT, all participants received an ophthalmological examination. Those participants whose fundus was suspected to have VFD as well as participants who reported abnormalities of the noise-field were examined by static perimetry. One hundred and sixty-two eyes perceived abnormalities of the noise-field; 58 eyes were judged as having VFD by an ophthalmologist and the defects were confirmed.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of endobronchial tuberculosis with high serum CA19-9 and SLX level].

A 43-year-old female was admitted to our hospital with mild cough, sputum and right chest pain. The chest X-ray revealed an inhomogeneous shadow in the right upper lung field and mediastinal lymphadenopathy. At first we considered the patient had bronchogenic carcinoma, as her serum CA19-9 and SLX levels were high and the right upper bronchus was obstructed by necrotic tissues. However bronchoscopic specimen showed necrotizing epithelioid cell granulomas and Mycobacterium tuberculosis was detected, and her disease was diagnosed as endobronchial tuberculosis. The case responded well to anti-tuberculosis chemotherapy. The differential diagnosis between bronchogenic carcinoma and endobronchial tuberculosis was very difficult in this case because of high serum level tumor marker and endoscopic findings, which turned to normal after treatment. We discussed the cause of high serum level of CA19-9 and SLX in nonmalignant lung disease.

[Thulium-holmium-chromium-doped YAG laser sclerostomy ab externo in uncontrollable glaucoma].

Laser sclerostomy ab externo, using thulium-holmium-chromium-doped YAG laser was performed in 40 eyes with refractory glaucoma. Preoperative intraocular pressure (IOP) ranged from 21 to 50 mmHg (mean +/- SD: 33 +/- 18.6) with maximum tolerable medications. In all cases, a full-thickness sclerostomy was performed and subsequently a filtering bleb was formed. Total energy levels to produce the sclerostomy ranged from 0.2 J to 12.9 J (3.9 +/- 2.4). Follow-up periods varied from 3 to 18 (9 +/- 4) months. IOP control rate calculated by a life-table method at 6 and 12 months after surgery was 54% and 46%, respectively. IOP for successful eyes ranged from 0 to 18 mmHg (12.3 +/- 2.4). As complications, corneal fold formation (all eyes), choroidal detachment (12 eyes), hyphema (10 eyes), and iris incarceration in the sclerostomy site (4 eyes) were found. Astigmatism caused by corneal fold formation disappeared spontaneously within 3 months. Iris incarceration causing IOP elevation was repaired by Nd-YAG laser treatment and IOP control was obtained in 2 of 4 eyes. No other complications were observed.

[Infiltrative peripheral neuropathy of acute monoblastic leukemia during hematologic remission].

A 38-year-old woman with acute monoblastic leukemia developed severe continuous pain in the left arm while she was in hematologic remission following both systemic and intrathecal chemotherapy. A nerve conduction study (NCS) showed marked decrease of amplitude in the left ulnar nerve, consistent with infiltration of leukemic cells. The pain in the arm was reduced by irradiation to the left brachial plexus, but right facial nerve palsy occurred. No improvement was achieved by systemic and intrathecal chemotherapy plus irradiation to the whole brain and right parotid. After sometime, she complained of pains in the legs and right foot drop. NCS showed amplitude decrease in bilateral peroneal nerve. Throughout the course, bone marrow remained in complete remission, and no signs of meningeal leukemia were obtained. A treatment with high dose Ara-C appeared to be effective for the pain in the legs. The foot drop, however, persisted and peripheral neuropathy progressed even after high dose Ara-C therapy. Peripheral nerve involvement in acute leukemia appears to be rare, and even more so in case of hematologic remission. The blood-nerve barrier may allow some malignant cells to escape from cytotoxic agents. Therefore, irradiation or high dose Ara-C therapy would seem to be rational approaches to the problem.

A case of peculiar plantar warts. Human papillomavirus type 60 infection.

BACKGROUND: Various kinds of viral warts have their own typical clinical appearances and histologic properties. From each of them, a corresponding unique type of human papillomavirus (HPV) has been detected. More than 60 types of HPV have been identified from the warts and other squamous cell epithelial tumors. Human papillomavirus type 60 has been identified in an epidermal cyst on the sole. We present a case of a peculiar plantar wart infected with HPV type 60. OBSERVATIONS: A 21-year-old male student complained of yellowish hyperkeratotic slightly elevated nodules on the right plantar arch. The surface of the nodules was not papillary, but a normal ridge pattern of the sole could be traced. Histologic findings included moderate acanthosis, papillomatosis, and eosinophilic intracytoplasmic large granules with or without surrounded vacuoles. These were noted in the middle and upper epidermis, including the hypertrophic stratum corneum. Virologic findings included HPV antigen detected by anti-HPV polyclonal antibody. The DNA of HPV type 60 was detected by in situ hybridization. CONCLUSION: A new type of plantar wart infected with HPV type 60 is reported. It shows unique clinical and histologic findings that can be distinguished from those of ordinary verruca or myrmecia on the sole.

[Effects of platelet activating factor on release of thromboxane B2 from human granulocytes and platelets].

We investigated the effects of platelet activating factor (PAF) on the release of thromboxane (Tx) B2 from human granulocytes and platelets. Stimulation of human peripheral whole blood cells by PAF (10(-5) M) resulted in the production of TxB2 (p < 0.05). When human granulocyte suspensions (2.0 x 10(6)/ml) were stimulated by calcium ionophore A23187 (10(-5) M), TxB2 levels in the supernatant fluid increased significantly (p < 0.01). However PAF (10(-5) M) did not increase the levels of TxB2 in granulocyte suspensions. In the presence of cytochalasin B (5 micrograms/ml), PAF (10(-8) M, 10(-7) M, 10(-6) M) did not induce any significant release of TxB2 from granulocytes. The levels of TxB2 in human platelet suspensions were significantly increased by PAF (10(-5) M) stimulation (p < 0.05). These results suggest that PAF induces TxB2 release from human platelets, but it may not stimulate the production of TxB2 in human granulocytes.
