Surgical management of cardiac myxomas.

Cardiac myxomas are rare cardiac lesions, though they are the commonest tumours of the heart. Seventeen cases of cardiac myxomas have been operated during the last one decade. Exertional dyspnoea, palpitation and chest pain were the main presenting symptoms. Echocardiographic assessment was the only definitive diagnostic investigation required prior to surgery. Early surgical excision was planned in all the cases. Irrespective of the exposure techniques, removal of the tumour with wide excision of its base was practised. There was one early death due to low cardiac output in a patient brought in a shock like state. Follow up study has revealed 14 patients in NYHA class I and two patients are having class II symptoms. Periodic echocardiographic follow up study has not revealed any recurrence till date. It is concluded that an early diagnosis and surgery gives excellent long term results in these cases.

Tibial and fibular sesamoid fractures on the same metatarsal: a review of two cases.

Fractures of the great toe sesamoids occur much less frequently than fractures of other bones, and, as such, the literature is sparse in dealing with this topic. Two cases of a rare occurrence of tibial and fibular sesamoid fractures on the same metatarsal are reported. Each case had a traumatic origin, and successful healing occurred in one case following 6 weeks of short leg casting. Total excision of both sesamoids was required in case 2 after complete healing was not achieved by short leg casting alone.

[Immobilization of glucose oxidase on silica-based supports].

Glucose oxidase (beta-D-glucose: oxygen 1-oxidoreductase, EC 1.1.3.4) was covalently coupled to silica-based supports containing aldehyde functional groups. The activity of the immobilized enzyme was about 1000 U/g support. The optimum pH of the catalytic activity was 5.5 for the soluble enzyme and 6.0 for the immobilized enzyme. With glucose as a substrate the Km value of the immobilized enzyme was higher than in case of the soluble enzyme. The immobilized enzyme was found to be more thermostable than the soluble one. The immobilization did not affect the stability of glucose oxidase against the denaturing effect of urea.

Duodenal stenosis from intramural cystic annular pancreas.

A case of annular pancreas presenting with duodenal obstruction due to overfilling of a large intramural duodenal cyst is reported. At 30 years of age, this man was an alcoholic with repeated attacks of pancreatitis which produced the enlargement of an annular pancreas and the development of a cyst probably through blocking of an aberrant duct. The surgical operation consisted of simply removing part of the external cyst wall and outer anterior layer of the duodenum. A postoperative acalculous cholecystitis necessitated subsequent cholecystectomy. Four years later, the patient had to be operated on for duodenal stenosis due to recurrent duodenal ulcers.
