Some properties of luminal sucrase and sucrase-isomaltase complex in rat small intestine.

Some properties of luminal sucrase-isomaltase complex and the effect of luminal fluid on their complex were studied in rat small intestine. Luminal contents were collected by flushing the small intestine with the buffered solution. The enzyme activity was observed in luminal contents and intestinal mucosa. Sucrase and isomaltase activities were located mainly in the intestinal mucosa. However, approximately 20% of sucrase and 10% of isomaltase activities of total small intestine were found in the luminal contents. A significant amount of sucrase without isomaltase activity, the molecular weight of which was estimated at about 140,000 daltons, was found in the luminal supernatant of the distal intestine in addition to the complexed form of sucrase and isomaltase. The luminal sucrase and sucrase-isomaltase complex had similar properties such as Km values, optimal pH, molecular weights and antigenicity against anti sucrase-isomaltase antibody compared with brush border membrane-bound sucrase-isomaltase complex. Furthermore, the supernatant of the luminal contents of the ileum had a degradative effect on the isomaltase moiety of the purified sucrase-isomaltase complex and a free sucrase without isomaltase also appeared in vitro as observed in vivo. These results suggest that the sucrase-isomaltase complex is released into the intestinal lumen from the brush border membrane and that a luminal factor affects the degradation step of this enzyme as well as the biosynthesis of sucrase-isomaltase complex.

Serial changes in sonographic appearance after transcatheter hepatic arterial embolization.

Serial changes in sonographic appearance after transcatheter hepatic arterial embolization (TAE) were studied in 22 patients who had hepatoma and one patient who had a hepatoblastoma. These changes were classified into three types. In cases of type 1, the internal echo of the entire tumor became remarkably echogenic with or without an acoustic shadow. In cases of type 2, echogenic areas or scattered echogenic spots with or without acoustic shadows were observed. In Type 3 cases, no changes in sonographic appearance were observed. Tumor diameters in all of the type 1 cases were less than 4.9 cm. Various size tumors were observed in the cases of type 2 and type 3. Transcatheter arterial embolization was completely effective or effective in 89 per cent of the type 1 and type 2 cases, but was not effective in 75 per cent of the type 3 cases. The sonographic changes correlated well with the effectiveness of TAE.

[One-shot therapy and transcatheter arterial embolization (TAE) therapy of unresectable hepatocellular carcinoma].

407 cases of unresectable hepatocellular carcinoma (HCC) occurring from 1970 to March 1985, including 107 cases receiving conservative therapy, 176 cases receiving one-shot therapy and 124 cases receiving transcatheter arterial embolization (TAE) therapy, were studied and the efficacy of chemotherapy was compared with that of TAE therapy. The results were as follows; One-year survival rate was 2.8% with a median survival time of 1.3 months in conservative therapy. In the 176 cases of one-shot therapy, one-year survival rate was 21.0%, two-year 6.8% and three-year 2.3% and the median survival time was 4.8% months. In 120 cases of one-shot therapy which were compatible with criteria for one-shot injection of anticancer drugs via the hepatic artery for HCC, one-year survival rate was 30%. However the rate was 1.8% in 56 cases which were not compatible with the criteria. In 37 cases in which Mitomycin C (MMC) and Adriamycin (ADR) were administered alternately, one-year survival rate was 41.7%, two-year 16.1% and three-year 4.3%. The highest survival rate was obtained by TAE therapy. One-year survival rate was 66.9%, two-year 33.8% and three-year 28.9%. Decrease of AFP after therapy was noted in 42.4% of cases given one-shot therapy and in 95.2% of cases given TEA therapy. The results suggest that alternate administration of anticancer agents produces good chemotherapeutic effects and that the best life-prolongation is obtained by TAE therapy.

[Sequential measurement of serum CA 125 levels in Krukenberg's tumor].

CA 125 is a tumor marker for ovarian cancer developed by hybridoma technology. In the present study, the levels of CA 125 were sequentially measured in the serum of five patients with Krukenberg's tumor originating in the gastrointestinal tract. Four out of the five (80%) had elevated serum CA 125 levels, i.e., greater than 35 U/ml and, in most cases, these levels decreased after resection of the ovarian tumor. Serum CA 19-9 and CEA levels were also increased in two (40%) and three (60%) patients, respectively, and, in these positive cases, reflected the clinical course. The results indicated the clinical usefulness of these cancer markers in Krukenberg's tumor.

Localized hyperthermia and radiation in cancer therapy.

Clinical researches in hyperthermia have recently expanded rapidly with the increase in our knowledge of the biological effects of heat on experimental systems. This article provides background information on the biological rationale and current status of technologies concerning thermometry and heating equipment for the application of hyperthermia to human cancer treatment. Much data has been accumulated recently in hyperthermia treatment with and without radiation to superficial tumours which are refractory to conventional treatments. In this paper the treatment results published recently have been surveyed. The complete responses of tumours treated by heat alone are in the range of 15 per cent as opposed to approximately 60 per cent for the combination of heat plus radiation. Clinical results so far published have demonstrated that local control is consistently better in the lesions treated with radiation plus heat than with radiation alone. The morbidity related to heat therapy is within tolerable limits. Several articles on the clinical results of deep-seated tumours treated by hyperthermia are reviewed. Problems to be solved in the application of heat to cancer therapy are discussed.

[The role of radiation therapy in the treatment of lung cancer].

An analysis of the effectiveness of radiotherapy was performed in 165 patients with non-small cell carcinoma of the lung who received more than 4500 cGy as a definitive treatment between April 1980 and March 1983 and survived longer than 2 months after the start of radiotherapy. Of patients with inoperable malignancy, 38.8% for stage II survived 3 years and 27.7% for stage III, while 4.1% of patients with stage IV disease were 2 year survivors. Median survivals of patients for each stage were 24.7, 18.7 and 5.9 months, respectively. As for further analysis of stage III, better results for year-survival rates and median survivals were observed in T1-2 N2 M0 than in T3 N0-1 M0. Based on these results, the role of radiotherapy for non-small cell lung cancer was discussed.
