Duodenal flow of microbial nitrogen estimated from urinary excretion of purine derivatives in calves after early weaning.

Duodenal flow of microbial N (MN) was estimated from urinary purine derivatives to examine age-related changes in MN in male Holstein calves. In Exp. 1, endogenous purine derivatives were determined by measurement of purine derivatives in five calves fed nucleic acid-free milk replacer alone. In Exp. 2, the ratio of urinary excretion as purine derivatives to purines administered via the reticular groove was determined in three calves weaned at 5 wk of age. As a result, endogenous purine derivatives were constant at 705 mumol/(kg BW.75.d), irrespective of the amount of milk replacer, and the ratios of purine derivatives to duodenal purines were estimated to be .549, .276, .363, and .466 at wk 1, 6, 11, and 20 after weaning, respectively. Using these variables and urinary purine derivatives, the duodenal flow of MN was estimated and its relation with N balance was examined in 15 calves weaned at 5 wk of age in Exp. 3. Digestible OM was lower at wk 1 after weaning and transiently higher at wk 6. The percentage of N absorbed to N intake, N absorbed, N retained, and estimated duodenal MN were also lower at wk 1, and rapidly increased for the first 6 wk. These findings suggest that the increases in N absorbed and N retained for the first 6 wk after weaning were due to augmentation of duodenal flow of MN and dietary N that escaped ruminal degradation.

Lysine deficiency in postweaned calves fed corn and corn gluten meal diets.

Holstein bull calves (n = 36) weaned at 6 wk of age were used in six trials to examine the response of N balance to postruminal administration of lysine with or without methionine in postweaned calves receiving diets based on corn and corn gluten meal. Calves were younger than 3 mo of age in Trials 1 and 2 but older than 3 mo in Trials 4 to 6. L-Lysine monohydrochloride was supplemented with or without DL-methionine twice daily through the reticular groove, except in Trial 4, in which N supplements were infused through duodenal cannulas. L-Glutamine was used as a nonspecific N source in every trial, and casein was a positive control in Trials 4 and 5. When daily CP intake from the diet was 3.9 g/kg BW, lysine was limiting for calves less than 11 wk of age (Trials 1 and 2) but not limiting for calves greater than 12 wk of age (Trial 3). No amino acid seemed to be limiting for calves greater than 20 wk of age (Trial 4) when daily CP intake was 4.1 g/kg BW, but lysine was limiting when CP intake was restricted to 3.0 g/kg BW when calves were more than 17 wk of age (Trial 5). However, lysine was not limiting above 18 wk of age (Trial 6) when CP intake was increased to 3.8 g/kg BW by adding urea to the diet. Results suggest that lysine may be limiting for corn and corn gluten meal diets only when ruminal microbial protein synthesis is restricted.

Subsets of activated T cells in patients with systemic lupus erythematosus: the relation to cell cycle.

The correlation among the various markers of activated T cells (soluble interleukin 2 receptor (sIL-2R), HLA-DR+ and HLA-DP+ T cells, proliferating cell nuclear antigen (PCNA) positive lymphocytes) were examined in patients with systemic lupus erythematosus (SLE), and related to the cell cycle. The concentration of sIL-2R and the proportion of HLA-DR+ T cells, HLA-DP+ T cells or PCNA+ lymphocytes were increased significantly as compared to that in normal subjects. And, the concentrations of sIL-2R correlated with the proportions of PCNA+ lymphocytes, but not with the proportions of HLA-DR+ T cells, HLA-DP+ T cells. The correlation between sIL-2R and PCNA+ lymphocytes was attributed to both indicators being increased during the G1B or S phase in normal T cells upon stimulation by phytohemagglutinin (PHA). Upon cell cycle analysis it was learned that activated T cells could be found in the G1A and the S phases.

Cloning and characterization of novel gene, DCRR1, expressed from Down's syndrome critical region of human chromosome 21q22.2.

The new gene, DCRR1, from the proximal part of the Down's syndrome critical region (DCR) was identified by the GRAIL analysis of the 97-kb nucleotide sequence of two P1 DNAs and the cDNA for DCRR1 gene was cloned. A 7.36-kb cDNA encodes the imcompleted open reading frame composed of 1941 amino acid residues (220.2 kDa). The deduced amino acid sequence contains the conserved domain for protein phosphatases at the N-terminus. The domain encoding the rod-like tail of a myosin heavy chain was also found near the C-terminal region besides the signature for an actin binding protein, profilin, suggesting its possible role as a microtuble-associated protein. Two different sizes (7.9 and 9.0 kb) of mRNAs were detected in the poly(A)+ RNA from abundant tissues by the Northern analysis. The smaller transcript was only transcribed at a high level in the testis. The imbalance of the DCRR1 gene dosage may contibute to the pathogenesis of Down's syndrome.

[Effective low-dose amiodarone therapy for ventricular tachycardia complicated with ischemic heart disease and poor left ventricular function in an elderly patient].

A 71-year-old man who had ischemic heart disease with poor left ventricular function and ventricular tachycardia was admitted to hospital for evaluation. Cardiac catheterization was performed on August 19, 1996, and right coronary arteriography revealed total occlusion at segment 3. Left coronary arteriography revealed total occlusion at segment 6, and a lesion at segment 13 was 75% occluded. Partial collateral flow from the right ventricular branch to the left anterior descending artery was demonstrated, and the left ventricular ejection fraction was 24%. Recurrent ventricular tachycardia followed by pre-syncope occurred from August 23, 1996, and the patient underwent emergency coronary artery bypass surgery to the left anterior descending artery and circumflex artery using saphenous vein grafts. Ventricular tachycardia followed by pre-syncope occurred frequently after the bypass surgery, and antiarrhythmic agents (Vaughan Williams classification Ia and Ib groups) were ineffective. He received amiodarone (100 mg/day after a loading dose of 200 mg/day for 2 weeks) from September 6, 1996. His symptoms of arrhythmia decreased, and side effects have not been observed. Low-dose amiodarone was effective in this case of ischemic heart disease with left ventricular dysfunction and sustained ventricular tachycardia.

[Cushing's syndrome due to small cell lung cancer with ectopic production of adrenocorticotropic and parathyroid hormone].

A 72-year-old man was admitted to our hospital because of a tumor-like shadow on a chest X-ray film. At the initial examination, he had clinical signs of Cushing's syndrome: moon face, central obesity, and hypertension. A computed tomographic scan of chest showed an abnormal shadow in the lung (5 x 6 cm) with involvement of the right paratracheal and anterior tracheal lymph nodes, and a right-sided pleural effusion. Small cell lung cancer (extended disease; T2N2M6 stage IV) was diagnosed after a transbronchial biopsy. The concentrations of adrenocorticotropic hormone, cortisol, and parathyroid hormone in plasma were markedly elevated, and there was no circadian rhythm (336 pg/ml. more than 60.1 micrograms ml. and 805 pg/ml, respectively). Fluid obtained by thoracentasis had malignant cells, and the levels of adrenocorticotropic hormone and parathyroid hormone in the effusion (1120 pg/ml and 1810 pg/ml, respectively) were higher than those in serum, which indicates that these hormones were produced by the tumor cells. The patient received chemotherapy and responded well, but he died of respiratory failure 26 months later. The response rate to chemotherapy in elderly patients with lung cancer is said to be comparable to that in younger patients, but treatment may be difficult because of poor performance status and diminished physical capacity. Although patients with lung cancer complicated by Cushing's syndrome have a poor prognosis, this patient survived for more than 2 years after the disease was diagnosed.

[Reoperation for malfunction of Ionescu-Shiley bovine pericardial bioprosthetic valves in the mitral position--special consideration with emergency operation].

Sixty-eight patients have had mitral valve replacement with Ionescu-Shiley bovine pericardial bioprosthesis from 1981 to 1984. Thirty five patients were required reoperation due to primary tissue failure of the bioprosthesis, 10 (28.6%) were on emergency basis and 25 (71.4%) were on elective. Hospital deaths were two (20.0%) on emergency and four (16.0%) on elective. Clinical features of emergency cases were as follows: progressive congestive heart failure in eight, severe hemolysis with massive hemoglobinuria in eight and acute renal failure in four. Cusp tears were observed in all valves, 8.1 +/- 1.5 mm in emergency group and 4.2 +/- 3.6 mm in elective group. Neointimal ingrowth over Dacron cloth of the inner surface of the stent was absent in 29 valves (89.2%). Calcification was observed in seven valves, however these were not dominant causes of primary tissue failure. Clinical symptoms were correlated with structural deterioration of explanted valves. Structural deterioration of the bioprosthesis may occur suddenly and progressively. Our experience demonstrated that life-threatening prosthetic valve failure may occur with a relatively high incidence and that careful follow-up is needed for Ionescu-Shiley bovine pericardial bioprosthesis.

Regulation of interleukin (IL)-1beta gene transcription induced by IL-1beta in rheumatoid synovial fibroblast-like cells, E11, transformed with simian virus 40 large T antigen.

OBJECTIVE: To investigate the process involved in the production of and responsiveness to interleukin 1beta (IL-1beta) in synovial fibroblast-like cells, we analyzed the enhancer region of pro-IL-1beta gene in a cell clone, E11, established from a patient with rheumatoid arthritis (RA). METHODS: A cell clone, E11, was derived from rheumatoid synovial fibroblast-like cells transformed with simian virus 40 large T antigen expression vector by electroporation. Responsiveness of E11 to IL-1beta was analyzed by [3H] thymidine incorporation and Northern blotting. IL-1beta responsive elements on pro-IL-1beta gene were analyzed by chloramphenicol acetyltransferase analysis. RESULTS: E11 resembled synovial fibroblasts based on morphological characteristics and phenotypic analysis. It also demonstrated marked enhancement of proliferation and rapid induction of IL-1beta mRNA expression by IL-1beta. We also identified IL-1beta responsive elements on the pro-IL-1beta gene at a position between -3134 and -3092 that contains the AP-1 binding site and between -2782 and -2729, which includes both AP-1 and nuclear factor-kappaB (NF-kappaB) binding sites. CONCLUSION: AP-1 and NF-kappaB binding elements were required for transcriptional regulation of the IL-1beta gene in the autocrine growth system of RA synovial cells.

Rheumatoid synovial fibroblasts are stimulated by the cellular adhesion to T cells through lymphocyte function associated antigen-1/intercellular adhesion molecule-1.

OBJECTIVE: To determine if T cells stimulate synovial fibroblasts to produce inflammatory cytokines through cellular adhesion in synovitis of rheumatoid arthritis (RA). METHODS: Immunohistochemical staining, flow microfluorometry, adhesion assay, ELISA, and Northern blot analysis to determine production of interleukin-1beta (IL-1beta) from RA synovium and RA synovial fibroblast-like cell line. RESULTS: We observed the following novel features of cellular adhesion of T cells to synovial fibroblasts, which suggest a role for induction of cytokine production in synovial fibroblasts: (a) CD11a (lymphocyte function associated antigen-1 alpha) positive T cells accumulated around CD54 [intercellular adhesion molecule (ICAM-1)] positive synoviocytes in active RA synovium, shown by immunohistochemical studies: (b) synovial fibroblastic cell line E11 expressed a single adhesion molecule ICAM-1, the expression of which was not affected by IL-1beta; (c) E11 adhered to phorbol myristate acetate (PMA) activated T cells within 30 min, not resting T cells, and its adhesion was completely inhibited by anti-LFA-1 monoclonal antibody (Mab); (d) pretreatment of E11 with IL-1beta did not affect the adhesion of E11 to PMA activated T cells; (e) IL-1beta production and IL-1beta mRNA transcription from E11 were induced by the addition of T cells in a cell number dependent manner and the induced production and transcription were inhibited by anti-LFA-1 Mab. CONCLUSION: T cells infiltrating the synovium may play a pivotal role in the pathogenesis of RA, by inducing IL-1beta production of synovial fibroblasts by sequential events, namely, T cell-synoviocyte cellular adhesion through LFA-1/ICAM-1, signal transduction, and production of IL-1beta induced by the cellular adhesion.

Comparative studies of CAM 123-6 and carcinoembryonic antigen for the serological detection of pulmonary adenocarcinoma.

Carcinoembryonic antigen (CEA) has been clinically used as a serum marker for lung cancer, especially for pulmonary adenocarcinoma. Since we had developed a tumor marker against pulmonary adenocarcinoma, CAM 123-6 belonging to KL-6 MUC 1 mucin, we compared its clinical value with that of CEA, in combination with squamous cell carcinoma-related antigen (SCC) and neuron-specific enolase (NSE). The subjects were 30 patients with adenocarcinoma, 23 with squamous cell carcinoma, 16 with small cell carcinoma, and 20 with benign lung disease. For discriminating lung cancer from benign lung disease, the combination of CAM 123-6, SCC, and NSE showed 64% diagnostic accuracy, and the combination of CEA, SCC, and NSE 65%. In differentiating pulmonary adenocarcinoma from other lung cancer, the diagnostic accuracy of CAM 123-6 was 81% and that of CEA was 74%, although there was no significant difference between them. These observations indicate that the diagnostic accuracy of CAM 123-6 is similar to that of CEA. Neither marker is sufficiently specific to differentiate pulmonary adenocarcinoma from other forms of lung cancer.

The 'sail sound' and tricuspid regurgitation in Ebstein's anomaly: the value of echocardiography in evaluating their mechanisms.

We describe a patient with Ebstein's anomaly in whom Doppler echocardiography was used to clarify the mechanism responsible for 'sail sound' and tricuspid regurgitation associated with this condition. Phonocardiography revealed an additional early systolic heart sound, consisting of a first low-amplitude component (T1) and a second high-amplitude component (T2, 'sail sound'). In simultaneous recordings of the tricuspid valve motion using M mode echocardiography and phonocardiography, the closing of the tricuspid valve occurred with T1 which originated at the tip of the tricuspid leaflets, while T2 originated from the body of the tricuspid leaflets. Using color Doppler imaging, the tricuspid regurgitant signal was detected during pansystole, indicating a blue signal during the phase corresponding to T1 and a mosaic signal during the phase corresponding to T2 at end-systole. Thus, 'sail sound' in patients with Ebstein's anomaly is not simply a closing sound of the tricuspid valve, but a complex closing sound which includes a sudden stopping sound after the anterior and/or other tricuspid leaflets balloon out at systole.

[Intraoperative re-rupture of gastric varix immediately after the start of an operation in a patient with liver cirrhosis].

Gastroesophageal variceal hemorrhage is a rare complication during an operation. We present a case of gastric variceal re-rupture during an emergent operation for devascularization of the gastric veins. The patient was a 72-year old man with liver cirrhosis, who developed gastric variceal hemorrhage on the day of surgery. Sclerotherapy with an endoscope was performed, and the hemorrhage was controlled four hours before entering the operating theater. The induction of anesthesia and tracheal intubation were done with rapid sequence because the patient was regarded as full stomach. Induction was completed successfully. However, gastric varix ruptured immediately after the beginning of the surgery. The hemorrhage into the stomach amounted to 2,165 ml. The patient developed hypotension of 40 mmHg of systolic pressure for 15 minutes. With fluid resuscitation, continuous infusion of dopamine and ligation of varix, the patient recovered from this hypotensive event. No neurological deficit developed postoperatively. Portal hypertension results from increased resistance to portal venous blood flow or increased portal venous blood flow. Therefore, increased intravascular volume may play a significant role in precipitating variceal hemorrhage. In this case, abrupt circulatory change due to inadequate depth of anesthesia may partly cause massive hemorrhage. In conclusion, since potential adverse effects of increased blood volume and hepatic resistance on variceal hemorrhage must be considered during anesthesia, patients with episode of variceal hemorrhage should be treated as full stomach although endoscopic findings before the surgery indicate controlled hemorrhage from varix.

[True "PICA communicating artery" aneurysm: a case report].

We present an unusual case of an aneurysm of the distal posterior inferior cerebellar artery (PICA). A 51-year-old female presented a subarachnoid hemorrhage with mild consciousness disturbance on August 6, 1992. Computed tomography (CT) on admission showed subarachnoid hemorrhage with thick hematoma in the cisterna magna and intraventricular hematoma in the 4th, 3rd and both lateral ventricles. The angiogram on admission revealed no definite vascular anomalies. Repeated angiograms on the 11th day after onset showed an aneurysm on anastomotic branch between the bilateral distal PICAs. The aneurysm was clipped successfully through a suboccipital craniectomy 14 days after the onset. In the literature reviewed, only one such aneurysm, located at an anastomotic vessel of the bilateral PICAs, has been reported by Hlavin et al in 1991. They reported that the aneurysm was associated with a unilateral PICA that supplied both cerebellar hemispheres and arose from an anastomotic vessel in the contralateral circulation. They called the aneurysm as "a PICA communicating artery" aneurysm. We assume that this "PICA communicating artery" is a remnant of a primitive lateral vertebrobasilar anastomosis, which appears in the embryo at the 9 mm stage. It is suggested that the pathogenesis may be not only the hemodynamic factor but also a congenital anomaly.

[Combination use of second generation cephem and isepamicin for the treatment of post-surgical infection of the lower digestive tract].

Effects of combination use of a second generation cephem and an aminoglycoside antibiotic isepamicin (ISP), for the treatment of post-surgical infections of the lower digestive tract were examined from a laboratory and clinical perspective. Thirty-three out of total 35 cases were included in the analysis of the test, while the other 2 cases did not meet the criteria for analysis. ISP was administered in combination with one of the second generation cephems among cefotiam (CTM), cefmetazole (CMZ), or cefuroxime (CXM) for 26 cases of wound infection and 7 cases of peritonitis. The overall efficacy rate was 88%; 92% in wound infection and 71% in peritonitis individually. There was no significant difference in efficacy among different groups. Bacteriological study showed the combination use of any one of the second generation cephems and ISP was very effective in all 14 cases of infections with a single species of bacterium, the efficacy rate was 100%. It was also effective in 9 out of 10 cases with mixed infections of 2 species of bacteria (90%), but effectiveness was diminished in only 6 out of 9 cases with mixed infections of 3 species (67%). Sixty-nine strains were isolated from the 35 cases, more than half of which (44 strains) were Gram-negative; 14 strains of E. coli (20%), 13 strains of E. faecalis (13%) and 6 strains of P. aeruginosa (9%). The MIC50 and MIC90 were, respectively, 1.56 micrograms/ml and 100 micrograms/ml for ISP, 6.25 micrograms/ml and 100 micrograms/ml for CTM, 12.5 micrograms/ml and 100 micrograms/ml for CMZ, and 25 micrograms/ml and 100 micrograms/ml for CXM. The MICs in any combination uses decreased synergistically according to ISP concentration. Adverse events were observed in 3 out of 35 cases, but they were not severe. The results indicated that the combination use of a second generation cephem and ISP was useful and should be one of the choices for the treatment of post-operative infections of the lower digestive tract.

The micronucleus assay of lymphocytes is a useful predictive assay of the radiosensitivity of normal tissue: a study of three inbred strains of mice.

Radiation sensitivity was measured by the micronucleus assay in T lymphocytes of the spleen and in fibroblasts derived from three inbred strains of mice, BALB/c, A/J and C3H/He. A linear correlation between the surviving fraction and the micronucleus frequency was obtained. There were clear differences in the radiosensitivity of T lymphocytes and of fibroblasts from the different strains of mice. T lymphocytes and fibroblasts from BALB/c mice were more radiosensitive than those from A/J and C3H/He mice. To explore the correlation between the radiosensitivity of T lymphocytes and of fibroblasts in vitro and the reaction of normal tissue to radiation, the survival ofjejunal crypt stem cells from the three mouse strains was determined using a microcolony assay. The jejunal crypt cells of BALB/c mice were more radiosensitive than those of A/J and C3H/He mice. Thus the order of radiosensitivity of T lymphocytes and fibroblasts in vitro corresponds to the sensitivity ofjejunal crypt cells in vivo. Our results suggest that determining the radiosensitivity of T lymphocytes and fibroblasts in vitro may be useful in predicting the magnitude of radiation-induced damage in the small intestine.

[A report of two cases of Carpentier-Edwards pericardial mitral valve malfunction due to neointimal overgrowth expanding to valve cusps].

Two cases of Carpentier-Edwards pericardial mitral valve malfunction due to neointimal overgrowth were reported. Case one was a 51-year-old female undergone redo mitral valve replacement at nine years after first operation. Removed valve showed remarkable overgrowth of neointima expanding to the valve cusps. Case two was a 67-year-old male. A valve removed at nine years after first operation and at 1.5 years after recovery of prosthetic valve endocarditis. Removed valve also showed neointimal overgrowth expanding to the valve cusps. Although we experienced only two cases of neointimal overgrowth, these findings were considered being important in durability of Carpentier-Edwards pericardial valve.

[Stenosis of Carpentier-Edwards pericardial bioprosthesis in the tricuspid position caused by adhesion of native valve cusps to the bioprosthesis].

We experienced a case of dysfunction of Carpentier-Edwards pericardial bioprosthesis in the tricuspid position. Explanted bioprosthesis showed overgrowth of neointima from ring to cusps and adhesion to remnants of native cusps. This restricted the motion of the pericardial leaflets resulting in orifice stenosis. Care must be taken in such valve replacement by this prosthesis leaving native valve tissue.
