Distress and self-soothing bedtime behaviors in hospitalized children with non-rooming-in parents.

The nature of behaviors during sleep onset latency (SOL) at night bedtime was studied in a group of 40 hospitalized children ages 3 to 8 years whose parents did not room-in. Research questions were: (a) What is the nature of behaviors during sleep onset latency of young children whose parents do not room-in? and (b) How are these behaviors related to family structure, social status, age, gender, previous hospitalizations, and number of days hospitalized? Child subjects were observed using the Senders, Signals, and Receivers (SSR) computer-compatible system to measure SOL, distress, and self-soothing behaviors for 2-3 consecutive nights. The median falling asleep time was 26 minutes. Variations in SOL and self-soothing behaviors were not significantly related to family structure, social status, gender, age, length of hospitalization, night of hospitalization, or previous hospitalizations. Greater distress behaviors were, however, significantly related to children of two-parent families and those from middle social status.

Renal oncocytoma: a benign tumour on the increase.

Renal oncocytoma is an uncommon tumour which is being diagnosed with increasing frequency in recent years. Traditionally, these renal tumours were considered to be histological variants of renal adenocarcinomas, with no prognostic significance of their own. Several recent retrospective studies have demonstrated that they are a separate benign entity; therefore, differentiation and identification of these tumours are of the utmost importance.

Anatomicomechanical study for the hydraulic line of a thermal left ventricular assist system.

A thermal left ventricular assist system currently under development consists of two separate major components, i.e., a pump/actuator module and an engine/thermal battery module. The possible implantation site of the engine/battery module is tentatively determined to be the iliac fossa, which requires a flexible interconnecting line to the pump/actuator module. A quantitative and biomechanical study was done on the effect of the implanted hydraulic line on the bendability of the torso. It revealed that the semirigid interconnecting line would not severely restrict the movement of the patient provided it has the proper prebend configuration. The cadaver fitting study proved the anatomical feasibility of the retroperitoneal iliac fossa as the implant site of the Thermal Ventricular Assist System 8 (TVAS 8) engine/battery module. The location and configuration of the interconnecting line of the TVAS 8 were determined using biomechanical analysis so that the restriction of the body movement due to the line could be minimized. The main route of the line was the left side of the trunk, and the line is W shaped in configuration. The line elongation is the most critical resistive factor and accordingly there may still be rigidity during lateral flexion.

A computer-compatible method for observing falling asleep behavior of hospitalized children.

A family-organizational linkage framework was used to conceptualize the nighttime separation behaviors of 18 hospitalized children, ages 3 to 8, whose parents did not room-in. All children were observed for three consecutive nights during the falling asleep period. A bedtime story, tape recorded by the parent, was played on the second and third nights for one group. Frequency and duration of falling asleep behaviors were recorded and analyzed with the Senders Signals and Receivers System, a computer-compatible method of recording behavioral data. Eight conceptual behavioral categories emerged from the data: sleepy, contact, active, inactive, neutral, communication, distress, and pleasure. The story-children fell asleep sooner (23 minutes vs. 28 minutes), exhibited more sleepy behaviors, and displayed fewer active behaviors. Duration of contact behaviors also was greater in this group (22 minutes vs. 14 minutes). Hospitalized children who hear a bedtime story recorded by a parent appear to use self-soothing behaviors to cope with the separation experience.

Natural rates of teacher approval and disapproval in the classroom.

Sixteen classroom observational studies were conducted to determine natural rates of teacher verbal approval and disapproval in the classroom. Rates of teacher verbal approval and disapproval were measured by the Teacher Approval and Disapproval Observation Record (TAD) over Grades 1 through 12. Teacher verbal approval rates dropped over grade, with a marked drop after second grade. In every grade after second, the rate of teacher verbal disapproval exceeded the rate of teacher verbal approval. These rates are interpreted in terms of reinforcement theory.
