A 65-year-old woman with bilateral renal parenchymal malakoplakia and renal vein thrombosis had severe azotemia, bilaterally enlarged kidneys, urinary tract infection, and sepsis. Angiographic studies demonstrated left renal vein thrombosis. A percutaneous kidney biopsy specimen showed the characteristic Michaelis-Gutmann bodies. After therapy with fluid replacement, antibiotics, and long-term anticoagulation, kidney function improved and both kidneys decreased in size. We suggest that, with adequate treatment of urinary tract infection and correction of potential insults to the kidney, progression of this peculiar disease can be prevented.
Histochemical and microscopic studies have shown that a characteristic renal response to lead exposure is the formation of discrete, dense, staining intranuclear inclusion bodies in renal tubular epithelial cells. Cytologic examination of urinary sediment showed that four of 19 (21%) lead workers had exfoliated inclusion-bearing cells of proximal renal tubular origin. These lead-induced inclusion-bearing cells appeared distinctly different from viral-induced inclusions, degenerative or nonspecific intranuclear inclusions seen with tubular necrosis, or macronucleoli seen in reparative renal tubular epithelium. While their presence indicates cytologic evidence of tubular injury, the clinical significance of these cells and their application to medical monitoring is not clearly understood.

To determine the effect of renal venous constriction on the localization of immune complexes in the kidney, we performed unilateral renal venous constriction in 19 male albino rabbits 7 days after they were given an i.v. bolus (250 mg/kg) of bovine serum albumin (BSA) to induce the development of serum sickness nephritis. The rabbits were sacrificed on day 12 after BSA administration. Renal histology was evaluated by (1) light microscopy, with semiquantitative grading and enumeration of glomerular nuclei and (2) fluorescent microscopy. Six rabbits were nonresponders and did not exhibit nephritis. The remaining 13 rabbits had differential degrees of severity of nephritis between the control and experimental kidneys as assessed by light microscopy (in 7 rabbits) and by fluorescent microscopy (in 12 and 9 rabbits when examined for deposits of IgG and C3, respectively). In all instances, the experimental kidney (with the constricted vein) showed less severe alterations. The degree of protection appeared to correlate with the degree of renal venous constriction, as manifested by tubulointerstitial changes [rs = 0.77, P less than 0.01). We concluded that renal venous constriction exercises a protective effect on the impaction of immune complexes in the glomeruli and development of acute serum sickness nephritis.

Fibronectin is a glycoprotein shown previously to be present on the surfaces of cells, connective tissue matrices, and in extracellular fluids. Its distribution in the human kidney was evaluated by immunofluorescence methods studying 65 renal biopsies with a variety of pathologic disorders. Fibronectin was found in the mesangium of the normal kidney and increased proportionately in the disease states characterized by mesangial expansion. It was visualized in glomerular capillary loops in some biopsies with membranoproliferative glomerulonephritis, diffuse proliferative glomerulonephritis, and membranous nephropathy. Additionally, fibronectin was detected in glomerular crescents, diabetic nodules, vascular thrombi, glomerular sclerosis, and variably in the interstitium.

Scintiphotographic demonstration of a pulmonary arteriovenous malformation in an 8-year-old child is presented. Using a data processor, the transit of radiopertechnetate through the lesion as well as various portions of the lung is shown. The "steal" of blood flow from the left lobe is documented.

Primary mucinous adenocarcinoma of the skin is a rare entity arising from eccrine sweat glands with a propensity for local recurrence and metastasis. This tumor has been widely reported in the ophthalmology literature, but not to our knowledge in the otolaryngology literature. A case of a 48-year-old man with a mucinous adenocarcinoma extensively involving the right eyelid and orbit necessitating craniofacial resection is presented together with a discussion of the pathology, clinical features, and management.
In this cytomorphologic study, the diagnostic criteria for recognizing renal epithelial fragments are given based upon urine sediment examinations with histologic correlation. The study included individuals without urine system disease and patients with acute allograft rejection, acute tubular necrosis, papillary necrosis and renal infarction. Exfoliated renal epithelial fragments are accurately identified by th following configurational characteristics: (1) epithelium attached to or molded by renal casts, (2) epithelial encasement of casts, (3) cylindrical ("sleevelike") epithelium, (4) epithelial sheets with "honeycomb" arrangement and (5) epithelium containing spindle or elongate cells.
Exfoliated epithelial fragments found in urinary sediment may originate in renal parenchymal or lower urinary tract epithelium. Cytologists must be aware of situations or diseases that frequently exfoliate epithelial fragments from these sites. In this case report, cytodiagnoses of both renal ischemic necrosis and low-grade papillary transitional cell carcinoma were made. Exfoliated renal epithelial fragments were accurately identified by their morphologic features and distinguished from exfoliated urothelial fragments of a low-grade bladder neoplasm.
The tocolytic effects, adverse reactions, cost, and duration of therapy of terbutaline and ritodrine used for treating preterm labor were compared using retrospective chart review. Of 614 pregnant women admitted to an obstetrics unit between May 1980 and June 1981 who did not deliver a child during that admission, 48 remained on the unit more than 12 hours, received a tocolytic agent, and had a diagnosis of preterm labor. Of these, 22 women received terbutaline or ritodrine. Eight women were excluded, leaving 14 women in the study group. Maternal and fetal characteristics were tabulated, and patients exposed to terbutaline (seven mothers and seven babies) and ritodrine (seven mothers and eight babies) were compared. There were no significant differences between the terbutaline and ritodrine groups for maternal or fetal characteristics, efficacy of treatment, adverse effects, or duration of therapy. The cost of terbutaline is substantially less than that of ritodrine. Terbutaline and ritodrine appear to be comparable tocolytic agents clinically. The use of terbutaline should be investigated further in view of its lower cost and comparable clinical effects.
