Rickets among children of a coastal area of Bangladesh.

Many children with rachitic deformities have been reported in southern coastal area of Bangladesh but the actual rate of prevalence was not known. A survey was conducted to determine the magnitude of rachitic problem among the children of Chakaria thana of Cox's Bazar district of Bangladesh. Nine hundred children between 1-15 years selected randomly from 30 villages of total 340 villages. Face to face interview of the parents was taken and the children were examined for evidences of rickets. Serum calcium, phosphorus, alkaline phosphatase (ALP) were estimated and radiology of limbs were done in all clinically suspected cases and in a control of every eighth child. Seventy eight children (8.7%) had physical features suggestive of rickets. Fifty eight (6.4%) children had 'clinical rickekts' (positive physical feature(s) but normal ALP and negative radiology), 12 (1.3%) children had 'biochemical rickets' (positive physical features and raised ALP but negative radiology) and 8 (0.9%) children had 'confirmed rickets' (positive physical features, raised ALP and positive radiology). Out of 78 children with rachitic feature(s), Pectus carinatum was found as the most common clinical feature in 26 (33.3%) children followed by genu valgum in 23 (29.4%) cases. Twenty two normal children (2.2%) had raised level of ALP (>300U/L). The prevalence of rickets is high in children of Chakaria and further study is needed to find out the exact aetiology of rickets in children there.

Obesity management in patients with type 2 diabetes mellitus.

Obesity infrequently associated with type 2 diabetes mellitus and may cause insulin resistance, hypertension and other complications of obesity. So obesity management in Type 2 diabetes mellitus is essential. Prevention is a logical first step in the management of the obese Type 2 diabetic patient, such programs have had little long-term success. Diet, exercise and behavioral modification still form the cornerstones of treatment and relatively small weight loss results in improvement of all major obesity related co-morbidity's, including Type 2 diabetes. The obese diabetic patient faces extra impediments to weight loss, including the adverse effects of diabetic medication, poor glycaemic control and diabetes related complications. New drugs may offer some additional help, in general by providing the benefit associated with the weight loss as such, Bariatric surgery can produce major long-term weight loss in the severely obese subjects.

An evaluation of the national food and nutrition policy of Bangladesh.

Food and nutrition policy activities directed toward improvement of the nutritional status of the people of Bangladesh began in the 1980s. The government formulated a national food and nutrition policy and approved it in 1997. Qualitative methods, including observational techniques, in-depth interviews of the key informants, and focus group discussions, were used to collect information on the strengths, weaknesses, opportunities, and threats (SWOT) of the policy. The information obtained has been transcribed and analyzed using this model. The strengths of the policy are that it is a consensus document that emphasizes human rights, was formulated by a multisectoral approach, complements other government policies, and has broad goals and wide-ranging objectives. The weaknesses include lack of implementation, monitoring, and evaluation guidelines; lack of strong government commitment; inadequate support of policy makers; perhaps an excessively ambitious target; and ignorance of past lessons learned. The opportunities include the scope of social mobilization, the wide scope of the policy, suggested programs and measures to improve nutritional status, a congenial policy environment, and the ability to modify the scope of the policy as needed. The threats to the policy are lack of knowledge of the policy, lack of resources to implement the policy, tension between technical people and bureaucrats, vested business interests, and, possibly, discontinuity of political commitment. The key to reducing the weaknesses of the food and nutrition policy of Bangladesh and minimizing the threats to it is for the stakeholders in the policy to coordinate efforts to use the strengths and opportunities of the policy to effectively implement it.

Accumulation of arsenic in tissues of iron-deficient rats.

Toxic concentration of arsenic (400 microg/kg body weight/day) was administered orally for 8 weeks to both iron-supplemented and iron-deficient rats. The results showed greater amount of arsenic in both the duodenum and liver of iron-deficient rats, compared to iron-supplemented ones. Duodenum containing high concentration of arsenic in iron-deficient rats showed significant reduction of ferrireductase (the enzyme responsible for the conversion of ferric iron to ferrous one) activity. Our results suggest that anemic rats are more prone to develop arsenic poisoning following chronic ingestion of high content of arsenic.

Development of cytochrome P450 17 alpha-hydroxylase (P450c17) mRNA and enzyme activity in neonatal ovaries of normal and hypogonadal (hpg) mice.

The cytochrome P450 enzyme 17 alpha-hydroxylase (P450c17) is required for androgen synthesis and therefore regulates substrate supply for aromatization. In this study, changes in P450c17 activity and mRNA levels were measured during ovarian development in the normal mouse and in the hypogonadal (hpg) mouse which lacks circulating gonadotrophins. At birth, low levels of P450c17 activity and mRNA were detectable in normal ovaries. This basal level of expression did not change until after day 10 at which time both enzyme activity and mRNA levels increased by six- to eightfold. In the hpg mouse, levels of P450c17 mRNA were normal at birth but did not change significantly during subsequent development and were significantly less than normal by day 15. Results show that there is a low level of gonadotrophin-independent expression of P450c17 in the ovary at birth and that gonadotrophins are required for the subsequent increase in expression between days 10 and 15. In the ovary, P450c17 is expressed solely in the thecal/interstitial compartment and interstitial cells arise in the mouse ovary around day 11. Changes in P450c17 are likely, therefore, to be related to gonadotrophin-dependent development of the interstitial tissue in the mouse. Treatment of adult hpg mice with LH and FSH showed that both gonadotrophins can act to increase P450c17 activity. Since FSH acts only on the granulosa cell compartment of the ovary it is likely that FSH acts through a paracrine mechanism to regulate thecal/interstitial cell activity.

Development of cytochrome P450 aromatase mRNA levels and enzyme activity in ovaries of normal and hypogonadal (hpg) mice.

The cytochrome P450 aromatase (P450arom) enzyme is required for bioconversion of androgen to oestrogen. In this study ovarian P450arom mRNA and enzyme activity have been measured during development in normal mice and hypogondal (hpg) mice which lack circulating gonadotrophins. A semi-quantitative reverse transcription-PCR (RT-PCR) technique was used to measure cytochrome P450arom mRNA levels and aromatase enzyme activity was measured directly. Using RT-PCR, P450arom mRNA was detectable in the adult mouse ovary and also in the uterus, kidney, brain and skeletal muscle but not in cardiac smooth muscle. In the normal mouse, P450arom mRNA was detectable in the ovary on the day of birth (day 1) and levels increased significantly up to day 15 with the most marked changes seen between days 1 and 5. Aromatase activity was also detectable at all ages in the ovary and increased significantly between days 1 and 7. In ovaries from hpg mice, normal levels of P450arom mRNA were present on day 1 but there was no significant change in P450arom mRNA at later ages up to day 15. These results show that in the newborn mouse ovary, which contains only primordial follicles, there is a basal expression of P450arom mRNA which is not gonadotrophin-dependent. After 1 day, however, gonadotrophins are required for normal expression of ovarian P450arom and this coincides with development of primary and secondary follicles.

Development of cytochrome P-450 side chain cleavage mRNA levels in neonatal ovaries of normal and hypogonadal (hpg) mice.

Cytochrome P-450 side chain cleavage (P-450scc) is vital for biosynthesis of gonadal steroids. In this study changes in P-450scc mRNA levels and cholesterol side-chain cleavage (CSCC) enzyme activity have been measured during development in the ovary of the normal mouse and the hypogonadal (hpg) mouse which lacks circulating gonadotrophins. Levels of P-450scc mRNA were measured using a semi-quantitative reverse transcription-polymerase chain reaction technique for which part of the 5' sequence of a mouse P-450scc cDNA was sequenced. On the day of birth P-450scc mRNA was present at low levels in the mouse ovary. Thereafter there was no change in P-450scc mRNA levels for 5 days after which time levels increased significantly to reach a peak around day 10. Activity of CSCC showed a similar pattern of development although activity was not detectable on days 1 and 3. In the hpg mouse P-450scc mRNA levels were normal on day 1 but did not increase thereafter up to 15 days. These results show that there is gonadotrophin-independent expression of P-450scc mRNA in the mouse ovary at birth when only primordial follicles and stromal tissue are present. As the ovary develops after 5 days changes in P-450scc mRNA levels become gonadotrophin-dependent and coincide with maturation of secondary follicles.

Steroidogenesis during postnatal development in the mouse ovary.

In the mouse, follicular formation and development is largely postnatal. Changes in ovarian steroidogenesis during early postnatal life are likely, therefore, to reflect changes in follicular activity during early folliculogenesis. In this study, basal progesterone and androstenedione production and responsiveness to gonadotrophins, dibutyryl cyclic AMP (dbcAMP) and 22R-hydroxycholesterol have been measured following short-term in-vitro incubations of ovaries from mice aged 1, 3, 5, 7, 10 or 15 days. On the day of birth (day 1), basal and gonadotrophin-stimulated progesterone and androstenedione production were undetectable although a response to dbcAMP and a low level of cholesterol side-chain cleavage (CSCC) activity (measured using 22R-hydroxycholesterol) were present. On day 3 progesterone and androstenedione production were undetectable under all conditions. On day 5 only a low level of CSCC activity was detectable but on day 7 there was an increase in ovarian steroid production. Basal progesterone and androstenedione were detectable and LH, but not FSH, increased the production of both steroids. These changes were associated with a marked increase of more than 80-fold in CSCC activity. Basal steroid output increased from days 7 to 15 and LH continued to stimulate progesterone accumulation although no effect on androstenedione was seen. Addition of FSH had no effect on steroidogenesis on day 10 but significantly increased progesterone production on day 15. Ovaries from the mice used in these studies contained primordial follicles and stromal tissue on day 1. By day 5 primary and secondary follicles were present and the major increase in steroid production between days 5 and 7 was associated with an increase in secondary follicles and increased differentiation of the thecal tissue.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of high-density lipoprotein on bovine granulosa cells: progesterone production in newly isolated cells and during cell culture.

It has been proposed that changes in steroidogenesis which occur during early development of the corpus luteum may be due to increased availability of lipoproteins. Bovine follicular fluid, however, contains significant amounts of high-density lipoprotein (HDL), and granulosa cells are exposed to this lipoprotein before ovulation. To determine whether bovine granulosa cells can utilize HDL the effects of this lipoprotein on freshly isolated, non-luteinized granulosa cells and on granulosa cells undergoing luteinization in serum-free culture were examined. Cells were isolated from non-atretic, antral follicles and cultured for 12 h in 10% (v/v) lipoprotein-deficient serum to allow cell attachment. After this time cells were cultured in serum-free medium. During culture the cells underwent functional luteinization as assessed by an increase in basal progesterone output (9.6-fold in 7 days) which was associated with a marked increase in activity of cholesterol side-chain cleavage and loss of aromatase activity. Dibutyryl cyclic AMP (dbcAMP) increased basal production of progesterone about twofold but HDL alone had no effect. Addition of HDL plus dbcAMP, in contrast, caused a very marked stimulation (up to ten times) of basal steroidogenesis. This trophic effect of HDL and dbcAMP lasted at least 2 weeks. Activity of cholesterol side-chain cleavage was stimulated (threefold over basal) by dbcAMP during culture but HDL was without effect, alone or with dbcAMP. Addition of HDL (in the presence or absence of dbcAMP) to freshly isolated granulosa cells had no significant stimulatory effect on progesterone production over 12 h in six experiments, and in two of these experiments a significant inhibitory effect was seen.(ABSTRACT TRUNCATED AT 250 WORDS)

Family, society, economy and fertility in Bangladesh.

"This paper examines the socio-economic and cultural conditions under which the large family represents a rational economic goal for parents [in Bangladesh]." The author notes that rural children provide valuable labor services to parents during childhood, grown sons continue to support their parents financially and in other ways, and sons are the most reliable source of security in old age. Daughters, however, remain at home and cost a significant amount for dowries at marriage. It is concluded that prevailing socioeconomic conditions in Bangladesh still provide substantial support for high fertility and son preference.

Ovarian steroid metabolism during post-natal development in the normal mouse and in the adult hypogonadal (hpg) mouse.

Ovarian steroid metabolism was investigated (i) during development in a normal inbred strain in which post-natal follicle growth has been described and (ii) in adult hypogonadal (hpg) mice which lack GnRH and have very low serum concentrations of gonadotrophins. Tissue was incubated with [3H]pregnenolone or [3H]androstenedione and metabolites separated by t.l.c. or h.p.l.c. Progesterone was the major metabolite formed at all ages while androstenedione was the major androgen. Between 7 and 21 days there was an overall increase in steroidogenic enzyme activity with a peak of 5 alpha-reductase between 21 and 29 days. The major metabolite of progesterone around puberty was 5 alpha-pregnane-3 alpha-ol-20-one. A sharp increase in 20 alpha-hydroxysteroid dehydrogenase was observed after 38 days due, presumably, to the appearance of corpora lutea. Unlike the rat, androstanediol levels were low at all ages. Oestradiol was the major oestrogen formed from androstenedione with a peak of production at 38 days. In the adult hpg mouse metabolism was similar to that of the 7-day normal mouse although 17-ketosteroid reductase and aromatase levels were very low compared to normal animals of any age, indicating that gonadotrophin stimulation is involved in the expression of activity by these enzymes.

Infective polyneuropathy (a report of 14 cases).

Fourteen cases of infective polyneuropathy were admitted in the Institute of Postgraduate Medicine and Research, Dacca, during a period of one year. The clinical features and laboratory investigations of these patients have been recorded. The management was done as usual but particular stress was given to the use of a short course of corticosteriod to all patients. The results of treatment was satisfactory and uniform for majority of the cases.

Cerebrovascular diseases. (A report of 53 cases).

Fifty-three patients suffering from cerebrovascular disease admitted to the Neurology unit of the Institute of Postgraduate Medicine and Research, Dacca, have been analyzed. It has been observed that the age incidence, sex ratio, causes and clinical pattern of cerebrovascular diseases are similar in developed and developing countries.
