Incomplete medical records: three case solutions.

The medical record is the wellspring of data for countless medical staff and administrative decisions. Even though the ancillary activities that depend on the medical record have received most of the attention in recent years, the most common problem encountered by medical and hospital staffs is probably as old as the concept of the medical record itself--completing it in a timely and accurate manner. This article details how three hospitals keep the number of incomplete charts in check.

Has the health care debate gotten out of hand?

Has the struggle between private providers and the government over the control of health care services gotten out of hand? Robert Derzon, former director of the Health Care Financing Administration, thinks that both sides have lost their perspective on the issues and need to regain a sense of balance and recognition of each other's rightful role in the health care delivery system.

Medical education faces a no-growth decade.

The nation's medical colleges are coming out of a decade of growth and expansion into what some perceive as a steady-state decade that will see a decline in financial support and an increase in regulation. Physicians and administrators gathered at the annual convention of the Association of American Medical Colleges discussed the effects of such a restricted environment on medical education and health care in general.

AMA withdraws from CME accrediting body.

The AMA House of Delegates voted to withdraw from the Liaison Committee on Continuing Medical Education and return to its own accrediting system. The move ends a situation that has irritated the state associations, but it opens the possibility of two CME accrediting bodies. The AMA also modified its stand on chiropractic in an attempt to stop its mounting legal problems.

FTC allegations cause rift in AMA house.

The AMA attempted to close ranks on a number of potentially devisive issues at its interim meeting in December. Among the issues disputed were alleged AMA antitrust violations dealing with chiropractors and advertising policies and the association's position on national health insurance. Other medical staff issues put before the delegates included voluntary cost containment, due process, liability insurance requirements, and PSRO data confidentiality.

How state medical record access laws are working.

Several states have passed laws granting patients the right to directly review their medical records. So far, such laws have caused some administrative headaches, but hospitals report no major problems. The biggest fear--increase in the number of malpractice suits--has not been realized.

Eastern equine encephalomyelitis in upstate New York: studies of a 1976 epizootic by a modified serologic technique, hemagglutination reduction, for rapid detection of virus infections.

An extensive outbreak of eastern equine encephalomyelitis (EEE) occurred in upstate New York during the summer of 1976, with 37 cases confirmed in horses by isolation of virus and/or by serologic examination. Other specimens collected in the affected area yielded 16 further isolates: 9 from 818 pools of 33,365 mosquitoes, 5 from tissues of 64 birds and 2 from 4 sentinel pheasants with serologic conversions. EEE antibodies were also detected in 81 of 499 wild birds tested. Our data implicate sparrows, cowbirds, and catbirds in the amplification of EEE virus and Culiseta melanura mosquitoes as vectors among avians. During the course of this epizootic a modified serologic technique involving hemagglutination reduction gave reliable diagnoses of EEE as early as 24 hours after receipt of field specimens.

Medical educators told to reduce enrollments.

HEW predicts that in the next decade, the United States faces an oversupply of physicians who will be of the wrong type and practicing in the wrong places. HEW Secretary Joseph Califano announced the government's plans to deal with this projected problem in an address before the Association of American Medical Colleges. Also released at the meeting was a report on minority student opportunities in medicine that outlined some of the problems faced by affirmative action programs and future directions.

AMA postpones debate on cost containment report.

"Aggressive moderation" to combat government's excess was the position suggested by John H. Budd, M.D., in his presidential address to the AMA's House of Delegates. Topics under discussion at this interim meeting included cost containment, primary responsibility for hospitalized patients, medical staff representation on hospital governing boards, PSROs, and the AMA's national health insurance proposal.

Cost control, primary care initiatives challenge teaching hospitals.

Pressures currently affecting medical schools and teaching hospitals are detailed. Although cost containment poses special problems for teaching hospitals, physicians can help hold down cost. Primary care initiatives and specialty redistribution, as well as the classification of "staff" and "private" patients, and planning agencies also are exerting pressures.

Risk management: new focus for traditional functions.

Physician cooperation and leadership are essential to risk control programs in hospitals. A recent AMA seminar on risk control in patient care dissected risk management into components familiar to all physicians--quality of care, infection control, medical records--with insight into how each segment relates to an overall program.

Physicians and hospitals: the times demand unity.

If the voluntary health care system is to continue effectively, then physicians, hospital administrators, and trustees must cooperate in working toward common goals. Ways to enhance communication between physicians and hospitals, including more complete participation in local health systems agencies and better understanding of the mechanism of Professional Standards Review Organizations, were discussed at a conference on physician-hospital relations.

AMA house backs voluntary controls, confronts government.

The AMA demanded more fiscal responsibility from government and strongly supported voluntary cost containment measures in actions at its annual convention. Of the 48 recommendations of the National Commission on the Cost of Medical Care, the house accepted those that were in agreement with AMA policy. Four commission recommendations that directly affect hospitals were passed: prospective rate setting, limitations on new hospital beds, decertification of existing health facilities, and strengthening physician price consciousness.
