Serous-lined extramural ileal valve as a new continent cutaneous urinary outlet: an experimental study in dogs.

A new technique for the construction of a cutaneous continent catheterizable outlet suitable for urinary reservoirs is presented. The technique entails the creation of an ileal reservoir outlet by implantation of a tapered ileal segment into a serous-lined extramural tunnel. The procedure was carried out in eight experimental dogs, and the results were compared with a control group of five dogs for which a nontunnelled tapered ileal outlet draining a similar reservoir was constructed. Clinical, radiological and urodynamic evaluation provided evidence that this new outlet is easily catheterizable. Its continence mechanism is more reliable than that of a simple tapered outlet.

Late malignancy in bowel segments exposed to urine without fecal stream.

OBJECTIVES: This study was constructed so as to screen malignant transformation after uroenteric reconstructions using bowel segments exposed to urine without fecal stream for more than 10 years. METHODS: Follow-up data were available for 186 patients who underwent various uroenteric reconstructions using bowel segments exposed to urine without fecal stream for more than 10 years. There were 68 eligible patients with isolated rectosigmoid bladder, 23 with bladder augmentations (15 ileocystoplasty and 8 colocystoplasty), 57 with ileal ureter, and 38 with ileal loop conduit. Besides routine laboratory and radiologic investigations, urine for cytology was obtained from all patients. Moreover, endoscopy and random biopsy of the part of bowel exposed to urine were carried out in all patients. RESULTS: Uroenteric malignancy was diagnosed in 4 patients (2%): 2 adenocarcinoma in an isolated rectosigmoid bladder, 1 transitional cell carcinoma following augmentation colocystoplasty, and 1 squamous cell carcinoma after ileal ureter. None of the patients developed tumors in ileal loop conduits. CONCLUSIONS: Malignant changes do not only occur after ureterosigmoidostomy but are also observed after different uroenteric reconstructions not exposed to fecal stream. Hematuria, ureteral obstruction, and abnormal urine cytology are warning signs of malignancy. Routine cytology is recommended at least yearly beginning 10 years after surgery.

Ethanol sclerotherapy for symptomatic simple renal cysts.

Twenty-eight patients with 30 symptomatic simple renal cysts were treated by percutaneous aspiration of the cyst and injection of 95% ethanol. All patients were available for follow-up during mean period of 19 (range 14-40) months. In all patients, successful resolution of the cyst with disappearance of the symptoms was observed within 1 month after treatment. During the whole period of follow-up, no patient experienced recurrence of symptoms or required repeat therapy. Of all renal cysts treated, resolution was complete in 25 (83%) and partial (recurrence of < 50% of original cyst volume with no symptoms) in 5 (17%). The degree of response to sclerotherapy (whether complete or partial) correlated significantly with cyst size (P > 0.0008). Major complications were not encountered, while microscopic hematuria was seen in two patients, and low-grade fever was observed in another two. Ethanol sclerotherapy is simple, noninvasive, and highly cost-effective and should be recommended for the treatment of symptomatic simple renal cysts.

Mutations in the p53 gene in schistosomal bladder cancer: a study of 92 tumours from Egyptian patients and a comparison between mutational spectra from schistosomal and non-schistosomal urothelial tumours.

Much of bladder cancer in East Africa and the Middle East is attributed to chronic urinary infection with Schistosoma haematobium ('schistosomiasis'). Most schistosomal bladder cancer (SBC) is squamous cell carcinoma (SCC) and occurs in the fifth decade of life. In contrast, nonschistosomal bladder cancer (NSBC) in Western countries usually occurs in the seventh decade of life and is largely transitional cell carcinoma (TCC). To shed light on the mechanisms underlying these different patterns of bladder cancer we looked for mutations in the p53 gene in SBC from 92 patients in Egypt, where schistosomiasis is hyperendemic. Patients' mean age at presentation of bladder cancer was 49.4 +/- 9.9 years and 90% had a clinical history of schistosomiasis and/or histological evidence of schistosomal eggs adjacent to the carcinoma. There were 53 SCC, 23 TCC, 13 adenocarcinomas and three other carcinomas. Thirty patients had tumours with mutations in exons 5-8 of the p53 gene: 17/53 SCC, 8/23 TCC, 4/13 adenocarcinomas and 1/3 other tumours. Of 19 mutations in SCC, 16 were base pair substitutions (BPS), two were deletions and one an insertion. Two tumours each contained two mutations. Of the BPS, nine were transitions at CpG dinucleotides and two were G-->T transversions. All the mutations in TCC were BPS: four were transitions at CpG dinucleotides and three were G-->C transversions. One TCC had two mutations. Of four adenocarcinomas with mutations, two had transitions at CpG dinucleotides. Of the 30 BPS mutations, 16 were transitions at CpG dinucleotides, of which 12 were C-->T. We combined these 33 mutations with six obtained from Egyptian SCC reported by Habuchi et al. (Cancer Res., 53, 3795-3799, 1993) to compile a mutational spectrum. This was compared with a NSBC spectrum assembled from 118 mutations reported in the literature. The proportion of BPS at CpG dinucleotides was significantly higher in SBC than in NSBC (18/34 versus 25/103, P = 0.003). There was also a bias away from mutations in exons 7 and 8 towards mutations in exons 5 and 6. We suggest that the excess of transitions at CpG dinucleotides in SBC results from nitric oxide (NO) produced by the inflammatory response provoked by schistosomal eggs. NO could produce such mutations directly, by deamination of 5-methylcytosine, and indirectly, following conversion to nitrate, bacterial reduction to nitrite and endogenous formation of urinary N-nitroso compounds. These produce O6-alkylguanines in DNA, leading to very high rates of G:C-->A:T transitions, a process possibly augmented by inefficient repair of alkylated bases at CpG dinucleotides.

Laparoscopic nephrectomy: Mansoura experience with 106 cases.

OBJECTIVE: To describe the technique and report our experience of laparoscopic nephrectomy. PATIENTS AND METHODS: Between August 1992 and December 1993, 106 patients underwent laparoscopic nephrectomy at the Mansoura Urology and Nephrology Center. RESULTS: Of the 106 patients, laparoscopic nephrectomy was performed successfully in 97 cases. Conversion to an open procedure was necessary in the remaining nine patients. CONCLUSION: For a selected group of patients, laparoscopic nephrectomy is a feasible and safe technique. The length of hospital stay and convalescence is short and return to work is rapid.

Further clinical experience with the ileal W-neobladder and a serous-lined extramural tunnel for orthotopic substitution.

OBJECTIVE: To report on the functional results following orthotopic substitution using an ileal W-neobladder with an extramural serous-lined tunnel for reflux prevention. PATIENTS AND METHODS: Sixty men (mean age 46 years) underwent a one-stage radical cystoprostatectomy and an ileal W-neobladder for invasive bladder cancer. The technique entails the creation of two serous-lined extramural tunnels in a detubularized ileal W-bladder fashioned from 40 cm of the terminal ileum. Fifty-one patients were evaluable, with a minimum follow-up of 2 years. Evaluation of patients included clinical, radiographic and urodynamic studies. RESULTS: There was no operative mortality and no gross morbidity. The upper tracts remain unchanged or improved in 97% of the implanted renal units. Reflux was not observed in any patient. The incidence of day and night continence was 90 and 80%, respectively. CONCLUSION: The technique provides a non-obstructed unidirectional uretero-ileal re-implantation, in a low-pressure system constructed from a short ileal segment. No staples were required.

Orthotopic bladder substitutes: histopathologic risk factors.

Prostate glands from 150 patients with carcinoma of the bilharzial bladder who underwent cystoprostatectomy were studied histopathologically by step sections. Prostatic urethral involvement by urothelial carcinoma was noted in 13 out of 96 (13.5%) and 5 out of 40 (12.5%) squamous and transitional cell tumors, respectively. None of the 12 adenocarcinomas and the two undifferentiated tumors showed involvement. Prostatic urethral involvement was as high as 19% in basal tumors and 26.7% in multifocal tumors compared to only 6.5% when the tumors occupied the bladder body. There was a significant increase in the incidence of prostatic urethral involvement from 9.5 to 35% when the prostate gland was involved. Prostate gland was involved in 20 out of 150 (13.3%). The bladder tumor was basal and infiltrating the prostate in 18 such cases. Seminal vesicles were infiltrated in 6 cases from the adjacent basal bladder tumors. We conclude that patients with basal or multifocal tumors are risky regarding bladder substitution and we recommend routine diagnostic transurethral prostatic biopsies and frozen sections from the site of urethral transection during cystoprostatectomy whenever bladder substitution controlled by the urethral sphincter is considered.

Further experience with the modified ileal ureter.

A total of 50 patients for whom an ileal ureter was indicated was prospectively randomized between 2 treatment groups according to the surgical technique used. In group 1 the standard ileal ureter procedure was performed, while in group 2 a tailored and valved (modified) ileal ureter was fashioned. Of the patients 19 in group 1 and 23 in group 2 were available for followup of 69 +/- 5.8 months (range 60 to 80). Among the 23 patients in group 2, 5 (22%) experienced reflux due to dessusception of the nipple valve and 4 (17%) had bladder stones. Metabolic acidosis was observed in 9 patients in group 1 (47%) and 4 in group 2 (17%). Urinary outflow obstruction occurred in 5 patients in group 1 and 1 in group 2. Although renal function was comparable among patients in both treatment groups, comparison between patients in group 1 and those without reflux in group 2 showed better renal function in the latter group. We conclude that reflux prevention is of central importance in preservation of renal function in patients with an ileal ureter. A distal nipple valve at the ileovesical anastomosis is associated with significant problems. These observations would urge urologists to search for a more suitable procedure that provides an efficient antireflux principle with minimal adverse sequelae.

Radioisotopic evaluation of renal function in cyclosporine-treated pediatric and adult renal transplant recipients and their living donors. A study of 152 donor-recipient pairs.

Renal function was studied in 2 groups of renal transplant recipients and their donors by technetium-99m diethylenetriamine pentaacetic acid and a gamma camera. The pediatric group (group A) comprised 40 children and their adult kidney donors. The adult group (group B) consisted of 112 consecutive adult renal transplant recipients and their adult donors. All patients received kidneys from living donors and were given the same immunosuppression protocol (PRED plus CSA). Donor glomerular filtration rate (GFR) was determined before nephrectomy and at a mean period of 30 (range 10-50) months after nephrectomy. The graft GFR was measured at 1, 3, 6, and 12 months and at the most recent follow-up visit. Moreover, the functional reserve of the graft was assessed by infusion of dopamine and an amino acid. The postnephrectomy GFR of donors in groups A and B were 74 +/- 18 and 72 +/- 20 ml/min/1.73 m2, respectively. The GFR of pediatric recipients was significantly lower than that of adult recipients at corresponding time points along the course of follow-up. The mean values of graft GFR were 47.6 +/- 20 and 63.8 +/- 29.6 ml/min/1.73 m2 for pediatric and adult recipients, respectively (P < 0.001). Moreover, the graft functional reserve was significantly lower in pediatric recipients. These data demonstrate that adult kidneys transplanted into pediatric recipients have lower GFR than those transplanted into adults, despite corrections for body surface area. Although the reason for this phenomenon is unknown, the observation may have important implications for management of pediatric recipients.

A novel uretero-ileal reimplantation technique: the serous lined extramural tunnel. A preliminary report.

A novel technique for an anti-refluxing uretero-ileal reimplantation entailing creation of 2 serous lined extramural tunnels in a detubularized ileal W-bladder is presented. The operation was done on 12 patients in whom an orthotopic bladder substitute was indicated. Mean followup was 18 months. Evidence indicated that this method could provide a nonobstructed unidirectional flow of urine in all of the examined renal units.

Accidental division of the transplanted ureter during laparoscopic drainage of lymphocele.

We report on the accidental division of the ureter of a transplanted kidney during laparoscopic marsupialization of a symptomatic lymphocele into the peritoneal cavity. Identification of the lymphocele was difficult because of a thick layer of overlying periperitoneal fibrolipomatous tissue. The complication was identified intraoperatively and stented reanastomosis of the divided ureter was performed. Postoperative course was uneventful. To avoid accidental injury to the transplanted ureter during laparoscopic drainage of lymphocele, we recommend that a ureteral catheter be inserted.

Tetracycline sclerotherapy for testicular hydroceles in renal transplant recipients.

OBJECTIVES: To study the efficacy of tetracycline sclerotherapy in renal transplant recipients with symptomatic hydroceles. METHODS: A total of 21 patients with symptomatic hydroceles following renal transplantation underwent aspiration of hydrocele and injection of tetracycline hydrochloride. Sclerosant solution was prepared by dissolving 1 g tetracycline hydrochloride powder in 10 mL 1% lidocaine. The amount of sclerosant used depended on the volume of the sac: 5 mL for a sac containing up to 100 mL and 2.5 mL of sclerosant was added for each increase of 100 mL in sac volume. RESULTS: Twelve patients (57%) required only one treatment and 9 patients (43%) had up to 3 injections. The larger the hydrocele, the more treatments were required. The resolution of hydrocele was complete in 12 patients (57%) and partial in 7 (33%) with 2 (10%) failures. Pain at injection was observed in one third of the patients. No major complications (fever, hematoma, infections, abscess, or scrotal necrosis) occurred in any patient. No changes in the structure or size of the testicles were found by ultrasound during an average follow-up period of 35 months. CONCLUSIONS: Tetracycline sclerotherapy is a safe, effective, and economical form of out-patient therapy that can be recommended as primary treatment for hydroceles in patients who have undergone renal transplantation.

The modified rectal bladder (the augmented and valved rectum) for urine diversion in children.

OBJECTIVES: Urinary diversion is usually the procedure of choice for children having complicated primary closure for bladder exstrophy. We introduce the modified rectal bladder as a low pressure and functionally isolated rectal reservoir as a bladder substitute for these cases. METHODS: Modified rectal bladder urinary diversion was done on 15 children as a low pressure and functionally isolated rectal reservoir via the adoption of sigmoid intussuscepted valve and the rectal patching with detubularized sheet of ileum. Fourteen of these children are currently evaluable, with follow-up ranging from 16 to 72 months (median 55 months). All of them are subjected to thorough history-taking, clinical examination, laboratory and radiologic investigations, and urodynamic study. RESULTS: A high rate of urinary continence was achieved and so far the upper urinary tract and the metabolic status were preserved. Reflux to the colon and kidneys was prevented. Urine samples from the renal pelvis through percutaneous needle aspiration revealed sterile cultures in 82% of the renal units (23 of 28). CONCLUSIONS: Our results demonstrate the distinct advantages of the modified rectal bladder over the conventional methods of urinary diversion to the rectum or the abdominal reservoirs coupled to the skin via continent catheterizable stomas.

Laparoscopic nephrectomy: an established routine procedure.

Between August 1992 and April 1993, 60 patients underwent laparoscopic nephrectomy in our institution for benign disease (35 hydronephrosis, 20 chronic pyelonephritis, 4 end-stage kidney, 1 renal hypoplasia). Conversion to open surgery was needed in six cases to overcome intraoperative bleeding or perirenal adhesions. The mean operative time was 3.5 +/- 1.3 hours, and the mean hospital stay was 3.2 +/- 2.1 days. No deaths occurred, but significant complications were encountered in four cases in the form of pulmonary embolism, a large hematoma, postoperative bleeding, and colonic perforation. Laparoscopic nephrectomy is a safe and effective alternative to open nephrectomy for benign renal conditions.

Elastase inhibitor elafin is a new type of proteinase inhibitor which has a transglutaminase-mediated anchoring sequence termed "cementoin".

Elafin was shown to be a new type of proteinase inhibitor which has an anchoring sequence. Human elafin, a potent inhibitor specific for elastase and proteinase 3, has a unique repeating sequence in its prosegment that is rich in Gln and Lys residues. The prosegment, termed "cementoin," exhibits high homology with the repetitive element of seminal vesicle clotting protein, which is known as a good substrate for prostate transglutaminase. The cross-linking of cementoin by tissue transglutaminase showed that the cementoin moiety is indeed a preferable substrate for transglutaminase. In addition, transglutaminase-mediated cross-linking between cementoin and laminin was observed in vitro, suggesting that cementoin has the ability to covalently attach to other extracellular matrix proteins. To determine whether or not this type of covalent gluing of elafin through the cementoin moiety occurs in vivo, we determined the molecular size of cementoin-elafin in the trachea mucous epithelium by Western blotting; the rationale of this approach is that (i) the trachea is the richest source of cementoin-elafin, as shown below, and (ii) if cementoin-elafin is covalently associated with other proteins, it should migrate as a higher M(r) species on SDS-polyacrylamide gel electrophoresis; cementoin-elafin immunoreactivity was indeed detected at a position corresponding to 50 kDa, a value much higher than that of its monomeric form. RNase protection analysis and immunohistochemical staining revealed that cementoin-elafin is densely distributed in the skin and trachea, and moderately in the stomach, duodenum and small intestine. These sites of localization are consistent with the locations where elastic fibers are abundant.(ABSTRACT TRUNCATED AT 250 WORDS)

The nutcracker syndrome: new methods of diagnosis and treatment.

OBJECTIVE: To study the pathophysiology of the nutcracker syndrome and to describe a new method of treatment. SUBJECTS AND METHODS: Computerized tomography (CT) was used to compare the anatomical relations of the left renal vein with the aorta and the superior mesenteric artery in three patients with the nutcracker syndrome and in a control group of 12 healthy kidney donors. RESULTS: Abnormal branching of the superior mesenteric artery from the aorta was identified as the cause of the nutcracker syndrome. Two patients were treated by autotransplantation of the left kidney, while the third refused surgery. In the treated patients haematuria ceased 4 and 6 weeks after surgery. Postoperative excretory urography and repeated isotopic studies revealed perfect configuration and function of the autotransplanted kidneys. The patients remain asymptomatic 6 and 12 months after surgery. CONCLUSION: In patients with haematuria from the left kidney, angiographic CT must be considered before phlebography and pressure measurement of the left renal vein. If the diagnosis of the nutcracker syndrome is established, autotransplantation of the left kidney is a treatment alternative.

The application of the modified rectal bladder in management of the compromized urethral damage.

Failure to repair the traumatic injuries of the lower urinary tract with loss of urethral sphincteric function usually puts forward the subject of urinary diversion. Ten males and five females presented with extensive urethral damage and with or without vesical fistulas. All attempts of repair were exhausted before the decision of diversion was planned. Continent urinary diversion using the modified rectal bladder (the augmented and valved rectum) was utilized. The procedure entailed functional isolation of the rectum without terminal colostomy and depended on colorectal intussuscepted valve. All the patients are currently evaluated with follow up periods ranging 10-56 months. Evidences proved the effectiveness of the colorectal valve in providing non impeded fecal stream to the functionally isolated rectal reservoir. Continent status is attributed to the ileal patching that produces a low pressure reservoir against the effective high anal resistance. The results support the distinct superiority of the modified rectal bladder to either the conventional methods of urinary diversion to the rectum or the abdominal reservoir with continent catheterizable stomas.
