Morphological and immunohistochemical aspects of the biological seal in Klöckner's dental implants: study of 15 cases.

The morphological and immunohistochemical aspects of the peri-implanted pathological soft tissue of 21 Klöckner implants are assessed, corresponding to 15 patients and three gingival samples from three cadavers. For the collagen and vascular walls assessment, the Picro-Sirio technique is used with normal microscopy as well as with polarized light, evaluating the decrease of collagen fibers in relation to the grade of inflammation. In cases when there is a plasmocyte inflammatory prevalence, antibodies are used contrasted with Kappa and Lambda light chains, observing in all cases a polyclonal plasmocytosis (reactive). The Ag. Ki-67 is used to mark cell proliferation that shows a normal or enlarged activity that can reach the infiltrated cells of the lamina propria. Epitheliotropism or a lymphoepithelial lesion is analyzed in relation to the inflammation, observing the tendency to be larger in K-type implants with a completely-submerged technique. The lesion that is usually observed is the periimplanted mucositis of the chronic or mixed type. The regenerated epithelium has a slight thickness with congestive and dilated vessels. In one case (as anecdotal value), there is the presence of coilocytes compatible with infection by HPV (Human Papilloma Virus).

[Spontaneous perirenal hematoma secondary to hydronephrosis in pelvic kidney].

Contribution of one case of Spontaneous Perirenal Haematoma (SPH) secondary to hydronephrosis accompanied by lithiasis in pelvic kidney. SPH can result from multiple causes, the most frequent ones being renal tumours, most particularly angiomyolipoma and hypernephroma. Hydronephrosis is an uncommon cause of SPH, and very few cases are described in the literature. The present paper addresses the diagnostic usefulness of the various radiological tests as well as the therapeutical approach to be followed.

[Bladder inverted papilloma. Can it be considered as a neoplasm with low degree of malignancy?].

We report a case of inverted papilloma of the bladder. The clinical features, diagnosis and treatment of this tumor type are discussed. We underscore the histology and histogenesis of the two types: trabecular inverted papilloma and glandular inverted papilloma. A review of the literature has shown that inverted papillomas do not show clinical or histological features of malignancy most of the time. The cases with synchronous or metachronous association with transitional cell carcinoma, recurrence or histologic signs of malignant transformation of a lesion may be considered to have a neoplasm of low grade malignancy and these patients must be followed closely.

[Rupture of the corpora cavernosa. Evaluation of 5 cases].

Five cases of fracture of penis with a mean follow-up of 52 months are described. Four underwent early surgical treatment and one was treated conservatively. This latter case developed penile curvature from fibrosis. The literature is reviewed with special reference to treatment. Like most of the authors, we advocate early surgical management as the treatment of choice for rupture of the corpora cavernosa.

[Early calculogenesis caused by ureolytic infection of the double-J ureteral catheter].

Stone formation in the double J ureteral catheter is an uncommon complication that generally appears in patients that are predisposed to stone formation and persistent infection of the urinary tract. We report on a 29-year-old female patient in whom stone formation was observed in both ends of the catheter, less than 3 months after it had been placed. Urine culture was positive for P. mirabilis. Percutaneous nephrolithotomy was warranted in the treatment of the pyelic stone and for removal of the double J catheter. We underscore the importance of doing urine cultures and radiologic control in this patient group and shortening the time the catheter is left indwelling.

[Comparative study of ureteral displacements].

A study ia mde of 15 patients with a known neoplasia: 2 retroperitoneal tumours, 5 lymphomas, 3 prostate neoplasias, 3 seminomas and 2 uterus neoplasias; with displacement of the ureter and in some cases ectasia of the excretion system. The displacement was always one-sided and most frequently found in the lower third of the ureter. Ectasia was found in two patients; we have never found this in any patients without a known neoplasia. Adenopathies were the most frequent cause of inferior ureter deviation but in only one case did they infiltrate into the wall and they were more constant on the right side. These cases are compared with other patients suffering from different pathologies, some of which are also neoplastic but in which no displacement or ectasia appeared even when the retroperitoneum was affected, and with another group of patients in whom there was found to be ureteral displacement or ectasia without any retro- or intraperitoneal pathology.

[Ectopic ureters].

Ten cases are presented of ectopic openings of the ureter. The authors stress the embryological considerations concerning this congenital malformation as well as the clinical and radiological characteristics and the means of diagnosis applied in our patients.
