Effect of high frequency transcutaneous electrical nerve stimulation on viability of random skin flap in rats.

PURPOSE: To determine the effect of high frequency Transcutaneous Electrical Nerve Stimulation (TENS) on viability of random skin flap in rats. METHODS: The sample of this study was 75 Wistar rats. The skin flap measured 10 x 4 cm and a plastic barrier was interposed between the flap and donor site. After the operative procedure, animals of all groups were maintained anesthetized one more hour with electrodes positioned in the base of the flap and submitted to treatment according of their respective group. This procedure was repeated on the two subsequent days. G1: sham stimulation (control), G2: TENS (f = 80 Hz and I = 5 mA), G3: TENS (f = 80 Hz and I = 10 mA), G4: TENS (f = 80 Hz and I = 15 mA), G5: TENS (f = 80 Hz and I = 20 mA). RESULTS: The average percentage of necrotic area was 43,11, 34,65, 49,44, 23,52, 45,10 in groups 1, 2, 3, 4 and 5 respectively. CONCLUSION: The amplitude of 15 mA presented a lower necrotic area than control group and Transcutaneous Electrical Nerve Stimulation was efficient in increasing the random skin flap viability.

Muscular subunits transplantation for facial reanimation.

PURPOSE: To present an alternative technique for reconstruction of musculocutaneous damages in the face transferring innervated subsegments (subunits) of the latissimus dorsi flap for replacement of various facial mimetic muscles. METHODS: One clinical case of trauma with skin and mimetic muscles damage is described as an example of the technique. The treatment was performed with microsurgical transfer of latissimus dorsi muscle subunits. Each subunit present shape and dimensions of the respective mimetic muscles replaced. The origin, insertions and force vectors for the mimic muscle lost were considered. Each subsegment has its own arterial and venous supply with a motor nerve component for the muscular unit. RESULTS: Pre and one year postoperative photos registration of static and dynamic mimic aspects, as well as digital electromyography digital data of the patients were compared. The transplanted muscular units presented myoelectric activity, fulfilling both the functional and cosmetic aspect. CONCLUSION: This technique seems to be a promising way to deal with the complex musculocutaneous losses of the face as well as facial palsy.

Basal cell carcinoma: an updated epidemiological and therapeutically profile of an urban population.

PURPOSE: To describe the epidemiological profile of basal cell carcinoma patients at a private hospital in São Paulo and to evaluate the treatment adopted. METHODS: A prospective study of 202 patients, on which 253 lesions were diagnosed for histopathological exam as basal cell carcinoma within the period of January 2001 to September 2003, in the Plastic Surgery Residency Program at the Hospital Jaraguá. The susceptibility factor of the host, the environment variables, the characteristics of the lesions and the efficacy of the treatment were examined. The data were statistically evaluated. RESULTS: The incidence of basal cell carcinoma was 126 cases per 100,000 patients in a period of 32 months (36 cases per 100,000 patients/year). The patients were evenly distributed in terms of sex: 48% male and 52% female. The greater incidence was in patients between the ages of 60 and 80 years and the average was 64 years. The survey revealed susceptibility factors such as white race and phototypes I and II in 95.5% of the patients. Exposition to ultraviolet radiation was reported by 77% of the patients and the most frequent location of tumors was on the face (71.2% of the cases). Actinic keratosis and a history of skin cancer were reported in 43.6% and in 25% of the cases, respectively. The adopted treatment was surgery in 99.4% of the cases and only one patient was treated with radiotherapy. Twenty lesions (8%) had incomplete excision. The recurrence rate was 2% (5 cases). There were no cases with metastasis or fatal outcome. CONCLUSIONS: The factors related to the development of basal cell cancer which were significantly present in the population surveyed were: older age, white individuals, phototypes I and II, presence of actinic keratosis, previous history of non-melanoma skin cancer and exposure to ultra-violet rays both in recreational and in occupational form. The surgical treatment employed was effective with a rate of incomplete excision and recurrence similar to those found in the literature.

Vital dye is enough for inguinal sentinel lymph node biopsy in melanoma patients.

PURPOSE: The importance of gamma probe detection (GPD) combined with vital dye for sentinel node (SN) biopsy is well accepted. We evaluated the efficacy of patent blue dye (PBD) in identifying inguinal SN. METHODS: Ninety-four cutaneous melanoma patients with inferior extremity lesions were submitted to SNB according to a established protocol. Patients were randomized in two groups: Blue group, where SN was identified by PPD and Probe group, where SN was identified by GPD. The median age was 44.2 years and median Breslow thickness was 2.1 mm. Preoperative lymphoscintigraphy, lymphatic mapping with PBD and intra-operative GPD was performed on all patients. Histological examination of SN consisted of hematoxylin-eosin and immunohistochemical staining. If micrometastases were present complete lymphadenectomy was performed. The SN was considered as identified by PBD if it was blue and identified by GPD if it demonstrated at least ten times greater radioactivity than background. RESULTS: It was explored 94 inguinal lymphatic basins, 145 SN were excised (70 guided primarily by blue dye and 75 guided primarily by probe). All SN identified by preoperative lymphoscintigraphy were excised. In the Blue group PPD identified all SN and all of them were hot. In the Probe group all SN were identified by probe and were blue. The coincidence of PPD and GPD was 100%. CONCLUSION: Patent blue dye is enough to identify superficial inguinal SN in cutaneous melanoma.

Understanding the finger-assisted malar elevation technique in face lift.

BACKGROUND: The finger-assisted malar elevation technique addressing the midface represents one of the new trends in surgical techniques for facial rejuvenation. Although Aston described it in 1996, until now there has been no detailed publication on this technique and its anatomical relations. This study focused on the anatomical structures and layers involved in it. METHODS: Ten cadavers were dissected from the orbicularis oculi muscle to the base of malar fat pad, which was detached. All visible anatomical structures were described. The finger-assisted malar elevation technique was used in clinical cases and the surgical technique was described. RESULTS: This digital maneuver is performed in a relatively avascular plane, over the muscles and underneath the malar fat pad. The safety of this technique is also reinforced by the absence of important anatomical structures. Two hundred fifty-three patients underwent this procedure, and minor complications were recorded in six patients. CONCLUSIONS: The supra-superficial musculoaponeurotic system plane is a safe and natural plane for the finger-assisted malar elevation technique. It also carries the advantage of allowing greater mobilization of the nasolabial fold, the superficial musculoaponeurotic system, and excess skin, although the major advantage is the facility of repositioning the malar fat pad to its original position, over the zygomatic body prominence.

Transcutaneous electrical nerve stimulation in viability of a random skin flap in nicotine-treated rats.

BACKGROUND: Skin flaps are extensively used in plastic surgery, the main postoperative problems being ischemia and necrosis. Nicotine has acute vasoconstrictive properties, diminishing blood flow and increasing the risk of flap necrosis. Transcutaneous electrical nerve stimulation (TENS) was effective in reducing necrosis in random skin flaps in rats. Therefore, the authors decided to test the efficacy of TENS to reduce necrosis in the presence of nicotine. METHODS: Sixteen adult male Wistar-Escola Paulista de Medicina (EPM) rats were distributed, at random, in 2 groups of 8 animals. The nicotine group (NG) received this drug subcutaneously (1.2 mg/kg/d), for 1 week before flap elevation. The nicotine plus TENS group (NTG) followed the same routine. Immediately after the operative procedure and on the next 2 postoperative days, the treatment group (NTG) was submitted to TENS during an hour under anesthesia. NG animals were sham exposed. After 7 days, the percentages of necrosis were determined. RESULTS: The NG had a mean value of 45% and the NTG, 21% of necrosis, a difference statistically significant (P < 0.001). CONCLUSION: TENS was effective in the reduction of necrosis in this flap model.

[Healing of colonic anastomosis with the use of extract aqueous of Orbignya phalerata (Babassu) in rats].

INTRODUCTION: Healing is a highly complex process with different phases. Because some medicinal plants have antiinflammatory properties, they could influence the wound healing process. There are many species of medicinal plants used for research, among them the Orbignya phalerata (Babassu). PURPOSE: To analyze, comparatively, the tensiometric and histologic alterations in the healing process of colon anastomosis with the use of aqueous extract of the babaçu mesocarp. METHODS: Forty Wistar male rats were used, divided in two groups of 20 animals named control and experimental group. Each group was further divided in two sub-groups of 10 animals and evaluated on the third and seventh post-operative days. Colotomy and one suture line coloraphy was performed in all animals. The control group received an intraperitoneal single dose of isotonic saline solution at the time of the surgery. The animals from the experimental group received babassu aqueous extract dose with 50mg/kg/weight, concentration of 25 mg/ml dose, with the same administration method. The following parameters were evaluated: a) macroscopic appearance of abdominal wall and cavity; b) tensiometric evaluation of anastomosis (bursting pressure); c) histologic characteristics of anastomosis. RESULTS: The macroscopic examination showed the presence of adherences in all animals with statistical significance in the ones from the experimental group at third day when compared to the control group. No difference was observed in the seventh-day group. No fistulae, abscesses and/or hemorrhage were observed. In regard to the tensiometric variation, the bursting pressure for the control and experimental groups, both for the third day (25.4 mmHg - 14.8 mmHg) and at seventh (183 mmHg - 175 mmHg), were similar with no significant difference. Histological analysis for the third day showed significant difference in the experimental group in relation to the controls, in regard the presence ofto mononuclear ones whereas at seventh day there was significant statistical difference in all variables from the experimental group compared to the control one. CONCLUSION: A favorable effect with administration of the aqueous extract of the babaçu mesocarp was seen in histological parameters of the healing process of colon anastomosis however there was no difference in the tensiometric evaluation between the control and experimental group.

[Evaluation of aroeira (Schinus terebinthifolius Raddi) extract on the healing process of gastroraphy in rats].

PURPOSE: To evaluate the healing process of gastric suture in rats using hydroalcoholic aroeira (Schinus terebinthifolius Raddi) extract. METHODS: Forty adult male rats, divided into two groups of 20 animals were operated and named as follows: aroeira group (Ga), and the control group (Gc). Each group was divided into two subgroups (SG) of 10 animals (SGa and SGc) according to the time of provoked death (three and seven days). The same surgical procedure was performed in all animals consisting in incision and simple suture of the stomach (Prolene(R) 6-0). The only difference was on the type of medical treatment. The aroeira group received a single 100 mg/kg of aroeira extract in an intraperitoneal dose and the animals from the control group received the same quantity in milliliters (ml) of the isotonic saline solution. The evaluated parameters were: macroscopic alterations, microscopic healing process and toleration to atmospheric air insufflation. RESULTS: All animals had good healing process of abdominal wall with no clinical evidence of infection, dehiscence, abscesses and peritonitis. Both groups presented adherences to gastric suture line area with surrounding organs, mainly the liver, lower intestines and the abdominal wall. Microscopic analysis showed only chronic inflammation significant difference between the aroeira and control groups on the third day of observation. Resistance tests did not present significant statistical differences in the studied groups. CONCLUSION: The use of aroeira (Schinus terebinthifolius Raddi) hydro-alcoholic extract did not alter the stomach healing process, considered on macroscopic, tensiometric and microscopic assessment.

[Extract of Passiflora edulis in the healing process of gastric sutures in rats: a morphological and tensiometric study].

INTRODUCTION: Many substances of vegetable origin have been used since the beginning of civilization with the purpose of improving the healing process. Among them, dry leaves extract from Passiflora edulis have been shown to have an anti-inflammatory effect in rats. PURPOSE: To analyze the effect of dry leaves extract from Passiflora edulis in the healing of gastric sutures in rats. METHODS: Forty male adult Wistar rats were divided into two groups of 20 rats, called Passiflora group (GP) and Control group (GC) which were divided into two groups of 10 according to moment of death, on day 3 or day 7 after the operation. All animals were submitted to a midline incision and a gastrotomy was performed on the anterior wall of the stomach followed by gastric suture with polypropylene 6.0 using four stitches on a single layer. Rats from GP were given, before closure of the abdominal wall, a solution of Passiflora edulis extract, 250 mg/kg/weight, while rats from the GC were given an isovolumetric isotonic saline solution. Macroscopic evaluation included the adhesion index proposed by Knightly. Bursting pressure was measure by an electronic device. Microscopic analysis was performed including inflammation parameters. RESULTS: All animals presented adequate healing of the abdominal wall with no clinical signs of infections or dehiscence. The adherence index was similar in both groups both on day 3 (p=0.734) and on day 7 (p=1.000). The gastric sutures presented leak with smaller insufflation pressure on the 3rd P.O. day in both groups as compared to the 3rd P.O. day. There was no significant difference of bursting pressure among the subgroups on the 3rd P.O. day (GC3=41.1 +/- 22.1 mmHg versus GP3=59.2 +/- 20.4 mmHg; p=0.074). On the 7th P.O. day, there was an increased mean bursting pressure in both groups, but there was no statistically significant difference between the two groups (p=0.850). Histologic parameters were similar in both groups, on P.O. days 3 and day 7, except for the fibroblastic proliferation, which was greater on the 7th day in GP (p=0.002). CONCLUSION: The intraperitoneal use of Passiflora edulis extract influences favorably the healing of gastric sutures in rats because of the increase in the fibroblastic proliferation on the 7th P.O. day.

[Topic use of aqueous extract of Orbignya phalerata (babassu) in rats: analysis of its healing effect].

INTRODUCTION: The mesocarp of Babassu (Orbignya phalerata) has been used in experimental studies trying to check its pro-inflammatory effect. PURPOSE: To analyse comparatively the histological changes made by the water extract of Babassu in skin surgical wounds. METHODS: Sixty Wistar adult male rats were used. The experimental procedure was a circle skin incision of 2 cm in diameter made with a metal punch. After this procedure done in all animals, they were randomized in two groups of 30. In the control group, the phytotherapeutic agent wasn't used but saline solution. In the experimental group, the water extract of Orbignya phalerata was used in the cut sites. All animals were followed and killed after seven, 14 and 21 days. Comparative histological analysis was made among the groups. RESULTS: In the microscopic view, a significant reepitelization effect on the healing process of the experimental group in relation to the control group in the 7th and 14th days, was recognized. CONCLUSION: The use of the mesocarp of Orbignya phalerata in skin surgical wounds contributed positively in the healing process in rats.

This paper provides elements to facilitate the creation of a research model. It also helps the researcher to identify which is the research model of an article. This article will enable the reader of a scientific paper to have a more accurate sense of criticism of the Methods of a study.
