Factors affecting automated syndromic surveillance.

OBJECTIVE: The increased threat of bioterroristic attacks and epidemic events requires the development of accurate and timely outbreak detection systems for early identification of anomalies in public health data. MATERIAL AND METHODS: We propose an automated outbreak detection system based on syndromic data. This system uses an autoregressive model with seasonal components to monitor, online, the daily counts of chief complaints for respiratory syndromes at the emergency department of two major metropolitan hospitals. We evaluate this system by estimating the false positive rate in real data under the assumption that there were no outbreaks of disease, and the true positive rate in real baseline data in which we injected stochastically simulated outbreaks of different shape and size. We then use directed graphical models to account for the effect of exogenous factors on the detection performance of the system. RESULTS: Our study shows that for a week-long outbreak, our model has an overall 84.8% true detection accuracy across all shapes of outbreaks, while the outbreak size influences the earliness to detection. The false and true positive rates are also associated with the exogenous factors and knowledge about these factors can help to improve the detection accuracy. CONCLUSION: This study suggests that the integration of multiple data sources can significantly improve the detection accuracy of syndromic surveillance systems.

Inhibition of vessel permeability by TNP-470 and its polymer conjugate, caplostatin.

Angiogenesis inhibitors, such as TNP-470 and the nontoxic HPMA copolymer-TNP-470 (caplostatin), are emerging as a class of anticancer drugs. We report that TNP-470 and caplostatin inhibit vascular hyperpermeability of tumor blood vessels as well as that induced in mouse skin by different mediators. Treatment with TNP-470 or angiostatin for 3 days was sufficient to reduce permeability of tumor blood vessels, delayed-type hypersensitivity, and pulmonary edema induced by IL-2. TNP-470 also inhibited VPF/VEGF-induced phosphorylation of VEGFR-2, calcium influx, and RhoA activation in endothelial cells. These results identify an activity of TNP-470, that of inhibiting vessel hyperpermeability. This activity likely contributes to TNP-470's antiangiogenic effect and suggests that caplostatin can be used in the treatment of cancer and inflammation.

Epstein-Barr virus lytic infection is required for efficient production of the angiogenesis factor vascular endothelial growth factor in lymphoblastoid cell lines.

Although Epstein-Barr virus (EBV)-associated malignancies are primarily composed of cells with one of the latent forms of EBV infection, a small subset of tumor cells containing the lytic form of infection is often observed. Whether the rare lytically infected tumor cells contribute to the growth of the latently infected tumor cells is unclear. Here we have investigated whether the lytically infected subset of early-passage lymphoblastoid cell lines (LCLs) could potentially contribute to tumor growth through the production of angiogenesis factors. We demonstrate that supernatants from early-passage LCLs infected with BZLF1-deleted virus (Z-KO LCLs) are highly impaired in promoting endothelial cell tube formation in vitro compared to wild-type (WT) LCL supernatants. Furthermore, expression of the BZLF1 gene product in trans in Z-KO LCLs restored angiogenic capacity. The supernatants of Z-KO LCLs, as well as supernatants from LCLs derived with a BRLF1-deleted virus (R-KO LCLs), contained much less vascular endothelial growth factor (VEGF) in comparison to WT LCLs. BZLF1 gene expression in Z-KO LCLs restored the VEGF level in the supernatant. However, the cellular level of VEGF mRNA was similar in Z-KO, R-KO, and WT LCLs, suggesting that lytic infection may enhance VEGF translation or secretion. Interestingly, a portion of the vasculature in LCL tumors in SCID mice was derived from the human LCLs. These results suggest that lytically infected cells may contribute to the growth of EBV-associated malignancies by enhancing angiogenesis. In addition, as VEGF is a pleiotropic factor with effects other than angiogenesis, lytically induced VEGF secretion may potentially contribute to viral pathogenesis.

Vector propagation of radially polarized Gaussian beams diffracted by an axicon.

On the basis of the vectorial Rayleigh diffraction integrals and stationary-phase method, the analytic expression describing the vectorial field distribution of radially polarized Gaussian beams diffracted by an axicon is derived. The theoretical analysis and simulation calculation show that the radial component of the diffraction field is the propagation-invariant first-order Bessel beam when the radially polarized Gaussian beam illuminates the axicon. However, the longitudinal component possesses no such behavior because of its intrinsic r dependence, and its central intensity is the maximum. The longitudinal component is related to the open angle and index of the axicon, which has to be considered when the open angle and index are large. For a small open angle and index, the longitudinal component can be neglected, and the scalar approximation is valid.

Coagulation and fibrinolysis in human acute lung injury--new therapeutic targets?

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are common, life-threatening causes of acute respiratory failure that arise from a variety of local and systemic insults. The need for new specific therapies has led a number of investigators to examine the role of altered coagulation and fibrinolysis in the pathogenesis of ALI/ARDS. This review summarizes our current understanding of coagulation and fibrinolysis in human ALI/ARDS with an emphasis on pathways that could be potential therapeutic targets including the tissue factor pathway, the protein C pathway and modulation of fibrinolysis via plasminogen activator inhibitor-1. The available data suggest that clinical ALI and ARDS are characterized by profound alterations in both systemic and intra-alveolar coagulation and fibrinolysis. Fibrin deposition in the airspaces and lung microvasculature likely results from both activation of the coagulation cascade and impaired fibrinolysis, triggered by inflammation. Modulation of fibrin deposition in the lung through targeting activation and modulation of coagulation as well as fibrinolysis may be an important therapeutic target in clinical ALI/ARDS that deserves further exploration.

[The fusion construction of HIV-1 Tat gene and efficient expression in E.coli].

AIM: To express high-level the Tat protein in E.coli. METHODS: Full-length HIV-1 Tat gene was amplified artificially by PCR and Tat gene was mutated site-specifically (substitution the codons AAG encoding the lysine at the 28th and the 50th site by the CAG encoding glutamine) in order to eliminate the transcriptional activity of Tat protein. The site-mutated Tat gene was fused with chaperone10 gene, and then was subcloned into vector pET28a. The recombinant plasmid was expressed in E.coli BL21(DE3). The expressed products were identified by Western blot. RESULTS: Full-length HIV-1 Tat gene was amplified successfully by three rounds of PCR. The recombinant plasmid pET28a-chaperone 10-Tat was expressed efficiently in E.coli BL21(DE3). Western blot analysis showed the expressed Tat fusion protein with relative molecular mass (M(r)) 24 000 could bind to anti-His-tag monoclonal antibody. CONCLUSION: Full-length HIV-1 Tat gene was cloned and chaperone 10-Tat fusion protein was expressed efficiently in E.coli BL21(DE3), which will lay the foundation for researching the pathogenic effect of HIV-1 Tat on AIDS.

[Observation on the protective effect of hyperoxia solution on the acute lung injury caused by phosgene poisoning.].

OBJECTIVE: To study the protective effect of hyperoxia solution on acute lung injury caused by phosgene poisoning by observing the changes of PaO2 and malondialdehyde (MDA) contents, superoxide dismutase (SOD) activity in serum and Glutathione (GSH/GSSG) contents in lung tissues. METHODS: The rabbits were divided into normal control group, hyperoxia solution (H0) and balance salt (BS) groups. Group HO and Group BS inhaled phosgene and the former was given intravenously hyperoxia solution (which was replaced by balance salt solution in Group BS). The content of MDA and the activity of SOD in serum were observed at different time points, the amount of GSH and GSSG in lung tissue were also measured. RESULTS: (1) The serum MDA contents increased and PaO2, SOD activity decreased significantly in Group HO and Group BS along with time increasing as compared with control group. The contents of GSH in lung tissue decreased in two groups compared with that in control group, however the contents of GSSG ascended instead. (2) At 3 and 8 h of the experiment, PaO2 of Group HO [(9.91 +/- 0.49), (9.15 +/- 0.46) mm Hg respectively] were significantly higher than those of Group BS [(9.03 +/- 0.76), (8.11 +/- 0.57) mm Hg respectively] (P < 0.01). The contents of MDA of Group HO (3.66 +/- 0.35), (5.31 +/- 0.15) micromol/L respectively] were lower than those of Group BS [(4.32 +/- 0.26), (7.4 +/- 0.33) micromol/L respectively] (P < 0.01). SOD activity in Group HO [(237.37 +/- 29.96), (208.10 +/- 18.80) NU/ml respectively] were higher than those of Group BS [(195.02 +/- 21.44), (144.87 +/- 21.26) NU/ml respectively] (P < 0.05 or P < 0.01). The content of GSSG lung tissue in Group HO (423.67 +/- 38.21) micromol/L were lower than those of Group BS (523.85 +/- 43.14) mol/L (P < 0.01). There were no significant differences in the content of GSH in lung tissues between Group HO and group BS. CONCLUSION: Hyperoxia solution can reduce acute lung injury of rabbits following phosgene poisoning.

[Microvessel density and expressions of survivin and vascular endothelial growth factor in non-small cell lung cancer and their correlations to clinicopathologic features].

BACKGROUND & OBJECTIVE: Survivin, an anti-apoptosis gene, expresses in most tumors, and takes part in tumor angiogenesis. This study was to investigate microvessel density (MVD) and expressions of Survivin and vascular endothelial growth factor (VEGF) in non-small cell lung cancer (NSCLC), and explore their correlations to clinicopathologic features of NSCLC. METHODS: MVD and expressions of Survivin and VEGF in 96 specimens of NSCLC tissues, 31 specimens of tumor adjacent tissues, and 20 specimens of benign lesions were detected by SP immunohistochemistry; their interrelations and correlations to clinicopathologic features of NSCLC were analyzed. RESULTS: Positive rate of Survivin was significantly higher in NSCLC than in adjacent tissues and benign lesions (69.8% vs. 16.1% and 0, P<0.05); its expression was related with differentiation and TNM stage of NSCLC. Positive rate of VEGF was significantly higher in NSCLC than in adjacent tissues and benign lesions (72.9% vs. 45.2% and 25.0%, P<0.05); its expression was related with lymph node metastasis and TNM stage of NSCLC. MVD was significantly higher in NSCLC than in adjacent tissues and benign lesions (24.44+/-7.79 vs. 19.37+/-5.26 and 11.83+/-6.25, P<0.05), and was related with lymph node metastasis and TNM stage of NSCLC. Survivin expression was positively correlated with VEGF expression and MVD. CONCLUSION: Survivin is overexpressed in NSCLC, which relates with differentiation and TNM stage of NSCLC and takes part in angiogenesis.

[Study on mitochondrial DNA genetic polymorphism of some yak breeds in China].

This study determined yak's complete sequence of mitochondrial DNA control region (D-loop) of 35 individuals in 5 yak breeds at the first time. The result showed that the length of D-loop in yak was 891 -895 bp. Content of nucleotide T, C, A, G were 28.5%, 25.3%, 32.5% and 13.7% respectively. There were 55 polymorphic sites (6.16% in total analyzed sites). The transition, transversion and insertion/deletion were found in this region.24 haplotypes was defined in this study, in which haplotype H4 and H6 were major haplotypes of Chinese yak. The distribution of all the haplotypes among the breeds was disequilibrium. The average diversity of haplotypes were 0.9697 +/-0.0180, indicating the abundance of haplotypes of yak's D-loop. The average nucleotide difference and the nucleotide diversity in Chinese yak breeds were 10.936% and 1.231% respectively. Nucleotide divergence and Kimura 2-parameter distance between yak breeds were 0.760% -2.155%, and 0.000-0.029 respectively. The results indicated that the genetic diversity of Chinese yak was very abundant. Analysis of molecular variance and network construction results indicated that there was significant divergence among Chinese yak breeds. The network construction indicated that Chinese yak had been divided into 2 types and had probably 2 maternal origins or 2 domesticated places.

[PPARdelta + 294T/C gene polymorphism related to plasma lipid, obesity and left ventricular hypertrophy in subjects with metabolic syndrome].

OBJECTIVE: To investigate the relationship between PPARdelta + 294T/C gene polymorphism and lipid profile, obesity and left ventricular hypertrophy (LVH) in patients with metabolic syndrome (MS). METHODS: This study was conducted in 300 patients with MS and 174 patients with essential hypertension (EH) and 143 patients with type 2 diabetes mellitus (T2DM). MS was diagnosed according to 1999 WHO criteria. Fasting insulin (FINS), fasting blood glucose (FBG), plasma lipids levels were measured, LVH was examined by Doppler echocardiography. The PPARdelta + 294T/C gene polymorphism were analyzed using polymerase chain reaction and subsequently digested by BSLI restriction endonuclease. RESULTS: The frequencies of the PPARdelta + 294T/C genotypes were not different among three groups. Compared with T2DM and EH, MS patients had significantly higher body mass index (BMI), plasma total cholesterol, TG and LDL-C levels (P < 0.01 or P < 0.05). LVM, LVMI and incidence rate of LVH were significantly higher in MS and EH patients than that in T2DM (P < 0.01). MS patients with CC genotype had significantly higher total cholesterol and LDL-C levels than those with TT and TC genotypes (total cholesterol in CC genotype: 6.13 +/- 1.86 mmol/L vs in TC genotype: 5.14 +/- 1.10 mmol/L, P < 0.05, and CC genotype: 6.13 +/- 1.86 mmol/L vs TT genotype: 4.99 +/- 1.42 mmol/L, P < 0.01; LDL-C in CC genotype: 3.82 +/- 1.52 mmol/L vs in TC genotype: 3.14 +/- 0.88 mmol/L, P < 0.05, and in CC genotype: 3.82 +/- 1.52 mmol/L vs in TT genotype: 2.90 +/- 0.87 mmol/L, P < 0.01). BMI and LVMI in MS patients with C allele carriers (CC + TC) were significantly higher than that of TT genotype (LVMI in CC + TC: 46 +/- 10 g/m(2.7) vs in TT: 44 +/- 10 g/m(2.7); BMI in CC + TC: 26 +/- 3 kg/m(2) vs in TT: 25 +/- 3 kg/m(2), P < 0.05). CONCLUSIONS: It is indicated that PPARdelta + 294T/C gene polymorphism in subjects with MS may be involved in the occurrence of obesity and dyslipidemia. MS patients with C allele had a predominant LVH than subjects with TT genotype.

[Effects of angiotensin receptor blocker and angiotensin-coverting enzyme inhibitor on vascular endothelial function in type 2 diabetic patients with atherosclerosis: a comparative study].

OBJECTIVE: To compare the changes of endothelial function in type 2 diabetic patients with atherosclerosis (AS) after treatment with angiotensin-coverting enzyme inhibitor (ACEI) and angiotensin II receptor blocker (ARB). METHODS: Totally 145 type 2 diabetic patients with AS were included in this study. According to completely randomized open unbalanced design, the patients were assigned into ACEI group (n=99) with ramipril treatment (starting dose of 2.5 mg/d and gradually reaching 5 mg/d), ARB group (n=25) with irbesartan treatment (with gradual increase of doses till reaching the maximum daily dose of 150 mg), and control group (n=21) without the above medication. After a treatment lasting for 3 months, the brachial artery endothelial function was evaluated with two-dimensional ultrasonography. RESULTS AND CONCLUSIONS: After therapy, endothelium-dependent vasodilation (EDV) of the brachial artery increased from (3.55+/-2.80)% to (5.62+/-4.18)% (P=0.001), and endothelium-independent vasodilation (EIV) increased from (12.62+/-7.48)% to (12.80+/-7.04)% (P=0.835) in ACEI group. EDV increased from (4.42+/-2.95) % to (6.28+/-3.26)% (P=0.039), while EIV decreased from (10.69+/-5.85)% to (9.54+/-4.78)% (P=0.229) in ARB group. No statistical changes in the indices were found after therapy in the control group (P>0.05). Comparison of endothelial function in the 3 groups showed no statistical significance in the changes of EDV between ACEI and ARB groups (P=0.853). No differences were found in the change of EIV in the 3 groups (P=0.644). The therapy with 150 mg irbesartan and 5 mg ramipril for 3 months produces similar effects on the endothelial function in these patients.

[Agrobacterium-mediated transformation of rice mature embryos and regeneration of transgenic plants with Metr gene].

The mature embryos of a Japonica rice, Guanglingxiangnuo, were used for the study on optimization of Agrobacterium-mediated transformation. Several factors affecting the transformation were investigated and a suitable transformation system was set up. For this transformation system, the HRM medium, based on the MS medium, was suitable for inducing callus from rice mature embryos. The suitable time span of initial culture in this medium was 7-8 days before co-culturing with Agrobacterium and suitable medium for selection was CC medium. Using this transformation system, the Metr gene was introduced into Guanglingxiangnuo, and many transgenic plants were obtained. Most of these transgenic rice plants were confirmed by PCR technique and basta resistance, indicating the T-DNA had been integrated into the genome of transgenic rice plants.

[A simple and repeatable model of subarachnoid hemorrhage in rats].

AIM: To build a simple and repeatable animal model of subarachnoid hemorrhage (SAH). METHODS: SAH was produced by passing a nylon thread up through the right internal carotid artery and piercing a hole in the right anterior cerebral artery. At 12 h and 24 h after SAH operation, the rats were evaluated with rotarod test and the behavior scale (5-point scale). RESULTS: The rats were trained through rotarod test and then randomly divided into three groups, including vehicle group treated with vehicle after SAH, nimodipine treated group (i.p. 0.25 mg x kg(-1), 5 min, 6 h, 12 h after SAH) and sham group. At the point of the perforation there was usually a capping clot. There was always blood in the basal cisterns with some spread over the hemisphere. After 12 h and 24 h of SAH operation, the time of rotarod test of rats decreased significantly and the rats had serious neurological deficit. Nimodipine could alleviate the neurological deficit after 24 h of SAH. CONCLUSION: To present a simple and reliable model of SAH in the rats, which allows evaluating novel compounds and new drugs for treatment of SAH.

Hepatitis E: an overview and recent advances in vaccine research.

Hepatitis E virus (HEV) is an unclassified, small, non-enveloped RNA virus, as a causative agent of acute hepatitis E that is transmitted principally via the fecal-oral route. The virus can cause large water-born epidemics of the disease and sporadic cases as well. Hepatitis E occurs predominantly in developing countries, usually affecting young adults, with a high fatality rate up to 15-20% in pregnant women. However, no effective treatment currently exists for hepatitis E, and the only cure is prevention. But so far there are no commercial vaccines for hepatitis E available in the world. Although at least four major genotypes of HEV have been identified to date, only one serotype of HEV is recognized. So there is a possibility to produce a broadly protective vaccine. Several studies for the development of an effective vaccine against hepatitis E are in progress and the best candidate at present for a hepatitis E vaccine is a recombinant HEV capsid antigen expressed in insect cells from a baculovirus vector. In this article, the recent advances of hepatitis E and the development of vaccine research for HEV including recombinant protein vaccine, DNA vaccine and the recombinant hepatitis E virus like particles (rHEV VLPs) are briefly reviewed.
