[Expression and significance of vascular endothelial growth factor in rat cornea after cautery with alkali].

OBJECTIVE: To investigate the expression of vascular endothelial growth factor (VEGF) in rat cornea after cautery with alkali. METHODS: In Sprague-Dawley rats, inflammatory corneal neovascularization was induced by cautery with alkali. VEGF was detected in corneal samples at different times by Western-blot, and immunohistochemistry method was used to investigate the distribution of VEGF at rat cornea after cautery with alkali. RESULTS: The normal rat corneas did not express VEGF. The expression of VEGF increased with time after corneal cautery. The rat corneas were infiltrated by massive inflammatory cells that, especially adjacent to the cautery lesion, showed staining for VEGF. CONCLUSION: VEGF production by inflammatory cells participates in inflammatory angiogenesis in rat corneas. VEGF has the effects on the induction and supporting of cautery-induced angiogenesis in rat cornea.

[Operative treatment of acetebular fractures].

OBJECTIVE: To discuss the diagnosis and operative technology of acetabular fracture. METHODS: 41 cases of displaced acetabular fractures were enrolled. CT and 3D-CT were performed before operation to understand the classification of fractures and to determine operative regimen. All cases were fixed by reconstruction plates and screws through the best approach. RESULTS: 39 cases were followed up for 1 - 4 years, with an average of 1.5 years. Curative outcomes were assessed by Matta's criteria. Clinically, 16 (53.3%) 30 fresh fractures were excellent, 11 (36.7%) good, 3 (10%) fair. The rate of excellence and goodness was 90%. In 9 cases of old fractures, 2 were excellent, 3 good, 2 fair and 2 poor. The rate of excellence and goodness was 55.6%. Complications consisted of sciatic never injury, necrosis of the femoral head, and heterotopic ossification. CONCLUSIONS: The time of operation, degree of injury, and quality of reduction of fracture are important factors affecting the postoperative function of the hip. The key to enhance the outcome is to verdict the type of fracture, select the proper approach before operation, and precise reduction and appropriate internal fixation during operation.

Psychological consequences derived during process of human hand allograft.

OBJECTIVE: To study the psychology and the treatment during the process of hand allograft. METHODS: The patients were interviewed to evaluate their states of mind and their abilities to manage stress during the selection of patients. The psychology of the two patients were trained before the operation and managed accordingly afterwards. RESULTS: One of 12 candidates was found to be unsuitable for the transplantation because of psychiatric problems. One week postoperatively, the two patients were anxious, lacked patience and were afraid of seeing the long-awaited grafted hand. After 1 week, the patients began to accept the new hand, with full acceptance of the hand 1 month later. With the recovery of hand sensation 4 to 5 months after the operation, the patients regarded the hand as their own. CONCLUSION: Psychological problems exist during preoperative selection of patients and postoperative rehabilitation, requiring psychologists in the hand transplantation team.
