Hepatic trisegmentectomy for 29 patients with huge liver neoplasms.

OBJECTIVE: To evaluate retrospectively the feasibility and effect of hepatic trisegmentectomy in therapy of huge neoplasms of the liver. METHODS: From July 1993 to October 1999, 29 patients with huge hepatic neoplasms underwent hepatic trisegmentectomy. Of these, 23 patients suffered from primary liver cancer, 1 hepatic infiltration of gallbladder cancer, 1 metastasis of colon cancer, 1 hepatic angiosarcoma, 1 hepatic neurofibroma, and 2 huge liver cysts. Twenty-six patients were subjected to right trisegmentectomy and the rest 3 left trisegmentectomy. All trisegmentectomies were performed under normothermic interruption of the porta hepatis at single time and these interruptions lasted 15 to 40 minutes. RESULTS: The relatively good effect was seen in our series. The 1-, 3-, 5-year survival rates for primary liver cancer patients were 63.6%, 36.4% and 27.3%, respectively. The survival period for the patients with hepatic infiltration of gallbladder cancer and liver metastasis of colon cancer was 6 months. Those with hepatic angiosarcoma, hepatic neurofibroma and huge liver cysts have been surviving 35, 26, 25 and 40 months, respectively. Major complications were noted in 5 patients, and one (3.4%, 1/29) died. CONCLUSION: Hepatic trisegmentectomy is safe and effective in treatment of huge hepatic neoplasms if its indications and operative techniques are properly mastered.

[Comparison of the toxic effect of methamidophos and acephate on acetylcholinesterase].

OBJECTIVE: To explore the inhibitory effects of highly toxic organophosphorus compound and its substitute (methamidophos and acephate) on acetylcholinesterase (AChE) and their toxic mechanisms. METHODS: Ellman method was used to measure AChE activity in vitro and vivo. RESULTS: Acephate and methamidophos could directly inhibit AChE activities in human erythrocyte membrane and rat brain synatosomal membrane in dose- and time-dependent manners in vitro, and this effect was irreversible. The IC50 of acephate and methamidophos affecting human erythrocyte membrane and rat synatosomal membrane were approximately 10(-4) mol/L and 10(-5) mol/L respectively and the Ki were 10(2) mol.L-1.min-1 and 10(3) mol.L-1.min-1 respectively. In vivo, after rats being administered with them for 5 d, the inhibitory rate of AChE activities in blood were increased to 68.24% and 54.80% respectively. When rats being administrated with acephate, there was 31.68% of inhibition on the brain stem, but no significant inhibition in other brain region was noticed, while methamidophos had a strong inhibitory effect on the activity of AChE in all brain regions, especially the cerebellum and brain-stem(71.51% and 61.85% respectively). CONCLUSION: Acephate and methamidophos could directly inhibit the AChE activities in vitro, but the inhibition degree was different. In vivo, both could also inhibit AChE activities in blood. The difference in inhibition on brain regions may be one of the reason of various toxic effect of them.

[Effects of deltamethrin on intracellular free Ca2+ concentration and apoptosis in rat neural cells].

OBJECTIVE: To study the effects of deltamethrin on intracellular free Ca2+ concentration and apoptosis in rat neural cells. METHODS: Wister rats were randomly divided into 4 groups(3 treated groups and 1 control). Intracellular free Ca2+ concentration in rat neural cells was measured by using the fluorescent Ca2+ indicator Fura-2/AM. Apopotic rate of neural cells was measured by using FACS420 Flow Cytometer. RESULTS: Intracellular free Ca2+ concentration at 5 h after deltamethrin exposure [hippocampus: (389.94 +/- 43.64) nmol/L, cerebral cortex: (449.33 +/- 23.23) nmol/L], at 24 h[hippocampus: (340.47 +/- 32.36) nmol/L, cerebral cortex: (311.62 +/- 25.48) nmol/L] and at 48 h[hippocampus: (287.13 +/- 24.29) nmol/L, cerebral cortex: (346.55 +/- 36.87) nmol/L] were all higher than those of the control group[hippocampus: (203.24 +/- 18.53) nmol/L, cerebral cortex: (226.85 +/- 14.81) nmol/L, P < 0.01]; Apoptotic rate in neural cells 24 h and 48 h later [hippocampus: (8.45 +/- 1.02)%, (9.44 +/- 1.14)%, cerebral cortex: (7.90 +/- 0.49)%, (8.01 +/- 0.87)%] were also higher than those of the control group[hippocampus: (2.97 +/- 0.36)%, cerebral cortex: (3.50 +/- 0.48)%, P < 0.01)] and increased with time prolonged. CONCLUSION: Exposure to high dose of deltamethrin would interfere with intracellular free Ca2+ concentration and apoptotic rate in rat neural cells, suggesting that there may be certain relation between them.

Determination of the Adsorbed Phase Volume and Its Application in Isotherm Modeling for the Adsorption of Supercritical Nitrogen on Activated Carbon.

Experimental excess adsorption isotherms of nitrogen on activated carbon were measured in the range of 103-298 K and pressures up to 10 MPa. A method for determining the volume of the adsorbed phase from the experimental data for the supercritical temperatures was proposed. Such a volume with a new isotherm equation was used in modeling experimental excess isotherms, and a satisfactory fit was achieved. Copyright 2001 Academic Press.

Evidence-based obstetrics in four hospitals in China: An observational study to explore clinical practice, women's preferences and provider's views.

BACKGROUND: Evidence-based obstetric care is widely promoted in developing countries, but the success of implementation is not known. Using selected childbirth care procedures in four hospitals in Shanghai, we compared practice against evidence-based information, and explored user and provider views about each procedure. METHODS: Observational study. Using the Cochrane Library, we identified six procedures that should be avoided as routine and two that should be encouraged. Procedure rate determined by exit interviews with women, verified using hospital notes. Views of women and providers explored with in depth interviews. The study sites were three hospitals in Shanghai and one in neighbouring province of Jiangsu. 150 women at each centre for procedure rate, and 48 in-depth interviews with women and providers. RESULTS: Vaginal births were 50% (303/599) of the total. Of the six practices where evidence suggests they should be avoided as routine, three were performed with rates above 70%: pubic shaving (3 hospitals), rectal examination (3 hospitals), and episiotomy (3 hospitals). Most women delivered lying down, pain relief was rarely given, and only in the urban district hospital did women routinely have a companion. Most women wanted support or companionship during labour and to be given pain relief; but current practice is insufficient to meet women's needs. CONCLUSION: Obstetric practice is not following best available evidence in the hospitals studied. There is a need to adjust hospital policy to support the use of interventions proven to be of benefit to women during childbirth, and develop approaches that ensure clinical practice changes.

[The Effects of Cyclosporin A on Leukemia Cells In Vitro]

To explore the mechanism of CSA in the reversal of drug-resistance of leukemia cells, K562 and K562/ADM cells were used for in vitro study. Drug sensitivity of cells was evaluated by MTT assay and cell survival by trypan blue exclusion. Apoptosis was detected by terminal-deoxynucleotidyl transferase mediated d-UTP nick end labeling (TUNEL) and agarose gel electrophoresis. The results showed that 1.0 mg/L CSA alone have no effect on the survival of K562 and K562/ADM cell lines, but it could enhance the chemotherapy sensitivity of the 2 cell lines, and accelerate the apoptosis induced by ADM. These effects were stronger in K562/ADM cell line, when the concentration of CSA > 1.0 mg/L, CSA itself could kill leukemia cells and induce the apoptosis of the 2 cell lines. It is concluded that CSA can reverse leukemia cells' drug-resistance by enhance apoptosis of the leukemic cells, CSA itself can induce leukemia cells' apoptosis and kill leukemia cell too.
