[The genetic polymorphism of melanoma-associated antigen 1 in Chinese normal donors and hepatoma patients].

OBJECTIVES: To investigate the expression of melanoma-associated antigen 1 (MAGE-1) in Chinese hepatocellular carcinoma (HCC) patients and to determine the existence and distribution of single nucleotide polymorphisms (SNP) of MAGE-1 gene. METHODS: Total RNA was extracted from cancer tissues and adjacent tissues from 19 HCC patients and the expression of MAGE-1 mRNA was examined by using RT-PCR. The PCR products were sequenced to analysis the gene variation. Genomic DNA was extracted from cancer tissues, adjacent tissues and peripheral blood cells of 19 HCC patients, as well as from peripheral blood cells of 23 healthy donors. The PCR product of MAGE-1 DNA was sequenced to determine the existence and distribution of SNPs of MAGE-1 gene. RESULTS: 9 of 19 (47.4%) tumor tissues and none of adjacent tissue from HCC patients expressed MAGE-1 mRNA. There were three kinds of gene variations of cDNA in 9 MAGE-1 mRNA positive patients. One type was named type I including 1 patient, which sequence is as same as that of the GenBank M77481. The other was named TGA type including 5 patients, which involved three nucleotide changes (C159T, A272G and G393A) and result in two amino acid changes (T32A and R72Q). Another one was named GTG type including 3 patients, which involved three nucleotide changes (A272G, C991T, A1125G) and result in only one amino acid changes (T32A). According to the analysis of genomic DNA, above three types were not specific mutations of tumor tissue, but were SNPs. These SNPs types were distributed in HCC patients and normal donors with the frequencies of 26.3% (5/19) and 60.9% (14/23) for type I, 57.9% (11/19) and 47.8% (11/23) for TGA type, and 21.1% (4/19) and 21.7% (5/23) for GTG type, respectively. The sequences of two new SNPs had been deposited in GenBank with the accession numbers of AF463515 and AY148486. In male population, the distributions of SNPs were not correlated to HCC suffering or MAGE-1 expression. Several new HLA-restricted epitopes were probably resulted from SNPs existing. The three-dimensional models of MAGE-1 proteins of type I and TGA type was established by using computer software. CONCLUSION: The expression rate of MAGE-1 gene in Chinese HCC patients is high. Three SNP types of MAGE-1 gene exist in Chinese population. The three-dimensional models of MAGE-1 proteins were obtained by computer processing. These results will be helpful to developing MAGE-1 peptide-vaccine for HCC immunotherapy.

[Evaluation of health-related quality of life in chronic hepatitis C women patients].

OBJECTIVE: To evaluate the health-related quality of life (HRQOL) in chronic hepatitis C (CHC) female patients, and to identify factors associated with impairment in HRQOL. METHODS: We enrolled 41 women cases with hepatitis C for 10 to 15 years and 51 health women in Guan area of Hebei province, China. HRQOL was assessed by the SF-36 by an interview. RESULTS: CHC female patients had significantly lower SF-36 scors in all subscales when compared to those of the non-HCV infected female population. CONCLUSION: CHC female patients have a reduced HRQOL. CHC alone is associated with significant impairment in HRQOL.

[Hepatitis c virus genotype research by ABC programs of 5'-NCR restriction endonuclease digestion].

OBJECTIVE: In order to fully understand hepatitis c virus (HCV) genotype 3b, 1a, 2b and 6a infection in China, We built HCV 5'-noncoding region (5'-NCR) of different genotypes and subtypes. METHODS: The classification HCV into variable genotypes (subtypes) was carried on by programs A, B and C A. Using a combination of three restriction endonuclease BHH' (BsrB I, Hae II, Hinf I) digestions at the same time. The distinct genotypes were classified into 5 groups: genotype 1 (1a, 1b), 6a, 2 (2a, 2b), genotype 3 (3a, 3b), genotype4 (4a). B. With regard to genotype 1, we could distinguish subtype 1a from 1b using BstU I digestion. C. Using restriction endonuclease Hae III, genotype 2a, 2b, 3b, 4a, 6a are differentiated respectively. RESULTS: (1) HCV genotype 1a, 1b, 2a, 2b, 3a, 3b, 4a, 6a are fully discriminated by comparison with the genotypes regular samples. (2) Of the 93 patients, HCV genotype distribution in China was 66.67% for 1b, 18.28% for 2a, 3.23% for 1b/2b, 3b, 2b respectively. 2.15% for 2a/2b, 1b/2a respectively. 1.08% for 1a. CONCLUSION: This research indicated that adoption of HCV 5'-NCR A B C restriction endonuclease digestions techniques, might be sensitive and efficient to detect HCV and discriminate HCV genotype (subtypes) 1a to 6a.

[Clinical outcomes of women with transfusion-associated hepatitis C after 10-15 years follow-up].

BACKGROUND: To investigate natural history of hepatitis C virus (HCV) infection and related factors in China. METHODS: Totally 41 women infected with HCV 10-15 years ago were evaluated in this study. Their clinical manifestations were recorded and sera tested for alamine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyltransferase (gamma-GT), alkaline phosphatase (ALP), total bilirubin (TBIL), direct bilirubin (DBIL), total protein (TP), albumin (ALB), and alpha-fetoprotein (alpha-FP), as well as virus markers. The B-type ultrasound examination was performed on the liver, gall bladder, spleen and pancreas. For virus markers, HCV RNA was detected with fluorogenic-quantitative polymerase-chain-reaction, HBsAg and anti-HCV antibody, with enzyme-linked immunosorbent assay (ELISA). RESULTS: Ten to 15 years after infection, the infection was cleared spontaneously in 8 cases (19.51%); 73%(30/41) cases had signs at present, 32% (13/41) cases had mild ALT and/or AST elevation. On ultrasound examination, 83%(34/41) had mild degree 17%(7/41) moderate degree changes of the viral hepatitis. CONCLUSION: Women with HCV infection for 13 +/- 1 (10-15) years after transfusion had slight hepatic inflammation. A part of them had cleared the virus spontaneously.

[Clinical outcomes of 283 patients of transfusion-related hepatitis C in the northern areas of China].

OBJECTIVE: To investigate the natural history and clinical outcomes in a cohort of transfusion-related hepatitis C in northern areas of China. Methods The authors studied 283 patients (137 males, 146 females, mean age 45.79+/-9.92 age) who became infected with HCV while donating plasma 12.25 years ago. These cases were subjected to ultrasonography and liver biochemical tests and serologic anti-HCV assays. Statistical analyses were performed using the SPSS software. RESULTS: Ultrasonographic findings suggestive of liver cirrhosis were 8.3% of cases. No decompensated cirrhosis or HCC was detected. The value of ALT was higher in severe chronic hepatitis (mean 62.07+/-50.87 IU/L) and cirrhosis (mean 115.50+/-108.41 IU/L) patients than in the other groups (mean 32.30+/-29.10 IU/L). The abnormal rate of ALT was 53.3% in severe group, 100% in cirrhosis group. CONCLUSION: The natural history of transfusion-related hepatitis C in the areas seemed to be relatively mild when compared with previous data. Our cases showed relatively low rate of positive findings in ultrasonography. None of this cohort had the decompensated cirrhosis or HCC. The factor of sex but not the age at time of infection was found being related to the outcomes. In the absence of liver biopsy, ultrasonography was a suitable and sensitive method for the diagnosis of the progressive hepatitis and cirrhosis.

[Adenovirus mediated expression of interleukin 12 regulating hepatitis C virus E2 gene immunization-induced immune response].

OBJECTIVE: To observe the regulating effect of hepatitis C virus (HCV) envelop (E) 2 gene immunization-induced immune responses by adenovirus mediated interleukin 12 (IL-12). METHODS: HCV E2 protein was expressed and purified from NIH 3T3 and then used as an antigen to detect antibodies against HCV E2. With 51Cr release, SP2/0 expressing HCV E2 was used as target cell to detect specific cytotoxic T lymphocytes (CTL) response; adenovirus recombined IL-12 was propagated by 293 cell. HCV E2 recombinant and adenovirus recombined IL-12 were injected into the quadriceps femoris muscles and abdominal cavities of 6-8 weeks old BALB/C mice. Sera were collected at 2, 3, and 4 weeks and detected for antibodies for E2. Spleen cells isolated at 4 weeks were analyzed for specific CTL response. RESULTS: It was found that expression of IL-12 at an undetectable level did enhance HCV E2 gene immunization-induced CTL activity and there was no effect on its hormonal immune response. CONCLUSION: Using adenovirus to express interleukin 12 was helpful for regulation of HCV E2 gene immunization-induced immune response. Combined HCV E2 and IL-12 can render a strong anti-HCV CTL activity and may be of use in the development of HCV gene vaccine in the future.

[Isolation, culture and multipotent differentiation of mesenchymal stem cells from human fetal livers].

OBJECTIVE: To isolate and culture mesenchymal stem cells (MSCs) from human fetal livers and describe their biological characteristics. METHODS: MSCs were acquired using an optimized method. Cell cycles and the immunophenotype of the cells were analyzed by flow cytometry. The osteogenic and adipogenic differentiations were induced and identified by specific stainings, and hepatic differentiation by morphology and RT-PCR. RESULTS: The target cells derived from human fetal livers adhered to the plate with fibroblast-like morphology, whose surface markers were CD90, CD44, CD147 positive, and CD34, CD45, HLA-DR negtive. In the differentiation study, these cells could be induced to differentiate into osteogenic, adipogenic and hepatocyte-like cells. CONCLUSION: Multipotent MSCs can be isolated and cultured from human fetal livers.

[Construction of recombinant replication competent plasmids containing HBV genome with mutations of precore and basic core promoter].

OBJECTIVE: To construct a series of recombinant replication competent plasmids of hepatitis B virus (HBV) full-length genome with varying mutations of precore and basic core promoter. METHODS: The plasmid containing the entire HBV (1.2 copies) genome was used to construct objective plasmids. The objective competent vectors were constructed by molecular cloning and PCR-based site-directed mutagenesis in vitro and confirmed with sequence analysis. After introducing the plasmids into hepatocellular carcinoma cell line (Huh7) by calcium phosphate transfection, the culture supernatant was collected to detect HBsAg and HBV DNA to analyze viral replication and expression. RESULTS: Ten competent vectors with varying precore and basic core promoter mutations were constructed. After transfecting into Huh7, the vectors replicated and secreted viral particles. CONCLUSIONS: Ten full-length competent HBV recombinants were obtained, which were harbouring varying mutations within precore and basic core promoter.

[Therapeutic effect of levamisole plus HBV vaccine and dipyridamole on patients chronically infected by HBV with precore mutation].

OBJECTIVE: To evaluate the incidence of precore mutation in HBeAg negative HBV infected patients and the therapeutic effect of the immune therapy (levamisole + HBV vaccine + dipyridamole) on patients chronically infected by HBV with precore mutation. METHODS: The precore region of HBV from the HBeAg (-) chronic hepatitis patients was sequenced and the patients suffered from HBV with precore mutation were treated with immune therapy. RESULTS: The precore mutation rate was 10/12. The therapeutic effect of the immune therapy on the precore mutation patients (5/7) was better than that on the HBsAg(+), HBeAg(+) patients (2/11), P less than 0.05. CONCLUSION: The precore mutation rate was quite high in the HBsAg(+), HBeAg(-) patients we studied. The immune-therapy has some therapeutic effects on the patients with precore mutation. But the number of cases was too small, further study is needed.

[Cationic liposomes enhance the inhibitory effects of antisense oligodeoxynucleotide on hepatitis B virus].

OBJECTIVE: To study the interaction between 3beta-[N-(N', N'-dimethylaminoethane)-carbamoyl] cholesterol (DC-chol), a cationic lipid, and antisense oligodeoxynucleotide (asODN), and the cytotoxicity to HepG(2) 2.2.15 cells and the inhibitory effects of asODN cationic liposomes on hepatitis B virus. METHODS: Extract efficiency was used to estimate the interaction between DC-chol and asODN under different conditions; MTT was applied to examine the infection of DC-chol concentration on the cytotoxicity of treated cells; By ELISA technology, we examined the contents of HBsAg, and HBeAg in the culture medium, and evaluated the accelerate efficiency of neutral liposome and cationic liposomes to the inhibitory effects of asODN on hepatitis B virus, and the infection of charge ratio on the inhibitory effects of asODN cationic liposomes. RESULTS: When the charge ratio >0.6, DC-chol could efficiently extract the asODN from the aqueous phase; The alkaline condition and the presence of Na(+), Cl(-) ions prevented the efficient interaction between DC-chol and the asODN; DC-chol cationic liposomes had some toxicity to HepG(2) 2.2.15 cells, at low concentration (0.84-26.94 mg x L(-1)), and the toxicity of DC/chol was linear with the concentration of DC/chol; Within 1.25-5.00 micromol x L(-1), the inhibition efficacy of asODN was correlated to its concentration; liposomes could enhance the inhibition efficacy of asODN on hepatitis B virus, and cationic liposomes could enhance much more (from 60% to 95%). CONCLUSION: Surface-modified DC-chol cationic liposomes can efficiently enhance the anti-hepatitis B virus efficiency of asODN in vitro.

[Clinical and virological course of dual infection by hepatitis B and C viruses in China].

OBJECTIVE: To investigate the clinical and virological course of dual infection by hepatitis B virus (HBV) and hepatitis C virus (HCV) in China. METHODS: We enrolled 40 patients with chronic HBV and HCV dual infection (Group BC), 16 patients with chronic single HBV infection (Group B) and 31 patients with chronic single HCV infection (Group C). They infected HBV and/or HCV during 1982 to 1989. The sera of all the 87 patients were collected in 1994 and 2002 respectively. We detected biochemical and virologic markers and quantitative serum HCV DNA and HCV RNA levels of all the patients. The B-type ultrasound detecting was performed in some patients. RESULTS: In Group BC, 67.5% of the patients have cleared HBsAg, and 92.5% of the patients have cleared HBeAg. The clearance rate of HBV DNA was 87.5%. There was no significant difference of HBV clearance between Group BC and Group B. In Group BC, the clearance rate of HBV-RNA was 87.5%, significantly higher than those in Group C. Less than 40% of the patients in all groups have elevated liver enzyme values. The highest value of alanine aminotransferase (ALT) was 218 u/L (normal range for ALT 0 - 40 u/L). In most patients the ultrasonogram presentations were changed mildly. CONCLUSION: The clinical presentation of patients with HBV/HCV coinfection seemed mild and occult. High clearance of HBV, easy to clear HBV in male patient was the characteristics of infection by HBV in adult time in China. HBV could inhibit HCV replication, but no evidence could be found in our data that HCV could suppress HBV replication.

[Influence of soybean-derived sterylglucoside and amphipathic polyethylene glycol on the cellular delivery and anisotropy of different cationic liposomes].

OBJECTIVE: To investigate the influences of different lipids on cellular delivery and fluidity of liposome membrane of cationic liposome. METHODS: The 3 beta [N-(N',N'-dimethylaminoethane) carbamoyl] cholesterol (DC-chol), a cationic lipid was synthesized, which is approved by FDA. Using fluorescein sodium (SF) as a model material, dipalmitoylphosphatidylcholine (DPPC), dioleoylphosphatidylethanolamine (DOPE) and cholesterol (ch) as help lipids, the DPPC/ch/DC-chol cationic liposomes, DOPE/ch/DC-chol cationic liposome and soybean-derived sterylglucoside (SG) or polyethylene glycol-distearoylphosphatidylethanolamine (PEG-DSPE) modified cationic liposomes were prepared, respectively. The neutral liposome DPPC/ch was also prepared. HepG2 2.2.15 was selected as an in vitro cell culture assay model to investigate the cellular delivery of different liposomes. The influences of different lipids on the fluidity of liposome membranes were investigated with the technique of fluorescence polarization. RESULTS: It is showed that the DC-chol has significant influences on the incorporation efficiencies (from 0.64% to 74.84%), cellular delivery (from 1.83% to 32.29%) and the fluidity of liposome membrane; SG can significantly increase the incorporation efficiency (from 52.60% to 63.37%), cellular delivery (from 20.18% to 47.48%) and fluidity of liposome membrane; PEG-DSPE can enhance the cellular delivery of cationic liposome (from 20.18% to 28.17%). CONCLUSION: DC-chol and SG can significantly increase the cellular delivery and fluidity of liposome membrane, PEG-DSPE can enhance the cellular delivery of cationic liposome.
