[Influence of fibrin glue on biliointestinal anastomotic scar formation].

Conventional suture technique in the reconstruction of the bile duct often result in anastomotic stenosis and eventual obstructive jaundice and biliary cirrhosis. To overcome the aforementioned drawbacks, fibrin glue (FG) was used instead in the biliointestinal anastomosis where indwelling stent was inserted to complete the experimental model. The experiment consisted of pathological study and ECVD evaluation of anastomotic specimens in order to follow up the whole process of anastomotic wound healing. The experimental studies dis-closed that FG promotes wound healing, limits scar formation, and accelerates scar softening and maturnation. FG plus 6-month indwelling stent can guarantee scar maturation. It is an effective method in the prevention of bile duct stricture.

[Retrospective analysis of 830 extrahepatic biliary carcinoma].

We retrospectively analysed clinical data of 830 patients with extrahepatic biliary carcinomas treated in our hospital from 1956 to 1995. Among them, 601 had gallbladder carcinomas, and 229 cholangiocarcinomas. The results showed that extrahepatic bile duct carcinoma accounted for 6.77% of bile duct diseases. Gallbladder carcinomas and cholangiocarcinomas accounted for 72.4% and 27.6% of extrahepatic biliary carcinomas, while the mean age was 56.55 and 56.44 years old, respectively. The sex ratio (male:female) was 1:2. 54 and 1. 46:1. The operation rate and resection rate were 65.1% and 29.4% in gallbladder carcinoma while 66.4% and 20.4% in cholangiocarcinoma. The incidence rate and resection rate of extrahepatic biliary carcinomas were elevated.

[The effects of Tripterygium wilfordii polyglycosidium on neuromuscular junctions of adult toad].

The effects of tripterygium wilfordii polyglycosidium (TWP) on the transmission at the neuromuscular junctions (NMJ) of sciatic nerve-sartorius muscle preparations of 20 adult toads (Bufo bufo gargarizans) in vitro were studied by means of intracellular microelectrode recording of endplate potentials (EPP). The results revealed that TWP could cause changes of EPP amplitude so as to influence the transmission at NMJ, and the effects apparently depended on the concentration of the drug. A proper concentration of TWP could produce a significant increase in EPP amplitude and therefore facilitate the transmission at the NMJ.

[Interleukin-6 and tumor necrosis factor-alpha in middle ear effusions].

Using enzyme-linked immunosorbant assay (ELISA), we measured levels of interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-alpha) in middle ear effusions (MEE) from patients with secretory otitis media (SOM) undergoing auripuncture to study the role of cytokines in the pathogenesis of the disease. Significant levels of IL-6 (> 62.5 ng/L) were found in 19 (86.36%) of 22 MEEs and those of TNF-alpha (> 37.5 ng/L) in 19 (70.37%) of 27 MEEs. The mean (+/- s) levels of IL-6 and TNF-alpha were 507.68 +/- 813.11 ng/L and 186.86 +/- 166.93 ng/L, respectively. It revealed that the shorter the course, the higher the concentration of IL-6 in MEE: the longer the course, the higher the concentration of TNF-alpha in MEE. These findings suggested that during the early stages of SOM, IL-6 might participate in the defensive reaction of organism, while TNF-alpha might be closely related to the persistence of SOM, resulting in an excessive inflammatory reaction with a potential for pathological changes. It is concluded that the immunological mechanisms probably play a significant role in the pathogenesis of SOM.

[Chemical constituents of fructus Amomi].

Seven compounds have been isolated from Fructus Amomi. They were identified as bornyl acetate, camphor, borneol, beta-sitosterol, vanillic acid, stearic acid and palmitic acid. The vanillic acid was found in this medicine for the first time. Fifty-seven components were identified from the volatile oil of Fructus Amomi on the basis of GC-MS analysis. Eight components in the volatile oil over 1% in content are bornyl acetate, camphor, borneol, limonene, camphene, myrecene, carene-3 and alpha-terpeneol.

[Analysis of high frequency X-ray images in 43 cases of breast cancer].

OBJECTIVE: To analyse a series of breast cancer images in high frequency X-ray for more accurate diagnosis of breast cancer. METHODS: Forty three high frequency X-ray images of breast cancer surgically confirmed were comprehensively analyzed. RESULTS: Of the 43 breast cancer cases, 42 were primary breast cancers and one secondary. Tumor mass was present in 38 cases, with burrs in 26. Malignant calcification was found in 13 cases, and structural derangement in 21 cases. There were 30 cases with abnormal blood vessels, and 7 cases with thick skins. Ductal and lymph drainage features were seen in 9 cases. There were 5 cases showing ox horn sign. The high frequency X-ray diagnosis of breast cancer conformed with pathologic diagnosis in 95.6% of the cases. CONCLUSION: The advantages of high frequency X-ray are its extremely high clarity and good contrast. Pictures can be taken form various axises which help detect occult and tiny cancer.

[Recombinant AAV-LMP-induced LMP specific cytotoxic response to autologous lymphoblastoid cell lines tranformed by Epstein-Barr virus].

Epstein-Barr virus is believed to be controlled in normal host by virus specific cytotoxic T lymphocytes (CTL). Although unable to eliminate EBV from the body, CTL seems to be essential in control of latently infected cells. Infusion of autologous EBV specific CTL, which can be produced in laboratory by separating lymphocytes from patients and stimulating them with EBV antigen, will provide an effective method of preventing and treating EBV-related diseases. We inserted the LMP gene of EB virus into an AAV vector pACP and packed it in Ad2 infected 293 cells by co-transfecting with plasmid Ad8, which produced the recombinant virus rAAV-LMP. The recombinant virus was used to infect stimulating cells and LMP antigen was expressed on the surface of these cells. Then the stimulating cells were irradiated and co-cultured with T lymphocytes. The EBV specific CTLs were obtained. The target cells were autologous LCLs from EBV-transformed B lymphocytes. The CTL activity was assayed by BLT activity method. The result indicated that all the four CTL strains could recognize and kill their target cells. This study has laid the technical basis for us to prevent and treat nasopharyngeal carcinoma in China with molecular biological methods.

[Studies on peripheral blood stem cells mobilization by short course and high-dose G-CSF].

OBJECTIVE: To explore the effect of short-course, high dose granulocyte-colony stimulating factor (G-CSF) on peripheral blood stem cell (PBSC) mobolization. METHODS: G-CSF 5microg/kg was injected subcutaneously twice daily for 3 consecutive days. On the first day and the fourth day the peripheral blood and bone marrow were collected for CFU-GM, BFU-E, CFU-mix assays and CD34/CD38 detection. A single apheresis of over 8000ml blood processing was performed with CS-3000 blood cell separator. The mononuclear cells were prepared for hematopoietic colony assays and CD34/CD38 detection before and after freezing. RESULTS: After adminitration of G-CSF, the white blood cell count increased significantly. The CFU-GM, BFU-E, CFU-mix yields and CD34+ CD38+ cell and CD34+ CD38- cells also increased significantly (P<0.05). The median of mononuclear cells was 1.56 x 10(8)/kg. After one week freezing, the recovery of mononuclear cells, CFU-GM, BFU-E yields and CD34+ CD38+ cells were about 80% (P>0.05) and CFU-mix yields was 60%; The CD34+ CD38- cells were decreased compared with that of pre-freezing (P<0.05). The median of the reinfused mononuclear cells was 1.27 x 10(8)/kg,CFU-GM 6. 7 x 10(4)/kg, BFU-E 1.6 x 10(4)/kg, CFU-mix 0.32 x 10(4)/kg, CD34- CD38+ cells 1.6 x 10(6)/kg and CD34+ CD38- cells 0.23 x 10(6)/kg. CONCLUSION: Short course and high-dose G-CSF can effectively mobilize the peripheral stem cells. The mononuclear cells, hematopoietic colonies and CD34/CD38 cells were satisfied for the peripheral stem cells rescue.

Cathepsin G, acid phosphatase, and alpha 1-proteinase inhibitor messenger RNA levels in keratoconus corneas.

PURPOSE: Keratoconus is characterized by thinning and scarring of the central region of the cornea. The authors have shown, in corneas obtained from patients with keratoconus, that lysosomal enzyme activities are elevated, whereas levels of protease inhibitors such as alpha 1-proteinase inhibitor (alpha 1-PI) are reduced. This study was undertaken to examine further the gene expression of cathepsin G, acid phosphatase, and alpha 1-PI in keratoconus corneas. METHODS: Corneal buttons were collected from patients with keratoconus, normal subjects, and patients with other corneal diseases. In situ hybridization was performed on paraffin sections using a tritium-labeled probe for cathepsin G or alpha 1-PI. Competitive polymerase chain reaction (PCR) was used to determine the messenger RNA (mRNA) levels for lysosomal acid phosphatase and alpha 1-PI in epithelial and stromal cells of keratoconus corneas. RESULTS: Silver grains, indicative of positive in situ hybridization products, were observed in all three cell types of normal corneas for both DNA probes. Compared with normal and other diseased controls, the labeling was enhanced for cathepsin G but was diminished for alpha 1-PI in the epithelium of keratoconus corneas. Competitive PCR showed that the mRNA level for acid phosphatase was higher and that the mRNA level for alpha 1-PI was lower in keratoconus corneas. CONCLUSIONS: These results indicate that the mRNA level for degradative enzymes in increased and that for alpha 1-PI it is reduced in keratoconus corneas. This study provides the first evidence that the altered expression of multiple enzymes and inhibitors in keratoconus occurs at the gene level. Furthermore, it implicates a possible role of coordinated transcriptional regulation of gene expressions in keratoconus.

alpha-Adrenergic activity and cardiovascular effects of besipirdine HCl (HP 749) and metabolite P7480 in vitro and in the conscious rat and dog.

Besipirdine displays potent adrenergic activity in a variety of pharmacological and behavioral tests. Based on this property, we evaluated the effects of besipirdine and its N-despropyl metabolite N-despropyl-besipirdine (P7480) on cardiovascular function in rats and dogs. Besipirdine and P7480 bind alpha-2 adrenoceptors (K(I): 380 and 10 nM, respectively) and facilitate the stimulated release of [3H]norepinephrine from rat cortical slices due to presynaptic autoreceptor blockade. In rat aorta rings and the pithed rat, P7480, but not besipirdine, also behaved as a postsynaptic alpha-1 adrenoceptor agonist. In conscious rats, besipirdine (2-10 mg/kg, p.o.) and P7480 (3-10 mg/kg, p.o.) produced dose-related increases in mean arterial pressure. Inhibition of hepatic cytochrome P-450 enzyme activity blocked the pressor effect of besipirdine, but not of P7480; therefore, P7480 mediated besipirdine's pressor effect. The bradycardia after either agent was unaffected. In conscious dogs, besipirdine (0.1-2 mg/kg, p.o.) also produced dose-related hypertension and bradycardia. The hypertension, but not the bradycardia, were sensitive to prazosin (3 mg/kg, p.o.), but not hexamethonium (10 mg/kg, p.o.). Muscarinic and beta-adrenergic receptor blockade studies in anesthetized dogs demonstrated the bradycardia to be due to withdrawal of cardiac sympathetic tone. These findings suggest that besipirdine's peripheral hypertensive effect is primarily mediated by the pressor metabolite P7480, although facilitated norepinephrine release may contribute. Besipirdine's bradycardic action appears to be centrally mediated, because both compounds lacked direct negative chronotropic activity on spontaneously beating guinea pig atria in vitro.

Catalysis of the cysteine conjugation and protein binding of acetaminophen by microsomes from a human lymphoblast line transfected with the cDNAs of various forms of human cytochrome P450.

We have previously found that for acetaminophen kinetic differences exist between the hepatic microsomal catalyzed protein binding and cysteine conjugation. We have also observed that the protein binding of acetaminophen is only to intralumenal proteins. Together these data suggested that two pools of the reactive metabolite, N-acetyl-p-benzoquinone imine (NABQI), are formed during the oxidative metabolism of acetaminophen: one on the cytosolic surface and the other within the lumen of the microsomes. This would indicate that some of forms of cytochrome P450 (CYP) catalyzing NABQI formation have their active site on the cytosolic surface and others on the lumenal surface. We have examined this question by comparing the rates of cysteine conjugation and protein binding of acetaminophen by microsomes from lymphoblasts transfected with the cDNAs for human CYPs. We found that CYP2D6 catalyzed only cysteine conjugation; CYP1A2 and 3A4 catalyzed only protein binding; CYP2E1 catalyzed both; and CYP1A1, CYP2A6 and CYP2B6 catalyzed neither. These data suggest that CYP2D6 has its active site only on the cytosolic surface; CYP1A2 and CYP3A4 only on the lumenal surface; and CYP2E1 has catalytic sites on both the lumenal and cytosolic surfaces of the membrane. In mouse studies we have found that ethanol administration increased acetaminophen protein binding by 265% but cysteine conjugation by only 61%. CYP2E1 and CYP2B increased, whereas CYP3A decreased and the others did not change. These data suggest that in control mice CYP2E1 catalyzes the bulk of protein binding, whereas CYP2D catalyzes slightly more cysteine conjugation than does CYP2E1.

Familial occurrence of hemangioblastoma of central nervous system.

OBJECTIVE: To study the familial occurrence of hemangioblastoma of the central nervous system and to search for an effective way to diagnose and manage the disease. METHODS: From 1974 to 1993, six patients with hemangioblastoma with family history underwent surgical treatment at our hospital. Computerized tomography (CT) or magnetic resonance imaging (MRI) technique was used for the diagnoses before operation. RESULTS: Each patient had more than two relatives with hemangioblastoma. Multiple lesions were found in two of the six familial hemangioblastoma cases. All the patients recovered well after operation. So far, no victims have been discovered in the third generation. CONCLUSIONS: The high occurrence rate of familial hemangioblastoma cases suggests an involvement of hereditary factors. We need a long follow-up study on the descendants of familial hemangioblastoma patients. MRI is of diagnostic value in determining the location and nature of hemangioblastoma in both brain and spinal cord. A thorough examination is necessary for hemangioblastoma patients, and microsurgery is important in removing the tumor.

Internal atrial defibrillation: lessons learnt from experimental studies.

Following the introduction of a transvenous biatrial electrode configuration and a biphasic waveform for internal atrial defibrillation in patients in 1992, it was realized that the standard principles of efficient defibrillation derived from decades of ventricular defibrillation research would not provide painless atrial defibrillation in conscious patients. Over the last five years extensive experimental studies have addressed the risk of ventricular proarrhythmia from synchronized atrial shocks with reassuring results and the influence of the preceding R-R interval on the safety of atrial shocks has been established. Experimental atrial defibrillation research is now aimed at developing waveforms which are less painful and at exploring hybrid therapies including percutaneous right atrial compartmentalization by catheter ablation prior to atrial defibrillation and attempts at multisite pace-entrainment prior to and immediately following the delivery of perithreshold shocks.

Catheter ablation for atrial fibrillation.

Catheter ablation for atrial fibrillation is based upon the critical mass of fibrillation hypothesis and aims to compartmentalize the atria by the creation of linear lesions, thereby reducing the amount of contiguous myocardium available for the propagation of multiple activation wavefronts. Early attempts at creating right atrial linear lesions with conventional catheter tip technology to treat patients with chronic and paroxysmal atrial fibrillation yielded disappointing results. Although more efficacious, the creation of extensive left atrial lesions has been associated with a high rate of thromboembolic stroke despite the administration of heparin and use of temperature feedback to control radiofrequency energy. Approaches to reduce the risk of stroke include: the creation of more continuous and effective right atrial lesions with linear array catheter technology and thereby reduce the requirement for left atrial ablation; the assessment of adjuvant pharmacological agents to inhibit platelet aggregation; guidance of radiofrequency energy delivery by intracardiac echo; assessment of transvenous cryotherapy as an alternative to radiofrequency energy in order to reduce endocardial disruption; and development of minimally invasive surgical approaches to left atrial epicardial ablation.

[Role of cytosolic calcium release on gastrin induced contraction of gastric muscle cells of rat].

The present study was conducted to investigate the contractile effect of pentagastrin (G5) on isolated smooth muscle cells of rat stomach and role of cytosolic-free calcium ([Ca2+]i) release during contraction of gastric muscle cells induceed by G5. The results were as follows: (1) G5 induces contraction of smooth muscle cells isolated from gastric body, anturm and pylorus, in a dose-dependent manner from 4 x 10(-8) mol/L to 16 x 10(-8) mol/L, with the maximal effectiveness in antrum, (2) The action of G5 can be blocked by proglumide and anti-gastrin serum, but not by atropine. (3) The effect of acetylcholine and G5 are addtive when administrated simultaneously (4) Inhibitor of intracellular Ca2+ release TMB-8 could also suppress the G5 effect. (5) The effect of G5 could be seen accompaning significant increase of intracellular Ca2+ concentration. The above results suggested that the G5 induced contraction of gastric smooth muscle cells of rat is mediated via a distinct, specific type of receptors and cytosolic calcium release.

[Use of fibrin glue in the prevention of secondary anastomotic stenosis from repair and reconstruction of the injury of the bile duct].

The secondary anastomotic stenosis is often occured from the repair and reconstructive operation of the injured bile duct. It is difficult to treat and the outcome is serious. In order to prevent this complication, the fibrin glue instead of traditional suturing technique combined with inner support was used. Fifty-four hybrid dogs were divided into 3 groups. Group A received Roux-en-y choledochojejunostomy with fibrin glue; group B received Roux-en-y choledochojejunostomy, with a fibrin glue combined support left permanently in the bile duct and group C received Roux-en-y choledocholejejunostomy with fibrin glue combined a support left temporarily in the bile duct. The amount of collagen in the scar was measured at 3/4, 3, 6, 9, 12 months respectively after operation. The results showed: 1. the mature period of scar was shortened from 12 months to 9 months when fibrin glue instead of suture was used in choledochojejunostomy; 2. the mature period of scar was further shortened from 9 months to 6 months when fibrin glue combined with inner support instead of fibrin glue was used in choledochojejunostomy. The conclusions were as follows: 1. fibrin glue could facilitate the healing of wound by inhibiting the formation of scar and accelerrate the maturation of scar; 2. when the inner support was used with fibrin glue in the operation, the mature period of scar could be further shortened; 3. the mechanism of action of the fibrin glue included minimizing the injury, avoiding foreign-body reaction, modifying organization of hematoma, preventing formation of biliary fistular and enhancing intergration and cross-linkage of collagen.

History and current status of percutaneous arthroscopic disc surgery.

STUDY DESIGN: The relevant literature and the author's experience with the management of lumbar disc herniation is reviewed. OBJECTIVES: To describe the history of percutaneous arthroscopic discectomy, and to discuss the indications, the procedure's scientific validity, and its outcome. SUMMARY OF BACKGROUND DATA: In contrast to nuclear-debulking procedures, arthroscopic microdiscectomy is target-oriented and capable of retrieving the compressive elements in a carefully selected patient population. The insult to myoligamentous stabilizing structures and intracanalicular tissues is minimal. Extraction of herniated fragments not only can be demonstrated under arthroscopic visualization but also via postoperative imaging studies. METHODS: Search of the literature and the author's experience with arthroscopic disc surgery. RESULTS: Arthroscopic microdiscectomy has a learning curve. In properly selected patients, a successful outcome similar to macro- or microdiscectomy may be achieved. CONCLUSIONS: Proper patient selection is paramount to a successful outcome of arthroscopic disc surgery. Sequestered migrated herniations and large central herniations at L5-S1 in individuals with elevated iliac crests will require open surgery.
