Adhesion molecules expression in noncrescentic acute post-streptococcal glomerulonephritis.

Clinicomorphological features of 11 cases of non-crescentic acute post-streptococcal glomerulonephritis (APSGN) were reviewed. Intraglomerular and interstitial leukocytes and their possible correlation with the adhesion molecules intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), and endothelial-leukocyte adhesion molecule-1 (ELAM-1/E-selectin) were investigated by an immunohistochemical method. Intraglomerular leukocytes were primarily granulocytes (11.4 +/- 10 cells/glomerular cross-section) and monocytes-macrophages (13.4 +/- 19.4 cells/glomerular cross-section). The granulocytes outnumbered monocytes-macrophages in 7 of 11 specimens. The number of intraglomerular leukocytes correlated with proteinuria at the time of renal biopsy. Intraglomerular ICAM-1 staining was strongly positive in all biopsies, especially when intraglomerular monocytes-macrophages prevailed. Expression of intraglomerular VCAM-1 and E-selectin in diseased kidneys did not differ from that in normal kidneys. Interstitial leukocytes were primarily monocytes-macrophages (158.9 +/- 96.8 cells/mm2) and T lymphocytes (102.2 +/- 63.9 cells/mm2). The number of interstitial leukocytes, especially monocytes-macrophages, correlated with serum creatinine level at the time of biopsy. Interstitial ICAM-1 staining was strongly positive on tubules, peritubular capillaries, and small vessels. The tubular positivity for ICAM-1 correlated with the number of interstitial monocytes-macrophages. Interstitial VCAM-1 and E-selectin were expressed as in normal kidney tissues. The data from this study demonstrate that APSGN is characterized by the presence of both intraglomerular and interstitial leukocyte infiltration, correlating respectively with proteinuria and serum creatinine at the time of renal biopsy. Among the adhesion molecules studied, ICAM-1 seems the most involved in leukocyte recruitment, especially in that of monocytes-macrophages.

A seroepidemiologic study of Chlamydia pneumoniae, Chlamydia trachomatis and Chlamydia psittaci in different populations on the mainland of China.

To evaluate and compare the prevalence of antibodies to 3 chlamydial species in various populations on the mainland of China, the MIF test was used to detect both serum IgG and IgM antibodies to these species in the normal population (n = 711), in poultry workers (n = 51), in female prostitutes (n = 106), in patients attending the sexually transmitted diseases clinic (n = 98), and in adult patients with pneumonia and bronchitis (n = 108). In the normal population, IgG antibodies against C. pneumoniae, C. trachomatis and C. psittaci were present in 61.5%, 9.3%, and 3.5%, respectively, and increasing with age. Prevalence and geometric mean titers (GMT) of IgG antibodies to C. trachomatis in prostitutes (54.7%) and female (55%), but not male (15.4%) sexually transmitted disease patients were significantly higher than in the normal adult female population. The prevalence indicating both previous and recent C. psittaci infections in poultry workers was no higher than in the others. The prevalence of IgG antibodies and GMT to C. pneumoniae in patients with pneumonia and bronchitis was not statistically higher than in the normal adult population. Evidence of cross-reactivities in the MIF test between different species was found in both prevalence and GMT. Serum IgM antibodies were usually absent in patients who met the serological criteria of recent C. pneumoniae infection, indicating that most of them could be considered as reinfections. On the basis of our survey, acute antibodies to C. pneumoniae by the MIF test, if they are detected, may be valuable in the diagnosis of recent C. pneumoniae infections.

Hispanic intermarriage in New York City: new evidence from 1991.

"This study [uses] 1991 marriage records from New York City [to examine] trends in marital assimilation among Puerto Ricans and the non-Puerto Rican Hispanic population. The prevalence of intermarriage varies among the six Hispanic national-origin groups. Changes in intermarriage patterns since 1975 are documented. Results show very high rates of intermarriage with non-Hispanics among Cubans, Mexicans, Central Americans, and South Americans. Considerable intermarriage among Hispanics of different national origins is characteristic of all Hispanics. Finally, Puerto Ricans and Dominicans have distinct patterns of intermarriage...."

[Supra-aortic arch esophagogastrostomy through esophageal bed in treating patients with middle and lower esophageal carcinoma].

A new surgical procedure of esophagogastrostomy was carried out in 162 cases with middle or lower thoracic esophageal carcinoma in our department from January, 1992 to December, 1994. Esophagogastrostomy was performed at supra-aortic arch site by pulling the stomach along the esophageal bed instead of putting it before aortic arch in the thoracic cavity. The procedure may keep the normal physiological function of the organs in the chest. It may help fully distend the left lung so as to reduce impairement of pulmonary function because the thoracic stomach was kept within the mediastinum. No resection mortality or anastomotic leakage occurred in the series. Since the esophagus and the stomach were anastomosed in a linear fashion without angulation, the evenly distributed intracavitary pressure may help prevent anastomotic leakage from occurrence. It is also easy to be dilated if anastomotic stricture occurs. In addition, the new procedure may be helpful in promoting gastric emptying, preventing gastric reflux, depressing and stopping the blood oozing in the mediastinum and air leakage from the ruptured lungs, as well as in distinguishing pleural effusion from gastric retention. Compared with other procedures, it is more difficult to perform the anastomosis. Once anstomotic leakage occurs, drainage is not easy.

The combination of melphalan, cyclophosphamide and cytosine arabinoside as a conditioning regimen for autologous bone marrow transplantation for acute leukemia.

OBJECTIVE: To explore more effective and less toxic conditioning regimen without total body irradiation for autologous bone marrow transplantation (ABMT) for acute leukemia (AL). MATERIAL AND METHODS: Twenty patients with AL were treated with ABMT, including 13 cases of acute myelocytic leukemia (AML) and 7 cases of acute lymphocytic leukemia (ALL). A median of 1.06 (range, 0.69-1.75) x 10(8) nucleated BM cells/kg was harvested and stored in normal salt solution containing heparin at 4 degrees C. The conditioning regimen (MAC) consisted of high-dose melphalan (M, 140-160 mg/m2), cyclophosphamide (CY, 120 mg/kg) and cytosine arabinoside (Ara-C, 2.0 g/m2). These 3 drugs were administered within 25 hours and the unpurged autologous marrow infusion began after another 24-hour interval. RESULTS: MAC regimen could result in myeloblastic efficacy in a week. All marrow cells were reinfused within 56 hours after the harvest so that hemopoietic reconstitutions could occur in all the patients. The median time to reach a neutrophil count of > 1.0 x 10(9)/L and a platelet count of > 50 x 10(9)/L was 20 and 26 days respectively. With a median observation period of 25 months, the median duration on continuous complete remission in our patients was 22 months, and the longest reached 56 months. The median survival was 33 months, and the longest was over 6 years. The event-free survival at 2 years had reached 72%. In seven patients with leukemic relapse, six (86%) relapsed within 8 months after ABMT. The relapse rate and mortality in AML patients were significantly lower than those in ALL patients. In 7 patients with M3, relapse had not yet been observed. The nonhematologic toxic effects of MAC conditioning regimen occurred mainly in the gastrointestinal tract. CONCLUSIONS: The preliminary results indicated that the ABMT using MAC conditioning regimen had some advantages in stronger antileukemic efficacy, less extrahematologic toxicity and earlier recovery of platelet and could greatly prolong the duration of remission and survival in some patients with AL.

Transduction of human interleukin-9 receptor gene into human cord blood erythroid progenitors increases the number of erythropoietin-dependent erythroid colonies.

We constructed a retroviral vector, pLhIL-9RSN, containing CDNA encoding the human interleukin-9 receptor (IL-9R) along with a neomycin phosphotransferase gene (Neo). In order to study the biological effects of the IL-9R, high titer (1-5 x 10(5) CFU/ml) viral supernatant, generated from the packaging cell lines, ecotropic GPE86 and amphotropic PA317, was used to transduce the IL-9R gene into sorted populations of CD34++ CD33-cells from human cord blood which are highly enriched for erythroid progenitor cells (BFU-E). Colony formation by BFU-E transduced with the IL-9R gene and grown without selection in G418 and in the presence of erythropoietin (Epo) and interleukin (IL)-9 was significantly increased up to three-fold and the size of the erythroid colonies was significantly increased 50-100% compared to colony formation by mock virus transduced cells. Moreover, colony formation by IL-9R-transduced cells was more sensitive to stimulation with lower doses of IL-9 and Epo. Individual colonies formed with or without selection in G418 were evaluated. Proviral integration and mRNA expression were respectively assessed by polymerase chain reaction (PCR) and reverse transcriptase (RT) PCR analysis and were apparent in 93% and 84% of the G418-resistant colonies and 52% and 48% of the colonies grown in the absence of G418. Our study demonstrates that a functional human IL-9R gene can be efficiently transduced into human cord blood hematopoietic progenitors using retroviral vectors with increased cytokine-dependent erythroid colony formation.

[The influence of pharmacological manipulation on postsurgical labial scar contraction and craniofacial growth in rabbits].

This study was designed to test the influence of pharmacological manipulation on scar contraction and craniofacial growth in rabbits after repairing surgically created lip defects by wide undermining of soft tissue. Two pharmacological agents, i.e. papaverine hydrochloride and hyaluronidase, were injected subcutaneously and submucosally in the wound. The preliminary results of this study indicated that the treatment with either papaverine or hyaluronidase for four weeks following lip repair with soft tissue undermining significantly reduced lip pressure and decreased tension of labial wound. Analysis of the data from the lateral head cephalography revealed that the rabbits treated with such pharmacological agents exhibited significantly more anteroposterior growth than the rabbits not treated with such agents.

[Correlative study on the storage time and quality of rhizoma Dryopteris crassirhizomae].

The contents of dryocrassin in Rhizoma Dryopteris Crassirhizomae were determined by TLC-scanning in a period of two years of storage. Phloroglucinol content was calculated by area normalization method. The results show that in contrast with the fresh crude drugs of Rhizoma Dryopteris Crassirhizomae, the contents of dryocrassin in the crude drugs of one year and two years of storage have reduced by 22% and 27% respectively, and phloroglucinol contents have reduced by 2% and 8% respectively.

[Clinical research of the bushen tiaojing decotion in promoting ovulation].

A comparative study on effect of promoting ovulation between Bushen Tiaojing Decoction (BSTJD) and clomifene was conducted. Results showed that the ovulation rate of BSTJD group was 66.67%, as compared with that of clomifene group (61.54%), the difference was insignificant (P > 0.05), However, there was a significant difference (P < 0.05) between the two groups in effective rate of menoxenia treatment (82.14% versus 66.67%). No toxicity or side-effect appeared. These results suggested the BSTJD could definitely promote the ovulation, and has a good effect in treating menoxenia and improving the symptoms. The mechanism of BSTJD might be regulating the function of hypothalamus-pituitary-ovary system to promote ovulation.

[Early efficacy of recombinant human erythropoietin in treatment of anemia].

A Before-after study based on self-comparison was conducted to assess the early efficacy of recombinant human erythropoietin (r-HuEPO) in 51 patients with uremic anemia who were subjected to dialyses. A significant elevation of levels of Hb and Hct was observed (P < 0.001, P < 0.005) after r-HuEPO therapy. And hemorheologic tests showed whole blood viscosity was obviously increased (P < 0.005), but no changes in plasmic viscosity and platelet adhesion rate were found (P > 0.05). No increase of blood pressure of the patients was noticed in the early period after treatment. The results revealed a satisfactory early efficacy with few sideeffects of the r-HuEPO treatment in uremic anemia.

[Experimental studies of segmental bone defects treated by skeletal external fixation].

Bony tissue of an average of 13.6% of the original length (OL) was resected from the tibia shaft in 26 mature goats. The fracture ends were compressively fixed with skeletal external fixation (SEF). Simultaneously, upper metaphysiotomy was performed to lengthen the tibia with a lengthening rate of 1.0 mm/d and a total lengthening 20.14% of OL. The restoration of blood circulation and the hemodynamic changes during the repair process of the compressively fixed bone ends and the metaphysiotomy site were studied with angiography, microvascular cast, Chinese ink permeated transparent thick slide, and radioactive microsphere measurement of regional blood flow (RBF). It was found that there were abundant vascular networks in the lengthened zone. The blood vessels not only could keep up with the same lengthening rate of growing, but also were gradually converted into the Haversian system in the newly formed cortex of the lengthened area. In the 2nd week after the cessation of bone lengthening, the RBF of the lengthened zone reached the maximum (54.56 +/- 8.53 ml/mm/100 g). It was also found that in the 7th postoperative week, the vessels of the cortex of the osteotomy shortening ends could grow across the space between them and anastomose with each other. The RBF at this time was 31.82 +/- 2.85 ml/mm/100 g. In the 12th week after operation, the RBF was still 2 times higher than that of the normal cortex (3.02 +/- 0.13 ml/mm/100 g). The primary healing could be observed under the wellreduction and stable compressive external fixation. These findings provide a theoretical basis for the clinical application of SEF for the treatment of tibial shaft defects or nonunion associated with limb shortening.

[Consolidation therapy with high-dose cytarabine and daunorubicin for prolonged disease free survival in acute myelocytic leukemia].

In order to study the effect of consolidation therapy with high-dose cytarabine (Ara-C) and daunorubicin (DNR) for prolonged disease free survival (DFS) in acute myelocytic leukemia (AML), 51 patients with AML in remission were treated with this method. Ara-C at a dosage of 1.3g/m2 administered in 2-hour intravenous infusion every 12 hours for 10 doses. After the treatment of Ara-C, DNR was given at a daily dose of 45 mg/m2 in rapid intravenous infusion for two days. The median duration of complete remission (CR) was 32 (8.2-110) months. The duration of DFS was more than 3 years in 21 patients (41.18%) and more than 2 years in 35 (68.63%). In a control group, however, the median duration of CR was 10 months and the rates of patients with DFS more than 3 and 2 years were 12.5% and 21.8% respectively. It is shown that high-dose consolidation chemotherapy is an effective method to prolong DFS in patients with AML. Toxicities of high-dose Ara-C and DNR can be tolerated by patients with AML.

[Effects of saponins from stems and leaves of Panax quinquefolium L. on the contraction of rabbit aortic strips].

It has been found out that the saponins from stems and leaves of Panax quinquefolium (PQS) can non-competitively antagonize the dose-response curves of norepinephrine (NE), potassium chloride and calcium chloride on the isolated aortic strips of rabbits, and obviously inhibit intracellular and extracellular Ca2(+)-dependent contraction of the aortic strips induced by NE. These effects of PQS are similar to verapamil.

[Studies on sapogenins of Aesculus wilsonii Rehd].

Three sapogenins were isolated from the hydrolysate of the saponin mixture extracted from the seeds of Aesculus wilsonii. They were identified as 21-angeloyl-protoaescigenin., aescigenin and protoaescigenin by physicochemical propertise and spectroscopic analysis.

[Nuclear matrix-lamina-intermediate filament system in PtK 2 cells].

Selective extraction, whole mount cell preparation and DGD embeddment-free section were involved in visualizing the nuclear matrix-lamina-intermediate filament system in PtK 2 cells. After extraction the anaphase chromosome residues adjoined to the cytoplasmic intermediate filament in some areas. Immunofluorescent staining showed us that the intermediate filament reacted with AE 1 and AE 3; McAb 223 could be localized specifically on the lamina while McAb C 23 could crossreact with cytoplasmic intermediate filament beside the lamina location, monoclonal antibody against lamin A (C) could also bind to chromosome residues. Antibody to 280 kD nuclear matrix protein which were positive stained in HeLa cells could not react with the nuclear matrix components of PtK 2 cells. 2-D electrophoresis demonstrated that there were some differences in the composition of the nuclear matrix-lamina-intermediate filament system of HeLa and PtK 2 cells. TdR treatment could lead to alteration of nuclear matrix proteins.

Cellular distribution of isoforms of protein kinase C (PKC) in pancreatic acini.

As in a previous study (Biochim, Biophys. Acta 1224 (1994) 127-138), we used quantitative immunoblot analysis and found that rat pancreatic acini possess four different isoforms of PKC-alpha, delta, epsilon and zeta. The phorbol ester 12-O-tetradecanoyl phorbol 13-acetate (TPA) caused translocation of each isoform from the cytosol to the membrane fraction. CCK-8 increased diacylglycerol (DAG) and caused translocation of PKC-sigma and PKC-epsilon but not that of PKC-alpha or PKC-zeta. L-364,718, a CCK receptor antagonist, prevented as well as reversed the effects of CCK-8 on DAG and on translocation of PKC-sigma and PKC-epsilon. To explore the possibility that different isoforms of PKC might have different distributions in rat pancreas, we used immunocytochemistry to determine the cellular distribution of different isoforms of PKC in intact pancreas as well as pancreatic acini. In intact pancreas, PKC-alpha and PKC-sigma were detected in islet cells but not in duct or acinar cells. PKC-epsilon was detected in the apical region of acinar cells and PKC-zeta was detected over the luminal surfaces of acinar cells and the ductules that extend from the acinus. Neither PKC-epsilon nor PKC-zeta was detected in islets. In pancreatic acini PKC-alpha and PKC-sigma were detected in islets or fragments of islets that contaminated the preparation but were not detected in acinar cells. PKC-epsilon was detected in the apical region of acinar cells and adding 1 microM TPA or 1 microM CCK-8 accentuated the immunostaining but did not alter its cellular distribution. L-364,718 reversed the changes in immunostaining caused by CCK-8. PKC-zeta was detected over the luminal surface of the acinar cells. TPA, but not CCK-8 or CCK-8 followed by L-364,718, increased the number of acini that showed staining of the luminal surfaces of acinar cells. Thus, the present results demonstrate that different isoforms of PKC are distributed differently in rat pancreas and that the different patterns of distribution can explain, at least in part, the different responses to CCK-8.
