ApoA-II maintains HDL levels in part by inhibition of hepatic lipase. Studies In apoA-II and hepatic lipase double knockout mice.

High density lipoprotein (HDL) cholesterol levels are inversely related to the risk of developing coronary heart disease. Apolipoprotein (apo) A-II is the second most abundant HDL apolipoprotein and apoA-II knockout mice show a 70% reduction in HDL cholesterol levels. There is also evidence, using human apoA-II transgenic mice, that apoA-II can prevent hepatic lipase-mediated HDL triglyceride hydrolysis and reduction in HDL size. These observations suggest the hypothesis that apoA-II maintains HDL levels, at least in part, by inhibiting hepatic lipase. To evaluate this, apoA-II knockout mice were crossbred with hepatic lipase knockout mice. Compared to apoA-II-deficient mice, in double knockout mice there were increased HDL cholesterol levels (57% in males and 60% in females), increased HDL size, and decreased HDL cholesteryl ester fractional catabolic rate. In vitro incubation studies of plasma from apoA-II knockout mice, which contains largely apoA-I HDL particles, showed active lipolysis of HDL triglyceride, whereas similar studies of plasma from apoA-I knockout mice, which contains largely apoA-II particles, did not. In summary, these results strongly suggest that apoA-II is a physiological inhibitor of hepatic lipase and that this is at least part of the mechanism whereby apoA-II maintains HDL cholesterol levels.

A novel hepatitis B virus variant S 129 (Gln-->Leu): lack of correlation between antigenicity and immunogenicity.

A point mutation has been detected in the "a" determinant of hepatitis B surface antigen (HBsAg) in an infant immunised with hepatitis B vaccine after exposure to hepatitis B virus (HBV). This A-to-T point mutation at nucleotide 540 resulted in a glutamine-to-leucine substitution at amino acid residue 129 (129L). The S gene fragment (nucleotide 58-1072) of this isolate was cloned and used to substitute the wild-type S gene in a plasmid (p3.8II), containing 1.2 copy of full-length HBV genome with expression and replication efficiency. This plasmid p3.8II-129L was used to transfect HepG2 cells. HBsAg expressed by p3.8II-129L showed higher binding efficiency compared with the original plasmid containing the wild-type clone. A panel of 24 anti-HBs monoclonal antibodies (MAbs) was used to characterise the binding efficiency of HBsAg expressed by p3.8II-129L. Eighteen showed higher binding to the antigen, whereas HBsAg expressed by p3.8II-145R gave a consistently lower absorbance or were negative. Surprisingly, when the immunogenicity of plasmid constructs was used for DNA immunisation in Balb/c mice, the anti-HBs response induced by p3.8II-129L was significantly lower than that of the wild-type p3.8II. The lack of correlation between the antigenicity profile (binding of expressed HBsAg to anti-HBs in vitro), and the immunogenicity (induction of anti-HBs by plasmid DNA in vivo) of HBsAg with leucine substitution at position 129 indicates that biological characteristics other than the binding efficiency of HBsAg to anti-HBs could occur in HBsAg variants. These different aspects of the biological characteristics of S mutants merit further investigation.

Formation of multinucleated variant endothelial cells in vitro and investigation of MVECs' features.

Multinucleated variant endothelial cells (MVECs) have frequently been observed in the human aorta, and the ratio of MVECs to typical endothelial cells correlates well with the severity of atherosclerosis. MVECs showed no capacity of proliferation in vitro, making their study extremely difficult. We attempted to obtain reproducible MVECs in vitro in order to understand their functional characteristics and their roles in atherosclerosis. This study was designed to derive MVECs from human umbilical cord vein endothelial cells (HUVECs) with different reagents such as Meso-4,4'-(2,3-butanediyl) bis (2,6-piperazinedione) (ICRF-193), low density lipoprotein (LDL), interleukin-4 (IL-4), polyethylene glycol (PG), H2O2, and linoleic acid hydroperoxide (LAHO). We found that 10 microM ICRF-193 was most effective in inducing MVECs. We then investigated the features of aortic endothelial cells (AECs) and ICRF-193 treated HUVECs (I-HUVECs) in the following four aspects: morphology, by light microscopy; cell cycle phase, by uptake of BrdU; expression of endothelial cell (EC) related markers such as von Willebrand Factor (vWF), endothelin-1 (ET-1), prostacyclin (PGI2) and intercellular adhesion molecule CD34 by immunocytochemistry; and biological activity by analyzing their uptake of low density lipoprotein (LDL). Furthermore, we compared aortic MVECs (AMVECs) and other aortic endothelial cells (A-others), as well as A-MVECs and ICRF-193 induced MVECs (I-MVECs) in every parameter examined. We found: 1. Compared with A-others, A-MVECs expressed more vWF (p < 0.01), more ET-1 (p < 0.05) and less CD34 (p < 0.01). In the uptake of LDL, A-MVECs took up more nLDL than A-others; 2. Both A-MVECs and I-MVECs contained multiple nuclei, but the nuclei differed in shape. A-MVECs and I-MVECs were similar in the nuclear incorporation of BrdU, in the uptake of nLDL and oxLDL, and in the expression of vWF, ET-1 and PGI2, but different in the expression of CD34 (p < 0.01). Our findings suggested that A-MVECs may transfer more plasma LDL to the subendothelial space because they took up more LDLs. I-MVECs were similar to A-MVECs morphologically and functionally. Thus, I-MVECs could be considered as substitutes in the study of A-MVECs.

Study on pathogenic mechanism of emotions in traditional Chinese medicine--an observation of hydrogen peroxide releasing function of celiac macrophages in rats under stress state.

The hydrogen peroxide releasing function of macrophages in rats under the stress state was observed with the animal model of "excessive anger impairing the liver". The results showed that the volume of hydrogen peroxide released from the macrophages in rats was decreased, while the corticosterone level in plasma increased after stress. It indicates that stimulation of harmful emotions could cause inhibition of immunoreaction of the organism, which might be related to the enhancement in excitability of the hypothalamus-pituitary-adrenal axis and hypersecretion of glucocorticoid hormone.

[Changes of insulin and C-peptide in pregnancy of gestational diabetes mellitus].

OBJECTIVE: To observe the different changes of plasma insulin and c-peptide in patients with gestational diabetes mellitus (GDM), gestational impaired glucose tolerance (GIGT), normal pregnant women, and to detect the function of beta-cell in patients with gestational impaired glucose metabolism. METHODS: Test the plasma insulin, c-peptide in 48 cases of GDM(group I), 39 cases of GIGT (group II), 39 cases of normal pregnant women (group III) and 22 cases of healthy non-pregnant women (group IV) by radioimmunoassay. RESULTS: (1) In third trimester of group III, insulin, c-peptide levels were higher than those of group IV (P < 0.01). They increased from 28-34 weeks to 38-40 weeks, peaked during delivery. Insulin levels of group I and group II were higher than those of group III (P < 0.01, P < 0.05), but tended to decrease by the end of the third trimester and peaked during delivery. During delivery, there was no significant differences among the three groups. At 28-40 weeks, c-peptide of group I was significantly higher than those in groups II and III (P < 0.01) and was lower than that in group III during delivery (P < 0.05). Insulin and c-peptide of these 3 groups dropped gradually within 2 months after delivery but in group I and II they were significantly higher than those of group III (P < 0.01). CONCLUSION: In third trimester insulin and c-peptide of group I-III increased till delivery, the reason maybe associated with insulin resistance. GDM had the same peaking secretion during delivery with normal pregnant women and GIGT group, this maybe due to the stress during delivery. Insulin levels of most GDM and GIGT were still higher than normal after two month postpartum, and should be followed up carefully.

[A brief history of discovery of Helicobacter pylori].

The theoretical background for the diagnosis and treatment of upper digestive ulcer, gastritis and stomach carcinema was challenged by H. pylori, a microbe seems only under the microscope. This paper makes a brief review on the brief history of discovery and recent conclusion on the microbe, and explores its challenge to the traditional conception.

[Immunohistochemical study of angiogenesis of axillary lymph node metastasis in human breast carcinoma].

OBJECTIVE: To study angiogenesis of axillary lymph node metastases (including micrometastatic foci and metastatic tumors) in breast carcinoma and the relationship between microvessel density (MVD) and metastasis. METHODS: Thirty-seven breast cancer tissues and 121 metastatic axillary lymph nodes were collected from breast cancer patients and studied immunohistochemically. MVD was counted by means of microvideo system under 100x magnification. The diameter of each micrometastasis was measured with a micrometer. RESULTS: The mean diameter of 13 micrometastatic foci was 210 +/- 37 microns. No blood vessel formation was found. MVD of the primary tumor and that of metastatic tumor in the axillary lymph node was 93.8 +/- 21.8 and 89.3 +/- 18.4, respectively (P > 0.05). The distribution of microvessels of the metastatic tumor in the lymph node and that of the primary tumor was similar, being higher at the periphery than at the center. CONCLUSION: Micrometastatic foci of breast carcinoma in the axillary lymph node do not have new blood vessel formation. Their further growth, however, depends on neo-angiogenesis. Treatment based on inhibition of angiogenesis may be efficacious in the prevention of micrometastatic foci from developing into metastatic tumor in lymph node.

[Diagnosis and surgical treatment of idiopathic slow transit constipation].

OBJECTIVE: To investigate the pathogenesis, the diagnostic criteria, and the therapeutic method of idiopathic slow transit constipation. METHODS: History recording, bowel transit, anorectal dynamic, electromyography of the pelvic floor and defecography were performed before subtotal or total colectomy for severe constipation resistant to conservative treatment. RESULTS: The natural stool frequency was decreased and the time of bowel transit was delayed. The rectal sensation and the ability to reflect inhibition of the pelvic floor when attempting to defecate were damaged significantly compared with normal controls. pathological findings showed that the argyrophilic neurons in the myentenic and submucous plexus reduced quantitatively. All the patients were followed up for an average of 32.2 months. Satisfactory functional outcome was obtained in 91.8% of the patients receiving the operation. CONCLUSIONS: The severe damage colon transit function is due to the impairement of the myentenic and submucous plexus of the colon. It causes vary. The diagnosis of the disease is dependent on the typical clinic features and the bowel transit test. The symptoms of constipation could be relieved effectively by removal of the pathologic colon.

[The study of ultrastructure of retinal pigment epithelium of transplantation].

OBJECTIVE: To investigate the ultrastructure of retinal pigment epithelium (RPE) for the base of clinical application. METHODS: Pigmented rabbit RPE was transplanted to the subretinal space of 10 unpigmented rabbits by internal route. The ultrastructure of transplanted RPE cells were observed by electron microscopy. RESULTS: The transplantation of RPE was successful. It was shown that the transplanted cells had polarity and attached to Bruch's membrane to form basal infoldings, and the relationships between transplanted cells and the host photoreceptors were also seen. CONCLUSIONS: The internal approach is a practical and useful method for transplantation of RPE. The transplanted RPE can vitalize, perform metabolism and form some normal ultrastructure.

[Secondary glaucoma after anterior chamber intraocular lens implantation].

OBJECTIVE: To investigate the intraocular pressure level, the change of anterior chamber angle and the occurrence of secondary glaucoma after anterior chamber intraocular lens (AC-IOL) implantation. METHODS: Sixty-two AC-IOL implanted eyes were followed up postoperatively for 3 months to 7 years (mean 31 months). The examination of anterior chamber angle was emphasized. The type of cataract mainly included senile and traumatic. The operation performed were lensectomy with anterior vitrectomy or extracapsular cataract extraction (ECCE) combined with primary or secondary AC-IOL implantation. RESULTS: The incidence of postoperative peripheral anterior synechia (PAS) of iris was 87.1% and that of the secondary glaucoma (mainly the angle-closure) was 11.3%. The incidence of the glaucoma after ECCE with primary or secondary implantation was 17.4%, while that after ECCE combined with anterior vitrectomy was 0%. CONCLUSION: The main causes of the secondary glaucoma are possibly the rupture of posterior capsule and vitreous loss during the surgery leading to postoperative severe uveitis and pupillary block. To prevent its occurrence, it is important to combine ECCE with anterior vitrectomy.

[The characteristics of luminance of color visual evoked potential in macular diseases].

OBJECTIVE: To objectively evaluate the luminance channel damage of macular diseases by color pattern reversal visual evoked potential (CPR-VEP). METHODS: CPR-VEP was performed in 38 eyes with age-related macular degeneration (ARMD) dry type, 24 eyes with ARMD wet type, 20 eyes with Stargardt's disease and 54 normal eyes. Color pattern stimuli were obtained with color monitor controlled by computer program. Color pattern reversal rate was 2 Hz and its spatial frequency was 0.53 cycle/degree. The color graphic system could present white/black (W/B), red/black (R/B), green/black (G/B), blue/black (B/B), yellow/black (Y/B). RESULTS: In normal eyes, CPR-VEP P(1) latency was negatively correlated with luminance, whereas P(1) amplitude positively correlated with luminance; there were linear regressions between the mean luminance and P(1) latency and between the mean luminance and P(1) amplitude. Also in ARMD dry type, the CPR-VEP showed that the characteristics of the luminance channel were similar to that in the normal eyes. However, these phenomena did not exist in ARMD wet type and Stargardt's disease. CONCLUSION: In ARMD dry type, there is no damage of luminance channel, but in ARMD wet type and Stargardt's disease, there is severe damage of luminance channel and the patients can not discriminate luminance difference.

[Resection of nerve of wrist combining replacement of lunate with bone cement prosthesis in treatment of Kienbock's disease].

OBJECTIVE: There are various methods used to treat the Kienbock's disease with various outcomes. This study investigated the effect of combining acrylic lunate prosthesis replacement with resection of articular branches to wrist from anterior and dorsal interosseous nerve. METHODS: From January 1992 to January 1997, six cases were included. All of the patients were in stage III according to Lichtman's classification. Besides replacement of the acrylic lunate prosthesis, segment of the pure sensory branches from anterior and posterior interosseous nerves were resected. RESULTS: Follow-up for 10 to 54 months (an average of 32 months) the pain was relieved in all patients except one. The range of movement of wrists were as followings: The extension of the wrist was 20 to 35 degrees (an average of 26 degrees) and flexion was 20 to 35 degrees (an average of 32 degrees). There was no improvement in radial and ulnar deviation. CONCLUSION: Lunate prosthesis replacement combined with resection of articular branches to wrist resulted in a painless wrist and was a satisfactory method.

[Ameloblastic fibrosarcoma: an immunohistochemical and ultrastructural study of the mesenchymal component].

To study the characteristics of the mesenchymal cells of ameloblastic fibrosarcoma (AFS), three cases of AFS were studied immunohistochemically and ultrastructurally. The results showed that the mesenchymal component of AFS consisted predominantly of fibroblastic cells with a small number of undifferentiated cells, a few histiocytes and occasionally myofibroblastic cells under electron microscope. The fibroblastic cells were Vimentin positive only, and myofibroblastic cells were positive for Vimentin, HHF35 and alpha-SMA. The histiocytes were positive both for kp1 and PG-M1, suggesting that these cells were infiltrating cells from peripheral blood rather than histiocytic differentiation of tumor cells. Compared with ameloblastic fibroma, AFS showed much higher PCNA labeling index, suggesting higher proliferation activity of AFS.

[Determination of sulfacetamide sodium and sulfanilamide in shao tang ling ointment by high performance liquid chromatography].

An HPLC method for the determination of sulfacetamide sodium and sulfanilamide in Shao Tang Ling ointment was developed. The chromatographic conditions were: column, Spherisorb C18, mobile phase, methanol-0.25% (V/V)-acetic acid solution (7:93); detector UV-257 nm. The average recoveries were 98.9% (n = 5), RSD = 0.64% for sulfacetamide sodium and 99.3% (n = 5), RSD = 1.58% for sulfanilamide. The analytical method is simple, quick, accurate and reproducible.

[Study of methods for measuring spontaneous activation of neutrophils in vitro].

From the viewpoints of cellular morphology and enzymic histochemistry, we adopted the pseudopod method and NBT test for the measurement of spontaneous activation of neutrophils in vitro The time course curves of spontaneous activation of neutrophils from healthy donor were drawn. After comparing the pseudopod method with NBT test, we conclude that pseudopod method is more sensitive and reliable than NBT test and it can be used for real time observation; and that the curves of time course ascend from lower level to the highest level within 3 hours after the samples are collected, and then they descend gradually. The methodology of this study is available for use in future studies on the relationship between activation of neutrophils and many diseases.

[Effects of anticoagulants, temperature and sex on the spontaneous activation of neutrophils in vitro].

From the view of cellular morphology and enzymic histochemistry, we used the pseudopod method and NBT test to measure spontaneous activation of neutrophils in vitro. The effects of anticoagulant, temperature, and sex on the percentage of spontaneously activated neutrophils were studied. The results suggest that heparin is a better anticoagulant than EDTA because EDTA because EDTA inhibits neutrophil activation; that a higher percentage of activated neutrophils can be obtained when incubated at 37 degrees C, compared with the percentage at 25 degrees C, and at 37 degrees C the highest percentage appears an hour earlier; and that females show a higher percentage of spontaneously activated neutrophils than males, and females' highest percentage appears an hour earlier than males'. These are of referential value for further studies.

Characteristics of indocyanine green angiography in exudative age-related macular degeneration.

OBJECTIVE: To observe the characteristics of indocyanine green angiography (ICGA) in exudative age-related macular degeneration (AMD). METHODS: Thirty-one cases (39 eyes) were diagnosed as exudative AMD by ocular examination, fundus color photography, fundus fluorescein angiography (FFA) and ICGA. Their ages ranged from 50 to 82 years and visual acuities from FC/30 cm to 0.7. We analyzed and compared the characteristics of ICGA and FFA in these patients. RESULTS: Of 26 eyes with occult choroidal neovascularization (CNV) by FFA, 15 (57.7%) had classic CNV by ICGA. Of 4 eyes with serous retinal pigment epithelial detachment (PED) without CNV by FFA, 1 had serous PED with classic CNV by ICGA. The hyperfluorescence of the scar staining was detecfed by ICGA. CONCLUSION: ICGA adds clinically useful information and is an important adjunct in the evaluation, classification, and laser treatment of patients with occult CNV in AMD.
