[Ifosfamide combination chemotherapy for advanced gynecologic malignancies].

OBJECTIVE: To evaluate the clinical efficacy and toxicity of ifosfamide (IFO) combination chemotherapy in patients with advanced gynecologic malignancies. METHODS: Thirty-four patients with gynecologic malignancies were included in this series. Of the 34 patients, 26 with epithelial cancer of the ovary were previously treated with cisplatin-containing combination chemotherapy but failed to respond or recurred after treatment. They were treated with IEP (IFO, VP16, PDD) regimen. The remaining 8 patients with uterine sarcoma (5 cases), squamous-cell carcinoma of the uterine cervix with metastases to the liver or bone (2 cases), and endometrial carcinoma with lung metastases (1 case) were treated with IFO combination chemotherapy. At least two courses of treatment were given unless tumor progression occurred after the first course. RESULTS: The overall response rate was 35.3% including 8.8% complete response. The response rate of 26 patients with ovarian cancer was 30.8%. Two patients with PDD-sensitive tumor all achieved complete response, which lasted for one year. The response rate of the remaining 24 PDD-resistant patients was 25% with a mean duration of 5.5 months. There was no complete response. Two patients with cervical carcinoma and two of five patients with uterine sarcoma responded to IFO combination chemotherapy. Relatively severe hematological toxicity was observed, including grade III and IV leucopoenia and thrombocytopenia. Two patients died from severe toxicity. CONCLUSION: IFO combination chemotherapy is effective in treating recurrent or progressive gynecologic malignancies, especially PDD-sensitive ones. Myelosuppression is relatively severe which may be due to prior long term and intensive chemotherapy.

[Effect of 2/100 Hz transcutaneous electrical nerve stimulation on sexual dysfunction and serum sex hormone of heroin addicts].

OBJECTIVE: To evaluate the effect of 2/100 Hz electrical stimulation produced by Han's acupoint nerve stimulator (HANS) in treating heroin induced sex dysfunction. METHODS: Thirty-three heroin addicts were randomly divided into 2 groups to receive either buprenorphine (BPN group, n = 16) or HANS plus small dose of BPN (HANS group, n = 17) for 14 days. The doses of BPN were tailored according to individual requirement that could totally abolish withdrawal syndrome. The frequency of HANS was 2 Hz and 100 Hz shifting automatically, each lasting for 3 seconds. The stimulation was applied to 8 acupoints, the Hegu and Laogong of one upper limb, Neiguan and Waiguan of another limb, Sanyinjiao and Xingjian of both lower limbs, for 30 min. The therapy was carried out 3-4 times per day in the initial two weeks of treatment, and reduced to 1-2 times per day in the following 2 weeks. RESULTS: (1) The urine analysis for morphine in both groups turned to negative 14 days after treatment. (2) After 2 weeks of treatment, the composite score (Visual Analog Scale, VAS) of sexual function in HANS group showed a 102% increase (P < 0.01) compared with before treatment and was 107% of that in the BPN group (P < 0.01). (3) After 4 weeks treatment the serum concentration of luteinizing hormone (LH) and testosterone (T) in the HANS group showed a 137% and 118% increase respectively compared with those before treatment (P < 0.05), and the level of LH was 79.6% of that in the BPN group (P < 0.05). CONCLUSION: The 2/100 Hz transcutaneous electrical stimulation produced by Han's acupoint nerve stimulator could improve the recovery of sexual function and raise the serum concentration of LH and T in heroin addicts.

[Correlation of intrauterine growth retardation with endocrine parameters].

OBJECTIVE: To investigate the relationship between intrauterine growth retardation (IUGR)and endocrine parameters and to assess the effects of the main endocrine factors on IUGR. METHODS: The concentrations of growth hormone (GH), insulin, triiodothyronine (T(3)), tetraiodothyronine (T(4)), and thyroid-stimulating hormone (TSH) were measured in maternal serum, umbilical blood and amniotic fluid. The samples were collected from 23 term pregnancies with IUGR, and 42 normal pregnancies served as control. Growth hormone and insulin were measured by radioimmunoassay, and T(3), T(4) and TSH by microradioimmunoassay. RESULTS: The concentrations of GH, insulin and T(4) in umbilical blood were lower in IUGR group than that in the normal group (IUGR: GH 4.63 microg/L vs 7.01 microg/L, insulin 10.68 mIU/L vs 31.44 mIU/L, T(4) 0.68 mg/L vs 1.07 mg/L. P < 0.05, 0.05, 0.05), the TSH level in umbilical blood was higher in IUGR than that in the control group (10.84 mIU/L vs 5.74 mIU/L. P < 0.01). The concentration of GH in maternal serum and insulin in amniotic fluid in IUGR group were also lower than that in normal group (GH 1.77 microg/L vs 2.74 microg/L, P < 0.01; insulin 5.84 mIU/L vs 15.64 mIU/L, P < 0.01). CONCLUSIONS: This study confirms previous observation that insufficiency of GH might be a pathogenesis of IUGR. The decrease of GH and insulin might compromise the basic metabolism of the fetus. Besides, the change of early hypothyrosis of fetus with IUGR might benefit them to live in unfavorable environment.

[Study on perinatal group B Streptococcus carriers and the maternal and neonatal outcome].

OBJECTIVE: To investigate the group B streptococcus (GBS) colonization rate and the relationship between vaginal colonization of GBS and the maternal and neonatal outcome. METHODS: 3% TH selective broth media were used for culture of GBS. The vaginal samples were collected during different pregnant periods (before 20 weeks' gestation; 20-28 weeks; after 34 weeks' gestation) and other samples from different sites after labor (including neonatal throat, ear, umbilicus and placenta, umbilical blood, maternal blood, etc). RESULTS: The GBS carrier rate in 1 039 pregnant women was 11.07% (115/1,039). The colonization rate of different treating times was 29.73% (test three times), 15.50% (test twice) and 9.86% (test one time) respectively. The carrier rate of three-time-test was significantly higher than that of two times test (P < 0.01) and that of only one time (P < 0.01). The carrier rate in three periods was 16.84% (< 20 weeks), 18.40% (20-28 weeks) and 14.90% (> 34 weeks) respectively. Obstetric complications associated with GBS carrier were similar with those of non-carrier. Neonatal carrier rate was 9.95%. The neonatal carrier rate in the group of carrier mothers was higher than that of non-carrier mothers (14.29%:7.26%). The rate of neonatal pneumonia and the upper respiratory tract infection in GBS-carrier-mother was higher than that of non-carrier mothers (11.01%:9.71%; 8.27%: 6.07%) while it was similar in neonatal carrier group and non-carrier group (20.00%:14.92%; 10.00%:4.97%). CONCLUSION: GBS might be one of vaginal pathogens in Chinese women, but it is not severe enough to threaten the health of pregnant women and their newborns.

[Allotransplantation of cultured fetal parathyroid gland cells in treating patients with hypoparathyroidism].

OBJECTIVE: To evaluate the therapeutic efficacy of transplantation of cultured human fetal parathyroid gland (chf-PTG) cells in treating patients with primary or secondary hypoparathyroidism. METHODS: Chf-PTG cells were allotransplanted into the renal adipose capsules of 6 patients with hypoparathyroidism under the ultrasonic guidance. The levels of serum parathyroid hormone (PTH) and calcium were monitored by radioimmunometric assay (RIA) and biochemical method, respectively. Both indexes of pre-and post-operation were compared and the data were analyzed. RESULTS: The levels of serum PTH and calcium were markedly elevated from three days to the first two weeks following transplantation of chf-PTG cells (P < 0.01). The PTH and calcium levels gradually stabilized from day 14 up to months 9-12, during which the symptoms of the patients alleviated or relieved. CONCLUSION: The transplantation of chf-PTG cells is a potential method for treating patients with primary or secondary hypoparathyroidism.

[Corneal thickness and intraocular pressure in cases with ocular hypertension and glaucoma].

OBJECTIVE: To investigate the relationship between central corneal thickness (CCT) and intraocular pressure (IOP) in patients with ocular hypertension (OH), normal tension glaucoma (NTG), primary open angle glaucoma (POAG) and normal subjects and the importance of CCT in the diagnosis of glaucoma. METHODS: The CCT was measured in 79 cases with NTG, 61 POAG, 73 OH and 50 normal subjects with an ultrasonic pachymeter. One eye in one subject randomly selected was used for inter-group comparison. The maximum IOP measured by Goldmann applanation tonometer without ocular hypotensive therapy during the follow-up was corrected with CCT by using Ehlers' method. RESULTS: The CCT of OH subjects (582 +/- 32) microm was significantly thicker than that of other groups (P < 0.001), while no differences were seen in CCT among normal, NTG and POAG groups, the thickness being (552 +/- 36) microm, (548 +/- 33) microm and (550 +/- 33) microm respectively. The corrected maximum pressure was significantly lower than that in the untreated ones in all the groups, and <or= 21 mm Hg in 100%, 97%, 55% and 50% of the eyes in the normal, NTG, OH and POAG subjects, respectively. In the normal subjects, the CCT was positively correlated with the IOP (r = 0.423, P = 0.004). CONCLUSIONS: There is a large variation in CCT of normal subjects, which is significantly positively correlated with IOP. The CCT is significantly thicker in OH subjects, which should be considered as an important variable in follow-up. No significant differences in CCT are shown among NTG, POAG and normal groups. It is suggested that CCT has little influence on the diagnosis of NTG and POAG.

[Expressional change of brain-derived neurotrophic factor in adult cat spared DRG].

UNLABELLED: This investigation was aimed to search the expressional change of BDNF in the normal and spared L6DRG. The L6DRGs from 20 adult male cats (5 normal cats, 15 unilateral L6 spared roots cats allowed to survive 3 days, 6 days and 12 days respectively) were embedded in paraffin, using immunohistochemical reaction to detect BDNF. RESULTS: 1. Normal DRG neurons were divided into three categories(large, medium and small) with the cell body diameter 32 microns and 44 microns as the dividing lines. 2. There are 50.87% BDNF positive neurons in normal L6DRG. After operation, the number of BDNF positive neurons of the operated side DRG significantly increased, especially 6 days(76.70%). Normal and postoperative BDNF positive neurons were mainly small neurons. After partial section of DRG, the expression of BDNF in spared DRG neurons increased, which might be involved in promoting collateral sprouting of spared root in spinal cord and rescuing the dorsal horn neurons from partial deafferentation.

[Determination of blood glucose by chemiluminescent flow-injection analysis with immobilized glucoxidase column].

Luminol-hemin system was used for chemiluminescent flow-injection analysis with immobilized glucoxidase(GOD) column to determine blood glucose. The method is fast, simple and sensitive. Blood samples were diluted to 1:1000 and pumped into the flow-injection analyzer for direct detection. The sampling frequency was 30 samples/h, with a detection limit of 7.2 x 10(-9) mol/L(1.30 micrograms/L). A column packed with 36 milligrams of CPG could be used nearly 200 times. The determination results of glucose in serum by this method were in good agreement with those obtained by the routine GOD spectrophotometric method (correlation coefficient r = 0.90, P > 0.05). Recoveries of this method were 94.0%-107.2%.

[A new resolution vector with cry1Ac10 gene based on Bacillus thuringiensis transposon Tn4430].

A new resolution vector with cry1Ac10 gene based on TnpI-mediated site-specific recombination system of Bacillus thuringiensis(Bt) transposon Tn4430 was developed. The gene cry1Ac10, encoding a protoxin against plutella xylostella larvae, and the gene ori1030, from a plasmid of wide type Bacillus thuringiensis, were inserted into two copy sets of RES sites, named pBMB801. When pBMB801 was introduced into crystal negative Bt host BMB171, antibiotic resistance genes and other non-Bt DNA can be selectively eliminated. This recombinant plasmid was found very stable without antibiotic selection. The resulting strain only contained Bt DNA and is free of antibiotic resistance genes. This strategy should facilitate regulatory approval for its development as a commercial biopesticide.

[Effect of lysophosphatidylcholine on endothelial cell surface expression of adhesion molecules].

Extensive monocyte recruitment is an early phenomenon associated with the development of atherosclerotic lesion, in which adhesion molecules expressed by endothelial cell play an active role. In this study we examined the effects of Lysophosphatidylcholine(Lyso-PC), a major phospholipid component of atherogenic lipoproteins, on the expression of adhesion molecules for monocytes, such as intercellular adhesion molecule-1(I-CAM-1), endothelial-leukocyte adhesion molecule-1(E-selectin) and vascular cell adhesion molecule-1(VCAM-1), in cultured human umbilical vein endothelial cells(HUVECs). After treating HUVECs with different concentration Lyso-PC at different times, the expression of adhesion molecules on the surface of HUVECs induced by Lyso-PC were analyzed using flow cytometry. The results showed that compared with control group, the surface expression of ICAM-1 and VCAM-1 was increased on HUVECs treated with Lyso-PC. ICAM-1 and VCAM-1 expression was maximally induced 1 h after stimulated with 50 micrograms/mg Lyso-PC, compared to basal values. At the 3 h after stimulation, ICAM-1 dropped to basal level while VCAM-1 dropped below to basal level. The effects of Lyso-PC on E-selection expression were negligible with in 3 h. However, at the 3 h of treatment with 50 micrograms/ml, the E-selectin expression on HUVECs began to be up-regulated significantly. We conclude that in addition to its monocyte chemoatractant activity, Lyso-PC can selectively induce ICAM-1 and VCAM-1 expression on the surface of HUVECs. Lyso-PC may play an important role in monocyte recruitment into athersclerotic lesions.

A comparative evaluation of Humphrey perimetry and the multi-channel pattern visual evoked potentials.

PURPOSES: To compare the multi-channel pattern visual evoked potentials to Humphrey perimetry in the assessment of central visual function in primary open angle glaucoma. METHODS: The multi-channel checkerboard reversal PVEPs waves to full-field and half-field stimulus of 25 normal persons and 74 patients with primary open angle glaucoma were recorded and analyzed, All patients were examined using Humphrey Field Analyzer. The area of visual field corresponding to the area of retina stimulated during multi-channel PVEPs testing were analysed, straight-line correlation and regression analyses of the various multi-channel PVEPs parameters and the total dB losses were performed. RESULTS: The multi-channel PVEPs demonstrated a low detection rate compared with Humphrey perimetry in the early glaucoma, absolute latency and field loss were correlated in the late stage of glaucoma, and absolute amplitude and field loss were not correlated. CONCLUSIONS: In relation to signalling "early" loss the multi-channel PVEPs was inferior to Humphrey perimetry, in late loss of primary open angle glaucoma, multi-channel PVEPs can provide a valuable, objective complement to Humphrey perimetry.

[The clinical application of different brightness and different saturation D-15 tests].

PURPOSE: To compare the clinical application of different brightness and different saturation D-15 tests. METHODS: Eighteen normal subjects (30 eyes), 19 cases (38 eyes) of congenital color vision defects and 36 cases (59 eyes) of eye diseases were tested with Panel D-15 test, Hann's double D-15 test, middle and low saturation CAS-PI (Psychological Institute, Chinese Academy of Sciences) D-15 tests. RESULTS: A few of the normal eyes made minor errors in the low saturation D-15 tests. All of the protanopes and deuteranopes could be detected correctly. Protanomalias and deuteranomalias showed normal arrange or some type of abnormal arranges in all of the 5 sets of D-15 tests. In sick eyes, the abnormal rates were the highest in the low saturation D-15 tests and the lowest in Panel D-15 test. CONCLUSION: The five sets of different brightness and different saturation D-15 tests had similar efficiencies of detecting congenital color vision defects. The abnormal rates of CAS-PI(4/5)D-15 tests and the CAS-PI(2/5)D-15 tests were similar to those of Panel D-15 test and Hann's Double D-15 test. Both tests can be used in the clinical setting.

[Uptake of drug-containing nanoparticles in dog catotid and femoral arteries].

OBJECTIVE: To study the uptake of drug-containing nanoparticles in artery for local therapy of restenosis. METHODS: Polylactic polyglycolic acid copolymer (PLGA) nanoparticles containing an antiproliferative agent 2-amino-chromone were formulated with an oil-in-water sonication emulsion/solvent evaporation technique. Arterial uptake of nanoparticles was assessed both in vitro and in vivo models of dog and rat carotid and femoral arteries. Epoxide, cyanoacrylate, fibrinogen, fibronectin, didodecyldimethylammonium bromide (DMAB), L-alpha-phosphatidylethanolaine, and lipofectin were selected to modify the surface of nanoparticles to enhance arterial uptake of nanoparticles. RESULTS: The nanoparticle size ranged from 100 nm to 200 nm, the drug loading in nanoparticles was about 15%, the nanoparticle morphology was observed by scanning electron microscopy and showed spherical shape with smooth surface. Arterial uptake of nanoparticles was enhanced greatly by surface modified nanoparticle with positively charged active agent DMAB. Once nanoparticles located in artery, a high drug level could be maintained at local site for 2 days in vivo model. CONCLUSION: The primary results of animal experiments suggested that nanoparticle could be used as drug carrier of local drug delivery for treating cardiovascular diseases.

[IR spectra and structures of two trinuclear molybdenum cluster compounds coordinated by binary carboxylates].

Two trinuclear molybdenum cluster compounds coordinated by binary carboxylates compounds Mo3S4 (DTP)3(phthalate) (Py)(A) and Mo3S4 (DTP)3(malonate)(Py)(B) have been synthesized. IR absorption peaks of Mo3S4 core and the ligands are assigned with reference to those of Mo3S4(DTP)4(Py)(C). The assignment of stretching vibrations of carboxylate (COO-) for both (A) and (B) reveals much information corresponding to their molecular structures.

Inactivation of the INK4A/ARF locus frequently coexists with TP53 mutations in non-small cell lung cancer.

Inactivation of the P16 (INK4A)/retinoblastoma (RB) or TP53 biochemical pathway is frequent event in most human cancers. Recent evidence has shown that P14ARF binds to MDM2 leading to an increased availability of wild type TP53 protein. Functional studies also support a putative tumor suppressor gene function for p14ARF suggesting that p14ARF or p53 inactivation may be functionally equivalent in tumorigenesis. To study the relative contribution of each pathway in tumorigenesis, we analysed and compared alterations of the p16, p14ARF and p53 genes in 38 primary non-small cell lung cancers (NSCLCs) (19 adenocarcinomas and 19 squamous carcinoma). The p16 tumor suppressor gene was inactivated in 22 of 38 (58%) tumors. Twelve of these samples (31%) had homozygous deletions by microsatellite analysis; eight of them (21%) had p16 promoter hypermethylation detected by Methylation Specific PCR (MSP) and the remaining two (5%) harbored a point mutation in exon 2 by sequence analysis. The absence of P16 protein in every case was confirmed by immunohistochemistry. Fourteen of the 22 tumors with p16 inactivation also inactivated the p14ARF gene (12 with homozygous deletions extending into INK4a/ARF and two with exon 2 mutations). Mutations of p53 were found in 18 (47%) of the tumors and nine of them (50%) harbored p14ARF inactivation. Thus, an inverse correlation was not found between p14ARF and p53 genetic alterations (P=0.18; Fisher Exact Test). Our data confirm that the p16 gene is frequently inactivated in NSCLC. Assuming that 9p deletion occurs first, the common occurrence of p53 and p14ARF alterations suggests that p14ARF inactivation is not functionally equivalent to abrogation of the TP53 pathway by p53 mutation.

Pre-T cell receptor (TCR) and TCR-controlled checkpoints in T cell differentiation are set by Ikaros.

T cell differentiation relies on pre-T cell receptor (TCR) and TCR signaling events that take place at successive steps of the pathway. Here, we show that two of these T cell differentiation checkpoints are regulated by Ikaros. In the absence of Ikaros, double negative thymocytes can differentiate to the double positive stage without expression of a pre-TCR complex. Subsequent events in T cell development mediated by TCR involving transition from the double positive to the single positive stage are also regulated by Ikaros. Nonetheless, in Ikaros-deficient thymocytes, the requirement of pre-TCR expression for expansion of immature thymocytes as they progress to the double positive stage is still maintained, and the T cell malignancies that invariably arise in the thymus of Ikaros-deficient mice are dependent on either pre-TCR or TCR signaling. We conclude that Ikaros regulates T cell differentiation, selection, and homeostasis by providing signaling thresholds for pre-TCR and TCR.

Enhanced expression, native purification, and characterization of CCR5, a principal HIV-1 coreceptor.

Seven-transmembrane segment, G protein-coupled receptors (GPCRs) play important roles in many biological processes in which pharmaceutical intervention may be useful. High level expression and native purification of GPCRs are important steps in the biochemical and structural characterization of these molecules. Here, we describe enhanced mammalian cell expression and purification of a codon-optimized variant of the chemokine receptor CCR5, a GPCR that plays a central role in the entry of the human immunodeficiency virus-1 (HIV-1) into immune cells. CCR5 could be solubilized in its native state as determined by its ability to be precipitated by 2D7, a conformation-dependent anti-CCR5 antibody, and by the HIV-1 gp120 envelope glycoprotein. The 2D7 antibody recognized immature and mature forms of CCR5 equally, whereas gp120 preferentially recognized the mature form, a result that underscores a role for posttranslational modification of CCR5 in its HIV-1 coreceptor function. The methods described herein contribute to the analysis of CCR5 and are likely to be applicable to many other GPCRs.
