Genetic relatedness among human rotaviruses.

The genetic relatedness of 81 clinical rotavirus isolates to the human rotavirus prototype strains Wa (subgroup 2, serotype 1) and DS-1 (subgroup 1, serotype 2) was examined by RNA hybridization techniques. Labeled single-stranded (+) transcripts of Wa or DS-1 virus were incubated with denatured genomic rotaviral RNAs, and the resulting hybrids were subjected to gel electrophoresis and autoradiography. Nineteen of the specimens contained subgroup 1 rotavirus with a "short" RNA migration pattern. These viruses were found to be closely related to the DS-1 strain and were associated with illness of short duration. The remaining 62 isolates belonged to subgroup 2 and exhibited a "long" RNA migration pattern. Fifty-four of these isolates exhibited significant hybridization with the Wa strain probe. Four isolates yielded multiple hybrid bands with the Wa probe but also possessed at least one gene segment homologous to the DS-1 strain. The remaining four subgroup 2 rotaviruses did not exhibit significant homology in the form of labeled hybrid bands when tested with either the Wa or DS-1 probe. These findings suggest that most clinical rotavirus isolates belong to one of two human rotavirus "families" defined as Wa-like or DS-1-like. Our observations also suggest that reassortment occurs in vivo between rotaviruses belonging to the two human rotavirus "families" and that there are one or more additional families of human rotavirus.

Seasonal variation of acute anterior uveitis: differences between HLA-B27 positive and HLA-B27 negative disease.

One hundred and seventy-five consecutive patients with acute anterior uveitis (AAU) were examined over a 24-month period. There was a significantly increased incidence of AAU during the months August to December (p less than 0.05). This increase was confined predominantly to the HLA-B27 negative group of patients (p less than 0.01). There was no significant monthly difference in incidence between males and females, between patients with first or recurrent attacks, or between patients with and without arthritis. These results suggest that seasonal environmental factors may play a role in the production of HLA-B27 negative AAU.

Preoperative ultrasound and gallium-67 evaluation of abdominal non-Hodgkin's lymphoma.

The diagnostic accuracy of abdominal ultrasonography followed by gallium (Ga)-67 scintigraphy in 21 patients, aged 1 to 14 years, appearing with abdominal non-Hodgkin's lymphoma (NHL) was analyzed. All cases were confirmed by biopsy; in a majority (16 patients), the tissue was obtained from an abdominal mass at the time of laparotomy subsequent to the imaging studies. Nineteen satisfactory abdominal ultrasound examinations were performed; 18 were interpreted as characteristic of NHL. Sixteen of these were of masses involving the gastrointestinal tract. All 21 patients had 67Ga scintigraphy that demonstrated abnormal radionuclide accumulation in the abdomen. In no instance was the final diagnosis different from the one predicted by the combined imaging studies. Ultrasonography is recommended as the initial test in the evaluation of clinical presentations consistent with abdominal NHL to expedite suitable management and prevent inappropriate surgery.

Rotavirus shedding by newborn children.

We studied the shedding of rotavirus by newborn children in the nurseries of a large maternity hospital in Caracas, Venezuela, throughout the year 1982. Sixty-two (57%) of 108 children examined shed the virus within the first few days of life. Four (6%) of the 62 children who shed rotavirus had diarrhea but only one of them required oral rehydration therapy. The rotavirus specimens were identified as subgroup 2 in an ELISA subgrouping assay that employs monoclonal antibodies. Analysis of the RNA extracted from 52 of the samples by electrophoresis revealed a similar migration pattern in all the specimens; their identity was confirmed by crosshybridization analyses which revealed a strong degree of genomic homology among the strains studied.

Cis-platinum-induced renal sodium wasting.

A 10-year-old girl with a malignant ovarian germ cell tumor was treated with cis-platinum-based chemotherapy. During the third and fourth courses of therapy, the patient developed hyponatremia, hypomagnesemia, and hypocalcemia. Renal sodium wasting was documented to be the cause of the hyponatremia. Despite normalization of serum electrolytes, hyponatremia again occurred during a period of stress following cessation of cis-platinum. Prior work has documented that cis-platinum-induced hypomagnesemia is related to renal tubular dysfunction with resulting magnesium wasting. A similar etiology for cis-platinum-induced hyponatremia and renal sodium wasting is proposed.

A new oral hygiene strategy.

This study was designed to see if toothbrush pressure could be considered a significant factor in the reduction of plaque among a group of orthodontic patients. Eleven orthodontic patients who had been identified previously as having chronic poor oral hygiene and the ability to tolerate only small toothbrush pressures (an average of 3 oz) were selected to participate in this study. At the beginning of the study, each of the patients was supplied with a specialized toothbrush feedback mechanism that was to be used in the routine home care. The feedback mechanism permitted the patients to gradually increase their tolerance to toothbrush pressure by moving from a target pressure of 2 oz to a target pressure of 16 oz over a period of several weeks. When the target pressure was reached, a light on the end of the brush mechanism came on, thus alerting the patient that the target had been reached. Only one of the eleven patients failed to improve her plaque score. There was, on the average, a 47% reduction in plaque scores. The statistical evidence indicates that the improved plaque scores were directly related to the higher toothbrush pressures. The present study was undertaken as an extension of a previous study that indicated good orthodontic toothbrushers used four and one half times more pressure than did chronically poor toothbrushers. This study suggests that poor toothbrushers can improve their oral hygiene significantly by increasing the pressure with which they brush.

Three examples of anti-Fy3 produced in Negroes.

We present three examples of anti-Fy3 produced by Negroes. Results of serologic studies were similar to those of previously reported examples of Negro produced anti-Fy3, including negative to weak reactivity with cord red blood cells, separable anti-Fya, and the presence of multiple unexpected alloantibodies. These findings reinforce the difference between anti-Fy3 produced by Negroes and that produced by non-Negroes. The rare formation of Duffy antibodies by Negroes may signify the presence of an amorphic gene at the Duffy locus.

Relative frequency of rotavirus subgroups 1 and 2 in Venezuelan children with gastroenteritis as assayed with monoclonal antibodies.

Monoclonal antibodies recently developed against the 42,000-dalton protein of two rotavirus strains were used in an enzyme-linked immunosorbent assay to determine the subgroup specificity of 252 specimens collected during a 45-month period from Venezuelan children with rotavirus gastroenteritis. Subgroup 2 rotavirus was shed by 85% of the children, whereas only 14% shed subgroup 1 rotavirus (one-half of them in a 3-month period). No differences were found in the occurrence of fever and vomiting between children shedding either rotavirus subgroup, but it appeared that the syndrome tended to last longer in children shedding subgroup 2 rotavirus. The monoclonal subgrouping enzyme-linked immunosorbent assay seemed to be more sensitive than an immune adherence hemagglutination assay, an enzyme-linked immunosorbent assay with polyclonal antibodies, or the electrophoretic analysis of RNA extracted from the virus. Overall, 99% of the specimens could be subgrouped by this assay.

A search for gram-negative enteric micro-organisms in acute anterior uveitis: association of klebsiella with recent onset of disease, HLA-B27, and B7 CREG.

Faecal samples obtained from patients with acute anterior uveitis (AAU) and healthy controls unconnected with hospitals were examined for the presence of Gram-negative micro-organisms reported to be associated with AAU. There was an increased recovery of Klebsiella spp. from AAU patients seen in the first and in the second week after onset of symptoms when compared with patients presenting three to four weeks after onset of symptoms (p less than 0.025 and p less than 0.05 respectively) or with healthy control subjects (p less than 0.005 and p less than 0.05 respectively). By the third to fourth week there was no difference in klebsiella recovery between AAU patients and controls. The increased faecal recovery of klebsiella in the first two weeks was predominantly in HLA-B27 positive patients or patients possessing antigens which cross-react with B27, namely, HLA-B7 CREG. The patients with faecal cultures positive for klebsiella had a higher mean ESR than patients with negative faecal cultures (p less than 0.05). Although there was an increase in recovery of faecal klebsiella from AAU patients with spondylarthritis when compared with AAU patients without arthritis, the difference was not statistically significant. These results suggest that some klebsiella micro-organisms may play a role in the aetiopathogenesis of AAU associated with HLA-B27 or HLA-B7 CREG.

Serum immunoglobulins in acute anterior uveitis.

Serum levels of immunoglobulin A, G, and M were examined in 99 patients with acute anterior uveitis (AAU). The mean level of serum IgA was raised (p less than 0.001) in AAU patients when compared with healthy control subjects. The levels of serum IgG and IgM were not significantly different from those in the control group. The raised levels of serum IgA were found to occur predominantly in the HLA-B27 positive group of patients (p less than 0.001) and were highest in HLA-B27 positive patients with sacroiliitis or associated spondylarthritic diseases (p less than 0.001). The HLA-B27 negative group of AAU patients did not have a significantly raised mean serum IgA. The raised level of serum IgA in HLA-B27 positive AAU patients suggests that there has been a response to an environmental or infectious agent(s) acting across a mucosal tissue in these patients. This agent could be responsible for the initiation of the acute ocular inflammation.

Hematoporphyrin derivative photoradiation induced damage to normal and tumor tissue of the pigmented rabbit eye.

Cytotoxicity induced by hematoporphyrin derivative (HpD) photoradiation in both normal and experimental tumor tissue of pigmented rabbit eyes has been examined. In addition, documentation of HpD induced fluorescence in ocular structures has also been obtained. Acute normal ocular tissue toxicity studies demonstrated that HpD (1-10 mg HpD/kg) followed 48 hours later by a transpupil irradiation of red light (635 nm, 36-90 J/cm2) resulted in demarcated areas of retinal damage. Long term (chronic) toxicity studies have shown that the initial damage to the retina was permanent but that no damage to the cornea, lens or vitreous could be observed during a 16 month follow-up. Visual and histological documentation have been obtained, following HpD photoradiation therapy (PRT), in rabbit eyes having heterotransplanted single nodule amelanotic melanomas. A toxic effect characterized by tumor blanching, edema and hemorrhage was observed within 24 hours of treatment. Histological examination obtained 24 hours following HpD PRT illustrated massive tumor tissue necrosis and vascular disruption. HpD PRT at clinically relevant doses was also shown to be effective in selectively curing the highly malignant amelanotic iris melanoma. It is concluded that HpD PRT may prove to be an effective modality for treating certain ocular tumors.

A multidisciplinary audit of diabetic care using criteria mapping.

A multidisciplinary audit of care delivered to diabetic patients was completed at the Royal Victoria Hospital, in Montreal. The audit of inpatient care was implemented using Criteria Mapping. A Criteria Map is based on decision-making logic and allows use of criteria relevant to individual patients. The Map includes 23 criteria: 18 medical, three dietetic, and one nursing. The charts of 17 patients discharged with a diagnosis of diabetes mellitus were reviewed. A score above 75% indicated that each discipline rendered adequate care. The medical compliance was 78.46%, dietetic 79.17%, and nursing 85.71%. Mean dossier score was 80.62%. The Criteria Map was highly satisfactory as an audit method; the results of the audit illustrated a need for more complete documentation of services, especially in the area of patient education.

Establishment and characterization of a human T-cell leukemia line (LALW-2) in nude mice.

Lymphoblasts from the peripheral blood of a 10-year-old boy who was treated for T-cell acute lymphoblastic leukemia were heterotransplanted intraocularly into nude mice. The resultant tumors and their metastases were serially passaged both intraocularly and subcutaneously in the mice. The mouse tumors closely resembled morphologically and cytochemically a human T-cell lymphoma. The primary cells, as well as cells from subsequent in vivo passages, were predominantly of a suppressor T-cell phenotype (OKT8-positive). No viral products were identified. Chromosome analysis revealed a near-diploid karyotype with a translocation between chromosomes 11 and 14. The tumorigenicity, morphology, cytochemistry, immunologic phenotype, and karyotypic pattern of the cells remained constant through six serial in vivo passages over a period of 10 months. This is the first report of direct heterotransplantation and long-term in vivo maintenance of primary human T-cells in immunologically unmanipulated nude mice.

A dot hybridisation assay for detection of rotavirus.

A "dot" hybridisation technique for the detection of rotavirus in stools and other biological materials is described. The assay is based on the in-situ hybridisation of labelled single-stranded RNA probes, obtained by in-vitro transcription of rotavirus particles, to heat-denatured rotavirus RNA immobilised on nitrocellulose membranes. The method is highly specific and allows for the detection of as little as 8 pg of viral RNA. Its use for the detection of rotavirus in stool suspensions and rectal swabs obtained from children with diarrhoea may facilitate epidemiological studies of rotavirus gastroenteritis.

Effect of total parenteral nutrition on marrow recovery during induction therapy for acute nonlymphocytic leukemia in childhood.

Ten well-nourished children with acute nonlymphocytic leukemia (ANLL) were randomly assigned to groups that received (a) total parenteral nutrition (TPN) throughout the period of induction therapy or (b) standard nutritional support. Body mass and skin hypersensitivity reactions were better maintained in experimental patients. Patients on TPN had higher total white blood counts, absolute granulocyte counts, and platelet counts than did control patients during the course. No difference was apparent in the frequency of febrile episodes, or other aspects of the patients' courses. This preliminary report suggests that intensive nutritional support may accelerate the recovery of normal marrow function during induction therapy for ANLL.
