[Phlebography of the testicular veins for varicoceles and their recurrences (author's transl)].

The clinical significance of varicoceles depends on their frequency and on the fact that they may produce infertility. The available operative forms of treatment are unsuccessful in about 10% of patinents. In these cases it is desirable to elucidate the very variable drainage pattern by means of transfemoral phlebography with the patient erect. The method was employed in 13 patients and demonstrated continuity of the testicular vein, or of one of its tributaries, in twelve. If the testicular vein appears to be absent, one must consider the possibility of a venous short circuit or drainage of the pampiniform plexus into the external iliac vein. In four patients out of the 13, scrotal phlebography in addition for complete definition was necessary.

[Malignant lymphoma of the testis--an urological or nonurological disease?[].

Malignant lymphomas of the testis have a special position among the tumours of the testis. Also in such cases, in which only the testis is perceptibly diseased, nevertheless must be started from a systemic disease which is clinically not yet recognizable. These special problems demand a multidisciplinary, oncological collaboration in diagnostics and therapy. After establishment of the diagnosis and semicastration by the urologist further diagnostics and therapy should be performed by the representatives of other specialities.

[Tuberculosis of seminal vesicle. A contribution to differential diagnosis of fructose deficiency in spermatic fluid (author's transl)].

The functional condition of seminal vesicles can be judged on the basis of spermatic fructose. In dealing with the causes of fructose deficiency, one must distinguish between androgen-dependent and androgen-refractory deficiencies. The main causes of androgenic-refractory fructose deficiency are specific and unspecific inflammations of the masculine adnexa. The example of spermatocystic tuberculosis demonstrates the importance which must be given to specific inflammation of the masculine adnexa, during fertility-consultation hours.

[Particular aspects of testicular tumor in the elderly (author's transl)].

Malignant testicular tumors mainly occur in early adult life. 14 (7.8%) of our 180 patients with testicular tumors, treated from 1966 to 1976, were between 50 to 68 years of age. The pathological anatomical classification showed 9 germinal and 5 non-germinal tumors. To find out which tumor classification is the best for prognostic information and assessment of therapeutic consequences, we recommend setting up a central testicular tumor panel and register and refer to the multilateral study already initiated.

The combined surgical and chemotherapeutic approach to non-seminomatous cancer of the testicle.

Of 250 testicular tumors treated up to May 1975 233 (93.2%) were germ cell and 17 (6.8%) non-germ cell tumors. Reexamining the histology of the last 123 patients only 29.3% had pure seminomas and 70.7% had non-seminomatous tumors. The results of regimens of combined surgical (extended semicastration and lymphadenectomy) and chemotherapeutic (triple-drug as well as Ifosfamid in combinations with low-dose radiafor stage II (37.5%) and stage II (16.7%). There were good results after 96 transperitoneal bilateral lymphadenectomies. But even in stages II and III results were achieved which would not have been possible without lymphadenectomy. It is pointed out that in the future, with improvements in cytostatic therapy, even better results including treatment of stage IV tumors can be expected.

Renal hamartoma: therapeutic aspects.

Hamartomas are benign renal tumors. In most cases they can be diagnosed by means of angiography. Treatment varies in accordance with clinical manifestation and progression. Symptom-free hamartomas need no treatment. In case of bleeding, embolization may be performed. Nephrectomy ought not to be performed save in emergency or if malignancy is suspected.

[Central register and combined study of testicular tumors (author's transl)].

To successfully combat cancer, research needs to be based on prospective investigations and central collation of data. An initial report is presented concerning one such investigation that has been in progress since September, 1976: "central register and combined study of testicular tumors--Bonn." The aims are to collate epidemiologic data, achieve a standardization of the stages of the cycle of the seminiferous epithelium and of classification, on the model of the WHO, as well as to attain maximum efficiency of diagnosis and therapy. Fourteen participating clinics in the German Federal Republic, Austria, and Switzerland, and 70 submissions of data up to the 1st of March, 1977, amply justify the efforts so far expended and encourage us to solicit the cooperation of previous nonparticipants.

[Coral calculus in the cystic kidney (author's transl)].

A clinical case is described featuring bilateral lithiasis of the renal pelvis and calices in conjunction with cystic kidney in a female patient. Progression was shown by tumor enhancement in the lower abdomen, a distension of the concretions in pyelogram, and an increment of creatine in the serum. In this case there was no indication for surgery.

[Sisomicin treatment of urinary tract infections (author's transl)].

In a study with 30 randomized patients the pharmacokinetics, efficacy and side effects of Sisomicin were determined. Half of the patients received one single intramuscular injection of 75 or 100mg (1.4 mg/kg) Sisomicin/day. The other cases were injected 50 or 75mg (1.9 or 2.1 mg/kg) b.i.d. The serum concentrations determined on the first and last days of treatment 1 hour after the application ranged between 4 and 5.3 mug/ml. After 6 hours the values decreased to 0.7-0.8 mug/ml. The elimination rate within 24 h amounted to 63-77%. Depending on the dosage group, the mean urinary concentrations/24 h amounted to 46-51 mug/ml or 63 to 92 mug/ml, respectively. The highest concentration measured during the first 8 hours amounted to 129 mug/ml after an administration of 1.4 mg/kg. In 18 of 29 cases a "cure" of urologic infections, most of which had been treated previously without success, could be achieved. Furthermore we observed 5 relapses, 3 failures and 3 reinfections, but no superinfection. The local and general tolerance of Sisomicin was good. 5 patients showed toxic reactions of the vestibular organ in the form of dizziness. In 1 case there was an excretion of granulated casts.

Quantitative parameters for light microscopic assessment of the tubuli seminiferi.

For quantitative, histologic assessment of the testis tubuli there are four suitable parameters: diameter of tubulus, area of tubulus, number of cell nuclei per tubulus, and area of nuclei of the various cells participating in spermatogenesis. For estimating areas, the integrative method of measurement, either with a grid or with the aid of the manual optical picture-analysis system, is proposed. The tubulus diameter can be determined rapidly and easily, although the tubulus size can be assessed more accurately by area estimation. An assessment of reproductive capacity in detail is possible by means of rather time-consuming determination of cell nucleus number and nuclear area, whereby the stages of the cycle of the seminiferous epithelium must be considered.

Stages of the cycle of the seminiferous epithelium in the dog.

In principle, the spermatogenesis of the dog resembles that of other animals. It consists of 6 consecutive, differentiating spermatogonia generations (A1-A4, Im, B). The number of ensuing spermatogenetic cell generations corresponds to that of other mammals. We divided the spermatogenesis of the dog into 10 stages in order to attain a precise systematization. The cell-divisions of the successive spermatogonia occur in stages II, III, V, VI, VII, and VIII. Stages V a and V c are characterized by the meiotic divisions.

[The simultaneous management of rupture of the urethra and the symphysis (author's transl)].

The combined injury-rupture of the urethra and the symphysis--is no respecter of the surgical disciplines, urology and accident surgery. Therefore, a combined operating team from both specialties should simultaneously undertake primary care. First comes the restoration of urethral continuity. The retropubic approach favors the gaping pubic symphysis. A stationary osteosynthesis of the symphysis with an AO-tension band plate follows. In that way the urethra and its suspension apparatus are brought into the right position. Immobilization damage is avoided through exercise stability which allows early maximum stress of the patient. A persisting instability of the anterior pelvic girdle through shearing force is a risk in reconstructive urethral surgery. Simultaneous primary care is contraindicated in cases of irreversible shock and life-threatening injury where the need for surgery is urgent.

Effect of human chorionic gonadotrophin on the dog seminiferous tubule, with and without experimental unilateral cryptorchism.

The effect on human chorionic gonadotrophin (HCG) on the testes of pubertal and postpubertal Beagle dogs was studied, in the one instance in bilaterally normotopic testes and in the other on the still scrotally located gonad of dogs which had been subjected to unilateral experimental cryptorchism. The area of the cross-sectioned seminiferous tubules served as a parameter for evaluating the effect of HCG. Administration of therapeutic doses of HCG produced, in animals with bilaterally normotopic testes, a marked diminution of the area of the seminiferous canals. This means that HCG application at normal dosage impairs the seminiferous tubules in the dog. In animals with unilateral cryptorchism, HCG application produced no significant change in the tubule area of the scrotally located testis beyond that which is ascertainable anyway on the orthotopic testis following translocation of the contralateral gonad to within the abdominal cavity. Hence, HCG does not influence the degenerative change in the orthotopic testis in experimental unilateral cryptorchism. Rather, an adverse effect of HCG on the seminiferous tubule is evident, independently of the hitherto published reports of successfully attained descent.
