Added value of automated clinical proton MR spectroscopy of the brain.

OBJECTIVE: A trial was conducted to establish the added diagnostic value of an automated proton MR spectroscopy (MRS) examination (PROBE). MATERIALS AND METHODS: The PROBE and MRS were compared for metabolite ratios of normal controls and 21 patients. In addition, PROBE was performed in either the occipital cortex (gray matter) or the parietal cortex (white matter) or, more rarely, within the confines of a focal lesion identified on MRI, using a GE Signa 1.5 T whole-body scanner, in 112 patients undergoing routine brain MRI. The trial was conducted in three different MR centers to establish percentage of positive findings with MRI vs. MRI plus MRS. RESULTS: Cerebral metabolite ratios (N-acetylaspartate/creatine, choline/creatine, myo-inositol/creatine) obtained by PROBE and MRS were similar. Metabolite profiles in dementia, head trauma, herpes encephalitis, hepatic and hypoxic encephalopathy, stroke, and tumor were identified using PROBE. The PROBE technique increased the number of positive findings ("added value") achieved by MRI; the added value was 28, 21, and 93% for the three trial sites. CONCLUSION: With only minor variations, PROBE reproduces the cerebral metabolite patterns obtained with MRS. It significantly increases the diagnostic yield of routine neuroimaging and might be incorporated as a standard sequence in a cost-effective manner.

The role of ultrasound in breast imaging.

Sonography has become an important tool for evaluating breast abnormalities, including inflammatory processes, hematomas, ductal ectasia, cysts and solid lesions. This article describes protocols for ultrasound imaging of the breast and the sonographic appearance of common breast conditions, with particular attention to differentiating benign from malignant lesions. Ultrasound's role in interventional breast procedures and radiation therapy planning also are discussed.

Breast cancer: diagnosis, treatment and prognosis.

This article reviews staging of breast cancer and treatment protocols for the disease, then examines several different types of breast cancer. Each type is described in terms of its mammographic, sonographic and histologic appearance, along with its treatment and prognosis. The author concludes with a discussion of magnetic resonance imaging's role in breast cancer detection and treatment planning.

Immunohistochemical localization of proliferating cells, basic fibroblast growth factor and Fos protein following iliac artery balloon angioplasty in hypercholesterolemic rabbits.

The restenotic process in iliac arteries of hyperlipidemic rabbits following balloon angioplasty was evaluated in a temporal sequence by monitoring smooth muscle cell (SMC) proliferation (proliferating cell nuclear antigen -PCNA marker), growth factor (basic fibroblast growth factor -bFGF) production, and Fos oncogene protein expression using immunohistochemical methods. The percutaneous transluminal angioplasty was performed in iliac artery of rabbits fed a 1% cholesterol diet, with a balloon catheter inflated from 4 to 8 atm for 2 min under cineangiographic control until an acceptable (at least 50% stenosis reduction) result was obtained. The morphometric analysis indicated that the number of PCNA-positive cells in contralateral control arteries was low, but was increased in balloon-injury. The bFGF-positive labeled cells were localized in the media and in recently-migrated SMCs within the intima. Two days following angioplasty these cells were more abundant and scattered in the entire neo-intima. After 14 days their number was still higher than that of the controls. Cellular Fos protein expression was detected in the intima and, to a lesser extent, in the media 2 hours after angioplasty. These data suggest that early expression of Fos protein and bFGF in iliac arteries of hyperlipidemic rabbits following balloon angioplasty could be involved in development of restenosis in this animal model.
