Cerebral amyloid angiopathy with dementia: clinicopathological studies of 17 cases.

OBJECTIVES: To study clinicopathologically 17 cases of cerebral amyloid angiopathy (CAA) with dementia and to investigate the possible neuropathology of this disorder. METHODS: Of 362 autopsy cases, 17 who had had dementia with proved CAA were studied. Hematoxylin and eosin, Congo red, Bodian and beta/A4 protein immunohistochemical stain were used. Brain sections were observed under light microscopy and polarized light microscopy. The amyloid was confirmed by positive Congo red stain with apple-green birefringence under polarized light and positive beta/A4 immunohistochemical stain. The severity of amyloid deposit was graded as slight, moderate and severe. RESULTS: In the 17 cases of CAA associated with dementia, Alzheimer's disease was found in 3 cases, Pick's disease in 1, multi-infarct dementia in 6, multi-hemorrhage dementia in 4, and pure CAA in 3. CONCLUSIONS: CAA not only is closely related to Alzheimer's disease, but also causes cortical multiple small infarcts and hemorrhagic lesions, which lead to dementia. Moreover, pure CAA may produce dementia through hypoperfusion of the cortex by CAA-related hyalinization and stenosis of the arteries, which lead to the loss of cortical neurons.

[Repairing the defects of volar surface of distal finger with a reverse insular fascial flap of dorsum of juxta positioned digit].

OBJECTIVE: To introduce a method for one-stage repair of skin defects of the volar surface of distal finger or fingers. METHODS: The reverse fascial flap was designed and created on the dorsum of the juxta positioned digit, and it was then rotated 180 degrees to cover the defects of the volar surface of the distal finger. Since May 1992, a total of 20 reverse fascial flaps were used to treat 14 patients. RESULTS: All flaps survived except one with partial necrosis. Follow-up of one half to one year showed satisfactory results functionally and cosmetically. The finger is plump but not swollen. CONCLUSION: The method is easy and effective. The surgical treatment requires short time and causes little pain. The flap has good blood supply, suitable thickness, softness and resistance to wear and tear.

[Analysis of the protein of Haemophilus influenzae].

OBJECTIVE: To understand Haemophilus influenzae special protein and to help for preparing Haemophilus influenzae Vaccine. METHODS: Proteins from isolates of Haemophilus influenzae were examined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). RESULTS: 23 isolates were from nasopharyngeal secretions of children with pneumonia and 33 from pharyx swab of healthy children. Conventional tests showed that 79% of fifty-six strains of Haemophilus influenzae were non-serotypable and all belonged to one of eight biotypes. The common biotypes were II (48%). All strains contained more than 10 proteins with molecular weights of 96,000, 86,000, 55,000, 51,000, 46,000, 40,000, 39,000, 31,000, 29,000, 25,000, and 16,000. Among them, three were major proteins (31,000, 51,000 and 96,000) and different proteins were mainly between peptide 31,000 and 51,000. A possible association between biotype and protein was unclear. CONCLUSION: Analysis of protein of Haemophilus influenzae is helpful for epidemiological study of non-serotypable Haemophilus influenzae and making Haemophilus influenzae vaccine.

Effect of WT1 gene expression on cell growth and proliferation in myeloid leukemia cell lines.

OBJECTIVE: To investigate the effects and mechanism of Wilms' tumor (WT1) antisense oligonucleotides (AS-oligomers) on proliferation and apoptosis in myeloid leukemia cell lines. METHODS: K562 and HL-60 cells were cultured in presence of WT1 oligomers. Both cell lines express WT1 gene with no p53 protein expression. Cells growth, apoptosis and expression of WT1, bcl-2 genes were analysed using 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenylmetrazolium bromide (MTT) colorimetric assay, flow cytometry and reverse transcription-polymerase chain reaction (RT-PCR) methods. RESULTS: WT1 antisense oligonucleotides inhibited cellular proliferation of K562 cells and the effect was concentration-dependent. When cultured at concentration of 200 micrograms/ml oligomers, growth inhibition was 46.2% for antisense oligonucleotide cultivated group and 28.1% for sense oligonucleotide cultured group (P = 0.008) respectively. WT1 antisense oligonucleotide can induce apoptosis of K562 and HL-60 cells. Percentages of apoptotic cells in antisense oligonucleotide and sense oligonucleotide treated groups were 30.88% versus 13.62% for K562 cells and 40.15% versus 4.23% for HL-60 cells. However the growth of HL-60 cells and expression of bcl-2 gene were unaffected. CONCLUSIONS: The WT1 gene is related with proliferation and apoptosis of leukemic cells. Effect of anti-apoptosis may be independent of the cellular p53 status and bcl-2 expression. WT1 gene may play an important role in leukemogenesis.

[The relation between polymorphisms of apolipoprotein B gene and atherosclerotic cerebral infarction].

OBJECTIVE: To explore the association of variance at XbaI site of apolipoprotein B (ApoB) gene and atherosclerotic cerebral infarction (ACI) in Chinese Han population. METHODS: Using polymerase chain reaction (PCR) techniques, we studied the restrict fragment length polymorphism (RFLP) at XbaI site of ApoB gene in 150 patients with ACI and 301 healthy age, sex-matched individuals from a population of Chinese Han nationality in Beijing. RESULTS: In both ACI group and control group, X-X- genotype was the most frequent one (frequency: 0.907, 0.948) and we did not find X+X+ genotype. The distribution of genotypes in the two groups was at the Hardy-Weiberg equilibriums. The frequency of rare allele X+ was significantly lower in Chinese Han than that reported in Caucasians (0.027 vs 0.418, 0.454, 0.479, P < 0.01). The higher frequency of rare allele X+ was found in the ACI group as compared with the control group (0.053 vs 0.027, P < 0.05). markedly increased levels of TC (5.3 +/- 1.3), (4.9 +/- 1.3) mmol/L (P < 0.05), and low levels of Apo-AI, Apo-AI/ApoB in the ACI group were observed (1.02 +/- 0.34), (1.26 +/- 0.40) g/L, (P < 0.01); X+X- genotype was associated with higher levels of ApoB compared with the levels of X-X- genotype in the ACI group (0.89 +/- 0.29) g/L, (0.78 +/- 0.17) g/L (P < 0.05). CONCLUSION: X+ allele of ApoB gene may be associated with ACI to some extent in the Chinese population, and presumbly through its effect on ApoB metabolism it increases the susceptibility to ACI.

[Effect of T lymphocyte in the acute rejection of liver xenograft].

OBJECTIVES: To study whether T cell take part in the acute rejection of liver corcondant xenograft. METHODS: The orthotopic liver transplantation model introduced by Hariharas was established. In grafts, CD4, CD8 and Fas-L were examined by immunohistochemistry at day 1, 3, 5, 7 post-transplantant respectively, and apoptosis in grafts was observed by histology and TUNEL (Terminal Deoxynucleotidyl Transferse-mediated dUTP nick-end Labelling,). RESULTS: In xenogenic grafts, T cell infilitration occurred at day 3, peaked at day 5, including CD4, CD8 subsets. The expression of Fas-L was found at day 4, peaked at day 5-6. The more severe the acute rejection is, the more the quantities of the expression of Fas-L is, the more the quantities of apoptosis is in grafts. CONCLUSIONS: T lymphocytes participate in the acute rejection of liver xenograft.

[Bolus administration of esmolol for preventing the haemodynamic response to tracheal intubation: a multicentre clinical study].

OBJECTIVE: To explore the dose-response relation and the safety of esmolol administered as a single i.v. bolus prior to the induction of anesthesia for preventing the haemodynamic response to tracheal intubation. METHODS: 1,830 patients from 20 centres were randomly divided into three groups: E1 group, receiving esmolol at a dose of 1 mg/kg; E2 group, at 2 mg/kg and E0 group, receiving 0.9% NaCl. Heart rate, systolic and diastolic, and mean artery blood pressures (MAP) were measured following the induction and 1 to 10 minutes following the intubation. RESULTS: The patients in the E0 group had greater HR and MAP values after anesthesia induction and tracheal intubation than the patients in the E1 and E2 groups (P < 0.05-0.01). The E1 group had higher HR than the E2 group (P < 0.05-0.01). The incidence of tachycardia after intubation was higher in the E0 group (63.8%) than in the E1 group (34.7%) and the E2 group (22.6%) (P < 0.01). The proportion of hypotention and bradycardia was higher in the two treatment groups than the E0 group (P < 0.01), but no severe adverse event was observed. CONCLUSION: A 1-2 mg/kg bolus of esmolol is effective and safe for preventing the haemodynamic response to tracheal intubation. The clinical and side-effects are all dose-related.

Effect of Astragalin on matrix secretion and beta 1 integrin mRNA expression in human mesangial cells.

OBJECTIVE: To investigate the effect of Astragalin on human renal mesangial cells. METHODS: Cultured human mesangial cells were treated with Astragalin and Astragalin serum in different concentrations in the presence or absence of PDGF-BB, the proliferation and type IV collagen secretion of mesangial cells were measured by MTT assay and ELISA, and expression of beta 1 integrin gene was estimated by reverse transcription-polymerase chain reaction (RT-PCR) method, suspectively. RESULTS: After 72 hours Astragalin or Astragalin serum treatment, the proliferation of mesangial cells induced by PDGF-BB was inhibited significantly in a dose-dependent manner compared with untreated controls (P < 0.05 and P < 0.01). After 24 hours of Astragalin or Astragalin serum treatment, the secretion of type IV collagen protein in presence of PDGF-BB was significantly decreased and beta 1 integrin mRNA level decreased significantly compared with untreated control (P < 0.05, P < 0.01). CONCLUSIONS: Astragalin inhibits cell proliferation and matrix over-synthesis which might be mediated, at least, partly by decrease of beta 1 integrin gene over-expression. The study suggested that Astragalin might play a role in preventing the progression of chronic renal diseases.

Evi-1 and MDS1-Evi-1 genes in pathogenesis of myelodysplastic syndromes and post-MDS acute myeloid leukemia.

OBJECTIVE: To investigate expression of Evi-1 and MDS1-Evi-1 genes in myelodysplastic syndromes (MDS) and post-MDS acute myeloid leukemia (post-MDS AML), and its role in pathogenesis or progression of MDS and post-MDS AML. METHODS: Expression of Evi-1 and MDS1-Evi-1 genes was examined in 31 MDS, 11 post-MDS AML, and 34 de novo AML patients by a semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: Evi-1 expression was not detected in bone marrow samples of 8 normal controls, but low MDS1-Evi-1 expression levels (MDS1-Evi-1/GAPDH < 0.1) were detected in 3 of the 8 controls. Evi-1 RNA was expressed in 1 of 8 RA, 8 of 13 RAEB and 6 of 9 RAEB-T patients, and the percentage of Evi-1 expression in RAEB(T) patients was higher than that in RA (P < 0.05). MDS1-Evi-1 expression was detected in 5 of 8 RA, 9 of 13 RAEB and 5 of 9 RAEB-T patients, and MDS1-Evi-1 expression levels (MDS1-Evi-1/GAPDH > 0.1) were markedly higher than those in the controls. Evi-1 expression was gradually increased in 4 of 5 RAEB-T patients with transformation from MDS to AML. The percentages of Evi-1 and MDS1-Evi-1 expression in post-MDS AML patients were significantly (P < 0.01 and P < 0.05 respectively) higher than those in de novo AML. The colonies of hematopoietic progenitor cells were decreased in Evi-1 and MDS1-Evi-1-positive MDS patients as compared with those in Evi-1 and MDS1-Evi-1-negative patients. CONCLUSION: Abnormal expression of the Evi-1 gene and overexpression of MDS1-Evi-1 gene may play a role in the pathogenesis or progression of MDS and post-MDS AML.

[Cisplatin plus 5-fluorouracil combination therapy in the treatment of advanced esophageal cancer: a report of 156 cases].

OBJECTIVE: To evaluate the results of combination chemotherapy with cisplatin (DDP) plus 5-fluorouracil(5-FU) in the treatment of advanced esophageal cancer. METHODS: From 1988 through 1997, 156 patients with advanced esophageal cancer were enrolled in this study. The median age of the patients was 60. Squamous-cell cancer was the most common type of malignancy. All but 16 patients received no previous chemotherapy. RESULTS: Complete response was observed in 26 patients and partial response in 71 patients, with an overall response rate of 62.2%. The response appeared to correlate with dose intensity, as 65.7% response rate was achieved with a dose of DDP 75-100 mg/m2 versus 43.7% response rate with DDP 50-70 mg/m2. The median response duration was 4.5 months and the median survival period was 8.2 months. Nausea/vomiting, neutropenia and phlebitis were the main adverse reactions in all treated cases. CONCLUSION: A high response rate was obtained in advanced esophageal cancer treated by DDP plus 5-FU with tolerable toxicity.

[Radiotherapy for early anal cancer: a report of 27 cases].

OBJECTIVE: To evaluate the results of radiotherapy for early stage anal cancer. METHODS: During 1960 through 1993, 27 patients with stage T1-2N0 anal cancer were treated by radiotherapy alone. Seven cases received pelvic irradiation plus local field boost, 20 cases received local field irradiation alone. Kaplan-Meier method was used in survival analysis. RESULTS: The overall 5-year survival rate in this series of patients was 79.1%. Eighteen of the 27 cases had their anal functions preserved. Local recurrence occurred in 7 cases and regional lymph node metastasis in one case. Five of them were salvaged, two by surgery and three by irradiation. CONCLUSION: Radiotherapy alone for early anal cancer is effective. Anal function is preserved in about two-thirds of the patients so treated. Local treatment failure can be salvaged by surgery or radiotherapy.

[Surgical treatment of esophageal leiomyosarcoma: a review of the literature and report of 11 cases].

OBJECTIVE: To discuss the clinical biocharacteristics, diagnosis, operation and prognosis of esophageal leiomyosarcoma (ELS). METHODS: Clinical materials and follow-up results of 11 surgically treated ELS patients were analyzed. RESULTS: All 11 cases received radical resection of tumor. The 5-year survival rate was 54.5% (6/11). Grossly, two types of ELS were observed: the polyp form (4 cases) and invasive form (7 cases). The 5-year survival rate was 75.0% and 42.9%, respectively. Two patients died of local recurrence, another 2 cases died of distant metastasis. CONCLUSION: In patients with ELS surgically treated, prognosis of polyp form ELS is better than that of invasive form ELS. Local recurrence and distant metastasis are the major cause of death.

[Clinico-pathological characteristics and prognosis of thymoma: an analysis of 159 cases].

OBJECTIVE: To investigate the clinicopathologic features of thymoma and assess prognostic factors. METHODS: Data of 159 patients operated for thymoma were collected. A retrospective analysis was performed, through comparison of survival rate computed by the actuarial method and rate of recurrence and metastasis. RESULTS: (1) The 3-, 5-, and 10-year survival rate was 78.9%, 63.7% and 46.3%, respectively. All patients treated by subtotal thymoma resection with postoperative radiotherapy survived more than 3 years. (2) Patient's age, location and image density of tumor play important role in differential diagnosis. (3) Tumors with epithelial predominance were more frequently seen in late stage than those of other histologic types(P < 0.01). (4) The prognosis of patients whose tumors had incomplete or no capsule was poor as compared to that of stage I tumor (P < 0.05). (5) According to multivariate analysis, the most important prognostic variables included completeness of resection (OR = 2.10) and clinicopathologic stage (OR = 1.73). Myasthenic crisis was the most important factor influencing operative death. Sex, symptoms and histologic classification did not influence prognosis. (6) Recurrence and metastasis occurred in all stages, but more frequent in the later stages of the disease. CONCLUSION: Complete resection helps decrease recurrence of thymoma. Prognosis of patients receiving palliative resection of thymoma may be improved by postoperative radiotherapy.

[Cloning of differentiation-related genes induced by all-trans retinoic acid(ATRA) from human lung cancer GLC-82 cell line].

OBJECTIVE: Cloning of differentiation-related genes induced by all-trans retinoic acid(ATRA) from human lung cancer GLC-82 cell line. METHODS: The difference in gene expression between human lung cancer cell GLC-82 before and 8, 24 hours and 4 days after ATRA treatment was investigated by an improved protocol of mRNA differential display with SYBR Green I, a highly sensitive fluorescent stain. RESULTS: There were obvious differences in gene expression between human lung cancer cells before and after ATRA treatment. The expression of some genes was decreased or inhibited while that of other genes was activated temporarily or persistently. Three cDNA fragments of the differentially expressed genes were cloned and analyzed. CONCLUSION: ATRA plays an important role in regulating the differentiation-related gene expression. ATRA-induced differentiation is a complex process with multiple genes involved.

[The changes of transforming growth factor beta 1 in periodontal tissue during orthodontic tooth movement].

OBJECTIVE: To observe the distribution and change of transforming growth factor beta 1 (TGF-beta 1) in periodontal tissue and understand the role of TGF-beta 1 in orthodontic tooth movement. METHODS: Apply force on right maxillary first molars in rats, and examine the decalcified alveolodental connected tissue in different stage of tooth movement with immunohistochemical staining technique (SP method). RESULTS: TGF-beta 1 exists in normal periodontal tissue. The expression of it in periodontium increases during orthodontic tooth movement. Especially from 5 to 10 days after force application, the expression at tension side is much more than those at pressure side. CONCLUSION: TGF-beta 1 plays an important role in remodeling of periodontal tissue during orthodontic tooth movement, and can powerfully promote bone formation.

[Observation on left ventricular remodeling in acute myocardial infarction].

OBJECTIVE: After an acute myocardial infarction (AMI), there will be regional dilation and global remodeling of the infarcted left ventricle. This study was aimed to observe the features of the ventricular remodeling after AMI. METHODS: Based on the time of onset of the symptoms, 37 patients (pts) with AMI were divided into 4 groups. The time of onset of AMI for group 1 (16 pts), group 2 (7 pts), group 3 (11 pts) and group 4 (3 pts) was < or = 3 hours (hrs), > 3 hrs - < or = 6 hrs, > 6 hrs - < or = 12 hrs and > 12 hrs respectively. The size of the left ventricle in all the patients were analysed with resting myocardial gated SPECT (GSPECT). Tc-99m MIBI was infused at admission and data of GSPECT were collected 2 hours later following the infusion. End-diastolic volume (EDV), end-systolic volume (ESV) and LVEF were shown by the system. Stroke volume (SV) was then calculated. RESULTS: EDV for group 1 to 4 was (98.5 +/- 31.9) ml, (99.4 +/- 55.6) ml, (128.1 +/- 55.1) ml and (140.0 +/- 25.3) ml, respectively. ESV for group 1 to 4 was (57.0 +/- 30.9) ml, (55.1 +/- 40.3) ml, (77.4 +/- 39.8) ml and (81.3 +/- 26.0) ml, respectively. SV for group 1 to 4 was (57.0 +/- 30.9) ml, (55.1 +/- 40.3) ml, (77.4 +/- 39.8) ml and (81.3 +/- 26.0) ml, respectively. LVEF for group 1 to 4 was (44.6 +/- 13.1) ml, (49.7 +/- 13.1) ml, (42.8 +/- 13.5) ml, and (42.7 +/- 8.6) ml, respectively. In pts with anterior infarction, EDV, ESV and SV after 6 hrs of the onset of AMI were greater than those in pts with in 6 hrs of the onset. It was the same in pts with inferior infarction. CONCLUSION: It is suggested that the left ventricular size increased in 3 hrs after AMI. Following the initial dilation of left ventricle, EDV, ESV and SV did not change significantly with in 6 hrs, but then increased gradually after 6 hrs from the onset of the symptoms. The early and significant dilation of left ventricle after AMI may be one of the risk factors of severe cardiac events.
