Fournier's classical description of infective genital gangrene has been frequently amplified. We present seven patients in whom the exact site of gangrene was related to the origin of the infection, either from the urethra or from the anorectum. A sixth case of diabetes presenting as Fournier's gangrene is described. The need for urological assessment in this condition is discussed.

Because play is extremely sensitive to environmental conditions, extended hospitalization may have adverse effects on normal play development in young children. This study compared the playfulness, as well as the level of play development, of three 2-year-old children who had been hospitalized most of their lives for tracheostomies, and three 2-year-olds living at home. Data on the children's play were gathered by videotaping in two standardized play settings and one free play setting. Statistically significant differences in the developmental level of play and in playfulness (i.e., the degree of liveliness and joy exhibited) were found between the two groups in all three settings, and the play age of all six children varied by setting. Quantitative data analysis was supported by qualitative findings. Although the differences between the groups cannot be conclusively attributed to hospitalization alone, certain features of the hospital environment appear to have hampered the development of play.
The induction of IgG synthesis by normal human lymphocytes in response to a range of B-cell activators has been studied by indirect cytoplasmic immunofluorescence using monoclonal antibodies to the four IgG subclasses. Considerable variation was found between cells from different donors tested with the same activator but the following trends were observed:- Epstein-Barr virus and Branhamella catarrhalis induced a predominantly IgG3 response, whereas the most prevalent cells in cultures stimulated with lipopolysaccharide and Staph. aureus were IgG2-positive; the subclass distribution obtained with pokeweed mitogen resembled that found in normal serum (IgG1 greater than 2 greater than 3 greater than 4).
Progressive systemic sclerosis is a disease of uncertain etiology associated with vascular/collagen tissue changes that may affect only the skin (scleroderma) or any and all internal organs (PSS). A disease primarily of Caucasian females, PSS is usually manifested in the fourth to sixth decade of life. Clinical and laboratory diagnostic testing is often confused by overlap phenomena, but recent research has increased diagnostic specificity. There is no known cure for PSS. Nevertheless, the knowledgeable clinician has an important role to play in the PSS patient's future. Symptom management and function maintenance will require an ongoing and understanding relationship between practitioner and patient.

1H, 2H, and 31P NMR methods have been employed in the study of dimyristoyl lecithin bilayers hydrated with D2O in the gel (L beta'), intermediate (P beta') and liquid-crystalline (L alpha) phases. For D2O/lipid molar ratios, n, in the range 7 less than or equal to n less than or equal to 11 discontinuities are observed in the deuterium NMR splittings at both main and pretransitions. A partial phase diagram based on NMR and differential scanning calorimetry data is presented. 1H NMR dipolar splittings are observed for macroscopically oriented samples in all three phases. Changes in the 1H splittings are correlated with 2H and 31P data and interpreted to show that the chain tilt in the gel phase undergoes a discontinuous change on transition to the intermediate phase, which brings the chain axes closer to the bilayer normal. An estimate of chain tilt in the gel phase is made on the basis of NMR data and found to be approximately 23 degrees for a sample with n = 11 at 18 degrees C.
The importance of the incorporation of microbiology information into the laboratory cumulative report implies that continuing efforts need to be made to enhance the readability and style of such reports. In addition, the optimizing of laboratory work flow around a computer record-keeping system in microbiology is essential to guarantee technologist satisfaction with and usage of the system. The efforts and results of extending a commercially available system in the microbiology laboratory are discussed and compared with another approach using the same system base. The project led to smooth laboratory work flow and highly successful combined reports. Necessary software modifications are summarized.
Factor VII inhibitor activity was identified in a 66 year old man with probable bronchogenic carcinoma. Initial studies indicated a normal partial thromboplastin time (PTT) and a prothrombin time (PT) prolongation which increased with incubation. PT correction failed to occur with 50 per cent normal plasma in vitro and 2 U of fresh frozen plasma in vivo, indicating inhibitor activity. Specific factor assay levels fell within normal limits except for a factor VII level of 30 per cent. Subsequent evaluation with antiglobulin studies suggested an inhibitor of immunoglobulin G [IgG] origin.

The serum of male squirrel monkeys was analyzed for levels of thyroxine (T4) and triiodothyronine (T3) at intervals corresponding to (1) preparations for breeding, (2) termination of breeding, and (3) well after the breeding season. Highest concentrations of hormones were found prior to breeding when body weight was also highest. Significant differences in hormone levels were found across sampling period. The interaction between gonadotropins and thyroid hormones is discussed.
A survey of nearly 250,000 citizens of Georgia and South Carolina conducted during the past twenty years has led to the detection of over 40 abnormal hemoglobins and several additional hemoglobinopathies. The presence of some of these hemoglobin abnormalities cause (severe) clinical symptoms but others remain undetected unless a specific search is initiated. The incidence of Hb S varies slightly among the populations of different areas, and appears to be the highest in the coastal counties of Georgia and South Carolina. A survey of over 17,000 persons of mainly high school and college age has shown that a significant number of cases with clinically significant hemoglobinopathies will remain undetected unless such surveys are actively promoted.
