During diabetic ketoacidosis, in 17 adult patients, significant decreases in serum TBG and total T4 levels were observed, without significant alteration of the T4 to TBG binding property. In addition, serum free T4 (FT4) was moderately elevated and the TSH response to TRH was markedly blunted. No correlation, however, was found between TSH blunting and FT4 elevation. Correlation of these serum anomalies required at least 5 days of adequate control of the diabetes. Thus, diabetic ketoacidosis in euthyroid patients is characterized by multiple alterations in thyroid function parameters and caution is recommended in the interpretation of thyroid tests during and in the days following this severe metabolic disorder.
A 50 year old man is described in whom primary hypothyroidism and isolated ACTH deficiency leading to adrenocortical insufficiency occurred simultaneously. Low thyroid hormone levels, elevated serum TSH values and high titers of antithyroid antibodies (ATA) were consistent with primary hypothyroidism. The diagnosis of secondary adrenocortical insufficiency was based on low cortisol and ACTH morning levels, low urinary steroids, a significant increase of cortisol levels after 0.25 mg tetracosactide iv, an absence of ACTH and cortisol release after insulin-induced hypoglycemia and a similar absence of ACTH and 11-deoxycortisol increase after metyrapone administration. After one year treatment and five days withdrawal of cortisol administration, the insulin-induced hypoglycemia still failed to elicit secretory responses of ACTH and cortisol, despite the normalization of thyroid hormone levels. The etiopathogenesis of these two coexisting endocrine deficiencies is discussed.
IGF-I and II genes expression has been localized by in situ hybridization in testis and epididymis of immature hypophysectomized rats treated in vivo with either pFSH, hLH, bGH, hPRL or with saline. IGF-I mRNA expression was found in both Sertoli and Leydig cells after treatment with either FSH or LH. IGF-I mRNA was highly expressed in germ cells after FSH stimulation and to a lesser extent after GH or LH treatments. However, its expression was very low in hypophysectomized control or PRL treated rats. IGF-I mRNA was also expressed in stromal cells of epididymis after LH treatment and to a lesser extent after GH stimulation. In contrast, IGF-II mRNA expression was detected in all testicular cell types whatever the hormonal treatment (FSH, LH, GH, PRL). For each hormonal treatment testicular sections were examined after immunohistochemical staining with specific antisera against IGF-I and IGF-II. Both in situ hybridization and immunohistochemical data were examined in order to determine the testicular sites of synthesis of IGF-I and IGF-II.

PRL and TSH were measured by radioimmuno-assay in 14 persons wth myxoedema aged 16 to 65 years. Three of them had galactorrhea. In 13 patients the circadian rhythm of PRL was studied, and in 8 patients PRL was measured after TRH administration. All these patients have a high value of serum TSH, with a mean value +/- SEM of 29,4 (24,3 - 35,6) mU/ml. Values of serum PRL were comprised in normal ranges with a mean value +/- SEM of 126(196 - 238) mU/ml. The circadian rhythm of PRL is not altered, since mean values of serum PRL are more elevated at 4 am than 8 am with highly statistical significance. After THR administration the PRL response is normal or exaggerated.
