[A surgical treatment method in syringomyelia].

The author provides pathogenetic evidence for surgical treatment of syringomyelia. Based on the experience gained with surgical treatment of syringomyelia, gives a critical assessment of the hydrodynamic theory of the formation of intraspinal cavities. Provides the data obtained during examination of 127 patients suffering from genuine syringomyelia with the aid of cystography and NMR tomography. The communicating and non-communicating disease pattern were established. The short- and long-term results of surgical treatment were elucidated in 109 patients operated on. It is assumed that derangement of hydrodynamics plays a definite part in the disease pathogenesis. The hydrodynamic alterations include not only circulatory disorders but also have the signs of general functional insufficiency of the CSF system. The surgical treatment method is aimed at the correction of the hydrodynamic alterations, being one of the constituent parts in the whole complex of treatment measures for syringomyelia.

[Effects of low-intensity laser irradiation on the state of blood proteins].

The action of a low-intensity 632.8 nm-wavelength laser radiation on blood elements was studied. The degree of binding the fluorescence probes 1.8 ANS by blood proteins and cells was taken to be the index of the response to irradiation. The change of fluorescence of the oppositely charged probes indicates that a He-Ne laser radiation causes processes leading to a change of charge in proteins of a blood plasma with the negative charge decreased in most cases. The effect depends on individual characteristics of a biological object, on the exposure dose and the incubation time following the irradiation.

[Clinical picture and early diagnosis of craniospinal tumors].

The authors analyzed 32 cases of verified craniospinal tumors (CST) which were observed at a neurosurgical department over the last decade. These tumors were subdivided into neurinomas (n = 14), meningiomas (n = 11) and gliomas (n = 7). All patients underwent surgery. The pathogenesis of symptoms observed in CST is discussed. Clinical symptoms of importance for the early diagnosis of craniospinal tumors are outlined. When CST is suspected the recommended procedures include analysis of the CSF, X-ray examination of the cervical portion of the vertebral column, ascending positive myelography and computer-aided tomography of the cranium and vertebral column.

[X-ray diagnosis of cranio-vertebral anomalies].

On the basis of an analysis of the radiological data of 66 patients with craniovertebral anomalies the following forms of them were distinguished: 1) basilar impression; 2) assimilation of the atlas; 3) coarctation of the atlanto-occipital segment; 4) anomaly of the C1-dens. The main radiological signs revealed in the patients with different forms of craniovertebral anomalies with the aid of craniometric techniques and linear and angular indices are presented. The importance of tomography and myelography in the diagnosis of medullar compression and of its concomitant anomalies is emphasized, pneumomyelography being preferred against positive myelography. A combined evaluation of the neurological and radiological data permits to determine the indications for surgery of congenital craniovertebral pathology.

[Electrostimulation of the phrenic nerves in neural resuscitation].

Electrostimulation of the phrenic nerves (ESPN) on the cervical level was conducted by the transcutaneous and direct methods in 30 neurosurgical patients in the postoperative period. Hypoventilation, hypoxemia, as well as microfocal pneumonia were relieved rapidly in 10 patients after transcutaneous ESPN. In 10 patients in whom the generally applied therapeutic measures proved ineffective, ESPN made it possible to cope with severe, rapidly progressing confluent pneumonia in the shortest time. The authors discuss the possibility of applying ESPN for restoration of spontaneous ventilation in central respiratory disorders (8 patients) and note the inefficacy of ESPN in pathological types of respiration and division of the phrenic nerve.

[New method of draining a syringomyelitic cyst].

A new method of operative intervention for syringomyelia is described in which after the syringomyelia cyst is opened its permanent drainage is accomplished by introducing into its cavity the free end of an adjacent posterior root transected before-hand directly at the site of the funnel-shaped transition of the dura mater from the spinal cord onto the cuff. As a result, all movements of the spinal cord are made together with this root, which prevents the draining root from falling out of the cystic cavity and does not create conditions for it to inflict damage to the cystic walls and the surrounding cerebral tissue. The method was used in operations on 9 patients.
