[A comparison of scintigraphy with radioiodinated MIBG and CT in localizing pheochromocytomas].

In order to define the diagnostic roles of MIBG imaging and CT in the detection of pheochromocytomas (pheos), the results obtained in 45 patients suspected of bearing pheo and studied with both modalities were analyzed and compared. Scintigraphy was correctly negative in 22/23 cases, correctly positive in 11/12 adrenal and 5/5 extra-adrenal pheos, and in 4/5 malignant pheos (metastases present in 2 cases were also identified). CT was correctly negative in 20/23 cases (a mass other than a pheo was detected in 3 patients); correctly positive in 12/12 adrenal and 4/5 extra-adrenal pheos and in 5/5 malignant pheos. Sensitivity, specificity and accuracy of scintigraphy and CT were 91% and 95.4%, 95.6% and 87%, 93.3% and 91.1% respectively (differences were not statistically significant). The overall data emphasize the complementary role of 123/131I-MIBG imaging and CT in the location of pheochromocytomas. A flow-chart essentially grounded on the combined use of both these diagnostic modalities is proposed which includes 123/131I-MIBG scintigraphy as a first choice examination.

Treatment of neuroblastoma with 131I-metaiodobenzylguanidine.

Seven patients with neuroblastoma (six children and one adult) were treated with therapeutic doses of high specific activity 131I-metaiodobenzylguanidine (131I-MIBG). Six patients were in stage IV and unresponsive to conventional treatment. One patient, in stage III, was treated at diagnosis, an approach never previously reported. Single doses of 131I-MIBG varying from 70 to 184 mCi split into two parts were administered by slow i.v. infusion (4 to 8 hours) at 2- to 4-day intervals. The following results were obtained in the six evaluable patients: two patients showed transient stabilization of the disease; three had an objective response, with shrinking of the primary tumor and/or regression of the metastatic lesions. Of these three patients, two suffered relapses at 2 and 7 months, respectively, from the first course of MIBG. The third patient, in whom the residual disease almost completely disappeared following MIBG therapy, is still alive in complete remission after autologous bone marrow transplantation with a follow-up of 14 months. The single patient treated at diagnosis showed a dramatic response to a relatively low dosage of MIBG, with histologically proved disappearance of the tumor mass. Our data indicate that MIBG may be useful in the treatment of neuroblastoma unresponsive to conventional chemotherapy. The complete response observed in the patient treated at diagnosis suggests that the full potentiality of MIBG therapy should be explored in untreated patients.

[A new medico-nuclear outlook for the diagnosis and treatment of pheochromocytomas: 131I-meta-iodobenzylguanidine (131I-MIBG). Comments on results obtained].

The diagnostic and therapeutic potential of 131I-MIBG, the new imaging agent with structural similarity to norepinephrine, was evaluated in 23 patients with a suspect pheochromocytoma. Seven out of 8 benign pheochromocytomas (2 ectopic) were correctly localized with 131I-MIBG scintigraphy (1 false negative). In 2 catecholamine-secreting malignant pheochromocytomas both the primary tumor and the metastases (1 case) were demonstrated. One uncommon case of dopamine-secreting malignant pheochromocytoma, as expected, was not shown. In the remaining 12 cases, found to be "normal" after completing the work-up, distribution of the radiopharmaceutical was evaluated using a semiquantitative grading system. 131I-MIBG scintigraphy proved to be a non invasive and accurate (95%) diagnostic modality, specific for neuroadrenergic tumors. The treatment with 131I-MIBG was also carried out in 2 malignant pheochromocytomas with encouraging results.

[Possibility of using 131I-meta-iodobenzylguanidine in the diagnosis of neuroendocrine tumors differing from pheochromocytomas].

The results obtained using 131I-MIBG to diagnose different neuroendocrine tumors in 17 patients are discussed. In 8 of the neuroblastomas studied the examination gave a wide spectrum of scintigraphic results according to the various stages of evolution of the tumors, thus supplying additional useful information for metabolic treatment. Contradictory data were obtained in 5 cases of medullary cancer of the thyroid. The overall results were unexpectedly negative for 2 carcinoid tumors and, as expected, negative for 2 ganglioneuromas. These data confirm that 131I-MIBG is a non invasive, reliable and easy-to-perform technique which is no longer limited to the diagnosis of pheochromocytomas, but can also be used for the diagnosis of other tumors derived from neural crest cells.

Scintigraphic localization of a disseminated malignant pheochromocytoma with the use of 131I-meta-iodobenzylguanidine.

Preliminary clinical studies with 131I-meta-iodobenzylguanidine, a newly synthesized radiopharmaceutical and guanethidine analog capable of imaging the adrenal medulla, have led to the identification of a case of disseminated malignant pheochromocytoma with the localization of brain and bone metastases. The result is of particular interest as the symptomatology in this case appeared rather equivocal and various investigations had led to a completely different diagnosis. This new scintigraphic technique has proved to be safe, specific and noninvasive, and it may have a clinical application as a complementary or alternative technique to conventional diagnostic tests.

Incidence of antibodies against rabbit liver specific lipoprotein (RLSP) in chronic active hepatitis.

In chronic active hepatitis (CAH) evidence exists that circulating autoantibodies against liver specific lipoprotein (LSP) could play a role in the development of hepatocellular injury. We evaluated the presence of autoantibodies in CAH against LSP using rabbit LSP, as antigen in a radioimmunoprecipitation test. Fifty-one patients with histologically diagnosed CAH were investigated. Among these 16 were HBsAg+, 15 were HBsAg-/anti-HBc+, 10 were non-A, non-B, 10 were autoimmune CAH. Anti-LSP were detected in six of 16 (37%) HBsAg+ (mean titre of 1:198); four of 15 (33%) HBsAg-/anti-HBc+ (mean titre of 1:246); two of 10 (20%) non-A, non-B (mean titre of 1:185); seven of 10 (70%) autoimmune CAH (mean titre of 1:307). No correlation was evident between the titre of anti-LSP and the values of AST, bilirubin or IgG. The findings seem to be consistent with the following conclusions: (a) CAH patients develop an humoral immune response to determinants in LSP which are not species specific. This is further evidence that rabbit LSP could be considered a suitable alternative to the human preparation in evaluation of autoimmunity in CAH and (b) the different behaviour of anti-LSP in patients with viral CAH (B, non-A, non-B) in respect of patients with autoimmune CAH suggests a variable importance of these antibodies in the mechanism of ongoing liver cell injury according to the various types of CAH.

The hepatic excretion of 131I-rose bengal and 99mTc-IDA derivatives in Rotor's syndrome.

Bilirubin kinetics and hepatobiliary excretion of some exogenous anions (BSP, 131I-rose bengal, diethyl and parabutly-IDA labeled with 99mTc) were studied in three patients presenting with Rotor's syndrome. Two were brothers; a nonjaundiced fraternal twin of one of them was also evaluated. The hepatic clearance of the radiopharmaceuticals was impaired in the affected patients but the degree of impairment was different among the tested anions, i.e., maximal for 99mTc-diethyl-IDA and minimal for 131I-rose bengal. Parabutyl-IDA was cleared better than the diethyl derivative. The metabolic derangement seems to be at the level of transfer from plasma to liver and of the hepatic storage, rather than at the level of hepatocyte excretory pathways, as in the case of Dubin-Johnson syndrome.

[Observations of the thyroid function after treatment of the laryngeal carcinoma with 60Co].

We have examined 16 patients with laryngeal carcinomas who had been submitted to laryngectomy and subsequent 60Co therapy (dose 60-70 Gy, thyroid exposure 50-60 Gy). After an interval of several years, some patients showed alterations of the thyroid function, i.e. a problematic hypothyreosis in two patients and a latent or compensated hypothyreosis in six patients. The latter could be demonstrated by several functional in-vitro tests (T3 saturation, T4, T3-RIA, TSHV, TRH). Especially the TRH test showed significant alterations in four patients. The results obtained from this relatively small number of cases correspond to the descriptions in literature of irradiation damages to the thyroid caused by the treatment of the laryngeal carcinoma. Despite the necessity of further control examinations of a greater number of patients, the results achieved hitherto suggest a prophylactic or substituting hormone treatment during the radiotherapy of the laryngeal carcinoma.

Functional study with 131I-rose bengal in chronic active hepatitis and nonactive cirrhosis using a three-compartment model.

The authors, using 131I-Rose Bengal, determined the fractional transfer constants of a three-compartmental model in 30 patients divided into two groups (chronic active hepatitis and nonactive cirrhosis) on a histological basis. Other functional indexes were also taken into consideration. In nonactive cirrhosis, the results show a greater impairment of Rose Bengal transfer from plasma to liver. In chronic active hepatitis K32 (fractional transfer coefficient from the second to the third compartment) appears significantly lower than in nonactive cirrhosis. No significant differences have been found for K12 and K03. The best differentiation between the two groups (consistent with histological data in 29 of 30 cases) was obtained by grouping K21, retention index (RI) and SGOT in the discrimination function: L=4.88 K21-0.016RI+0.0034SGOT. The ratio K12/K21 seems meaningful for diagnostic purposes and in the evaluation of the hepatocellular damage in single cases.

Bilateral pseudocystic pheochromocytoma.

A case of bilateral pheochromocytoma presenting as primarily cystic suprarenal masses is reported. The computed tomographic manifestations, possible etiology, and differential diagnosis of these cystic suprarenal lesions are discussed.

Growth hormone response to propranolol and L--dopa in obese subjects.

Oral administration of propranolol and L-dopa produced a marked increase in plasma growth hormone values in 12 obese subjects who had failed to respond to L-dopa alone. GH values after propranolol and L-dopa in obese subjects were not significantly different from those of normal subjects after L-dopa at 60, 90, and 120 min. Association of propranolol and L-dopa appears to be a simple means of investigating GH response in obese subjects.
