[Quantitative determination of total urinary protein utilizing the principle of Coomassie Brilliant Blue G250 binding to protein (author's transl)].

Coomassie Brilliant Blue G250 was used for the determination of total urinary protein and compared with the biuret-procedure. In the Coomassie Brilliant Blue G250-assay 0.10 ml urine are added to 5.0 ml Coomassie Brilliant Blue G250 reagent and the sample is read against the reagent blank. The Coomassie Brilliant Blue G250 method seems to be superior to the biuret-procedure because of better reproducibility, higher sensitivity and simpler handling. The upper limit or urinary protein excretion in 49 healthy subjects was 120 mg/24 h. To evaluate the clinical significance of the Coomassie Brilliant Blue G250 method the urinary protein concentration of 134 patients with metabolic, systemic and organ diseases was compared with the biuret-procedure. The regression line was y = 0.827 X + 8.713 mg/l with a correlation coefficient of r = 0.966. The type of proteinuria (mixed, glomerular, tubular) had no influence on the protein value measured with the Coomassie Brilliant Blue G250 method. However in selective proteinuria, like Bence-Jones protein excretion, there is no correlation between the two methods, because the concentration of Bence-Jones protein is underestimated. Lower protein values were also frequently obtained for patients with diabetes mellitus. Some specimens from patients with chronic renal failure treated by haemodialysis showed elevated values. In spite of these limitations the Coomassie Brilliant Blue G250 method might become the method of choice for the determination of total urinary protein.

[Haemoscreen in comparison with haemoccult as screening test for colorectal carcinoma (author's transl)].

Faeces of 1600 outpatients were tested for occult blood both with Haemoscreen and Haemoccult. A positive test was found with both methods in 35 patients (2.2%), a further 36 patients (2.3%) were positive only with Haemoscreen and 8 patients (0.5%) only with Haemoccult. In the first group six patients were found with colorectal carcinoma and 12 with large polyps, in the second group were one patient with carcinoma and four patients with polyps and in the third group two patients with polyps. Among patients with both tests negative carcinoma (n = 1) and polyps (n = 18) were diagnosed. Faecal testing was shown to be a simple and successful screening method for early recognition of intestinal carcinoma.

Dying as failure.

With the changes over the past forty years in the way people die and in the ways they are cared for when ill, the attitudes toward death have changed. When death seemed a metaphysical event, it commanded a kind of respect. Today, with the process of dying protracted--sometimes for years--it seems an evidence of failure.

[Trophoblastic tissue pulmonary embolus in a molar pregnancy (author's transl)].

We report a case of pulmonary embolus which is presumed to have been of trophoblastic tissue in a woman in her second pregnancy who had a hyperactive hydatidiform mole. (The size of the uterus was 27 cm after thirteen weeks of amenorrhoea. The level of urinary H.C.G. was greater than 2 million international units per litre.) One hour after the uterus had been evacuated under chemotherapy a picture of acute dyspnoea appeared, and this made it necessary to transfer the patient to an intensive care unit and to incubate her and ventilate her. The analysis of the blood gas showed desaturation and hypoxaemia leading to a diagnosis of massive pulmonary embolus. These complications were resolved in 72 hours and the levels of H.C.G. went down later under the influence of chemotherapy which was started one month after the evacuation and continued for fourteen weeks (for six weeks after the plasma H.C.G. levels had become negative).

[Present day problems in the diagnosis of diabetes (author's transl)].

The results of a longterm study showed that only a relatively small percentage of patients with limiting disturbance of glucose tolerance progressed in the direction of diabetic manifestation. There seems therefore to be grounds for the diagnostic term diabetes to be used only for clinically overt diabetes mellitus and to allow the definitions "subclinical diabetes, chemical diabetes, latent diabetes and symptomatic diabetes" to fall into disuse. On the question of quantity for the glucose tolerance test a 75 g load was proposed as a compromise because of the occasional poor tolerance of the 100 g load. For reasons of international comparability it seems better to adhere to the 50 to 100 g load as before.
