Coagulation studies in children and young adults with cerebral ischemic episodes.

One hundred and one patients below 45 years and showing objective signs of cerebral ischemia were studied retrospectively for pathogenic factors. Twelve were below 15 years; the male to female ratio was 1:1. Factors known as predisposing (heart disease, hypertension, hyperlipemia, diabetes mellitus or infectious diseases) and other possible factors (e.g. trauma, abuse) were found in 41 patients. Among women using contraceptive pills there might be an increased risk of development of cerebral thrombosis, but the material was not large enough to warrant statistical analysis. In 64 patients one or more abnormal coagulation values were found, the most frequent being a deficient vessel wall fibrinolysis, which was noted in 38%. We therefore consider it worthwhile to investigate the fibrinolytic defence mechanism of the vessel wall in patients with cerebral thrombosis, since it is possible to treat this condition with specific fibrinolytic stimulating agents.

Antithrombin III concentrate: its catabolism in health and in antithrombin III deficiency.

The catabolism of purified radiolabelled antithrombin III (AT III) concentrate was studied in two normals and two patients with congenital AT III deficiency both alone and combined with warfarin. The radiolabelling with iodine monochloride did not change the quality of the concentrate. The half-life varied between 3.4 and 4.8 days. No difference between normals and patients with congenital deficiency in non-acute stage could be observed in the catabolic parameters; nor was there any influence with warfarin.

Treatment with a pure factor IX concentrate in a patient with moderate hemophilia B undergoing bilateral total hip replacement.

Recently, a pure factor IX concentrate, licensed in Sweden as Immunine, was prepared through ion exchange and hydrophobic chromotography. Virus inactivation included two steps of steam treatment. Reconstituted Immunine could be kept at room temperature for at least 7 days without any substantial loss of activity. A patient with moderate hemophilia B undergoing bilateral total hip replacement was continuously infused with Immunine by use of a pump. There were no bleeding complications or signs of thrombosis, and the operation markedly improved the patient's range of motion and quality of life.

Recombinant factor VIIa in an infant with haemophilia A and inhibitors.

Post-traumatic bleeding from the gingiva in a 16-month-old boy with severe haemophilia A did not stop after treatment with factor VIII concentrate and tranexamic acid, and it was demonstrated that he had developed antibodies to factor VIII. Treatment with recombinant factor VIIa 60-90 micrograms/kg body weight four to eight times daily, together with local measures, stopped the bleeding and no complications were seen.

D-dimer and thrombin/antithrombin III complex--diagnostic tools in deep venous thrombosis?

In order to evaluate the usefulness of determining D-dimer and thrombin-antithrombin III complex (TAT) in the diagnosis of deep venous thrombosis (DVT), three D-dimer assays were tried: one ELISA and one latex test from Diagnostica Stago and one new latex method from Biopool. TAT was assessed using an ELISA (Behringwerke). We studied 96 consecutive outpatients with suspected DVT, of whom 36 had phlebographically confirmed DVT. Statistical calculations showed high sensitivity and a negative predictive value for the D-dimer ELISA (97% for both), confirming the results obtained by others. The new latex method (Biopool) showed similar figures (96% for both). The latex method from Diagnostica Stago and TAT showed lower sensitivity and negative predictive values. No differences in the D-dimer results were found with or without antifibrinolytics in the tubes for blood sampling. Our data suggest that negative results when using the new simple and cheap latex method (Biopool) may reduce the number of phlebographic examinations.

Femoral artery wall morphology, hemostatic factors and intermittent claudication: ultrasound study in men at high and low risk for atherosclerotic disease.

The aim of this study was to examine whether there was a relationship between ultrasound-assessed morphology of the femoral artery wall and hemostatic factors, and whether these factors were associated with intermittent claudication. One hundred and thirty men at high cardiovascular risk and 51 men at low risk were examined. The subjects (high- and low-risk) with moderate/large plaque (n = 96) had higher fibrinogen, thrombin/antithrombin complex and von Willebrand factor, compared to subjects with small/no plaque. The maximum intima-media thickness of the femoral artery was significantly associated with fibrinogen. These associations were independent of current smoking habits. Clinical atherosclerosis was associated with fibrinogen, von Willebrand factor, thrombin/antithrombin complex, plasminogen activator inhibitor activity, mean and maximum intima-media thickness and plaque status of the femoral artery. In conclusion, fibrinogen, von Willebrand factor and thrombin/antithrombin complex were related to plaque occurrence in the femoral artery. Clinical atherosclerosis was associated with fibrinogen, von Willebrand factor, thrombin/antithrombin complex and plasminogen activator inhibitor activity.

Autotransfusion of whole blood in massive bleeding. An experimental study in the pig.

An experimental model for controlled massive bleeding in pigs was developed to evaluate a new method for intraoperative autotransfusion of concentrated whole blood. The animals were injured in the descending thoracic aorta, creating a massive bleeding into the left pleural cavity. By means of a new system for autotransfusion, the animals were kept alive by retransfusing the blood at the same rate as it was shed. All animals were in good condition after autotransfusion of bleeding corresponding to three blood volumes. The postoperative hematological status including hemoglobin, hematocrit, platelet counts and electrolytes was not significantly different from the values before the experiment. The coagulation assays, including platelet adhesiveness, were normal after the experiment. In one pig the urine was positive for hemoglobin using a Hemostix test although it was macroscopically clear. This new method allows autotransfusion of whole blood with a desired hematocrit, with intact platelets and coagulation factors.
