A sensitive method for gas-chromatographic assay of barbiturates in body fluids.

The Authors present a gas-chromatographic method which allows for the rapid identification and assay of barbiturates and antiepileptics in body fluids. The Kupferberger procedure for extraction is used. The identification is obtained on two different columns: 10% Dexil 300 GC on Chromosorb W-HP and 3% OV 17 on Chromosorb G AW-DMCS (80-100 mesh). A considerable and interesting reduction of the barbiturates adsorption in the columns is obtained by preliminary conditioning with tetraethylorthosilicate. In this way, phenobarbital and cyclobarbital may also be assayed up to 1 microgram/ml in blood. For the simultaneous assay of therapeutic levels of phenobarbital and hydrantoins methylderivates are better employed: blood concentrations as low as 0.1 microgram/ml are easily detected.

Comparison of GLC-EMIT analysis for the assay of methadone and its major metabolite in urine.

The enzyme-multiplied immunoassay technique (EMIT) was compared with the gas-liquid chromatographic (GLC) method in the analysis of 130 urine samples from subjects receiving methadone treatment and non-methadone controls. The GC method allowed the quantitation of 2-ethylidene-1,5-dimethyl-3,3-diphenylpyrrolidine, and major metabolite of methadone. A good correlation was found between the two methods. The advantages of using the EMIT system are discussed.

Solid-phase extraction and HPLC-UV confirmation of drugs of abuse in urine.

A series of six liquid chromatographic methods were developed to confirm the presence of six classes of drugs of abuse in urine. The chromatographic separations were performed with a reversed-phase C8 column, except in the case of morphine, which was separated on a normal phase column. Isocratic and gradient elutions, ion pair technique, and UV detection were employed. Sample pretreatment involved the extensive application of solid-phase extractions and liquid-liquid extractions on solid supports. The specificity and sensitivity enabled the confirmation of morphine, benzoylecgonine, THC-COOH, amphetamine, and methamphetamine, six barbiturates, and nine benzodiazepines screened positive by EMIT in urine.

[Procedures for staging primary gastrointestinal lymphoma].

The non-Hodgkin's lymphomas (NHL) are usually generalized diseases and can involve almost any organ or tissue. Nevertheless also primary extranodal disease is frequent in NHL and some localizations are accomplished by a worse prognosis (brain, testicle) other, in particular primary gastrointestinal involvement, can be cured in a high percentage of cases with surgery, chemotherapy and radiotherapy. The gastrointestinal localization's symptomatology is not characteristic: abdominal pain is frequent and other symptoms can be present in other gastrointestinal diseases. Endoscopy, radiology and surgery are the most important diagnostic procedures. The first gives important information about the disease and it permits pre-operative diagnosis. The second is useful to detect both nodal extension (CT scan and MRI imaging) and intrinsic stomach or bowel involvement (contrast radiology of the gastrointestinal tract). The third is an important diagnostic and therapeutic moment. Some Authors recently don't agree with this invasive procedures routinely because of its potential morbidity and mortality. The determination of the extent of disease in patients with NHL is very important and serves multiple purposes: information regarding the imminence of potential complications, indication of prognosis and treatment planning. The most used staging system is the Ann Arbor scheme, originally designed for HG disease. This scheme is inadequate in particular for primary gastrointestinal NHL. Others, like Blackledge or Mushoff schemes show more correlation between tumor burden, nodal and extranodal involvement. An exact diagnosis, precise staging and a correct treatment bring to a potential curability.
