Evolution of cardiorespiratory variables in myxedema coma--a case report.

Normalization of hemodynamic, oxygen transport and oxygen consumption indices occurred within a week of treatment in a patient with myxedema coma. Ventilatory weaning and extubation was achieved using aminophylline, physical therapy and pleural drainage. Before extubation, naloxone was administered without any significant ventilatory improvement or change in endorphin levels.

Effect of nifedipine on TRH stimulation of TSH and PRL release by the pituitary gland.

Studies in vitro and in vivo have shown that thyrotropin-releasing hormone (TRH)-induced calcium ion changes in the adenohypophysial cells play an important role in release of hormones by the anterior pituitary. To determine the effect of the calcium blocker nifedipine on TRH-induced thyroid-stimulating hormone (TSH) and prolactin (PRL) release, TRH stimulation tests were performed before and after 74 hours of nifedipine therapy in ten patients. Although the magnitude of the TSH and PRL mean peak increase above baseline was slightly lower during calcium blocker administration (TSH 14.1 +/- 4.8 SEM v 16.4 +/- 4.5 SEM; PRL 37.7 +/- 4.5 SEM v 41.7 +/- 5.4 SEM), this was not statistically significant. Use of nifedipine in clinically effective doses does not appear to significantly interfere with TRH-stimulated release of TSH or PRL, in vivo.

Chylothorax review.

Chylous leakage from the thoracic duct into the pleural space may occur after any type of thoracic surgery; however, there are few reports of this condition after coronary artery bypass grafting. A case of chylothorax after combined coronary bypass and mitral valve replacement is reported to illustrate a discussion of its pathologic basis, diagnosis and management.

Recurrent unilateral lung disease.

A 25-year-old patient, with multiple pulmonary metastases from osteogenic sarcoma who two years ago developed respiratory failure with unilateral left lung involvement and was successfully treated with independent lung ventilation, was readmitted to an ICU with recurrent unilateral lung disease. Conventional therapy failed to reverse the respiratory failure; independent ventilation temporarily improved the patient's oxygenation. At autopsy, different involvement of the two lungs by the tumor was evident so that blood flow of the right lung and lymphatic flow of the left lung were impaired. When respiratory failure unresponsive to conventional therapy develops, anatomical abnormalities should be considered.

Non-invasive mechanical ventilation: the benefits of the BiPAP system.

Many of the complications with endotracheal intubation and invasive mechanical ventilation can be avoided with the use of non-invasive mechanical ventilation (NIMV). This technique has been especially successful in treating patients with acute respiratory failure (ARF). NIMV improves gas exchange, avoids complications caused by endotracheal intubation, and allows patients to talk and take medications orally. This article reviews our experiences treating 27 patients with ARF with a BiPAP (bi-level positive airway pressure) ventilator. This is a portable unit which allows for selection of different modes of ventilation and adjustment of inspiratory and expiratory pressures. Non-invasive mechanical ventilation should be considered in patients presenting with ARF who are hemodynamically stable and in whom spontaneous breathing is preserved.
