Afferent arteriolar adenosine A2a receptors are coupled to KATP in in vitro perfused hydronephrotic rat kidney.

Adenosine is known to exert dual actions on the afferent arteriole, eliciting vasoconstriction, by activating A1 receptors, and vasodilation at higher concentrations, by activating lower-affinity A2 receptors. We could demonstrate both of these known adenosine responses in the in vitro perfused hydronephrotic rat kidney. Thus, 1.0 microM adenosine elicited a transient vasoconstriction blocked by 8-cyclopentyl-1,3-dipropylxanthine (DPCPX), and 10-30 microM adenosine reversed KCl-induced vasoconstriction. However, when we examined the effects of adenosine on pressure-induced afferent arteriolar vasoconstriction, we observed a third action. In this setting, a high-affinity adenosine vasodilatory response was observed at concentrations of 10-300 nM. This response was blocked by both 4-(2-[7-amino-2-(2-furyl)[1,2,4]triazolo[2,3-a][1,3, 5]triazin-5-yl-amino]ethyl)phenol (ZM-241385) and glibenclamide and was mimicked by 2-phenylaminoadenosine (CV-1808) (IC50 of 100 nM), implicating adenosine A2a receptors coupled to ATP-sensitive K channels (KATP). Like adenosine, 5'-N-ethylcarboxamidoadenosine (NECA) elicited both glibenclamide-sensitive and glibenclamide-insensitive vasodilatory responses. The order of potency for the glibenclamide-sensitive component was NECA > adenosine = CV-1808. Our findings suggest that, in addition to the previously described adenosine A1 and low-affinity A2b receptors, the renal microvasculature is also capable of expressing high-affinity adenosine A2a receptors. This renal adenosine receptor elicits afferent arteriolar vasodilation at submicromolar adenosine levels by activating KATP.

A within-subject comparison of mandibular long-bar and hybrid implant-supported prostheses: evaluation of masticatory function.

Sixteen edentulous subjects participated in a within-subject crossover clinical trial to test the hypotheses that a long-bar overdenture attached to 4 implants gives greater patient satisfaction and masticatory efficiency than a two-implant hybrid overdenture. All subjects were given a new maxillary conventional denture. Ten received mandibular long-bar overdentures first and six the hybrid overdentures. Two months later, psychometric assessments and functional tests were repeated 3 times at one-week intervals. The mandibular prosthesis was then changed, and recordings were repeated after another 2 months. Mandibular movements and electromyographic activity of jaw muscles were recorded while subjects chewed standard-sized pieces of 5 foods: bread, cheese, apple, sausage, and carrot. Measurements included masticatory time, cleaning time (the time between the end of mastication and the last swallow), and duration and amplitude of masticatory cycles and phases. Multilevel analyses were performed. No significant differences in masticatory time were found between prostheses for any test food. However, cleaning time for carrot [estimated mean of difference (delta) +/- SE: 1.6 sec +/- 0.7] and bread (delta = 1.0 sec +/- 0.4) was slightly but significantly longer for subjects wearing long-bar overdentures. Cycle duration was longer with the long-bar overdenture only for subjects chewing carrot. The opening phase was shorter and the closing phase longer with the long-bar overdenture for almost all test foods. Vertical amplitude was significantly less with the long-bar overdenture for cheese (delta = -2.6 mm +/- 1.1), apple (delta = -2.6 mm +/- 1.0), and sausage (delta = -2.9 mm +/- 1.3). These results suggest that mastication with the 2 prostheses is equally efficient, although clearance of some foods from the mouth is longer with the long-bar overdentures. They also indicate that patients adapt their masticatory movements to the characteristics of different prostheses.

ACE inhibition and fibroblast growth factor in cultured human vascular smooth muscle.

The effects of the angiotensin converting enzyme (ACE) inhibitor, enalaprilat, and basic fibroblast growth factor (bFGF) on DNA synthesis and expression of ACE mRNA were examined in human vascular smooth muscle cells cultured from saphenous vein and internal mammary artery. DNA synthesis was estimated using 3H-thymidine uptake, and ACE mRNA was estimated by rt-PCR. Enalaprilat (0.125 microg/ml, 48 h) decreased 3H-thymidine uptake to 66+/-12% (SE) of the control without enalaprilat (p < 0.05). Basic FGF (10 ng/ml, 24 h) increased uptake by 41 +/- 12% (p < 0.05) while enalaprilat pretreatment (24 h) decreased uptake to 56 +/- 12% of this augmented value (p < 0.025). Basic FGF increased ACE mRNA, a process that was time dependent with an approximately 50% increase after 24 h exposure. Pre-exposure to enalaprilat (24 h) before bFGF reduced ACE mRNA to approximately 50% of that found in the presence of bFGF alone. The results indicate that ACE mRNA is present in human vascular smooth muscle cells and that exposure to an ACE inhibitor reduces DNA synthesis. Basic FGF stimulates DNA synthesis and ACE mRNA expression, and both of these effects are reduced by an ACE inhibitor. The results are consistent with the effects of bFGF being exerted through, or alternatively in concert with, angiotensin II. Further, they suggest that ACE inhibition can reduce the activity of the renin-angiotensin system by inhibiting the production of ACE, or at least the expression of ACE mRNA, in addition to producing enzyme inhibition at the ACE level.

Evaluation of recombinant chitinase antigen in serological diagnosis and surveillance of lymphatic filariasis.

Apply recombinant chitinase fusion protein antigen, enzyme-linked immunosorbent assays examined anti-filarial antibodies and evaluated of useful value in serological diagnosis and surveillance of lymphatic filariasis. The test jirds were immunized and infected by chitinase and B. malayi third stage larvae respectively. Functional protein molecular of chitinase was analyzed by SDS-PAGE, Western blot. The result shown that jirds from microfilaremia (mf) and donors with Mf were directly to react with chitinase antigen that positive rate was 100%, but Mf-xt antigen was only 80%. Normal jirds and persons sera from unepidemic control donors all were negative. False positives of 5% and 20% reacted with chitinase and Mf-xt antigens respectively. The results indicate that recombinant chitinase antigen is suitable for detection of active occult or patent lymphatic filariasis with daytime blood samples in residents of endemic areas, is easy to be performed and inexpensive.

[Experimental study of influence of yiqi huoxue serial recipes on basement membrane in membranous nephritis in rabbits].

OBJECTIVE: To clucidate the mechanism of the therapeutic effect of Yiqi Huoxue (YQHX) serial recipes on membranous nephritis. METHODS: Forty-nine New Zealand male rabbits were made to menbranous nephritis model by cation bovine serum albumin and divided into 5 groups, the group A (treated by Qingre Moshen granule), B (treated by Bushen Moshen granule), C (treated by steroid), D (the control group) and E (the normal group). Twenty-four hours' urinary protein content of the animals was determined every week, and plasma albumin, blood lipid, renal function and prostaglandins were tested by the end of experiment. And pathological changes of basement membrane were observed by using light, electronic and immunofluorescent microscopy with polyethylene imine stain. RESULTS: The 24 hours urinary protein content, plasma albumin and blood lipid in the group A and B were lower than those in the control group significantly, P < 0.01 or 0.05, while those in the group C and the D were similar, P > 0.05. In comparing the group A and B with the group C, the difference was also significant, P < 0.05. Light, electronic and immunofluorescent microscopic examination all showed that the pathologic changes in the group A, B and C were lesser than that of the control, the effect was in the order A > B > C. CONCLUSION: YQHX serial recipes can reduce urinary protein content, elevate plasma albumin level, restore the charge barrier effect of and attenuate the immune complex deposition on the basement membrane of glomeruli.

[Changes of the interleukin-1 in dysfunction of central nervous system in myasthenia gravis].

OBJECTIVE: Recent studies have demonstrated that acetylcholine receptor antibodies (AChRab) present in the serum of the patients with myasthenia gravis (MG) not only block acetylcholine transmission at the neuromuscular junction, but also cause dysfunction of central nervous system (CNS). The mechanism remains unclear. In the present study the concentration of interleukin-1 in the brain, thymus and blood was investigated to reveal the mechanism of CNS dysfunction in MG. METHODS: The concentration of interleukin-1 in brain, thymus and blood was detected after establishing the experimental MG model of central nervous system dysfunction induced by injection of AChRab purified from MG sera into the rat cerebral ventricular system. RESULTS: Interleukin-1 level in brain, thymus and blood started to increase one week after injection of AChRab. The level of interleukin-1 in brain tissue showed a very significant increase, as compared to that in thymus and blood till the end of 2 weeks after injection, where as in thymus and blood the increasing rate was relatively slow. CONCLUSION: It is concluded that interleukin-1 increased in rat brain, thymus and blood may play an important role in generating dysfunction of CNS in MG.

[Evaluation of DNA recombinant chitinase fusion protein antigens in serological diagnosis for filariasis].

OBJECTIVE: To evaluate DNA-recombinant chitinase fusion protein antigen in serodiagnosis for Bancroft's filariasis in jirds rats with microfilaremia and normal jirds rats with ELISA, and its prospect in the market to be spread. METHODS: Oligonucleotide was synthesized with filaria monoclonal antibody (Mf(1))-recognized microfilaria chitinase fusion protein and basic primer recognized by original clonal nucleosides, and amplified with polymerase chain reaction (PCR). PCR products were expressed in Pmal-c vector and purified by affinity chromatography. Experimental animals were inoculated with chitinase fusion protein antigen and infected with larval stage filaria Brugia malayi, respectively. Functional protein molecule of chitinase antigen was analyzed with SDS-PAGE and western blot. Serum level of specific antibody to chitinase antigen was determined by ELISA, and compared with that to microfilaria xt antigen (Mf-xt). RESULTS: Specific function protein molecule of the above-mentioned chitinase antigen has been identified as 69,000. Serum antibody against chitinase antigen was 100% positive for jirds and patients with microfilaremia, but, that against Mf-xt only 80% positive. Serum antibody against chitinase antigen was negative for all normal jirds and normal persons, but that against Mf-xt had 5% false positive in normal serum. For those with microfilaremia from non-filariasis-prevalent areas, false sero-positivity was 5% and 20% with chitinase antigen and Mf-xt, respectively. CONCLUSION: ELISA with DNA recombinant fusion protein antigen is a sensitive and specific method in serological diagnosis and surveillance for filariasis, with simple, easy and convenient manipulation. It can be used in field serologic surveillance and diagnosis for filariasis during the late phase in control of filariasis and can be spread in good prospects.

[Detection of bcr/abl gene expression on bone marrow cell colonies in chronic myelogenous leukemia by reverse transcriptase-polymerase chain reaction].

The t(9;22) (q34;q11) between abl and bcr genes plays a pivotal role in the diagnosis and pathogenesis of chronic myelogenous leukemia(CML). To explore the bcr/abl fusion mRNA expression on hematopoietic precursors, reverse transcriptase-polymerase chain reaction(RT-PCR) was applied to detect bcr/abl mRNA expression on bone marrow cell colonies. Meanwhile, bcr/abl mRNA expressions on 14- or 28-day colonies using HPP-CFC and CFU-GM semisolid agar culture assay were also determined in 4 cases of confirmed Ph-positive CML by karyotyping analysis. The results showed that the bcr/abl mRNA expressions on 14-day colonies and some 14- or 28-day colonies detected singly were positive at presentation by RT-PCR, in agreement with results by karyotype analysis. Thus, a sensitive and powerful technique was offered for studying gene expression on hematopoietic precursors, diagnosis and therapeutic monitoring of CML. Furthermore, this can be used as an ideal method for revealing molecular mechanisms of pathogenesis of CML and screening anti-CML drugs.

[Three-dimensional finite element analysis of stress in supporting bone of mandibular posterior fixed bridge. I. Stress analysis of the surface of alveolar bone around bridge abutments].

OBJECTIVE: Stress levels of the surface of alveolar bone around bridge abutment of posterior fixed bridge were studied. METHODS: The three-dimensional finite element model was developed on the basis of a try adult mandible. 6 vertical and 1 horizontal loadings were applied respectively, and the Maximum stress(epsilon max) and the Minimum stress(epsilon min) of the surface of alveolar bone around bridge abutment were calculated. RESULTS: The stress concentration exhibited at the apical and cervical regions. The stress of the bicuspid was about 2.1 times as high as that of the molar. When transverse loading was applied, the stress level was about 3.5 times as high as that when vertical loading was applied. CONCLUSION: The results were greatly valuable to design of posterior fixed bridge.

OBJECTIVE: Stress distribution of the cortical bone beneath the pontic of the fixed bridge was observed. METHODS: The three-dimensional finite element model was developed. 6 loadings were applied respectively and the Maximum Stress (epsilon max) and the Minimum Stress(epsilon min) of the cortical bone beneath the pontic of the fixed bridge were calculated. RESULTS: The stress values were very small before the bridge was inserted. After the bridge was inserted the stress increased, but the stress level in the cortical bone was much lower than that in the alveolar bone. The stress values of the cortical bone were only 9.5% (vertical loading) and 2.2% (horizontal loading) of the alveolar bone's. CONCLUSION: The stress value of the cortical bone beneath the pontic was very small. The load was born mainly by supporting tissue around the bridge abutment.

[Methodological study of measuring flow field uniformity downstream of mitral valve using Doppler echocardiography].

The objective of this study was to establish a quantitative measurement of flow field uniformity of mitral valve in vivo. The flow field uniformity downstream of mitral valve in normal persons and patients with mitral stenosis were studied using advanced color Doppler flow imaging technique. The results showed that a significant difference was found in the quantitative items of flow field uniformity between normal persons and patients with mitral stenosis (P < 0.05). There was significant correlativity between the items of flow field uniformity and other routine items of Doppler echocardiography. The authors conclude that the method is safe, non-invasive, accurate and repeatable, and it can really reflect the flow field uniformity of normal persons and patients with mitral stenosis.

[Evaluation of immunochromatographic test in the diagnosis of Plasmodium falciparum and Plasmodium vivax].

AIM: To evaluate the effectiveness of immunochromatographic test(ICT) in detecting Plasmodium falciparum and Plasmodium vivax in malaria endemic areas. METHODS: ICT was used to detect P. falciparum and P. vivax among patients with fever in the outpatient clinics by comparason with thick blood smear method. RESULTS: The sensitivity of ICT to detect P. falciparum and P. vivax was 96.7% and 90.4%, respectively. The specificity of ICT was 98.6%, and the coincidence rate was 94.7%. There is no cross reaction between P. falciparum and P. vivax. CONCLUSION: ICT could detect P. falciparum and P. vivax simutaneously, being more rapid and simple than blood smear method.

[A pilot study on malaria control by using a new strategy of combining strengthening infection source treatment and health education in mountainous areas of Hainan province].

AIM: To explore a new malaria control strategy that fits current epiodemiological condition and coincides with modern medicine model and the principle of cost-benefit. METHODS: The new strategy highlights the risk villages and risk population as the focal point and integrates health education with behavioral intervention. The main anti-malaria measures consists of carrying out health education in risk villages, giving mass drug administrations in risk population staying overnight in the mountain, following up malaria cases for implementing radical cure, but without using traditional residual spraying or impregnating bednets with insecticides. RESULTS: After having adopted the new strategy and taken the control measures, the people's knowledge about malaria increased to a higher level and the indices of malaria reduced to a lower level. The rate of bednet-using in the population was increased from 26.8% to 72.6%. The annual parasite incidence (API) of malaria was declined from 3.5% in 1994 to 1.1% in 1996 and 0.8% in 1997, and the API of falciparum malaria was declined from 1.0% to 0.3% and 0.3% respectively in the townships at the same time. The parasite rate(PR) of malaria was declined from 7.2% in May, 1995 to 2.1% in November, 1996 and 1.2% in October, 1997 and the PR of falciparum malaria was declined from 1.2% in May, 1995 to 0.1% in October, 1997. The proportion of villages without malaria cases was increased from 18.6% in 1994 to 54.2% in 1997, and the number of risk villages with a malaria incidence above 5% was reduced from 14 to 2 at the same time. The ratio of cost/benefit was 1:2.4 in 1995-1996 and 1:4.4 in 1997, showing a better economic benefit. CONCLUSION: The expectant result has been obtained, thereby providing new experience for the malaria control in the mountainous areas of Hainan Province.
