Intestinal permeability to 51Cr-EDTA and orocecal transit time in cystic fibrosis.

Intestinal permeability was investigated in 16 children with cystic fibrosis and 14 age-matched controls, making use of 51Cr-EDTA as probe molecule. Orocecal transit time was also evaluated using the lactulose/hydrogen breath test. There was an increase in permeability to 51Cr-EDTA in patients with cystic fibrosis (mean 13.62%) compared with the control group (mean 1.08%). The orocecal transit time was prolonged in patients with cystic fibrosis compared with controls. Mean fasting breath hydrogen concentration was significantly greater in cystic fibrosis patients (13 ppm) compared with control patients (3 ppm). There was no correlation between the percentage of 51Cr-EDTA excreted and the orocecal transit time in either control or cystic fibrosis patients.

[Cervico-facial heterotopic brain tissue with a mandibular deformity].

Case report of a premature baby girl who presented with a right cervico-facial mass which caused severe respiratory difficulty. The patient underwent subtotal resection of the mass whose pathologic specimen report revealed heterotopic cerebral tissue compatible with encephalocele. A head and neck CT Scan performed could not evidence a connection between the mass and the cranial cavity, reason for which the possibility of encephalocele was disregarded. After subtotal resection of the mass, the tumor began to grow from soft palate and the patient died from respiratory complications at four months of age. Autopsy reported heterotopic cerebral tissue from neck and soft palate; right lung pneumonia and infection by CMV. Although the heterotopic cerebral tissue was reported as benign, the clinical characterization of this mass is compatible with a malignant behavior due to the aggressiveness of its growth.

The prognostic value of right atrial pacing after acute myocardial infarction.

We performed right atrial pacing in 90 consecutive patients 10 to 30 days (mean 16.8 days) after acute myocardial infarction. Right atrial pacing was normal in 28 patients, depression of the ST segment occurred in 27 patients, systolic blood pressure fell below control values in 20 patients and, in 15 patients, right atrial pacing was non-diagnostic. Follow-up was from 12 to 28 months (mean = 17.3). Global mortality was 11.1%, with none of the patients with normal tests dying, 11% of those with ST depression, 30% of those with induced hypotension (P less than 0.01) and 7.1% of those in whom pacing was non-diagnostic. Patients with high clinical risk at discharge in Peel Class III-IV, showed 41.2% mortality during the period of follow-up. None of those had shown normal responses to pacing, but those dying included 50% of the patients with ST depression and 66.7% of those in whom right atrial pacing induced hypotension. Development of new angina during the period of follow-up was more frequent among the patients with ST depression (33.3%) (P less than 0.001). Thus, our results showed that right atrial pacing was useful in predicting mortality after acute myocardial infarction. In patients at high risk, we observed that a fall of systolic blood pressure was the best predictor of mortality.

[Myocardial fibrosis in 2 children with cystic fibrosis].

We describe the cases of two patients of 9 and 15 months of age who presented episodes of congestive heart failure at the age of 9 and 10 months respectively and who were diagnosed of cystic fibrosis. Death occurred during the first year after diagnosis due to heart failure. Necroscopic study showed pancreatic affectation and heart lesions consisting in areas of myocardial fibrosis and necrosis. We discuss the different hypotheses relating pancreatic affectation to myocardial fibrosis.

[Acquired immunodeficiency syndrome in childhood. Report of 3 cases and review of the literature].

Three infants with the acquired immunodeficiency syndrome (AIDS) are reported. Two cases belong to infants died with AIDS; one of them have as infection source multiple transfusions of blood during the neonatal period and the other case was an haemophilic children transfused several times with commercial factor VIII. The third case belong to an infant, four months old, with AIDS-related complex, born of mother intravenous drug abuser. The authors comments some characteristics about pediatric AIDS and the present problems.

[Plasmatic levels of gastrin in children under seven years (author's transl)].

Gastrinemia was determined in 100 healthy children whose ages ranged from three months to seven years, who were separated in three groups: from three months to two years, from two to four years, determining the gastrin by radioinmunoassay twice in each case. As no significant difference was found among the three groups, total results were grouped together to confect a graph ranging from the 5th to the 95th percentile. Average percentage of gastrinemia was determined at 76.81 pcg./ml. (50 percentile), with extreme values between 48.31 pcg./ml. (5th percentile) and 105.31 pcg./ml. (95th percentile).

[The influence of personality on suicidal behavior].

With the purpose of studying the influence of personality on suicidal behavior, 63 female suicide attempters were assessed by means of standardized interviews, Eysenck's personality questionnaire (EPQ-R) and other psychometric scales. Personality disorders (DMS III-R) were found in almost 70 per 100 and associated with previous attempts, higher hopelessness, suicide ideation, and to a more severe attempt. The most relevant dimensions were psychoticism and extraversion. The first one correlated to repetition, hopelessness, and suicide ideation; the second exerted a moderating effect, showing association with feelings of conformity towards survival. Neuroticism showed a rather unspecific effect on studied factors. Results confirm a relationship between personality disorders and suicidal behavior, and suggest possible mediating variables.
