Evaluation of abdominal pain in the AIDS patient.

Acquired immune deficiency syndrome (AIDS) is a recently recognized entity characterized by a deficiency in cell mediated immune response. The syndrome is manifested by the development of otherwise rare malignant neoplasms and severe life-threatening opportunistic infections. Case histories of five AIDS patients evaluated for abdominal pain are presented to demonstrate the unusual spectrum of intra-abdominal pathology that may be encountered in the AIDS patient. As the number of patients with AIDS continues to escalate, surgical evaluation and intervention will be required more frequently. An understanding of this syndrome and its complications is mandatory for the surgeon to adequately evaluate AIDS patients with abdominal pain.

Prenatal diagnosis in 3,000 women for chromosome, X-linked, and metabolic disorders.

In 3,000 women referred for prenatal diagnosis, 110(3.7%) abnormal fetuses were detected and 85 therapeutic terminations were performed. These were five main reasons for referral. Among the 2,227 women referred because of maternal age 35 years and older, there were 51 (2.3%) who had aneuploid fetuses. In the 297 women referred because of a previous child with Down syndrome, 3 aneuploid fetuses (1.0%) were detected. Of the 55 couples where one spouse was a carrier of a balanced chromosome rearrangement, 6 chromosomally abnormal fetuses were found (10.9%) (all the offspring of maternal carriers). In the latter group, five of the six heterozygotes with abnormal findings were carriers of tdic (13;21) translocations. In the 82 cases with a history of X-linked disorders, there were 40 males (49%). Thirty-five women were referred because of inborn errors of metabolism: 10 affected fetuses were found (28%). There was a greater proportion of sex-chromosome aneuploids as compared to trisomy 21 fetuses in the 35 to 39 year maternal age group. This was reversed in the group of women 40 years old and older. Of the 25 abnormal fetuses not terminated, 6 were sex chromosome aneuploids and 10 involved X-linked conditions where the progeny could be further prenatally monitored (eg, X-linked hydrocephalus) or treated (eg, hemophilia). In the remaining 9 the parents expressed divers reasons for their choice. Repeat amniocentesis was required in 2.9% of cases. One case of maternal cell contamination and one case of unconfirmed mosaicism were the only diagnostic errors found in the study. In the last 1,000 specimens referred, the average time necessary for a karyotypic result was 15.6 +/- 5.6 days after amniocentesis.

Growth induction in cystic fibrosis fibroblasts with low dexamethasone concentrations. Experience with application to genotyping.

Dexamethasone (DM) resistance was evaluated in fibroblasts from a pool of five patients with cystic fibrosis (CF) homozygotes, ten of their parental obligate heterozygotes, and seventeen age-matched controls of both sexes. The CF heterozygotes showed a mean DM resistance greater than homozygotes and both groups exhibited a higher mean DM resistance at every DM concentration than controls. However, substantial interassay variability rendered these differences in the total pooled data to non-significance. One control showed a consistently increased resistance and was possibly a covert heterozygote. It was concluded that the phenomenon of DM resistance was exhibited by CF heterozygotes and homozygotes but was not discrete enough for genotyping in the prenatal diagnosis of CF.

Measurement of ocular pursuits in normal children.

Ocular pursuits have been evaluated by occupational therapists (OTRs) in many settings, but normative information has been lacking. In this study, a standardized method of testing and recording ocular pursuits was developed and pilot tested on both normal and handicapped children. Normative data were then collected by using a cooperative research method in which 28 OTRs in the state of Colorado were trained, in a one-day workshop, to administer and score the test, achieving .98 inter-rater reliability for total test score. Each qualified tester evaluated approximately l5 normal children between 3 and ll years of age. The total sample was 489 subjects. There were some age-related trends, and most behaviors demonstrated a ceiling effect, particularly with older children. The results imply that many normal children have some immaturity in ocular pursuit skills until 6 or 7 years of age.

Social support and social adjustment: implications for mental health professionals.

The general importance of an individual's support network has been recognized in the field of community mental health; yet a more detailed understanding of how a client's available social ties may contribute to his or her adjustment is presently lacking. This study used network analysis to examine differences in the social networks of mental health clients to identify factors associated with positive social adjustment. Subjects were selected from three different types of mental health programs as well as from the general population of Marion County, Oregon. Results generally revealed that subjects from the community sample more often would look to immediate family members for support. Better functioning chronic clients emphasized professional contacts, whereas more poorly adjusted chronic clients would look to friends for support. The results have implications both for understanding the nature of the support available to a client and mobilizing the support resources of the existing network of relationships to aid adjustment to community living.

Using peer-group intervention with problem students in a secondary school.

Twenty adolescent girls whose records showed poor academic performance and frequent truancies and referrals to the guidance office were selected to participate in a pilot project to provide a peer-group experience for students having difficulty at school. Half the students were placed in a control group and received regular services from the school. Students in the experimental group attended 12 discussion sessions held during the school day. A one-year follow-up showed that seven in the experimental group and one in the control group had improved.

The "coast" hydrostatic bed.

The water bed has ushered in a new era in prophylactic care for patients suffering from the sequetiae of spinal cord injury and other debilitating diseases which have rendered the skin susceptible to decubitus ulceration. A commercially available water bed is described which has resulted from improvements to a prototype bed first used in the spinal injuries unit of Prince Henry Hospital in 1969 (Jones and Burniston, 1971).

Analysis of alpha-lipoprotein cholesterol in 50 microl of plasma.

A micromethod for quantitation of alpha-lipoprotein cholesterol was devised for gas-liquid chromatography to minimize plasma sample size and facilitate lipid studies using capillary blood from children or small animals. alpha-Lipoprotein cholesterol was measured by gas-liquid chromatography in 20 micro l of the centrifuged supernate obtained after addition of 5 micro l of mixed heparin-manganese chloride solution 1:1 (v/v) to 50 micro l of plasma. Comparison of venous alpha-lipoprotein cholesterol measured by gas-liquid chromatography with venous alpha-lipoprotein cholesterol measured by conventional heparin-manganese precipitation and ferric chloride (colorimetric) cholesterol determination gave a correlation coefficient of 0.98 for 80 plasma samples. Capillary alpha-lipoprotein cholesterol and venous alpha-lipoprotein cholesterol were closely correlated in 31 patients (r = 0.97).
