Plasma protein concentration and recovery from anaesthesia in man.

The effect of the concentration of plasma protein on the recovery from anaesthesia induced by equipotent doses (UD95) of thiopentone and Althesin was studied in 48 patients. The recovery time (RT) wa plotted against the concentrations of total plasma proteins (TPP) and albumin (Al). A small decrease in TPP concentration from normal range affected the recovery time minimally; a small decrease in TPP concentration from a low basal value markedly increased the recovery time with both agents. A large decrease in TPP concentration increased the recovery time from thiopentone and Althesin markedly.

The effect of histamine and H1 and H2 receptors on prolactin and luteinizing hormone release in humans: sex differences and the role of stress.

The present experiments were performed to investigate the possible role of histamine and its receptors, H1 and H2, in the control of PRL and LH release in normal adult humans of both sexes. Histamine infusion (200 microgram, iv, in 15 min) induced PRL and LH release in men; in women, histamine inhibited LH release without affecting PRL release. Two H1 antagonists, dexchlorpheniramine (10 mg, iv) and promethazine (50 mg, im), reduced PRL release in both sexes, stimulated LH release in men, and inhibited LH release in women. Cimetidine, an H2 antagonist (400 mg, iv), elicited PRL release in both sexes, more consistently in females than in males, and was without effect on LH release in either sex. These data suggest that in humans, the effect of histamine on PRL release is linked to H1 and H2 receptors, which respectively stimulate and inhibit PRL release independently of sex. The effect of histamine on LH release appears to depend on sex and to be mediated only by H1 receptors. To rule out the possibility that the effects of histamine are merely due to a nonspecific stress reaction, we have evaluated PRL and LH release in otherwise normal men and women undergoing surgery for gallstones. Surgery was accompanied by PRL release in both sexes, more evident in women, and by LH release only in men. These results indicate that the effect of histamine on PRL and LH release in humans is linked to sex and H1 and H2 receptors and is not due to stress; further studies are required to clarify the possible mechanism and site of action of histamine in modifying PRL and LH release in humans.

[Effect of proteinemia on recovery: a comparison between thiopentone and alphaxolone (althesin)].

The effect of proteinaemia and albuminaemia changes on recovery time following administration of an equipotent dose (DPC95) of thiopentone and alphaxolone has been evaluated. The study was carried out on two groups of 24 patients premedicated with atropine alone. Recovery time was more closely related to variations in albuminaemia compared to variations in total proteins: this results that albumins are the main binding-site of i.v. anaesthetics. Variations in total proteins and albuminaemia affect recovery time to a lesser extent when alphaxolone is used, compared with thiopentone. The result is related to the lower alphaxolone binding fraction. The clinical implications of protein binding fraction of anaesthetics is discussed.

Some technical modifications of surgical treatment of lumbar disc lesions.

The author's experience in the surgical treatment of lumbar disc lesions is reported. The following technical devices have been adopted: -- curved unilateral incision into deep fascia --interlaminar space widening by chisels and gouges, avoiding the use of rongeurs -- sodium succinate methylprednisolone injection into dural sac. -- early ambulation (within the first 18 hours). Benefits obtained by these procedures are: -- earlier disappearance of pain -- remarkable reduction of stay in hospital -- earlier and more complete recovery.

The relative potencies of thiopentone, ketamine, propanidid, alphaxalone and diazepam. A statistical study in man.

The potency of five i.v. anaesthetics (thiopentone, ketamine, alphaxalone (Althesin), propanidid and diazepam) was studied by determining the dose required to produce unconsciousness in 50% (UD50) and 95% (UD95) of patients. UD50 and UD95 were determined using the probit method. The effect of age on the anaesthetic requirement has been studied also. Relative to thiopentone, the potency was alphaxalone 9.22, diazepam 5.24, ketamine 4.37 and propanidid 1.006.

Primary sellar chondromas.

Two cases of chondromas arising primarily from the sella turcica are reported. The eight cases reported in the literature are reviewed. The basic clinical and radiological findings of these tumors and the difficulties of diagnosis and treatment are discussed.

Giant intramedullary ependymoma. A case report.

After a review of the international literature, the authors report a case of intramedullary ependymoma extending from the 1st cervical level to the 1st lumbar level completely removed in a one stage operation. The good functional recovery of the patient is emphasized.

[The pseudo-vascular syndrome of intracrânial metastases (author's transl)].

Our statistic consist of 162 cases of metastatic tumors of the brain (M.T.B.), of which 120 verified under surgery or at autopsy, and 42 diagnosed in the pre-operative phase and not undergone surgery. The pseudo-vascular form has been found in 58 cases (36%). If we only consider the supratentorial localizations, this percentage rises to 43,5%. This onset of symptomatology is quite predominant in males (52 cases 42%) compared to females (6 cases: 16%). The MTBs that in our statistics produce a greater frequence of pseudo-vascular forms are the MTBs of melanomas (66%), those of unknown origin (42%) and those of pulmonary origin (35%). These neoplasms are distinctly more frequent in males. On the contrary the mammary metastases responsible for most of the female MTBs originate to a pseudo-vascular form in only 22% of the cases. The etio-pathogenetic mechanism is double; it may be the manifestation either of arterial occlusion by neoplastic cells (embolia neoplastica attiva) or of disruption of the vascular wall and hemorrhage. As far as the diagnosis is concerned this syndrome can be quite easily recognised only when an extraneuraxial tumor is found, especially with a marked neuraxial tropism.

Ditazole and platelets. II. Effect of ditazole on in vivo platelet aggregation and bleeding time in rats.

Ditazole (4,5-diphenyl-2-bis-(2-hydroxyethyl)-aminoxazol) is a new drug shown to inhibit prostaglandin release from rat platelets. The present study indicates that ditazole, at doses similar to those inhibiting prostaglandin formation in rat, inhibits in vivo collagen--but not ADP-induced rat platelet aggregation. In addition, ditazole does not prolong the bleeding time in rats, but even tends to shorten it. This effect could be due to the inhibition of prostaglandin formation at the side of the vascular injury produced to induce bleeding.
