[ST-segment trend in patients with acute myocardial infarction treated with antithrombotic drugs (author's transl)].

Fifty-five patients were hospitalized in a Coronary Care Unit within the first 24 hours after onset of symptoms of an acute myocardial infarction. The sum of positive and negative ST-segment deflections of their twelve leads electrocardiograms (epsilon ST12) showed a maximum within the 3rd hour from the onset of symptoms. Thereafter, in the first 24 hours, there was a marked reduction in epsilon ST12, with a steep and significant fall within the 7th hour from symptoms. In the following nine days of this study, the patients showed 4 different epsilon ST trends, but there was no significant correlation with CPK curves, or with the kind of therapy the patients underwent. An irregular trend of epsilon ST12 or a secondary late rise (after 36 hours from symptoms) suggest an unfavorable prognosis (1 death in the epsilon ST-3 group, and 2 deaths + 1 ventricular fibrillation in the epsilon ST-4 group). No significant difference results between the patients treated with high doses Heparin plus Acetyl-Salicylic-Acid (A.S.A.), and the patients treated with A.S.A alone, though the first treatment seems to reduce the values of epsilon ST12 more rapidly; perhaps this behaviour is due to the use of antiplatelet drug A.S.A in both groups, and to the relatively small number of patients.

In vivo platelet release reaction in patients with heart valve prosthesis.

Platelets play an important role in the genesis of thromboembolic episodes frequently observed in patients following cardiac valve replacement. No simple and reliable method as yet exists for quantitating platelet function in vivo. Platelet-specific proteins, serum beta-thromboglobulin related antigen and platelet factor 4 as well as thromboxane B2 were measured in 50 healthy subjects and in 100 patients who had cardiac valve replacement at least 6 months previously; these were related to the type, site and number of valves replaced. Highly significant differences were observed in the mean plasma beta-thromboglobulin related antigen, platelet factor 4 and thromboxane B2 levels in patients compared with healthy controls. The elevated levels of platelet release products observed 6 months after cardiac valve replacement suggest a continuous activation by the prosthesis. No significant differences were observed in patients with different types and site of valve prosthesis.

Warfarin treatment during pregnancy: a clinical note.

The case history is presented of a woman aged 38 who received warfarin treatment because of a mitral valve replacement. During the first trimester of pregnancy, her prothrombin time was 20% of normal and kept at 40% until the 38th week of gestation. Then the patient received heparin until a few days after delivery of a normal child.

[Atrial myxoma. Clinical, diagnostic, surgical and pathological features of 13 subjects (author's transl)].

The clinical, surgical and pathological features of 13 patients (3 males, 10 females) with cardiac myxoma are reported. Eleven masses were situated in the left atrium and 2 in the right one. The incidence of signs and symptoms usually ascribed to cardiac myxoma is evaluated. Particular emphasis is given to embolization phenomenon which occurred in 3 patients with villous-surfaced tumors. The results of phonocardiographic, echocardiographic and isotopic angioscintigraphic examination are illustrated and the use of these incruent investigations is highly recommanded. Final diagnosis may be achieved by cardiac catheterization and angiocardiography. Successful surgical excision was carried out in 12 patients. Structural profile of atrial myxoma is discussed. Particular attention is given to the surface of the tumors (smooth or villous), to the pseudovascular structures on the light microscopy and to the endotelial like features, together with secretory activity of the myxoma cells, on the electron microscopy. The morphologic findings support the neoplastic origin of the myxoma. The distinction between embolization and malignant metastasis is underlined.

Short heparin thrombin clotting time, increased fibrinogen and platelet aggregates count in coronary disease: a probable hypercoagulable state.

Several indices of plasmatic and platelet coagulability (H.T.C.T., AT III, fibrinogen and Wu-Hoak test) were studied in 82 patients with acute and chronic coronary artery disease. The results were compared with those obtained in a control group. The most interesting result is the consistent shortening of H.T.C.T. as compared to the control group, particularly in patients with acute myocardial infarction. H.T.C.T. was always inversely correlated with the fibrinogen level. Those data suggest an important influence of fibrinogen levels on H.T.C.T., but this observation does not rule out the possibility that the heparin neutralizing activity (PF 4) will also influence the test. No positive correlation between H.T.C.T. and AT III could be observed. The elevated levels of AT III in acute myocardial infarction did not confirm the existence of a consumption of AT III due to chronic intravascular coagulation in these patients. The Wu-Hoak test increased only in patients with acute coronary disease, never in the other groups. In conclusion, H.T.C.T. may be assumed to be a valid test for indicating the existence of a possible hypercoagulability state.

[Rheologic, coagulative parameters and study of fibrinolysis in patients with coronary heart disease before and after aorto-coronary by-pass (author's transl)].

A study has been made of 31 patients with coronary heart disease where diagnosis had been clearly ascertained both from a clinical and strumental point of view. All patients had undergone aorto-coronary by-pass surgery. The controls of the parameters under observation (whole-blood and plasmatic viscosity, hematocrit, fibrinogen, euglobulin lysis, T protothrombin, T of partial thromboplastin, thromboelastogram antithrombin III, plasminogen, alfa2-macroglobulin and fractions C'3c, C'3c, C'4 of the complement) were carried out as follows: basic sample taken, I control (8th-10th day), II control (15th-20th day), III control (45th-50th day), IV control (85th-90th day) after surgical operation. A global examination of our results showed significant changes in the rheologic coagulative and fibrinolytic parameters after an aorto-coronary surgical operation. The slight tendency toward hypercoagulability met with in the basic blood sample (slight increase of whole-blood viscosity, hyperfibrinogenemy, inhibition of fibrinolytic activity) does not change significantly after surgical operation. This seems to indicate that the coronary by-pass does not in any way alter the evolution of arteriosclerosis.

Decreased thrombotic mortality and morbidity after valvular cardiac surgery as a consequence of adequate coumarin anticoagulation. A retrospective study in 516 patients.

Two almost identical groups of 258 patients each who underwent surgery for the substitution of one or more than one cardiac valve were taken into consideration. After surgery all patients, with the exception of those who received a porcine allograph, were given coumarin drugs for 1 year for thrombosis prophylaxis. In the first group of patients operated in 1972 and 1973 the laboratory follow-up and the therapeutic range were neither constant nor uniform. In the case of the second group, which included patients operated in 1974 and 1975, adequate laboratory follow-up and constantly adequate therapeutic ranges were always kept. This retrospective study has shown a statistically significant decrease in mortality due to thromboembolism and to unknown causes in the second group of patients (x2 = 8,313, "p" less than 0,0025). A decrease of the non-lethal embolic complications was also noted in the second group of patients (x2 = 6,813, "p" less than 0,0025). On the other hand, bleeding complications were greater in the second group of patients (x2 = 3,485, "p" less than 0,05) and this was interpreted as secondary to the higher anticoagulation administered to the latter patients. The results indicate that an adequate coumarin anticoagulation seems able to decrease the incidence of lethal or non-lethal thromboembolic complications after cardiac valve replacement significantly.

[Hematic and plasmatic viscosity in cyanotic congenital heart diseases (author's transl)].

57 patients with cyanotic congenital heart diseases were studied. 39 patients presented t. of Fallot, 18 patients had patent ductus arterious with pulmonary hypertension and right to left shunt. The average whole blood viscosity in patients with t. of Fallot was 6.86+/-0.32 cps. This parameter was correlated with hematocrit, fibrinogen, euglobulin lysis time, pulmonary output, systemic output, and O2 saturation. The value of "r" resulted as statistically significant only for the correlation between viscosity and hematocrit and between viscosity and fibrinogen (t=13.22, p less than 0.0005; t=3.35, p less than 0.001 respectively). The average value of plasma viscosity in patients with t. of Fallot was 1.61+/-0.024 cps. The correlations of this parameter with fibrinogen, euglobulin lysis time, pulmonary output and systemic output did not result as statistically significant. The average fibrinogen value was 256.10+/-20.63 mg%, whereas that of euglobulin lysis time was 12,67+/-1.50 hours. In patients with patent ductus arteriosus the average blood viscosity was 6.65+/-0.56 cps. Such parameter was correlated with hematocrit, fibrinogen, euglobulin lysis time, pulmonary output, systemic output and O2 saturation. The value of "r" resulted as statistically significant only between blood viscosity and hematocrit and between blood viscosity and O2 saturation (t=9.30, p less than 0.0005; t=5.800, p less than 0.001 respectively). The average value of plasma viscosity was 1.61+/-0.04 cps. This parameter was correlated with fibrinogen, euglobulin lysis time, pulmonary output and systemic output. The correlation index "r" never resulted as statistically significant. The average fibrinogen value was 299.20+/-20.30 mg. The average euglobulin lysis time was 16.02+/-2.73 hours.
