[Complete rectal prolapse in a patient with Hirschsprung disease: a clinical case].

The disease of Mya-Hirschsprung (HD) it's rare and congenital, usually diagnosed in child age, but that it can also remain unknown until the adult age. It's associated to genetic mutations and it can involve other pathologies and malformations. The variability of the anatomopathological and clinical phenomena is correlated to the length of the aganglionic segment. The Authors describe the clinical case of one young female patient, who presented chronic constipation (less than 3 evacuations to week), tenesm, meteorism, abdominal pain. To the inspection of the anal region after the evacuation the presence of complete rectal prolapse (3) cylinders of the length of 30 cm was appraised, with presence of ulcerations of the mucosa. The patient came subordinate to diagnostic study that they demonstrated the presence of HD. The Authors operated the patient with the technique of Frykman and Goldberg. In the follow-up the patient had the complete remission of the symptomatology and resumption of the rectoanal inhibitory reflex, remarkable diminished in the preoperative manometric examination. The pathogenesis of the association of HD and rectal prolapse goes searched in the presence of ultrashort HD (aganglionic defect of a limited segment of rectum), pathological variety somewhat rare that can determine subocclusive phenomena and that it favors the prolapse of the rectum for the continuous evacuating strains.

[Leydig-cell tumors of the testicle].

Leydig's cells tumor account for 1-2% of all testicular tumors. The authors report a rare case of benign Leydig's cells tumor in a young man with gynecomastia. They examine the embryogenesis, the hormonal modifications, the criteria of malignancy, the treatment and results. They present a review of the literature pertaining to this rare tumors.

[Video-laparoscopic cholecystectomy: first years of experience].

Laparoscopic cholecystectomy is became the elective operation in the treatment of symptomatic lithiasis of gallbladder, and it represent the surgical choice in 96% of cases. The authors on the base of their first years experience analyzes the results of literature with particular reference to the complications, like lesion of principal biliary tract and of other organs or vessels, underlining how the right selection of patients can be reduce morbidity. In this direction the subdivision of contraindication, in relative and absolute, already described in literature, represent an obliged chose to respect the mini-invasive principle which laparoscopic technique mean.

[Differentiated cancer of the thyroid: our experience].

The authors explain their experience about 136 patients affected by differentiated thyroid carcinoma. They compare different surgical procedure (total thyroidectomy vs conservative surgery). Although the surgical significance above the survival is discuss again, the authors, analysing the prognostic factors and the long term results, identify the total thyroidectomy maybe associated to lymphectomy and radioiodine therapy like the best treatment for this pathology.

[Morgagni-Larrey hernia: surgical treatment with Marlex. A case report].

The authors described a case of hernia of Morgagni-Larrey in aged patients with an acute sub-occlusive symptomatology. This type of diaphragmatic hernia is uncommon and characterized by an aspecific symptomatology which realize different problems in children and adult patients, in whom diagnosis can be made incidentally, executing analysis for other pathologies. Using Marlex mesh to repair hernia represents an improvement of surgery, because it enables to repair a great hernia with a better result, and no recurrence.

[Hürthle cell neoplasms: diagnostic and therapeutic evaluations].

Hürthle neoplasms have a low incidence, moreover diagnosis and clinical course are still unclear. As for follicular neoplasms the differential diagnosis between adenoma and cancer is possible only by the means of definitive histologic examination (capsule's infiltration, angioinvasiviness). The Authors analyze their experience about the treatment of 15 patients observed during 1986 and 1990 and work out a review of the literature. Hürthle neoplasms are similar to follicular neoplasms for many aspects: diagnosis based on the definitive histology, metastases via blood route, need for long-term follow up. Moreover they are considered as a unique pathological entity, biologically more aggressive than follicular neoplasms and the patients are considered as a "class at high risk". Total lobectomy and isthmectomy is the treatment of choice because an accurate diagnostic evaluation is impossible before surgery. In case of cancer a total thyroidectomy associated with lymphadenectomy reduces the incidence of local relapse. Long term survival is not influenced by surgical treatment. Independent prognostic variables are the capsule's infiltration and the growth's pattern only.
