Cosmetic oral and maxillofacial surgery.

The dental specialty of oral and maxillofacial surgery over the last fifteen years has been intimately involved in not only altering the bony facial skeleton, but also the esthetic defects of the facial soft tissue drape. These procedures are currently being taught, in many of our residency programs, along side the art and science of dentoalveolar surgery. When one combines the modification of both the skeletal and soft tissues, the resultant effect can be greater than the sum of its parts. Should one take this a step further and add to the list, proper arch form as prepared by the orthodontist along with cosmetic shape and appearance of the dentition by the restorative dentist using the latest in cosmetic dental techniques, what transforms is a result that has never been seen heretofore in the annals of dentofacial care. To follow are a number of case reports illustrating some salient points.

Oral and maxillofacial surgery: a new wrinkle.

A number of facial altering surgical procedures have been presented. Several areas are important to note. Most of these procedures were devised by practitioners lost to antiquity and individuals in a plethora of different fields, including orthopedics, dermatology, oral surgery, plastic surgery, and otolaryngology. Just as in the other specialties that perform these procedures, not all surgeons of the specialties perform the entire scope of their specialty. However, dentistry now has the capability to change the jaws, soft tissue drape of the face, neck, nose, and eyes, as well as smile lines and the shape and color of the dentition. No other profession in the history of human-kind has ever had this potential to make such facial changes.

From a humble, nascent beginning as a specialty to advance the then modern concept of alveolar surgery to its present state, oral and maxillofacial surgery is today vibrating with excitement. To the credit of dentistry, the profession now has a specialty that not only has the capability to alter the bony facial framework, but also can alter the collateral collagenous and superficial cutaneous structures. Combined with dental alteration of the dentition and gingival apparatus, the profession is nearing its zenith relative to having the capability of changing an entire face.

Total treatment planning for esthetic problems of the face: a team approach.

Oral and maxillofacial surgeons now have the capabilities to plan a total treatment for the entire face. In some instances, a team may be formed to deal with ancillary areas of expertise. Patients may be directed to members of the team who are cognizant of each other's capabilities. In this manner, the most complete type of treatment plan may be suggested to the patient.
