The effects of a load of phenĭlalanine on glucose metabolism.

The clinical and experimental hyperphenylalaninemic conditions seem associated with disorders of carbohydrates metabolism. Examples are the increased liver glucose turnover and the glycogen depletion under Phenylalanine load and the hyperexcretion of glucose in the urine of PKU children. We studied the effects of an acute load of Phenylalanine on Glucose-3-H3 kinetics and plasma glucose levels in adult male Wistar rats. The effects of the load were very slow and after 40 minutes we observed an increase in the rate of plasma glucose production (Ra), that in normal animals in an almost exclusive liver function. As result of this glucose production there was a modest increase in plasma glucose levels.

Mellituria screening in some metabolic diseases.

Mellituria was studied in 83 subjects: 25 normal adults and children and 58 patients with several metabolic diseases. In comparison to the controls, no significant differences were found in 9 patients with cystinuria and in 2 patients with Apert's syndrome. The large excretion of glucose was the only important pattern of the 11 patients with insulin-dependent diabetes. In 23 patients with porphyria cutanea tarda a statistically significant increase in the excretion of pentose was observed. In 16 children with the classical form of phenylketonuria, a significant hypoexcretion of glucose was found. This latter observation could be explained by the carbohydrate metabolic alterations described in experimental hyperphenylalaninemia.

A comparison of subcutaneous and intramuscular administration of human growth hormone in the therapy of growth hormone deficiency.

The sc and im administration of human GH (hGH) was compared in the therapy of GH deficiency. The peak and integrated concentrations of hGH in the plasma of the patients were similar after sc and im injection of an initial dose (0.1 U/kg) of hGH. The peak hGH concentration occurred at 2 h in both groups. The posttreatment height velocity and the change in height velocity at 3-month intervals were also similar in the im and sc groups. The somatomedin generation test resulted in a higher mean peak of somatomedin C after sc injection; however, if the individual peaks of somatomedin C were averaged, there was no difference between sc and im injection. A cross-over at 9 months of therapy to determine patient acceptance of im vs. sc injections indicated overwhelming acceptance of the sc route. The antibody responses to hGH were similar in both groups. We conclude that sc injection of hGH is an effective and safe mode of therapy for GH deficiency. The lipoatrophy that occurred infrequently at the injection site can be eliminated by rotation of sites. Subcutaneous administration of hGH will be more acceptable by the patients with less pain and less noncompliance.

[Limits of M-mode echocardiography in the diagnosis of cardiac tamponade].

The Authors examined the M-mode echocardiographic recordings concerning 15 patients with pericardial effusion; 2 of them showed clinical findings of cardiac tamponade. Some echocardiographic signs, related in literature as typical of cardiac tamponade, were analyzed: mitral E-F slope less than 50 mm/sec., right ventricular end-diastolic dimension at end expiration greater than 10 mm., reciprocal respiratory variations in right and left ventricular dimensions, notch on the right ventricular epicardial echo and/or interventricular septum during early systole. None of these signs was determinant in order to identify the patients with cardiac tamponade. Nevertheless one patient without cardiac tamponade showed all echocardiographic signs. The Authors believe that the diagnosis of cardiac tamponade remains eminently clinical; the utility of echocardiography consists in showing pericardial effusion.

[Echocardiographic and haemodynamic correlations in a case of hypoplasia of the right ventricle (author's transl)].

The authors correlate echocardiographic and haemodynamic reports of a 12 year old patient with congestive heart failure and enlarged heart, secondary to fibrotic cardiomyopathy which effected the right ventricle (RV) and left ventricle (LV) apex. The echocardiographic M-mode examination showed: dilatation of the RV; ectasis and akinesis of the apex of the LV; the left auricle small for hypoafflux; tricuspid valve very noticeable with defective end-diastolic closure, delayed 0,80 msec as regards to the mitral valve, and in which it was not possible to identify the proto-diastolic opening; typical movement of the pulmonary valve, according to French's description, with synchronous opening to the atrial systole; paradoxical movement of septum, with the peculiarity of showing the maximum posterior movement relating to the atrial systole. Catheterism indicated same pressures in the right auricle, RV and pulmonary artery, as is typical of right ventricle ipoplastic. Angiography confirmed echocardiographic reports revealing an enlarged right auricle and an ectasic and akinetic RV; the tricuspid valve was normally established and it remained almost constantly open; the apex of the LV was ectasic and akinetic. In conclusion, for the akinesis of the RV, the blood flow in the pulmonary artery was secured by atrial systole; such a haemodynamic situation had already been indicated by the echocardiographic examination for the absence of the real systolic closure of the tricuspid valve that the authors indicate as the most characteristic sign of such a table, together with the opening of the pulmonary valve with atrial systole.

[Effects of sodium salicylate on blood sugar].

The effect of constant perfusion of sodium salicylate on blood glucose has been studied in anestethyzed normale Wistar rats. The sodium salicylate perfused at the rate of 0,148 mg/min for 30 minutes has not statistically significant effect on glycemia.

[Metabolism of glucose-3-H3 during perfusion of sodium salicylate].

The effect of sodium salicylate perfused at constant rate for 30 minutes on Glucose-3-H3 kinetics has been studied in anestethyzed normal Wistar rats, controlling blood glucose levels, Ra and Rd. While blood glucose levels and Rd were not affected, the Ra values were influenced between the 20th and the 30th minute of salicylate perfusion.

[Free carbohydrates of the normal liver].

A separation of liver carbohydrates was carried by means of paper chromatography on deproteinized ethanolic extracts of frozen livers. The carbohydrates most frequently observed were Ribose, Xylose, Fructose, Glucose, Maltose, Maltotriose, Maltotetrose; isomaltose was found less frequently.

Artificial excretion of weakly bound middle molecules by hemoanadiafiltration.

The concept of a subtractive therapeutic intervention in several diseases (shock, acute liver, failure, poisoning) to remove toxic middle molecules (MM) has led to the devising of a particular ultrafiltration system (anadiafiltration) to clear weakly bound MM. Clinical hemoanadiafiltration (HADF) with a polyacrylonitrile filter and in vitro experiments with a series of marker substances, have shown HADF efficacy in clearing MM, irrespective of their molecular weight, with protein binding K between 50 and 150.

[Scoliosis and mitral valve prolapse. An epidemiological and etiopathogenetic screening (author's transl)].

The Authors have developed an epidemiologic screening among 299 subjects girl's secondary school (average 16.5 years) to verify the incidence of idiopathic mitral valve prolapse (MVP) and associated thoracic-skeletal abnormalities in young female people. The MVP was assessed in 10 subjects (3.3%), scoliosis in 41 subjects (13.7%). The MVP incidence among scoliotic subjects was 19.5% (8 among 41). To verify a possible common systemic connective tissue disorder between scoliosis and MVP, three groups of subjects (control, scoliosis, MVP), were visited by orthopedist, considering the incidence of following clinic parameters: recurvated knee, flat or valgus foot, pectus excavatum, lofty scapulas, joint and skin laxity. A significative increasing incidence of these parameters was found from the control group to the MVP group. Considering these parameters as index of a probable systemic connective tissue disorder, the hypothesis of a mesenchimal etiopathogenesis of MVP is confirmed.

[The action of cyclic somatostatin on hypoglycemia due to prolonged fasting in normal animals].

The authors have examined the action of cyclic Somatostatin on blood glucose levels in normal rats and in rats starved for 36 and 50 hours. The infusion of 0,235 gamma/min. of Somatostatin for thirty minutes in the normals induced a slight increase in blood glucose levels that was statistically non significative. Under the same condition, the cyclic Somatostatin increased, in a statistically significant way, the levels of plasma glucose in both starved groups of rats.

[Paper chromatography of the urinary carbohydrates in normal subjects: the aldoses].

The authors describe a method that utilizes paper chromatography for the separation and identification of urinary carbohydrates, especially the Aldoses. Using 5 ml. of urinary extract, it is possible to separate Lactose, Maltose, Galactose, Glucose, Fructose, Mannose + Arabinose, Xilose + Ribose, Glucuronolactone and other unidentified compounds.
