Favorable toxicity profile of raltitrexed in elderly patients treated for colorectal cancer: a case series.

BACKGROUND: Age-related physiological changes may lead to an increased toxicity of chemotherapy in the elderly, thus making tumor treatment difficult in this increasing subset of patients. OBJECTIVE: Since many trials claimed a favorable therapeutic index with raltitrexed, the aim of our preliminary study was to evaluate the anticancer activity and the toxic profile of this drug in the elderly. METHODS: Thirteen elderly patients with colorectal cancer, aged 75-90 years, were enrolled in a monochemotherapy treatment with raltitrexed. Due to their advanced age, the drug was administered with a 33% reduction of the dose. RESULTS: One partial response, four disease stabilizations, and two disease progressions were observed in 7 patients with advanced colorectal cancer. The patients with response or disease stabilisation had a satisfactory time to progression. Four out of 6 patients treated in the adjuvant setting for Dukes' C colorectal cancer remain disease free at observation periods of 15+ to 29+ months. Toxicity was virtually absent in all patients. CONCLUSIONS: The activity of monochemotherapy with raltitrexed appears to be appealing, above all because it is observed in the absence of toxicity. Though recent reports suggest some concern about severe complications of treatment with raltitrexed, administration of reduced doses of this drug seems to be a putative therapy for those patients who, because of their age, are highly susceptible to the adverse effects of chemotherapy.

Analysis of lung damage induced by trichloroethylene inhalation in mice fed diets with low, normal, and high copper content.

Copper is both an essential nutrient required for the activity of several enzymes and a toxic element able to catalyze free radical formation. Trichloroethylene (TCE) is a xenobiotic that generates epoxidic intermediates by bioactivation through the cytochrome P-450 system. In this study, the influence of a dietary copper imbalance on the TCE-induced lung damage was investigated. Weaning mice were fed copper-deficient, copper-sufficient, and copper-excessive diets. After 4 wk, mice were exposed for 30 min to 6,500 ppm of TCE and euthanatized 48 hr later. Lung damage in the TCE-treated mice consisted of vacuolations of Clara cells and was quantitatively evaluated by counting the vacuolated cells per micrometer of basal lamina. At the ultrastructural level, vacuolations appeared as the result of hydropic swelling of the endoplasmic reticulum cisternae. The copper-deficient mice presented the highest number of vacuolated Clara cells. These mice also showed alteration of the capillary endothelium and interstitium and decreased pulmonary copper-zinc-superoxide dismutase activity. Occurrence of oxidative stress in lungs of both copper-sufficient and copper-deficient mice following TCE treatment was indicated by a decrease in reduced glutathione and an increase in its oxidized form.

Automatic speech recognition in vitreo-retinal surgery. A project for a prototypal computer-based voice-controlled vitrectomy machine.

In the past half decade automatic speech recognition techniques, software and hardware technology have matured enough to support sophisticated medical applications. The project described aimed at introducing a computer-based, voice-controlled prototype system in a simulated vitreo-retinal surgery scenario. The aim was to provide the surgeon with a tool that could significantly improve the quality and ease of work and shorten the duration of intervention. The speech recognition system allows voice entry of simple commands to simulate surgical instrument control, including the infusion pump, vitreous cutter and diathermy. The project relies on a Markov-based, speaker-dependent, commercial isolated-word recognizer, and consists of a specific recognition vocabulary and application software, created and developed by the authors. Results have been encouraging. The system performed well under the test conditions, proving robust, simple to use and accurate (over 97% average word recognition rate). On the basis of their experience, the authors believe that automatic speech recognition technology, though suffering from some limitations such as the need for training, speaker dependence and a relatively small vocabulary, and requiring extensive testing under operating conditions, merits further development and opens new perspectives for a possible new generation of surgical instruments.

Hospitalizations and costs of treatment for protease inhibitor-based regimens in patients with very advanced HIV-infection (CD4 < 50/mm(3)).

PURPOSE: To describe the cost of hospitalization and treatment in patients with very advanced disease who tart different regimens based on a protease inhibitor (PI). METHOD: An observational retrospective analysis was performed on data from a 48-week randomized, multicenter study. Analysis was based on a subgroup of centers that were geographically defined. Costs of ordinary hospital admissions and of antiretroviral treatment were considered. Incidence of hospitalization and number of days free from hospitalization during the period of observation were calculated. Cost and hospitalization measures were compared among patients receiving three different therapeutic regimens: only PI, PI plus one nucleoside, or PI plus two nucleosides. A multivariate analysis was used to assess cost differences, controlling for variables potentially able to influence outcome. RESULTS: Overall, among 166 patients starting PI (PI plus two nucleosides, 71;PI plus one nucleoside, 65; only PI, 30), 162 ordinary hospital admissions were observed during about 1 year of follow-up. Monthly rates of admission per person and incidence of first hospitalization on 100 person-months showed a clear inverse relationship with the number of drugs comprising the baseline treatment regimen, with the lower rates for the triple therapy group (0.06 and 3.9, respectively), intermediate values for the dual therapy group (0.10 and 8.1, respectively), and higher rates for the PI monotherapy group (0.15 and 13.7, respectively). The average number of days free from hospitalization per month was 29.5 in the triple therapy group, 28.6 in the dual therapy group, and 27.9 in the monotherapy group. The results of cost analysis showed, despite higher cost of antiretroviral treatment, that global costs were progressively lower using regimens of increasing potency: Compared to PI monotherapy, global cost (costs of antiretroviral treatment and of hospitalizations combined) per month per patient was 31.9% lower for the triple therapy group and 19.3% lower for the dual therapy. Global cost for the triple therapy was 15.7% lower compared to global cost for dual therapy. After adjustment for CD4 count, AIDS status, and Karnofsky score, both hospitalization costs and global costs were significantly lower for triple therapy compared to monotherapy (p =.002 and.039, respectively). CONCLUSION: In advanced and nucleoside-experienced patients, PI-containing regimens have a differential impact according to the overall strength of the regimen, with the best effects on both hospitalizations and treatment costs obtained using PI within potent combination regimens.

[Health care inequalities: hospitalization and socioeconomic position in Rome].

Although the interest for equity is growing, scanty attention has been reserved so far in Italy to health care inequalities. The relation between hospitalization and socioeconomic position in Rome has been studied by evaluating overall heterogeneity and differences in access to effective non-discretionary treatments or at high degree of generic or specific inappropriateness. An area-based socioeconomic index was assigned to 86.4% out of 554.168 discharges of Rome residents identified during 1997 through the hospital information system. The analysis was performed by comparing standardized hospitalization rates across socioeconomic groups through linear trends and risk ratios. A significant inverse relation of overall hospitalization with socioeconomic position was observed for both acute admissions (+44% for most deprived males) and day hospital (+25%). No difference was found in use of effective treatments such as admissions in coronary care units for acute myocardical infarction or surgery for hip fractures. The inverse relation between socioeconomic position and acute hospitalization blunted in day hospital for inguinal hernia repair and actually reversed for cataract removal among females. The hospitalization risk for minor skin diseases, an ambulatory care sensitive condition, resulted inversely associated to socioeconomic position. An excess of hospitalization was also observed for poorest females undergoing appendectomy. Results indicate that observed heterogeneity between socioeconomic groups does not depend only on different health needs but also on an unequal utilization of services: although disadvantaged groups have equal access to treatments of non-discretionary effectiveness, they hardly use innovative services and are more vulnerable in receiving unnecessary treatments.

Sudden and unexpected infant death due to an hemangioendothelioma located in the medulla oblongata.

Herein, we report the case of a 4-month-old male infant dying suddenly and unexpectedly. Post mortem examination was requested with clinical diagnosis of Sudden Infant Death Syndrome (SIDS). Histological examination showed instead the presence of an hemangioendothelioma located in the medulla oblongata. Sudden unexpected death is the commonest form of death among babies between 1 month and 1 year of age. Although the vast majority of these fatalities are related to SIDS, a very small percentage is due to primary neoplasm. Necroscopy studies of sudden infant death should always include an accurate histological examination of the medulla oblongata on serial sections but seldom do.

[Disordered differentiation of mouse embryonic lungs in organ cultures under the action of nitrosomethylurea].

We have studied the effect of nitrosomethylurea (NMU) on differentiation of early rudiments of mouse embryonic lungs (12th day of embryogenesis) explanted into an organ culture. We have demonstrated that nontoxic doses of NMU are capable of accelerating normal lung differentiation both at the early (increase in the number of epithelial buds) and at the late (increase in the number of explants with regions of well-developed alveoli) stages of cultivation. However, NMU induces disturbances of differentiation, which appear as polycystic structures and hyperplastic nodules generally absent in the control.

Detection of crossing vessels at the ureteropelvic junction with fast MRI.

OBJECTIVE: Minimally invasive surgery is an alternative to open surgery in the treatment of ureteropelvic junction obstruction (UPJO) in selected pediatric patients. Anomalous crossing vessels represent a contraindication to endopyelotomy and may also interfere with the videosurgical pyeloplasty. We investigated the usefulness of magnetic resonance imaging (MRI) without contrast in the preoperative evaluation of anomalous crossing vessels in children. MATERIALS AND METHODS: From January 1999 to December 1999, 14 patients with diagnosis of hydronephrosis were prospectively evaluated with the aim of accurately plan the operative approach. The children underwent MRI study with HASTE sequences, heavily T2 weighted. Diagnosis was made without IV contrast material. No sedation was used. RESULTS: 4 out of the 14 patients studied were diagnosed as having anomalous crossing vessels at the ureteropelvic junction. They underwent open dismembered pyeloplasty and the diagnosis was confirmed at the operation. There was 1 false negative. CONCLUSIONS: The fast T2 weighted MRI allows a non invasive preoperative assessment of crossing vessels at the UPJO which is of paramount importance in the choice of the appropriate surgical approach. Nevertheless, the high cost and the low sensitivity (80%, 4/5) make this technique still not routinely applicable in the preoperative work-up of children with UPJO.

Modulation of DNA damage by vitamin A in developing Sprague-Dawley rats.

Prenatal and neonatal Sprague-Dawley rats were given a diet deficient in or with an excess of Vitamin A and at the age of 55 days female progeny were treated with a single i.g. dose of 80 mg/kg DMBA or 50 mg/kg MNU. Under these experimental conditions it was found that the exposure of perinatal rats to a diet containing an excess of Vitamin A caused a decrease in the amount of DMBA- and MNU-induced DNA damage in the mammary gland and the liver of the female offspring. When diets were deficient in Vitamin A there was a dual effect in terms of DNA damage detected in the same organs, namely DMBA caused an amount of DNA damage comparable to controls, while the extent of DNA damage induced by MNU greatly increased in both organs. These results indicate that Vitamin A can permanently change the sensitivity of adult progeny to chemically induced DNA damage when it is given to pregnant and lactating females.

[Changes in the cardiovascular chemo-baroreceptors as a possible basis for sudden infant death (SIDS)].

A male sixty days old infant, dying suddenly, has been diagnosed as sudden infant death syndrome (SIDS). Autopsy visualized plurifocal haematic suffusions, typical for an asphyxial status. Abnormalities were not found in the conduction system, but aorticopulmonary paraganglia hyperplasia with mild inflammatory characters has been noted. The role of aorticopulmonary paraganglia, as baro-chemoreceptors, in the cardiovascular regulation, has been anatomical substrate for explaining a sudden infant death, according to a multifactorial theory of SIDS.

[From the introduction to the possibility of moving beyond immunization obligation: considerations on the need for a guided procedure].

In Italy until sec. XIX, the beginning of vaccination duty allows to obtain some weighty aims in Public Health, like small pox eradication, polio and diphtheria elimination, or like the great reduction of tetanus and hepatitis B incidence. At the some, however, the vaccination duty doesn't allow to develop a "vaccination conscience" to do to accept the vaccines like the most important instrument, for effectiveness and utility, in the infectious diseases prevention. So, if from a side it's necessary to leave the vaccination duty from the another side it's necessary to make e leaded way of health education in the people and it's necessary to make a share of resources and aims in the sanitary world.

[Indoor allergens in office. Evaluation of the work station].

House dust mite and other indoor allergens play a prominent role in the pathogenesis of asthma and other allergic diseases. Several studies have shown a close relationship between sensitisation and/or onset of asthmatic symptoms and levels of indoor allergen exposure. Aim of the study was to investigate the concentration of specific markers of the indoor allergenic pollution, such as Der p 1, Der f 1, Mite Group 2, Fel d 1 and Bla g 2. Dust samples were taken using a standard method by means of a 1200 W vacuum cleaner connected with a dust-sampling device (MITEST). A standard A4 size area has been vacuumed four times during 2 min. The concentrations of Der p 1, Der f 1, Mite Group 2, Fel d 1 and Bla g 2 were determined in dust samples from 53 different sources (office chair and carpet) using a commercial kit (DUSTSCREEN). House dust mite allergens were not always detectable in the offices. Indoor allergen concentrations (Der p 1, Der f 1, Mite Group 2, Fel d 1) were significant higher in the work station (chair) than in the carpet (p < 0.0001). Der 1 exceeded the current threshold for sensitization in about 1/4 of the samples. Der f 1 was predominant over Der p 1 according to other studies. A good correlation between the results of Der p 1 and Der f 1 was observed both in carpet and work station. Cat allergen was ubiquitous and predominantly detected in the chairs because of the employees' clothes. No appreciable levels for Mite Gr 2 and Bla g 2 were detected. Such an exposure for 8 hours in every working day may be an important occupational risk for the development of sensitization/elicitation symptoms to house dust mite. To reduce mite allergen levels are necessary preventive measure by means of specific techniques and products as barriers for preventing the direct contact with allergens.

[The definitions and classifications of carcinogenesis, mutagenesis and teratogenesis used by national and international agencies and institutions].

Classifications of carcinogenesis, mutagenesis and teratogenesis used by the National Toxicological Advisory Committee in Italy (CCTN), the Commission of the European Communities (CEC), the Environmental Protection Agency in the USA (EPA), the International Agency for Research on Cancer (IARC), and the National Toxicology Programme in the USA (NPT) are examined and commented on, with a synopsis of several allocations made by the above organizations. However, the authors consider it advisable to redefine the categories and create 5 groups of substances (designated C1 to C5), on the basis of given definitions, with the aim of rendering the allocations comparable. This procedure is used solely for carcinogenesis and excludes the NTP. From such comparison, good agreement was found between CCTN and EPA and between CCTN and CEC, whereas agreement between IARC and other organizations was poor.
