[Diaminoxidase of the blood and blister fluid in burned patients].

Serous diaminoxidase (DAO) was measured in 28 patients affected by burns of various extents 3-8 hours after the lesion. In 32.1%, increases in DAO were encountered and these were greater and more frequent in more extensively affected subjects. After 7-10 days, DAO values were back to normal. Measuring DAO simultaneously in the serum and in the phlyctena fluid, it was observed that the latter constantly contains greater concentrations of the enzyme. This suggests that DAO is released or newly formed at lesional level although it cannot be excluded that the internal organs may also play a part in the release and neoformation of DAO as happens in experimental burns, after i.v. injection of heparin and after subcutaneous inflammation due to terpentine.

Left posterior hemiblock in acute myocardial infarction.

Fifteen cases of left posterior hemiblock associated with acute myocardial infaction were studied. In 5 cases the left posterior hemiblock was the only intraventricular conduction defect, while in the other 10 cases it was associated with complete right bundle-branch block. Left posterior hemiblock proved to be an early complication, appearing within a few hours from the onset of the acute episode, and an ominous sign, since hospital mortality rate was 87 per cent. Cause of death was mainly pump failure. In most of these cases ther was electrocardiographic evidence of infarction involving both anterior and inferior ventricular walls. Infarction of most or all of the ventricular septum was a common finding in the cases examined anatomically. Histologically, acute changes involving mainly the posterior septal and midseptal fibres were observed in 6 of the 8 cases studied. On the basis of these findings and of other published findings an alternative physiopathological mechanism for so-called left posterior hemiblock is proposed.

The human atrioventricular junctional area. A morphological study of the A-V node and bundle.

A study has been conducted into the morphological arrangement of the atrioventricular junctional area of human heart. The area was investigated in infantile, child, young adult, middle-aged, and old nodes. Although marked individual and ageing variations were observed, a general pattern of nodal architecture could be distinguished. The junctional area was therefore divided into four areas: (1) a transitional zone, (2) the compact node, (3) the penetrating bundle, and (4) the branching bundle. The transitional zone was intermediate between atrial myocardium and compact nodal specialized myocardium, but itself exhibited specialized characteristics. An important connection of the zone was to the myocardium of the left side of the interatrial septum. The compact node was itself composed of two segments which approached each other anteriorly from the mitral and tricuspid extremities of the septal anulus fibrosus. The junction of compact node and penetrating bundle could not be distinguished using cytological criteria. It was arbitrarily defined as the last point to make contact with transitional cells. Tissue distal to this was considered as penetrating bundle. A bypass tract was defined as any fiber contacting the bundle distal to this point, but such tracts were not observed in normal hearts. The branching bundle originated at the point of bifurcation of the penetrating bundle.

Ventricular preexcitation. A proposed nomenclature for its substrates.

The working party on ventricular preexcitation of the European Society of Cardiology has debated nomenclature of the syndrome at several meetings. This communication presents the terminology proposed by the morphologists within the study group on behalf of the other members. It is suggested that the problem should be approached on the basis of possibilities of avoiding the delay-producing area of the atrioventricular junctional region or short-circuiting the specialized ventricular conduction pathways. Thus we propose that the following pathways or contingencies are relevant to preexcitation: (1) accessory atrioventricular muscle bundles; (2) accessory nodoventricular muscle bundles, (3) atriofasicular bypass fibers, (4) fasciculoventricular accessory connections, (5) intranodal bypass fibers, and (6) nodal malformations. Each of these possibilities is defined, or, where only hypothetical, is discussed. In the case of the first two definitions, anatomical variations are indicated which satisfy the basic definition. Thus accessory atrioventricular bundles can be either septal or parietal, and can be composed of either specialized or nonspecialized myocardium. Nodoventricular bundles can connect either the transitional cell zone or the compact nodal region of the junctional area to the ventricular septum. Comparison is made of the new nomenclature with terms in present usage.

A histological survey of pre-excitation syndrome and related arrhythmias.

The scanty available data in histology of accessory AV pathways, namely James', Mahaim's and Kent's fibres, together with Paladino's bundle, defects in the fibrous annuli and "Ring tissue", were considered for their bearing on old and new pathophysiological questions about ventricular pre-excitation and complicating tachy-arrhythmias. The histological findings in nine hearts allowed for a realistic and synthetic schematization of all the main types of anomalous AV bypasses, which have been emphasized in discussing the subject. The inadequacy of basic knowledge concerning the anatomoclinical significance of James and Mahaim fibres and "Ring tissue" in ventricular pre-excitation is pointed out, and the importance of Kent bundles in pathogenesis of WPW is confirmed. In a case of supraventricular paroxysmal tachycardia, without pre-excitation, a Kent bundle was demonstrated, which suggests the possible existence of latent forma of WPW with antegrade-blocked AV bypass, underlying reciprocating tachycardias of this kind. New trends in morphological investigation on the AV junction and anatomophysiological bypasses in the area are outlined.

[Toxic polyneuropathy in shoe-industry workers. A longitudinal electromyographic study (author's transl)].

A longitudinal electromyographic study was carried out on fifteen shoe-industry workers (one man and fourteen women) affected with toxic polyneuropathy. The mean values of age, duration of exposure and lenght of the follow-up period were 23.2 years, 4.2 years and 12.2 months, respectively. Twelve subjects (80%) had a sensory-motor involvement of both upper and lower limbs; in the remainder, only the latter were affected. From a clinical standpoint, the polyneuropathy was classified as mild in seven subjects (46.6%), moderate in four (26.7%) and severe in four. On the basis of electromyographic criteria (maximal motor nerve conduction velocity; features of voluntary contraction tracings; existence of spontaneous activity recorded in different points of the same muscle), the polyneuropathy could be defined as mild in two patients (13.3%), moderate in three (20%) and severe in 10 (66.7%). In eight subjects (53.3%) the electromyographic abnormalities disappeared completely, through a gradual improvement in six and alternative phases of improvement and worsening in two. As for the remainder patients (7, viz. 46.6%), improvement and worsening in two. As for the remainder patients (7, viz. 46.6%), four showed a reduction of their electromyographic abnormalities, whereas no significant changes occurred in the last three. The obtained results are discussed and the following conclusions drawn: 1. the lower limbs are mostly affected, both clinically and electromyographically; 2. the clinical symptoms do not parallel the electromyographic findings; 3. the course of the disease appears to be independent of the initial electromyographic pattern; 4. recovery appears to occur in most instances within approximately eighteen months since the onset of symptoms, generally without significant residual deficit. Finally, the role of electromyography in the diagnosis and the follow-up study of the observed toxic polyneuropathy is underlined.
