Multifocal Purkinje-like tumor of the heart. Occurrence with other anatomic abnormalities in the atrioventricular junction of an infant with junctional tachycardia, Lown-Ganong-Levine syndrome, and sudden death.

A 13-month-old boy with intermittent preexcitation (Lown-Ganong-Levine type) and paroxysmal tachycardias of 280 to 300 beats per minute died suddenly with cardiac arrest. At necropsy the only abnormalities were in the heart. There were multifocal Purkinje cell tumors in the conduction system, including one directly at the bifurcation of the His bundle. In addition, a fault in the central fibrous body was filled with an accessory communication between the mid-portion of the atrioventricular node and both the interventricular septum and the His bundle. This accessory communication connected with the Purkinje cell tumor. Although an atrioventricular nodal bypass was thus present and could account for the preexcitation and for reentrant tachycardias, there is also the anatomic basis for an automatic arrhythmia originating within one or more of the Purkinje cell tumors, particularly the one directly within the His bundle. Dispersion of elements of the conduction system within the central fibrous body resembled the normal fetal pattern. Whether this persistence of fetal dispersion of the atrioventricular node and His bundle within the central fibrous body is causally related to the presence of multifocal Purkinje cell tumors or is only coincidence merits further consideration.

[Changes in the parameters of hepatic function in biliary lithiasis].

Our study investigates liver involvement serum parameters in 71 subjects with cholelithiasis and choledocholithiasis, without primary liver diseases. As a control group 118 healthy subjects were studied. All patients have been examined for serum transaminases, alkaline phosphatase and gamma-glutamyl-transpeptidase, total bilirubin and for liver enlargement. The results point out the presence of increased alkaline phosphatase and gamma-glutamyl-transpeptidase levels in more than half examined patients, and in nearly half of those without hyperbilirubinemia. This fact confirms that the existence of biliary lithiasis itself (even if limited to gall-bladder, in absence of jaundice) may account for the occurrence of a subclinical cholestatic liver impairment, that should be carefully considered in long-term prognosis of these patients.

[CT in the evaluation of the extension of bronchogenic carcinoma].

The computed tomographic patterns in 52 patients with histologically proven lung cancer are reported. The AA. underline the not sobstitutive role of C.T. scan in pre-operative determination of bronchogenic carcinoma extension. C.T. revealed a very high diagnostic accuracy in assessing the primary tumor extension and in evaluating distant macrometastases; however, in showing hilar and mediastinal nodal involvement has not the same accuracy. The AA. think that, at the present time, a combinate C.T. study of the chest, upper abdomen and brain is, in the most number of cases, a rapid, accurate and practical method to evaluate the extension of lung cancer.

Stage-dependent induction of prenatal tumors in mice by the Kirsten and Moloney strains of murine sarcoma viruses.

The Moloney (MoMSV) and Kirsten (KiMSV) strains of murine sarcoma viruses are known to induce mesenchymal sarcomas upon infection of newborn rodents. To determine their activity in mouse embryos, 11- to 15-day-pregnant CD-1 mice were laparotomized, and the single implants were inoculated into the abdominal portion of the embryonal body with an average of 15 and 1500 focus-forming particles/g of body weight of the MoMSV and KiMSV viruses, respectively. Another group of less than 1-day-old pups was given a comparable amount of either virus. Tumors appeared in the young within the first few weeks of life with incidences and histological types dependent on the gestational day and the viral strain inoculated. Mixed mesenchymal sarcomas at or near the site of inoculation and vascular tumors of the brain were by far the most frequent neoplasms observed in the newborn. With MoMSV there was an increased incidence of sarcomas with advancing age at treatment, being 0% at 11 days of pregnancy and 96% in newborn (P for trend, less than 0.025). By contrast, KiMSV caused an incidence of sarcomas below 20% throughout (P for trend, greater than 0.05). Brain tumors were identified in the several MoMSV and KiMSV groups, with a peak value of 43% following the inoculation of both viruses into 13- and 15-day-old embryos, respectively. While the total incidence of these tumors was significantly different from controls, no positive trend by day of treatment was found among the MoMSV and KiMSV viruses (P less than 0.05). The tumors were mainly capillary angiomas, but a few cavernous angiomas were also detected. In addition, eight pups which were given injections of both viruses at developmental Days 11 to 13 had tumors of the choroid plexus. In many instances, newborn pups were affected by multiple vascular abnormalities of the brain, including capillary telangiectases and multiple hemorrhagic areas. No such lesions nor tumors at any site were found among the control animals. The present results are important not only because of the evidence that Swiss embryos respond selectively to the carcinogenic effects by murine sarcoma viruses, but also because they offer the opportunity to dissect directly in vivo the mechanisms underlying the stage-related sensitivity of prenatal mice to oncogenic retroviruses.

[Prediction of the presence and severity of coronary arteriopathy and left ventricular dysfunction by discriminant multivariate analysis of the exercise test].

266 male p., with chest pain and without previous myocardial infarction were studied by exercise test and coronary arteriography. 17 had no coronary disease (group C-0), 22 had less than or equal to 50% coronary stenosis (C-1), 65 had severe 1 vessel disease (C-2), 73 had 2 vessel disease (C-3), 89 had 3 vessel disease (C-4). 95 had normal ventricular function (group V-0), 111 had hypokinetic ventricular segments (V-1) and 60 had akinetic ventricular segments (V-2). 11 exercise test variables were stepwise selected to discriminate the C-groups and the V-groups. For C-groups significant variables were: test result, heart rate increment, number of leads, work load, rate-pressure product, systolic pressure increment. By 4 discriminant functions, 41% p. were correctly classified. For V-groups significant variables were: hear rate increment, test result, effort angina, rate-pressure product, number of leads, ST depression, ST configuration, age. By 2 discriminant functions 53% p. were correctly classified.

Liver function in drug addicts: hepatitis B markers and cell-mediated immunity.

Liver function tests, hepatitis B virus (HBV) markers and cell-mediated immunity were evaluated in 100 drug addicts and in 54 healthy controls. Liver damage prevalence was much higher in the drug group, both in HBV marker-positive and in marker-negative subjects. Moreover HBV marker distribution was different among addicts in comparison with controls, in which the patterns of infection overcoming were more frequent. Cell-mediated immunity assessment showed a deep depression in the addict group. These data suggest that liver damage in addicts may be due mainly to the immunologic defect, which would have a negative influence on the action of HBV and of other viral and non-viral agents.

[Identification of patients with stenosis of the main trunk of the left coronary artery by ergometric variables].

In 354 patients with coronary artery disease (9 with greater than or equal to 90%, 42 with 50-75% and 303 with no left main (LM) stenosis, prevalence of ergometric criteria of severe disease was considered. In patients with LM stenosis were more frequent: work load less than or equal to 75 watt (p less than 0,001); ST depression greater than 2 mm (p less than 0,01); 3 or more leads involved (p less than 0,05); no increment of heart rate (p less than 0,01) or of blood pressure (p less than 0.01). Work load had the best predictive value (30%) of LM stenosis, associated with a reasonable sensitivity (55%). Combination ST depression + heart rate or work load + blood pressure had a predictive value of 100%, but minimal sensitivity (8%). In detecting greater than or equal to 90% LM stenosis heart rate had the best predictive value (13%), associated with high sensitivity (78%). Presence of 2 any criteria could detect all greater than or equal to 90% LM stenosis (sensitivity 100%, predictive value 11%). ST depression + heart rate had a predictive value of 100% and a sensitivity of 44%. It is concluded that no single or associated exercise variable has both high sensitivity and high predictive value of LM stenosis.

[Histopathology of aortocoronary glomera: modern anatomo-clinical approach to the understanding of cardiocirculatory disorders].

Recent advances in experimental and human physiology call for an updated anatomoclinical assessment of juxtacardiac neuroreceptors in cardiocirculatory disorders. An approach to this correlative side of the problem, hitherto neglected, has been made upon the basis of observations in normal, comparative, and morbid anatomy of the paraganglia in the cardiac and coronary plexus. Original topics are pointed out, in selectioned cases, aortocoronary bypass surgery included.

Prognostic significance of clinical and laboratory parameters in liver cirrhosis. A multivariate statistical approach.

Successive clinical and laboratory data of 73 cirrhotics, who died during a follow-up period, were evaluated. The relationships between the actual survival time and 22 potentially prognostic variables were studied by means of the log-rank test and the likelihood ratio test. Seven variables (presence of ascites, hepatic encephalopathy and oesophageal varices; serum albumin, total bilirubin, gamma-globulin percentage and cholesterol) showed a strict correlation with survival time, and were included in a multiple regression model in order to compute the specific "prognostic weight" of each parameter. Results show that the parameters directly related to liver cell biosynthesis or indicating a structural liver damage should be considered more reliable for predicting theoretical survival in patients with liver cirrhosis.

Histologic observations after closed chest ablation of the atrioventricular conduction system.

Postmortem examination of the heart was done in a case with disabling supraventricular tachyarrhythmia successfully treated by closed-chest ablation of the His bundle. A ventricular aneurysm developed as a result of a previous myocardial infarction, and death occurred suddenly five months after the procedure. Histologic studies of the atrioventricular (AV) junctional tissue demonstrated severe damage to the nearby AV node and penetrating His bundle. No damage to the branching His bundle or either bundle branch was observed.
