Sonography of small renal masses with indeterminant density characteristics on computed tomography.

Nine small renal masses with attenuation coefficients greater than 20 HU on computed tomography (CT) were termed indeterminate for cyst or renal cell carcinoma. Correlative sonography identified the fluid-filled internal architecture in four of five cysts and correctly characterized as solid four small renal cell carcinomas. The sonogram was particularly valuable in assessing masses that were indeterminate on CT when enhancement was equivocal or when intravenous contrast could not be administered. We conclude that sonography is extremely useful as a noninvasive procedure in evaluating the occasional small renal mass with CT number greater than 20 HU.

Sudden visual loss associated with clostridial bacteraemia.

A patient with endogenously acquired Clostridium septicum panophthalmitis is presented. The patient exhibited a striking sequence of signs and symptoms associated with this devastating ocular infection. Intensive antibiotic therapy was ineffective and enucleation of the globe was required. The microscopic pathology of the enucleated globe showed extensive infarction and necrosis of ocular structures in association with the panophthalmitis. In addition thrombosis of the central retinal artery and of choroidal vessels was observed.

Cutaneous histoplasmosis in the acquired immune deficiency syndrome--a report of three cases from Trinidad.

Three cases are reported of patients with the Acquired Immune Deficiency Syndrome (AIDS) and cutaneous histoplasmosis. Their initial presentation was that of a generalised maculopapular rash. Two patients were bisexual males and the third was an unmarried female. The range of opportunistic infections seen in AIDS patients in Trinidad is mentioned and clinicians are alerted to the fact that in areas endemic for Histoplasma capsulatum maculopapular rash in patients with AIDS may suggest disseminated histoplasmosis. The value of skin biopsy is mentioned.
